•One-10th of trials in preterm birth meets at least half of the usefulness items evaluated.•Recent trials (published from 2010 onward) have an increase in composite or surrogate (less informative) outcomes.•Use of informative outcomes, patient centeredness, pragmatism, and transparency should be key targets for future research planning.
