•Diagnostic algorithms are used to risk-stratify patients requiring imaging for DVT. •Age-adjusted & fixed D-dimer threshold >1000 had high sensitivity for proximal DVT. •Imaging was reduced for age-adjusted (4.4 %) & fixed D-dimer threshold >1000 (12.2 %).
