EP29.20: The rare case of “two left ovaries”

We present a case of a 41-year-old patient. The patient was three times pregnant, gave birth once and had history of two miscarriages. The patient was examined two weeks after the last miscarriage. Normal ovary was present on the right side of the uterus. In the assessment of left side of the pelvis, two structures adjacent to each other resembling ovaries were found, each with a maximum size of 29mm. None of these ovaries contained growing follicles as miscarriage occurred two weeks earlier. A more detailed analysis of the two “left ovaries” showed their independent mobility, therefore it proved separation of both analysed structures. The power Doppler analysis presented typical blood supply to the medially located left ovary and atypical, stronger, centralised vascularisation in laterally located left ovary. On further examination, it turned out that the lateral “ovary” was a structure located inside the large intestine (sigmoid colon) and suggested colorectal cancer. In the next day the patient underwent colonoscopy with resection of the tumour. Pathological diagnosis was high-grade tubulovillous adenoma. This case draws attention to the value of transvaginal ultrasound examination not only in the assessment of gynecological abnormalities of the pelvis, but also intestinal pathologies, including deep endometriosis and rectal cancer.
