Pre-clinical prevention, at its core, relies on a clear understanding of the mechanisms of the disease process itself. Many cardiovascular diseases result from the presence of diabetes mellitus and systemic hypertension, which in turn can be a manifestation of lifestyle. Heart transplant therapy is now well established, and the outcomes from such interventions are now generally very good following patient selection and donor genetic matching along with matching of other more physical constraints. Moreover, given that the chassis of the pacemaker will be fitted under the skin, the potential to load other sensors that may help optimise the management of cardiac conditions, such as heart failure, will almost certainly not be overlooked as these devices continue to be developed in the coming years.
