•Illustrated how active screening of rectal swabs reduces mortality of haematological patients with CRO BSI.•Preemptive positive rectal culture results and anti-infection treatment could significantly lower 30-d mortality for haematological patients complicated with CRO BSIs.•Antibiotic regimens based on CAZ/AVI were superior to those based on TGC or PMB.
