Giant caudate lobe in secondary biliary cirrhosis

Hepatic cirrhosis can lead to significant liver dysmorphia such as hypertrophy of the caudate lobe (hepatic segment I). This hypertrophy can complicate surgery and must be recognized and anticipated to plan the appropriate surgical technique. We report the case of a 16-year-old girl who was admitted to the hospital with secondary biliary cirrhosis due to chronic biliary obstruction. Computed tomography revealed evidence of portal hypertension and a giant caudate lobe. Given the context of end-stage liver disease, the patient underwent liver transplantation with uneventful postoperative course. Here, we will focus on the tendency of the caudate lobe to hypertrophy in cirrhotic livers and elucidate the reasons fot its enlargement.
