There is limited data on efficacy and safety of testosterone in women. The objective of this study was to evaluate the efficacy of transdermal testosterone in the treatment of hypoactive sexual desire disorder (HSDD) in all women, regardless of menopausal status, and its side effect profile. METHODS: A PROSPERO-registered systemic review of the literature was performed. The Ovid MEDLINE database was searched up to August 13, 2023 for randomized, placebo-controlled trials of transdermal testosterone formulations with adult women with HSDD. We performed a meta-analysis to assess whether efficacy and safety of transdermal testosterone was affected by menopausal type by standardized mean difference (SMD) and odds ratio (OR). RESULTS: We included 9 studies, containing 1476 patients in the testosterone group and 1496 patients in the control group. Use of transdermal testosterone significantly improved libido ratings in postmenopausal women (SMD 0.71, 95% CI 0.62-0.80, p<0.00001), including in subgroup analysis of surgical and natural postmenopausal cohorts (Figure 1). The overall adverse event rates between the testosterone and control groups did not significantly differ (OR 1.05, 95% CI 0.88-1.25, p=0.58), irrespective of subgroup analysis of premenopausal, and surgical and natural postmenopausal cohorts (Figure 2). The most common adverse events were application site reactions and mild androgenic effects of acne and hirsutism. Serious adverse events were rare. CONCLUSIONS: Our analysis highlights the significant and positive impact of transdermal testosterone on libido. This effect was observed across various cohorts, including those who reached menopause naturally or surgically. Our findings also provide valuable insights into the safety profile of transdermal testosterone therapy in this population.
