INTRODUCTION:The French National Agency for Medicines and Health Products Safety implemented in 2021 a nationwide experimentation for medical cannabis (MC), designed to assess the feasibility of implementing a new public health policy. A dedicated registry was established to support the notification of adverse drug reactions (ADRs). The objective of this study was to describe the safety data collected during the first 3 years of experimentation. METHOD:Data related to MC use were extracted from the registry and the French pharmacovigilance and addictovigilance database between March 26, 2021, and March 31, 2024. RESULT:Among the 3164 included patients, at least one ADR was reported in 1186 patients, including 81 patients (3%) with serious ADRs (SADRs). The most common ADRs were neurological (37.2%), gastrointestinal (16.9%), and psychiatric (15.2%). Among SADRs, six cases of acute coronary syndromes were reported, along with eight cases of suicidal thoughts including one suicide attempt. Half of these psychiatric events occurred in patients without a prior psychiatric history, and no cases of psychosis were documented. Regarding addictovigilance, only 12 ADRs were analyzed: seven withdrawal syndromes, three cases of tolerance, and two cases of THC-containing inflorescence misuse, including one case of MC use disorder. DISCUSSION:During the experimentation, 3% of patients treated with MC experienced at least one serious ADR. The safety profile was dominated by neurological, gastrointestinal, and psychiatric ADRs. Only one case of MC misuse was identified with inflorescences. This study is observational and non-comparative and cannot provide a comprehensive assessment of the risk associated with MC use.
Introduction Certains progestatifs sont des facteurs de risque de méningiome intracrânien toute localisation. Le méningiome sphénoorbitaire (MSO) est une tumeur intracrânienne rare, avec une composante osseuse notamment, difficilement accessible par chirurgie. Les objectifs étaient d’évaluer le risque de MSO associé à l’utilisation des progestatifs et de décrire le pronostic. Méthodes Nous avons réalisé une étude cas-témoins à partir des données du SNDS. Les cas incidents de MSO opérés entre 2011 et 2023 ont été sélectionnés et appariés à 10 témoins, selon la méthode du risk-set sampling, sur l’année de naissance, le sexe et le département de résidence. L’exposition à chaque type de progestatif et leur durée ont été recherchées. Les odds ratios (OR) et leur intervalle de confiance à 95% ont été estimés pour chaque progestatif par régression logistique conditionnelle. Résultats 4023 cas (âge moyen : 57,3 ans, femmes : 80,2%) ont été appariés à 40 230 témoins. Chez les 35 508 femmes, un surrisque de MSO était associé à une exposition actuelle d’acétate de cyprotérone (ACP) (39,4 [28,4-54,7]), de medroxyprogesterone (54,5 [6,0-493,4]), de chlormadinone (11,8 [9,4-14,8]), de nomegestrol (10,6 [8,7-13,0]), medrogestone (6,2 [2,7-14,1]), promegestone (2,7 [1,3-5,8]), desogestrel (2,5 [2,0-3,1]), contraceptifs combinés au lévonorgestrel (1,7 [1,4-2,2]). Chez les hommes, l’OR pour l’ACP était 25,0 [7,8-79,7]. Une relation dose-effet était retrouvée avec par exemple des ORs de 90,0 [11,4-710,4] et 88,1 [54,4-142,6] pour les hommes et les femmes avec une utilisation ≥ 5 ans de l’ACP. Le nombre de cas de MSO attribuables aux progestatifs chez les femmes était 694/3228 (21,5%). Au niveau du pronostic, 16,9% des cas étaient sous antiépileptiques après trois ans, 5,9% étaient réopérés au niveau sphénoorbitaire et 17,7% décédaient dans les 10 ans après la première opération. Discussion/Conclusion Un surrisque de MSO pour les utilisations de huit progestatifs a été démontré, avec une relation dose-effet. Plus d’un cas sur cinq de MSO était attribuable à l’utilisation des progestatifs. Le pronostic du MSO après la chirurgie montre la sévérité de cette tumeur.
Deprescribing, defined as the supervised reduction or discontinuation of inappropriate medications with the aim of improving quality of care, has become a major public health issue. Long guided primarily by clinical and economic considerations, deprescribing now increasingly incorporates an environmental dimension, particularly with regard to the ecological footprint of the pharmaceutical life cycle. This article presents a synthesis of key insights from an interdisciplinary workshop held at Sciences Po in December 2024, which brought together researchers, clinicians, institutional representatives, and health system stakeholders. The discussions highlighted that France remains a country with high levels of medication consumption, exposing patients to substantial risks of iatrogenesis while generating significant environmental impacts. Speakers emphasized the existence of structured international deprescribing initiatives, whose implementation in France would require adaptation to the specific features of the national health system. Despite institutional efforts led by the French National Health Insurance, the National Authority for Health, the National Agency for the Safety of Medicines and Health Products Safety, and regional observatories, several obstacles persist, including the lack of consolidated data, deeply rooted prescribing cultures, organizational constraints, and ambivalent patient expectations. Key levers for action were identified, such as greater recognition of medication review activities, the development of decision-support tools acceptable to health care professionals, the strengthening of territorially based multiprofessional dynamics, and the active involvement of patients. The case of psychotropic medications particularly illustrates the complexity of deprescribing practices. Beyond existing approaches, the discussions point to the need for a structured national strategy to embed deprescribing within a sustainable health framework, articulating quality of care, equity, and ecological transition.
Introduction L’objectif de cette étude était d’évaluer la sécurité du vaccin contre le virus respiratoire syncytial Abrysvo® administré chez les femmes enceintes au cours de la campagne de vaccination 2024–2025 pour une immunisation in utero des enfants. Méthodes À partir du SNDS, toutes les femmes ayant accouché après 22 semaines de gestation entre le 16 septembre et le 31 décembre 2024 ont été incluses. Les femmes vaccinées par Abrysvo® ont été appariées 1:1 avec des femmes non vaccinées. Différents risques maternels et fœtaux ont été modélisés à l’aide de régression de Poisson pondérées par score de propensions permettant d’estimer les rapports d’incidence pondérés (wIRR) et leurs intervalles de confiance à 95 %. Résultats Parmi les 29 032 femmes vaccinées, 24 891 (87,4 %) ont été appariées avec succès à 24 891 témoins non vaccinées. L’âge maternel moyen était de 30,9 ans (±5,0), 3,2 % présentaient des antécédents d’accouchement prématuré, 0,6 % avaient une grossesse multiple et 21,8 % avaient reçu une vaccination antigrippale. Aucun excès de risque significatif n’a été observé pour l’accouchement prématuré (wIRR = 0,97 [0,89–1,06]), la mortinatalité (0,77; [0,45–1,32]), la césarienne (1,00 [0,96–1,03]), le petit poids pour l’âge gestationnel (1,01 [0,96–1,07]), l’hémorragie du post-partum (1,03 [0,97–1,10]), la prééclampsie (1,02 0,85–1,22]) ou les événements cardiovasculaires majeurs (0,60 [0,26–1,40]). Dans les analyses en sous-groupes, parmi les femmes vaccinées à 32 semaines de gestation ou avant, une augmentation du risque d’accouchement prématuré, à la limite de la significativité, a été observée (1,13 [0,98–1,31]). Discussion/Conclusion Aucun signal de sécurité majeur associé à la vaccination par Abrysvo® pendant la grossesse n’a été mis en évidence sur les paramètres étudiés. Un risque potentiellement accru d’accouchement prématuré chez les femmes vaccinées à 32 semaines de gestation ou avant, bien que non statistiquement significatif, suggère un rappel de fenêtre recommandée de vaccination entre 32 et 36 semaines d’aménorrhée.
Purpose NUT carcinoma is an ultra-rare and highly aggressive malignancy lacking authorised systemic therapies. Birabresib, a bromodomain and extra-terminal (BET) inhibitor, has shown preliminary clinical activity; however, its development was discontinued and no GMP-grade active pharmaceutical ingredient or finished product is available. This work describes the hospital-based pharmaceutical development enabling authorised therapeutic use of birabresib in France. Methods Within the French ANSM temporary usage protocol (PUT), a quality- and risk-based Chemistry, Manufacturing and Controls (CMC) strategy inspired by ICH Q8-Q10 was implemented to convert a research-grade material into a qualified drug substance for compounding. An initial batch of birabresib dihydrate underwent comprehensive CMC-like characterisation, including purity, identity, structural confirmation, solid-state properties, residual solvents, and forced degradation to establish a primary chemical reference substance and a stability-indicating analytical method. Results The generated data supported acceptance criteria for subsequent batches and for 20-mg capsules compounded under Good Preparation Practice in a centralised hospital pharmacy. Finished product controls complied with pharmacopoeial and ICH requirements, and capsules were enrolled in a prospective stability programme under PUT-defined storage conditions. Conclusions This risk-proportionate, CMC-oriented hospital framework enabled the first authorised therapeutic use of birabresib in NUT carcinoma and may be extended to other discontinued small molecules used in regulated access programmes for ultra-rare diseases.
