Introduction Les pénicillines (PEN) sont des antibiotiques (ATB) de 1ère intention pour de multiples infections et prophylaxies. Cependant, de nombreux patients sont étiquetés à tort allergiques, entraînant l'utilisation d'ATB de 2ème ligne plus toxiques, moins efficaces ou à plus large spectre, augmentant le risque de sélection de mutants résistants ainsi que la morbi-mortalité. L'objectif principal de ce travail est d'évaluer l'impact du pharmacien sur la réévaluation et la « délabélisation » des patients faussement renseignés allergiques aux PEN. L'objectif secondaire est de renseigner les familles d'ATB que peuvent recevoir les patients dont le statut allergique a été confirmé. Matériels et méthodes Une étude rétrospective, monocentrique, observationnelle sur 6 mois a été réalisée par un interne et un pharmacien. Les patients étiquetés allergiques aux PEN ont été identifiés par l'intermédiaire de 2 méthodes; le module « allergie » du dossier patient informatisé (DPI) et via les ATB alternatifs aux PEN. Les données concernant l'allergie ont été recueillies grâce au DPI, au patient et à la pharmacie d'officine (PO). Les conclusions de « délabélisation » ont été émises en s'appuyant sur un algorithme validé avec l'équipe d'infectiologie. Résultats Sur la période de 6 mois, 331 patients ont été inclus dans l'étude dont 78,5% (n=260) étaient des femmes (p <0.001). Après analyse du DPI, 75,5% (n=250) des patients ont bénéficié d'un entretien téléphonique et la PO a été sollicitée pour 32,4% (n=81). Au total, 10,6% (n=35) des patients ont présenté des manifestations non allergiques et 2.7% (n=9) ont déclaré ne pas être allergique à ces ATB. La réaction a été caractérisée de non sévère et sévère pour respectivement 47,7% (n=158) et 20,3% (n=67), et est survenue il y a plus de 5 ans pour 89,9% (n=258). Les patients ont été réexposés à une bêta-lactamine (BL) dans 46,7% (n=134) des cas; 43,3% (n=58) ont reçu uniquement des PEN; 33,6% (n=45) uniquement des céphalosporines (CL) et 23,1% (n=31) les deux. Une réaction allergique a été notifiée chez 27,0% (n=24) des patients réexposés à une PEN et une notion de réaction a été retrouvée pour 7,9% (n=6) des patients réexposés à une CL. Cette étude a permis de « délabéliser » 56,8% (n=182) des dossiers dont 29,3% (n=91) totalement et 27,5% (n=91) partiellement. Ces chiffres étaient respectivement de 64,6% (n=214) et 8,2% (n=27) pour les autres classes de BL. Cette « délabélisation » a été possible dans 75.5% des cas grâce à l'interrogatoire patient (p <0.001). Pour 29,6% (n=98), les PEN ont été contre-indiquées (CI) dont 36.7% (n=36) de manière absolue et 63.3% (n=62) en l'absence de bilan allergologique et réexposition aux PEN. Les BL ont été CI pour 14,5% (n=48) des patients. Conclusion La réévaluation des allergies aux PEN est primordiale pour optimiser la prise en charge des patients et limiter l'utilisation inappropriée des ATB. Cette étude a montré qu'un entretien patient permettait fréquemment de désétiqueter les patients renseignés à tort allergiques et que le pharmacien clinicien, en tant que professionnel du médicament, avait toute sa place dans ce processus.
Gamma heavy-chain disease (gHCD) is a rare lymphoproliferative disorder characterised by the production of truncated gamma heavy chains that are not associated with light chains. We present the case of a 79-year-old man with a known history of chronic lymphocytic leukaemia who presented with acute confusion, hypercalcaemia and ascites. Workup revealed monoclonal gamma heavy chains in serum without corresponding light chains. Treatment with a combination of rituximab, bortezomib and prednisone resulted in complete remission, sustained over two years. This case highlights the diagnostic and therapeutic challenges of gHCD and underscores the importance of individualised management.
Hematology patients require central venous catheters for cancer treatment and nutrition, which increases their risk of intravascular device-associated bacteremia. In the absence of recent data, we investigated intravascular device-associated bacteremia in this specific context. A three-month surveillance was conducted annually in 27 hematology wards, using a protocol derived from the HAI-Net ICU ECDC protocol (2020–2024). We analyzed the characteristics of infected patients, the sources and microorganisms of healthcare-associated bacteremias, and intravascular device-associated bacteremias (including catheter type: central venous catheter (CVC), implantable venous access device (IVAD), peripherally inserted central catheter (PICC), midline catheter (MID), and peripheral venous catheter (PVC)), the time between catheter insertion and infection onset, and bacteremia incidence rates per 1,000 patient-days. Over the five-year period, 1,835 patients experienced healthcare-associated bacteremia. No significant changes were observed in patient characteristics over the five-year period. Autistic children (AT) are known to exhibit distinct postural control patterns compared to neurotypical (NT) children. However, identifying and interpreting these differences can be complex due to the wide range of variables used to analyse Centre of Pressure (CoP) trajectories. This study aims to elucidate the specific characteristics of postural control in AT children by identifying the most discriminative CoP variables that distinguish them from NT children. The study evaluated 24 AT and 24 NT children while they stood on a force plate for 30 s under three conditions: eyes open (EO), eyes closed (EC), and the feet on a foam pad with eyes open (EOF). A total of 75 variables-including frequential, linear, and non-linear variables-were extracted from the CoP trajectory. These variables, expressed as the rate of change between the EC and EOF conditions relative to the EO condition, were compared between the AT and NT groups. A best-subsets approach was used to identify the most discriminative variables, and Pearson correlations were calculated to assess their relationship with age and Social Responsiveness Scale (SRS) scores. Of the 75 variables analysed, 15 showed significant differences between the AT and NT groups. The best-subsets analysis and the correlations revealed that variables such as the rate of change between the EOF and EO conditions, and the root mean square of the trembling component of the CoP trajectory, were particularly discriminative. Autistic children demonstrated a more rigid and regular CoP trajectory, particularly in the EO condition, compared to NT children. These findings suggest that AT children have greater difficulty integrating multisensory information and an increased reliance on supraspinal processes for postural control.
Deletions of the 3q26.33q27.2 region appear to correlate with a distinct phenotype, although there are few reported cases. Here, we present seven previously unreported individuals carrying de novo 3q27 deletions (under 5 Mb), which include the AP2M1 (adaptor-related protein complex 2, mu-1 subunit) gene and summarize data from 12 previously reported cases from the literature. The overall cohort of 19 individuals demonstrates almost universal intrauterine growth restriction, intellectual disability, and post-natal microcephaly, along with common features of hypotonia, post-natal short stature, and facial dysmorphisms. Newly identified features include bicuspid aortic valve, atrial septal defect, congenital malformation of the mesentery, and metopic craniosynostosis, present in a subset of individuals. These seven newly identified individuals allow narrowing of the previously reported smallest region of overlap to 430 kb at 3q27.1. This region includes 20 protein coding genes. We propose AP2M1 as the most likely contributor to the neurodevelopmental phenotype, based on its predicted intolerance to haploinsufficiency, functional evidence in murine models, and similar phenotypes associated with other adaptor-protein-complex-family members. Furthermore, we report the first individual with a de novo loss-of-function nonsense single nucleotide variant in AP2M1 with neurodevelopmental features including severe epilepsy. We discuss the implications of this finding in the context of previously reported epileptic encephalopathy in individuals with the recurrent p.Arg170Trp variant in AP2M1. In conclusion, our study expands the phenotypic spectrum of 3q27 microdeletions and highlights the potential importance of AP2M1 in its clinical presentation.
