    1Le Groupe FEMMIR : Du Constat Aux Actions Pour L’égalité Femmes-Hommes En Réanimation

    Le groupe FEMMIR s’est formé en 2019 pour se pencher sur les problématiques de genre en réanimation : égalité femmes-hommes, discrimination et violences ressenties par les femmes médecin. La première étape de son travail a été d’établir un état des lieux : le nombre de femmes et d’hommes est relativement semblable parmi les praticien-nes de médecine intensive et réanimation, mais la proportion de femmes diminue aux postes-clés (chef-fes de service, professeur-es, intervenant-es en congrès, expert-es intervenant dans les médias) ; de nombreuses femmes ont déjà vécu par ailleurs des discriminations ou des comportements sexistes voire du harcèlement ou des agressions sexuelles. La deuxième étape a été d’entreprendre des actions pour promouvoir l’égalité des genres et la sensibilisation aux discriminations : engagement pour la parité au sein des commissions, des congrès et des journaux, création d’une liste d’expertes, publication d’articles de recherche à propos du genre chez les patient-es ou les praticien-nes, mis en place d’un enseignement auprès des internes et de formations aux professionnel-les de santé, communication sur les réseaux sociaux et création de réseaux inter-spécialités et internationaux. Le groupe va poursuivre et consolider son action sur ces sujets dans les années à venir.

    2The French Clinical Research in the European Community Regulation Era

    Knowledge of the occurrence and outcome of admissions to Intensive Care Units (ICU) over time is important to inform healthcare services planning. This observational study aims at describing the activity of French ICUs between 2013 and 2019.Patient admission characteristics, organ dysfunction scores, therapies, ICU and hospital lengths of stay and case fatality were collected from the French National Hospital Database (population-based cohort). Logistic regression models were developed to investigate the association between age, sex, SAPS II, organ failure, and year of care on in-ICU case fatality.Among 1,594,801 ICU admissions, the yearly ICU admission increased from 3.3 to 3.5 per year per 1000 inhabitants (bed occupancy rate between 83.4 and 84.3%). The mean admission SAPS II was 42 ± 22, with a gradual annual increase. The median lengths of stay in ICU and in hospital were 3 (interquartile range (IQR) = [1–7]) and 11 days (IQR = [6–21]), respectively, with a progressive decrease over time. The in-ICU and hospital mortality case fatalities decreased from 18.0% to 17.1% and from 21.1% to 19.9% between 2013 and 2019, respectively. Male sex, age, SAPS II score, and the occurrence of any organ failure were associated with a higher case fatality rate. After adjustment on age, sex, SAPS II and organ failure, in-ICU case fatality decreased in 2019 as compared to 2013 (adjusted Odds Ratio = 0.87 [95% confidence interval, 0.85–0.89]).During the study, an increasing incidence of ICU admission was associated with higher severity of illness but lower in-ICU case fatality.

    La décision est au cœur du métier de réanimateur. Elle est partie intégrante de son activité au quotidien. Cet exercice s’avère d’autant plus difficile lorsque, en période de crise sanitaire, les médecins sont confrontés à une majoration du flux des patients en inadéquation avec la capacité d’accueil en réanimation. Il est nécessaire de s’interroger sur les mécanismes qui entrent en jeu dans cette démarche de décision. L’analyse étymologique aide à une meilleure compréhension de ces mécanismes. Décider n’est pas choisir. On ne peut réduire la décision à un calcul mathématique. L’angoisse, l’incertitude font partie intégrante de ce processus. Les récents travaux sur les biais cognitifs ainsi que la recherche en neurosciences ont mis l’accent sur sa profonde complexité. Le partage de décision, exercice bien développé en soins critiques, permet de soulager en partie du fardeau qu’elle représente. Si les avantages de ce partage sont indéniables, il ne faut pas en négliger ses inconvénients. Elle reste un fardeau indispensable car témoin de l’engagement du soignant envers le soigné et de l’expression de sa responsabilité morale. Le fardeau est une charge que l’on porte, que l’on assume. S’il pèse dans la décision, il lui apporte aussi toute sa valeur. Décider c’est soigner. L’avènement de nouvelles technologies pourrait considérablement modifier l’approche décisionnelle. Il est nécessaire, en amont, de définir ce que représente la décision en réanimation afin d’intégrer au mieux ces technologies sans se positionner comme technolâtre ou technophobe mais plutôt technosophe.

    5Prevalence of Pressure Injuries among Critically Ill Patients and Factors Associated with Their Occurrence in the Intensive Care Unit: the PRESSURE Study

    Background: The prevalence of pressure injuries (PIs) in critically ill patients has been extensively studied, but there is uncertainty regarding the risk factors. The main objective of this study was to describe the prevalence of PIs in critically ill patients. Secondary objectives were to describe PI, use of preventive measures for PI, and factors associated with occurrence of PI in the intensive care unit (ICU). Material and methods: This was a 1-day point-prevalence study performed on a weekday in June 2017 in ICUs in France. On the same day, we noted the presence or absence of PI in all hospitalised patients of the participating ICUs, data on the ICUs, and the characteristics of patients and of PI. Results: Eighty-six participating ICUs allowed the inclusion of 1228 patients. The prevalence of PI on the study day was 18.7% (95% confidence interval: 16.6-21.0). PIs acquired in the ICU were observed in 12.5% (95% confidence interval: 10.6-14.3) of critically ill patients on the study day. The most frequent locations of PI were the sacrum (57.4%), heel (35.2%), and face (8.7%). Severe forms of PI accounted for 40.8% of all PIs. Antiulcer mattresses were used in 91.5% of the patients, and active and/or passive mobilisation was performed for all the patients. Multiple logistic regression analysis identified longer length of stay in the ICU, a higher Simplified Acute Physiology Score, higher body weight, motor neurological disorder, high-dose steroids, and absence of oral nutrition on the study day as factors independently associated with occurrence of PI in the ICU. Conclusion: This large point-prevalence study shows that PIs are found in about one of five critically ill patients despite extensive use of devices for preventing PI. Acquisition of PI in the ICU is strongly related to the patient's severity of illness on admission to the ICU and length of stay in the ICU. (c) 2020 Australian College of Critical Care Nurses Ltd.
