Introducción: Las inmunoglobulinas son moléculas efectoras de la respuesta inmune humoral, utilizadas en el tratamiento de enfermedades infecciosas. Por su efecto antiviral, se usó en el tratamiento de la enfermedad causada por SARS-CoV-2. Se realizó una búsqueda de artículos sobre el uso de inmunoglobulinas intravenosas en la infección por SARS-CoV-2, en las bases de datos PubMed/Medline, Scopus, Scielo y Lilacs, y mediante el buscador Google académico, en idioma español, inglés y portugués. Objetivos: Analizar el uso de las inmunoglobulinas intravenosas en la infección por SARS-CoV-2. Desarrollo: Las preparaciones de inmunoglobulinas intravenosas tienen niveles significativos de anticuerpos contra patógenos humanos y pueden usarse para tratar una variedad de enfermedades infecciosas, así como enfermedades de etiología desconocida. Esta terapia, cuando se administra en el curso de la enfermedad (dentro de las 72 horas posteriores al inicio de los síntomas) y con títulos elevados (> 1:160) de anticuerpos neutralizantes anti-SARS-CoV-2, se asocia con beneficio clínico. Conclusiones: Las inmunoglobulinas intravenosas son efectivas en diferentes enfermedades debido a sus propiedades inmunomoduladoras, antivirales y antinflamatorias. Las reacciones adversas principales se relacionan con alta velocidad de infusión y baja purificación. Los preparados de gamma hiperinmune a títulos altos y neutralizantes administrados en fases tempranas de la infección favorecen la mejoría clínica y estancia hospitalaria.
BACKGROUND/AIMS:The extent to which low- and middle-income countries have implemented Advance Care Planning (ACP) and Advance Directives (AD) remains unclear. We aimed to map the current status of ACP/AD in Latin America. METHODS:This cross-sectional, mixed-methods survey of ACP/AD in LA comprised interviews with 18 key informants from 18 out of 20 countries, most of whom were appointed by national Palliative Care Associations. Online interviews were conducted with each informant, covering a range of relevant topics from AD regulations to the use of ACP/AD in the context of end-of-life clinical decision making. We performed member checking and data triangulation to confirm our findings. RESULTS:Only eight (44%) countries have some form of ACP/AD regulations. Most regulatory frameworks tend to adopt a legalistic pattern heavily influenced by the North American model. Despite that characteristic of AD regulations in LA, the leading strategy used by patients to avoid unwanted treatment at the end of life is through conversations with their families, whereas the least common strategy was consulting with a lawyer. In six (33%) countries, informants believed it was common for patients to grant their families permission to modify their previous choices regarding future treatments. The religiosity/spirituality of populations play an important role in the implementation of ACP in the region. Additionally, respecting patients' preferences of care at the end of life appears to be tied more to aspects related to the characteristics of doctor-patient relationship, and the degree of integration of palliative care into the healthcare system than the existence or content of AD regulations. There was consensus that none of the countries provide sufficient education about ACP/AD to healthcare professionals. CONCLUSIONS:Our findings encourage rethinking ACP/AD in LA from a decolonial perspective, considering characteristics such as the preference for a relational model of autonomy in several countries and the importance of taking the religiosity/spirituality of individuals into account during ACP conversations. Our data also suggest that honoring patients' preferences of care at the end of life entails integrating palliative care into health care systems, educating healthcare professionals and the population, and fostering longitudinal trusting relationships between those professionals, patients, and their families.
Introducción: En el cáncer de pulmón se presentan alteraciones genéticas, incluida la mutación del receptor del factor crecimiento epidérmico. El gefitinib y osimertinib están aprobados para el tratamiento del cáncer de pulmón de células no pequeñas con dicha mutación, los cuales demuestran elevadas tasas de respuestas al tratamiento y supervivencias con respecto a la quimioterapia. Objetivo: Evaluar la efectividad de los inhibidores tirosina quinasa, en pacientes con adenocarcinoma pulmonar, con mutación del receptor del factor de crecimiento epidérmico. Método: Se realizó un estudio prospectivo en 29 pacientes con cáncer de pulmón de células no pequeñas, con histología de adenocarcinoma, con mutación del receptor del factor de crecimiento epidérmico, tratados con los inhibidores tirosina quinasa. Las variables estudiadas fueron: respuesta al tratamiento, eventos adversos, supervivencia libre de progresión y global. Se utilizaron medidas de resumen de estadística descriptiva y estimaciones de supervivencia por método de Kaplan-Meier y el test de log-rank en la comparación de las curvas. Resultados: Se obtuvo una tasa de respuesta objetiva de 70,3 %. Las medianas en meses de supervivencia global y supervivencia libre de progresión fueron 19,9 (IC95 %: 12,6‑27,2) y 14,8 (IC95 %: 3,6-25,9). Los eventos adversos se clasificaron como ligeros. Conclusiones: El tratamiento con inhibidores de la tirosina quinasa dirigidos a la mutación del receptor del factor de crecimiento epidérmico, es efectivo y seguro en pacientes con adenocarcinoma pulmonar con mutación del receptor del factor de crecimiento epidérmico.
Introduction: Schwannomas are tumors composed of Schwann cells of the sheath of nerves and their most usual location is in the head, neck and trunk. Sacral compartment tumors are relatively rare. Objective: To demonstrate the key role that the correct imaging interpretation plays in the process of patient care with presumptive pelvic mass Schwannomas. Case presentation: We present the case of a 29-year-old male patient with a history of having been operated 2 years ago in another health institution by presenting a pelvic retroperitoneal mass, which was detected during clinical diagnosis. Its origin was not conclusive after performing the initial studies. New imaging exams that included a computerized axial tomography and a magnetic resonance imaging, which showed a mass located in the left pre-sacral area in direct contact with the second sacral hole, were then carried out in the National Institute of Oncology and Radiobiology of Havana. Several weeks later, the patient underwent surgery with successful results. The pathological anatomy report confirmed the presumptive diagnosis of a benign Schwannoma. Conclusions: Schwannomas exceptionally manifest as pelvic masses, as in the case reported here in which a retroperitoneal liposarcoma was initially diagnosed, but it was then confirmed as a benign retroperitoneal sacral Schwannoma with the appropriate interpretation of diagnostic imaging techniques.
