Metabolic dysfunction–associated steatotic liver disease (MASLD) as the main cause of chronic liver disease worldwide is associated with increased mortality. Especially in patients with fibrosis, an increased risk of adverse drug reactions (ADRs) might be present. Therefore, the impact of high-risk for liver fibrosis on the outcome of ADRs was analyzed. Patients of a multicenter cohort study in maximum care emergency departments (ED) with admissions due to ADRs with available FIB-4 score parameters were analyzed. The cohort was divided into patients with low (FIB-4 < 2.67) and high (FIB-4 ≥ 2.67) liver fibrosis risk. BACKGROUND:The directives of the Federal Joint Committee (G-BA) define tiered emergency care structures, but their influence on patient decision-making has not been sufficiently studied. OBJECTIVE (RESEARCH QUESTION):This study examined patient-related determinants of utilising emergency departments (ED) of different emergency levels in Bavaria. MATERIALS AND METHODS:From September to November 2024, we carried out a standardised, cross-sectional survey in 18 ED at basic, extended and comprehensive care levels (n = 7527 participants). Data were analysed descriptively and with multinomial logistic regression. Subjective urgency, reason for visit, travel time and mode of arrival were included as influencing factors. RESULTS:15.4% of respondents who visited the ED independently (n = 5300) chose to do so because of the expected specialization. Patients in comprehensive EDs used emergency medical services significantly more often, rated their urgency higher and more frequently reported neurological, urological or ophthalmological complaints. They accepted longer travel times and were less likely to have sought outpatient care beforehand. DISCUSSION:The choice of emergency level is chiefly influenced by subjective urgency, specialty-related complaint types, mode of arrival and institutional structure. Even the existing heterogeneity among institutions exerts a steering effect. Future management concepts-such as integrated emergency centres-should incorporate these patient preferences and regional care realities to enable need-oriented direction without undersupply.
Orthogeriatrische Patienten weisen eine hohe Prävalenz von Mangelernährung oder eines entsprechenden Risikos auf. Diese ist mit erhöhter Komplikationsrate, verzögerter Wundheilung, funktionellen Einschränkungen und erhöhter Mortalität assoziiert. Eine strukturierte Ernährungsdiagnostik und -therapie sind daher essenziell, werden jedoch in der Praxis häufig noch unzureichend umgesetzt. Darstellung evidenzbasierter Strategien zur Optimierung des Ernährungsmanagements entlang des kompletten orthogeriatrischen Versorgungspfads – von der präoperativen Erhebung über perioperative Interventionen bis zur poststationären Weiterführung. Narrative Übersicht aktueller Leitlinien, Studien sowie praxisrelevanter Versorgungskonzepte. Ein Fallbeispiel illustriert Screening, GLIM-Diagnostik und ernährungstherapeutische Maßnahmen bei hüftnaher Fraktur. Mangelernährung entsteht multifaktoriell und wird mittels validiertem Screening sowie GLIM-Kriterien diagnostiziert. Frühzeitige, individualisierte Ernährungstherapie – inklusive energie- und proteinreicher Kost, orale bilanzierte Trinknahrung und systematischem Monitoring – verbessert Mortalität, funktionelle Resultate und den Rehabilitationsverlauf. Ergänzende Maßnahmen wie Prähabilitation, Carboloading und standardisierte postoperative Versorgung erhöhen die Erfolgswahrscheinlichkeit. Trotz klarer Leitlinien besteht Verbesserungsbedarf in Screeningdurchdringung, interdisziplinärer Zusammenarbeit und Einbindung qualifizierter Ernährungsfachkräfte. Ein durchgängiges, sektorenübergreifendes Ernährungskonzept kann den klinischen Verlauf signifikant positiv beeinflussen und sollte als integraler Bestandteil orthogeriatrischer Behandlung implementiert werden.
Background:Nonmissle penetrating brain injuries (PBIs) are rare in civilian settings and present significant diagnostic and surgical challenges, particularly when involving the skull base and orbit. Case Description:A 28-year-old male sustained a penetrating stab wound to the left temporal region following an interpersonal assault. Imaging revealed a transcranial trajectory extending to the orbital apex and ethmoidal cells in proximity to the internal carotid artery and optic nerve, without definitive vascular injury. The patient underwent emergency frontotemporal craniotomy with controlled removal of the foreign body and evacuation of intracranial hematoma. Postoperatively, neurological deterioration necessitated reoperation and placement of an external ventricular drain. Following intensive care management, the patient stabilized and was transferred to a specialized rehabilitation center. Conclusion:PBIs require rapid imaging, careful surgical planning, and multidisciplinary management. Even in the absence of major vascular injury, involvement of the skull base necessitates vigilant monitoring for complications.
Bei Patientinnen und Patienten mit chronischer Herzinsuffizienz sollen Therapieziele frühzeitig festgelegt und im Verlauf regelmäßig überprüft werden. Angesichts des oft schwer vorhersehbaren Krankheitsverlaufs ist eine offene, wertorientierte Kommunikation über Prognose, Belastungen und Erfolgsaussichten medizinischer Maßnahmen essenziell. Das Konsensuspapier betont die Bedeutung einer strukturierten Entscheidungsfindung im Sinne des Advance Care Planning (ACP) und einer engen multiprofessionellen Zusammenarbeit zwischen Kardiologie, Hausärztinnen und -ärzten, Pflege und Palliativmedizin. Dabei sind sowohl medizinische als auch ethische und rechtliche Aspekte zu berücksichtigen, insbesondere die Ermittlung und Beachtung des Patientenwillens. Palliative Therapieansätze sollen frühzeitig in die Versorgung integriert und nicht erst in der terminalen Krankheitsphase berücksichtigt werden. Regelmäßige Reevaluationen von Therapieentscheidungen und eine klare Dokumentation individueller Werte, Wünsche und Behandlungsgrenzen fördern eine patientenzentrierte, konsistente Versorgung. Eine Anpassung bestehender Versorgungs- und Vergütungsstrukturen wird empfohlen, um herzinsuffiziente Patientinnen und Patienten sektorenübergreifend adäquat und würdevoll zu begleiten.
