Colitis-associated cancer (CAC) is an aggressive subtype of colorectal cancer that can develop in ulcerative colitis patients and is driven by chronic inflammation and oxidative stress. Current chemotherapy for CAC, based on 5-fluorouracil and oxalipltin, is not fully effective and displays severe side effects, prompting the search for alternative therapies. Dimethylfumarate (DMF), an activator of the nuclear factor erythroid 2-related factor 2 (NRF2), is a potent antioxidant and immunomodelatrory drug used in the treatment of multiple sclerosis and showed a strong anti-inflammatory effect on experimental colitis. Here, we investigated the chemotherapeutic effect of DMF on an experimental model of CAC. Male NMRI mice were given two subcutaneous injections of 1,2 Dimethylhydrazine (DMH), followed by three cycles of dextran sulfate sodium (DSS). Low-dose (DMF30) and high-dose of DMF (DMF100) or oxaliplatin (OXA) were administered from the 8th to 12th week of the experiment, and then the colon tissues were analysed histologically and biochemically. DMH/DSS induced dysplastic aberrant crypt foci (ACF), oxidative stress, and severe colonic inflammation, with a predominance of pro-inflammatory M1 macrophages. As OXA, DMF30 reduced ACF multiplicity and crypt dysplasia, but further restored redox status, and reduced colitis severity by shifting macrophages towards the anti-inflammatory M2 phenotype. Surprisingly, DMF100 exacerbated ACF multiplicity, oxidative stress, and colon inflammation, likely through NRF2 and p53 overexpression in colonic inflammatory cells. DMF had a dual effect on CAC. At low dose, DMF is chemotherapeutic and acts as an antioxidant and immunomodulator, whereas at high dose, DMF is pro-oxidant and exacerbates colitis-associated cancer.
Trans Aortic Valve Implantation (TAVI) has become the primary treatment for aortic stenosis in patients over 75 years old. Despite its clinical efficacy, it's adoption in emerging countries remains low due to the high cost of prostheses and limited healthcare funding resources. This leads to prolonged waiting times for the TAVI procedure, which may lead to complications; these data are missing particularly in emerging countries.
A thyroid nodule is a formation well differentiated from the rest of the thyroid. It represents the most frequent thyroidpathology, and the majority of these nodules are benign. Assessment of patients with thyroid nodules includes history,clinical examination, TSH measurement and thyroid ultrasound.In this article, we will detail the criteria for suspicion, including the place of ultrasound in the evaluation of thyroidnodules, based on the EU-TIRADS criteria
The complex and evolving landscape of drug addiction poses significant public health challenges in Algiers, Algeria. With growing concerns about drug addiction and its associated consequences, it becomes imperative to comprehensively understand drug consumption patterns among individuals grappling with addiction in this region. Drug addiction is a multifaceted issue influenced by various factors such as drug availability, sociodemographic characteristics, and personal choices. The prevalence of drug addiction continues to rise, warranting a thorough examination of the specific substances that dominate the local addiction landscape. A descriptive retrospective analytical study was conducted, analyzing 92 cases of drug consumption profiles among addicts in the Algerian province of Algiers. Urine samples collected between January 1, 2020, and October 15, 2022, from drug-addicted patients in Algiers, were examined using Gas Chromatography-Mass Spectrometry (GC-MS). The meticulous analysis of this diverse population revealed that THC and pregabalin are the two most frequently consumed substances, often used together in poly-drug combinations. Surprisingly, there is a notable prevalence of opioid consumption, especially within families, raising concerns about a potential opioid crisis in Algiers.
