Self assembled video laryngoscope is a good alternative to commercial video laryngoscope, being cost effective. Intubation time is higher when best glottic view of laryngoscopy is used for intubation using this device. Partially restricted glottic view of laryngoscopy can significantly decrease the intubation time. Fifty patients of either sex aged 18 to 65 years were included. After induction of general anaesthesia, patients of Group I (n = 25) were intubated using best glottic view of laryngoscopy and patients of Group II (n = 25) were intubated using partially restricted glottic view of laryngoscopy. The primary objective was total intubation time. Secondary objectives were time for glottic visualisation, grade of glottic visualization, time for tube insertion after glottic visualization, first attempt intubation success, number of attempts for intubation and use of additional maneuvers. Continuous variables were compared using the independent sample t-test and categorical variables were compared using the Chi-square test. Oral squamous cell carcinoma is often diagnosed at advanced stages with inflammation and stromal remodeling considered important early events. Our study evaluated eosinophil infiltration and collagen fiber characteristics in oral epithelial dysplasia, early invasive carcinoma and oral squamous cell carcinoma using special histochemical stains, to assess their histopathological significance. In this retrospective observational study, 67 archival tissue samples comprising 25 oral squamous cell carcinoma, 25 oral epithelial dysplasia, and 17 early invasive carcinoma cases were analysed. Congo red stain was used for eosinophil assessment, while Picrosirius red under polarized microscopy was used to evaluate collagen characteristics. Total eosinophil count increased significantly from oral epithelial dysplasia to early invasive carcinoma to oral squamous cell carcinoma (p < 0.001). Collagen fibers displayed a significant shift in polarizing color, from reddish-orange in oral epithelial dysplasia and early invasive carcinoma to greenish-yellow in oral squamous cell carcinoma (p < 0.001). Total eosinophil count showed a significant negative correlation with the polarizing color of collagen fibers (p < 0.001). The findings suggest associated inflammatory and stromal alterations during oral carcinogenesis. Special histochemical stains may provide supportive information during routine histopathological assessment.
Purpose: This study evaluated correlation between knowledge of COPD, health related quality of life, level of physical activity and dyspnea score in patients with COPD from Haryana as these factors are known to influence disease progression and further the BCKQ score were compared across different sociodemographic variables, including locality, previous disease knowledge, smoking status and level of education. Materials and Methods: Prospective cross-sectional study was done on 200 stable COPD patients in a tertiary care teaching hospital. The Bristol COPD Knowledge Questionnaire was used to evaluate knowledge of the condition. St. George Respiratory Questionnaire (SGRQ) was used to measure health-related quality of life. IPAQ (International Physical Activity Questionnaire) was used to measure physical activity, while the Modified Borg Scale was used to measure shortness of breath. Results: BCKQ score was negatively correlated with SGRQ sub-components and also with SGRQ total score, with significant Pearson correlation SGRQ symptom (r = -.156; p<0.05), SGRQ activity (r = -.147; p<0.05), SGRQ impact (r = -.214; p<0.05) and with SGRQ total score (r = -.221±; p<0.01). Weak Positive correlation with significant Pearson correlation was found between IPAQ and BCKQ score (r = 0 .146, p<0.05). Weak negative correlation with significant Pearson correlation was found between dyspnea score and BCKQ score (r= -0.235, p<0.01). Conclusion: In conclusion, the study highlights that increase in disease knowledge is significantly associated with better health-related quality of life, higher physical activity levels, and reduced dyspnea. Introduction: The haemodynamic alterations occurring during laryngoscopy and intubation presents a significant challenge for Anaesthesiologists. It causes a transient but marked rise in Heart Rate (HR) and blood pressure which is highly undesirable. Aim: To analyse the comparative role of nebulised magnesium sulphate and nebulised lignocaine in managing the pressor response triggered by laryngoscopy and endotracheal intubation under anaesthesia. Materials and Methods: The present randomised controlled was carried out in the Department of Anaesthesia and critical care at PGIMS, Rohtak,India from September 2023 to August 2024 .The study included 80 patients scheduled for elective surgery requiring general anaesthesia. Group M (n=40) received nebulisation with 5 mL of a solution containing 1 mL of 50% (500 mg) magnesium sulphate diluted in 4 mL of saline. Group L (n=40) was nebulised with 5 mL of a 2% lignocaine hydrochloride solution. Baseline haemodynamic parameters (V0) were recorded, and subsequent parameters were measured after nebulisation (V1), prior to laryngoscopy (V2), and at intervals following endotracheal intubation: one minute (V3), three minutes (V4), and 5 minutes (V5). Data was expressed as frequency(n) or mean with standard deviation and appropriate statistical tests were applied for intragroup and intergroup comparison. Results were considered significant if the p-value was less than 0.05. Results: The demographic and clinical profiles of patients (age, sex, Body Mass Index (BMI), American Society of Anaesthesiologists (ASA) and Mallampatti Grading (MPG) were statistically similar across both groups. Baseline measurements for haemodynamic parameters were comparable between the groups. Group M had higher SBP values just before laryngoscopy (p=0.004) while Group L demonstrated significantly higher mean Systolic Blood Pressure (SBP) values than Group M at multiple time points: 1st minute (p=0.003), 3rd minute (p=0.001), and 5th minute (p=0.001) after laryngoscopy and endotracheal intubation. Similarly, Group L exhibited higher mean Diastolic Blood Pressure (DBP) values at comparable time intervals (p=0.03, p=0.001, p=0.001 and p=0.003). A more pronounced rise in mean MAP was observed in Group L compared to Group M at 1st min (p=0.001) and 3rd min post-intubation (p=0.001). Group L also had higher HR values than Group M after nebulisation (p=0.03), before laryngoscopy (p=0.04), and at 1st minute (p=0.006) and 3rd minute (p=0.001) postintubation. Conclusion: The present study revealed that Group L exhibited significantly greater haemodynamic responses to laryngoscopy and endotracheal intubation than Group M at multiple time points in patient receiving general anaesthesia when compared with nebulised lignocaine. So The authors concluded that nebulised magnesium sulphate provided more persistent and effective attenuation for the pressor response.
Hyalinizing clear cell carcinoma (HCCC) is a rare, low-grade salivary gland tumor that poses diagnostic challenges due to its morphological and immunohistochemical overlap with other clear cell neoplasms of the head and neck. EWSR1 gene rearrangements, particularly EWSR1-ATF1 fusion, have been identified as distinctive molecular markers; however, their diagnostic significance remains unclear. This systematic review and meta-analysis, conducted in accordance with PRISMA guidelines and registered in PROSPERO (CRD42022382551), evaluated the diagnostic role of EWSR1 rearrangements in HCCC. Comprehensive searches of PubMed, Scopus, EMBASE, Cochrane Library, and Google Scholar up to July 2024 identified 14 studies (2011-2023) encompassing 174 HCCC cases. Patients were predominantly female (F:M ratio = 2.05:1; mean age 53.3 years), with tumors commonly involving the palate, base of tongue, and nasopharynx. Immunohistochemistry showed strong positivity for Pan-CK (96.9%), p40 (100%), p63 (92.7%), and CK7 (86.4%), while myoepithelial and melanocytic markers were negative. Molecular testing revealed EWSR1 rearrangements in 86.7% (137/158) by FISH and 57.1% (8/14) by RT-PCR, with EWSR1-ATF1 fusion confirmed in 38 cases. Pooled analysis demonstrated high heterogeneity (I² = 90.98%), and subgroup analysis showed no gender-based difference. Compared with 144 controls, HCCC exhibited a 2.16-fold higher likelihood of EWSR1 positivity, though not statistically significant (p = 0.10). These findings highlight EWSR1, particularly EWSR1-ATF1 fusion, as a sensitive molecular marker that improves diagnostic accuracy when integrated with histopathology and immunohistochemistry.
