This article presents the latest recommendations for the promotion of health and prevention. This article is primarily addressed to public health actors and stakeholders. The recommendations are contextualized to the local reality, based on Geneva-specific data from the longitudinal Specchio studywhich evaluates the health status of the Geneva population and its determinants. In this article, are mentioned data on the prevalence of physical activity, sedentary behavior, nutrition, obesity, smoking and alcohol consumption, as well as related recommendations. The insights from the Specchio results help guide health promotion and prevention efforts adapted to the local context.
Acute gastroenteritis is a frequent reason for medical consultations and antibiotic prescriptions. In most cases, antibiotics are not needed, even for bacterial gastroenteritis. Microbiological diagnosis should be performed in conjunction withantibiotic prescription to better target the pathogen. Multiplex polymerase chain reaction panels offer short turnaround times for results, which should however be interpreted with caution due to possible false-positive results. The general principle is: "Test if you are going to treat, and treat only if you have tested."This article summarizes the situations in which microbiological diagnosis is needed and the situations in which empiric antibiotic treatmentis recommended before microbiological results are available, as opposed to situations in which targeted antibiotic treatment can be introduced once a pathogen has been identified.
Abstract Chimeric antigen receptor (CAR) T-cell therapy for multiple myeloma targeting B-cell maturation antigen (BCMA) induces high overall response rates. However, relapse still occurs and novel strategies for targeting multiple myeloma cells using CAR T-cell therapy are needed. SLAMF7 (also known as CS1) and CD38 on tumor plasma cells represent potential alternative targets for CAR T-cell therapy in multiple myeloma, but their expression on activated T cells and other hematopoietic cells raises concerns about the efficacy and safety of such treatments. Here, we used CRISPR/Cas9 deletion of the CD38 gene in T cells and developed DCAR, a double CAR system targeting CD38 and CS1 through activation and costimulation receptors, respectively. Inactivation of CD38 enhanced the anti–multiple myeloma activity of DCAR T in vitro. Edited DCAR T cells showed strong in vitro and in vivo responses specifically against target cells expressing both CD38 and CS1. Furthermore, we provide evidence that, unlike anti-CD38 CAR T-cell therapy, which elicited a rapid immune reaction against hematopoietic cells in a humanized mouse model, DCAR T cells showed no signs of toxicity. Thus, DCAR T cells could provide a safe and efficient alternative to anti-BCMA CAR T-cell therapy to treat patients with multiple myeloma.
Introduction : Le paludisme pendant la grossesse représente un problème de santé publique majeur, avec des risques élevés pour la mère, le fœtus et le nouveau-né. L’Organisation Mondiale de la Santé recommande un calendrier d'au moins quatre consultations prénatales lors de la grossesse. Notre étude s’est fixé ainsi l’objectif d’analyser le rôle des matrones dans le TPI en milieu rurale de Sélingué au Mali. Matériels et méthodes : Nous avons procédé à une analyse du rôle des matrones dans le TPI-SP à partir des CPN. En effet, les matrones étaient identifiées par les enquêteurs qui disposaient de la liste de tous les personnels des différents CSCom, et dès que les femmes enquêtées citaient le nom de celle qui avait effectué leur CPN, l’enquêteur vérifiait aussitôt sa qualification sur la fiche qui lui avait été donné. Résultats : Au total pour une population de 1021 femmes, Sur un total de 896 CPN, les matrones ont effectué 691(99,5%), pour un total de 798 CPN 3 ou plus réalisé les matrones arrivent avec 635(89,1%) contre 163 femmes pour les autres personnels(P<0,05). TPI effectué par les matrones nous donne presque dans les même proportion TPI-SP1 (591/772) 86,3% POUR ARRIVER A TPI-SP3 ou plus a (368/455) 61,7% contre 78 avec les autres personnels(P<0,05). Conclusion : Au vu de nos résultats, il est nécessaire de tenir compte dans les enquêtes nationales des données issues d’activités menées par les matrones en milieu rurale ou elles jouent encore un rôle important dans la plupart des activités de soin de santé maternels.IL faut Renforcer les capacités en apportant les compétences à tous les niveaux de la pyramide sanitaire en particulier au niveau rural. Réaliser d’autres études qui intégrerons l’ensemble des acteurs communautaires liés au TPI-SP.
