
Purpose. Ovarian cancer in the early stage requires a complete surgical staging, including radical lymphadenectomy, implying subsequent risk of morbidity and complications. Sentinel lymph node (SLN) mapping is a procedure that attempts to reduce radical lymphadenectomy-related complications and morbidities. Our study evaluates the feasibility of SLN mapping in patients with ovarian tumors by the use of intraoperative Technetium-99m-Phytate (Tc-99m-Phytate) and postoperative lymphoscintigraphy using tomographic (single-photon emission computed tomography/computed tomography (SPECT/CT)) acquisition. Materials and Methods. Thirty-two patients with ovarian mass participated in this study. Intraoperative injection of the radiopharmaceutical was performed just after laparotomy and before the removal of tumor in utero-ovarian and suspensory ligaments of the ovary just beneath the peritoneum. Subsequently, pelvic and para-aortic lymphadenectomy was performed for malignant masses, and the presence of tumor in the lymph nodes was assessed through histopathological examination. Conversely, lymphadenectomy was not performed in patients with benign lesions or borderline ovarian tumors. Lymphoscintigraphy was performed within 24 hr using tomographic acquisition (SPECT/CT) of the abdomen and pelvis. Results. Final pathological examination showed 19 patients with benign pathology, 5 with borderline tumors, and 6 with malignant ovarian tumors. SPECT/CT identified SLNs in para-aortic-only areas in 6 (20%), pelvic/para-aortic areas in 14 (47%), and pelvic-only areas in 7 (23%) cases. Notably, additional unusual SLN locations were revealed in perirenal, intergluteal, and posterior to psoas muscle regions in three patients. We were not able to calculate the false negative rate due to the absence of patients with involved lymph nodes. Conclusion. SLN mapping using intraoperative injection of radiotracers is safe and feasible. Larger studies with more malignant cases are needed to better evaluate the sensitivity of this method for lymphatic staging of ovarian malignancies.
Colon cancer is a type of cancer that begins in the large intestine. In the process of efficacy evaluation, postoperative recurrence prediction and metastasis monitoring of colon cancer, traditional medical image analysis methods are highly dependent on the personal ability of the doctors. In the process of patient treatment, it not only increases the workload and work pressure for doctors, but also has some problems with traditional medical image analysis methods. Moreover, the traditional medical image analysis methods have problems such as insufficient prediction accuracy, slow prediction speed, and the risk of errors in prediction. When analyzing 18F-FDG PET/CT images by traditional medical image analysis methods, it is easy to cause problems such as untimely treatment plans and errors in diagnosis, which will adversely affect the survival of colon cancer patients. Although 18F-FDG PET/CT images have certain advantages in image clarity and accuracy compared with traditional medical imaging methods, the analysis method based on 18F-FDG PET/CT images also has certain effects in predicting the survival of colon cancer patients, but there are still many shortcomings: the 18F-FDG PET/CT image analysis method overly relies on the technical advantages of 8F-FDG PET/CT images; in the analysis and prediction of image data, it has not gotten rid of the dependence on the personal medical quality of the doctors; traditional medical image analysis methods are still used when analyzing and predicting images; there is no breakthrough in image analysis effects. In order to solve these problems, this paper combined deep learning theory, using three algorithms of the improved RBM algorithm, image feature extraction method based on deep learning, and regression neural network to analyze and predict 18F-FDG PET/CT images, and applied some algorithms to analyze and predict 18F-FDG PET/CT images, and also established a deep learning-based 18F-FDG PET/CT image survival analysis prediction model. Four aspects survival prediction accuracy, survival prediction speed, survival prediction precision, and physician satisfaction were studied through this model. The research results have shown that compared with traditional medical image analysis methods, the prediction accuracy of 18F-FDG PET/CT image survival analysis prediction model based on deep learning is improved by 0.83%, and the prediction speed is improved by 3.42%, as well as the prediction precision increased by 6.13%. The research results show that the deep learning-based 18F-FDG PET/CT image survival analysis prediction model established in this paper is of great significance to improve the survival rate of colon cancer patients, and also promotes the development of the medical industry.
This study aims to investigate the relationship between the detection performance of an artificial intelligence (AI) algorithm and pathology in chest computed tomography (CT) images. In this study, a new pulmonary nodule (PN) detection algorithm was designed and developed on the three-dimensional (3D) connected domain algorithm. The appropriate grayscale threshold of CT images was selected, the CT images were converted into black-and-white images, and the useless images were removed. Then, the remaining lung images were formed into a 3D black-and-white pixel matrix. Labeling statistics was carried out, and the size, property, and location of PN could be measured and determined. A self-built database of PNs undergoing chest multislice spiral CT examination was retrospectively selected, and 150 cases were randomly selected by SPSS 22.0. Image processing was performed according to the algorithm and compared with the PN detected by radiologists; finally, the detection results were counted. There were 560 benign and malignant PNs, 312 malignant, and 248 benign. The algorithm detected 498 cases, of which 478 cases were detected accurately, and the sensitivity was 95.98%. The radiologist detected 424 cases, 364 cases were accurate, and the sensitivity was 85.85%. Compared with the detection results of radiologists, the algorithm detection results of nodules in solid nodules and ground glass nodules were more accurate. The detection results of nodules in the pleural connection type, peripheral type, central type, and hilar type were more accurate and statistically significant ( P < 0.05 ). The malignancy, size, property, and location of different nodules could be accurately determined through CT images under this algorithm. It provided important support for the pathological research of lung cancer and prejudged the future development of PN in patients more accurately.
