Bei technisch korrekter Anlage können moderne Versorgungssysteme die Lebensqualität deutlich verbessern. Dennoch sind Stomata weiterhin mit Komplikationsraten von 20–50
Postoperative Komplikationen stellen einen wesentlichen prognostischen Faktor beim kolorektalen Karzinom dar und sind mit erhöhter Morbidität, Mortalität sowie schlechteren onkologischen Ergebnissen assoziiert. Die perioperative Medizin zielt darauf ab, durch strukturierte Behandlungskonzepte postoperative Komplikationen zu reduzieren und die Voraussetzungen für eine erfolgreiche onkologische Therapie zu verbessern. Analyse des Einflusses perioperativer medizinischer Strategien auf postoperative Morbidität, Mortalität und onkologische Ergebnisse beim Kolorektalkarzinom. Narrative Übersichtsarbeit auf Basis einer Literaturrecherche in PubMed und den AWMF-Leitliniendatenbanken. Einbezogen wurden randomisierte Studien, Metaanalysen, prospektive Kohortenstudien und aktuelle Leitlinienempfehlungen ohne formale Selektionskriterien. Postoperative Komplikationen sind mit einer erhöhten Mortalität und einer reduzierten Gesamtüberlebensrate assoziiert. Evidenzbasierte perioperative Strategien reduzieren nachweislich die Komplikationsrate und ermöglichen eine frühere Einleitung der adjuvanten Therapie. Dadurch werden sowohl kurzfristige klinische Ergebnisse als auch langfristige onkologische Prognosen verbessert.
Die Standardtherapie des lokal fortgeschrittenen Rektumkarzinoms mit Rektumresektion oder -exstirpation und totaler mesorektaler Exzision (TME) nach neoadjuvanter Vorbehandlung ist oft mit deutlichen funktionellen Einbußen verbunden. Daher wurde in den letzten Jahren das Watch-and-wait-Konzept für Patienten entwickelt, die nach neoadjuvanter Therapie eine klinische und bildmorphologische Vollremission zeigen. Hier wird unter intensiver Überwachung auf eine operative Therapie verzichtet. Nach bisheriger Datenlage ist dieses Vorgehen onkologisch sicher. Zwar ist die lokale Tumorkontrolle schlechter als nach operativer Therapie. Dies scheint jedoch keinen Nachteil darzustellen, da in aller Regel eine Salvage-Operation mit TME durchgeführt werden kann und sich die Prognose im Vergleich zur primären TME wohl nicht unterscheidet. Zur Diagnose und Überwachung der klinischen Vollremission werden rektal-digitale Untersuchung, Endoskopie und eine Magnetresonanztomographie (MRT) des Beckens in T2- und Diffusionswichtung herangezogen. Die Beurteilung der Remission kann schwierig sein, so dass eine entsprechende Expertise der Untersucher vorhanden sein sollte. Da ein sog. Regrowth vor allem in den ersten 2 bis 3 Jahren auftritt, sind insbesondere in diesem Zeitraum engmaschige Kontrolluntersuchungen erforderlich.
Obwohl Duplikaturen im Bereich des gesamten Gastrointestinaltrakts vorkommen, ist die Analkanalduplikatur mit weltweit 110 publizierten Fällen eine äußerst seltene, angeborene Fehlbildung, die häufig als Analfistel fehlinterpretiert wird. Sie äußert sich als zusätzliche Öffnung des Analkanals in der Mittellinie bei 6 Uhr Steinschnittlage (SSL) ohne Kommunikation mit dem Anorektum und bleibt häufig initial asymptomatisch. Symptome können verzögert auftreten, sodass die Diagnose teils erst im Erwachsenenalter gestellt wird. Die Diagnose erfolgt durch eine differenzierte klinische Untersuchung, die Histopathologie bestätigt den Verdacht. Wichtig ist der Ausschluss von assoziierten Fehlbildungen.
Die Stomamarkierung gehört zu einem der wichtigsten präoperativen Vorbereitungsschritte vor solchen Eingriffen, bei denen eine Stomaanlage fest eingeplant oder zumindest wahrscheinlich ist. Die Grundregeln der Stomamarkierung sind: Einhalten eines Mindestabstandes zu Narben, Wunden, Falten, Knochenstrukturen und Nabel. Es muss darauf geachtet werden, dass die Patienten die markierte Stelle gut sehen und erreichen können. Die Chirurgen müssen jedoch auch mit eventuell erforderlichen Anpassungen vertraut sein, z. B. Stomamarkierung bei adipösen Patienten, Patienten mit Hernien, Patienten mit mehreren Stomata sowie in Notfallsituationen bei kritisch Kranken. In der vorliegenden Arbeit werden außerdem Tipps für die Wahl der Stomaposition bei Patienten, die präoperativ nicht markiert wurden, zusammengefasst.
Anale hochgradige squamöse Intraepitheliale Läsionen (HSIL) stellen präkanzeröse Veränderungen dar, die insbesondere immunsupprimierte Individuen betreffen. Bei Behandlungsoptionen wie Exzision, ablativer oder topischer Therapien sind Rezidive häufig. Wiederholte Eingriffe bedeuten eine erhebliche Belastung. Behandlungsalternativen wie die Strahlentherapie (RT) werden selten eingesetzt und sind wenig untersucht. Die Studie untersucht die Akzeptanz und Wahrnehmung der RT als alternative organerhaltende Behandlungsoption bei rezidivierender HSIL aus Sicht von Koloproktologen und Patienten. Explorative, anonymisierte Querschnittsbefragung mit 13 Betroffenen mit rezidivierender HSIL und 40 Koloproktologen aus Deutschland. Erfasst wurden therapeutische Erfahrungen, Krankheitsbelastung, Nebenwirkungen der Therapien sowie Einstellung zur Strahlentherapie. Patienten berichteten über wiederholte operative Eingriffe und empfanden die Erkrankung als stark belastend. Vor allem Schmerzen, Inkontinenz sowie wiederholte chirurgische Behandlungen wurden hervorgehoben. Aus ärztlicher Sicht wurden relevante Rezidivraten nach Standardtherapien beschrieben. Die RT wurde als potenziell sinnvolle Behandlungsalternative bewertet, insbesondere bei rezidivierender HSIL. Koloproktologen könnten sich den Einsatz der Strahlentherapie in ausgewählten Fällen vorstellen – nur wenige verfügten über praktische Erfahrung. Auf Patientenseite war die Kenntnis der RT als Therapieoption begrenzt. Ein relevanter Anteil zeigte Offenheit gegenüber dieser Behandlung, vor allem im Rezidivfall. Aufgrund der hohen Krankheitslast und der relevanten Rezidivraten bei HSIL sollten die gängigen Therapiekonzepte überdacht werden. Die RT könnte aus Sicht von Betroffenen und Koloproktologen eine Alternative nach ausgeschöpfter Behandlung darstellen. Retrospektive Analysen und weiterführende Studien zu Wirksamkeit, Sicherheit und patientenbezogener Endpunkte sind jedoch hierzu notwendig.
Background: The surgical treatment of acne inversa (hidradenitis suppurativa) requires consistent radical excision of the affected tissue. This frequently results in extensive soft tissue defects in functionally relevant regions, such as the axilla, groin and perineum. The choice of reconstructive strategy has a decisive impact on functional and esthetic outcomes, recurrence rates and consequently on the patients' quality of life. Objective: To present and systematically categorize established plastic reconstructive surgical procedures following radical excision of hidradenitis suppurativa, taking defect-specific and patient-specific factors into account. Material and methods: Narrative review based on current guidelines and published literature on the surgical management of hidradenitis suppurativa. Systematic presentation of reconstructive options including secondary wound healing, primary closure and local flaps, split-thickness skin grafting, regional pedicled flaps, and free microvascular flaps. Results: Secondary wound healing and split-thickness skin grafting are indicated for small to medium-sized defects or in cases with limited reconstructive options but are associated with scar formation and potential functional impairment. Regional flaps enable tension-free defect coverage with well-vascularized tissue in larger defects and represent the reconstructive standard in appropriately selected cases. Free microvascular flaps are reserved for complex, extensive or multiple previously surgically treated defects as an escalation strategy. Discussion: Plastic surgery reconstructive defect coverage is an integral component of the surgical treatment of hidradenitis suppurativa. Stable long-term results require radical disease eradication, structured defect analysis and careful consideration of individual risk factors. Early interdisciplinary planning enables optimal alignment of the resection extent and reconstructive strategy.
Anastomotic leakage remains one of the most significant complications of colorectal surgery. Because adequate blood supply is a key prerequisite for anastomotic healing, near-infrared fluorescence angiography with indocyanine green (ICG) has become an adjunct to conventional visual assessment. The current review summarizes the pharmacological and physical principles, dosing and timing, practical workflow, and interpretation of fluorescence patterns (intensity, homogeneity and inflow time) as well as limitations and sources of error. Randomized controlled trials (RCTs) report heterogeneous effects: while selected studies demonstrate lower leak rates, RCT-based meta-analyses suggest a moderate overall benefit, particularly in left-sided resections and/or low anastomoses. Contraindications, documentation of strategy changes, and future developments are discussed.
Die Hidradenitis suppurativa (HS) ist eine chronisch-entzündliche, ätiopathogenetisch noch unvollständig geklärte Hauterkrankung, die schmerzhafte Abszesse und Fisteln in vornehmlich den intertriginösen Arealen verursacht. Die neue S2k-Leitlinie von 2024 spiegelt eine umfassende Verbesserung im Verständnis der Pathogenese wider, einhergehend mit neuen Therapiemöglichkeiten. Die psychisch und sozioökonomisch sehr belastende Erkrankung bei vornehmlich jungen Erwachsenen stellt eine große therapeutische Herausforderung dar. Die Therapie richtet sich nach Stadium und Aktivität. Topische, physikalische und multimodale systemische Therapien in Kombination mit chirurgischen Verfahren sollten zum Einsatz kommen. Ziel ist eine dauerhafte Entzündungsreduktion zur Verhinderung des Krankheitsprogresses.
Rectal cancer (RC) remains a major contributor to the global cancer burden, with many patients diagnosed at locally advanced stages. Although neoadjuvant chemoradiotherapy (NCRT) followed by surgery has improved local control, individual treatment responses vary widely. Identifying accessible predictive biomarkers remains a clinical need. Systemic inflammatory markers such as the lymphocyte-to-monocyte ratio (LMR) and platelet-to-monocyte ratio (PMR) have emerged as promising candidates. This retrospective study evaluated 90 patients with histologically confirmed locally advanced rectal cancer (LARC) treated with standardized NCRT and surgery. Pretreatment blood counts were used to calculate inflammatory indices. Tumor regression was graded pathologically according to the American Joint Committee on Cancer (AJCC, 8th ed) criteria. Receiver operating characteristic (ROC) curve analyses identified optimal cut-offs for LMR and PMR to predict pathologic response (PR). Internal hemorrhoids are commonly managed using office-based procedures, with rubber band ligation (RBL) and infrared coagulation (IRC) among the most widely used minimally invasive treatments. In clinical practice, their comparative effectiveness is increasingly influenced by differences in post-procedural pain, tolerability, and recovery time. This study aimed to systematically evaluate randomized controlled trial evidence comparing the clinical effectiveness, post-procedural pain, and recovery outcomes of RBL versus IRC for internal hemorrhoids. A systematic review and meta-analysis of randomized controlled trials was conducted using PubMed, Scopus, Web of Science, and Google Scholar. Postoperative length of stay (LOS) after colorectal cancer (CRC) surgery is an important indicator of recovery, discharge readiness, patient experience, and healthcare utilization. Although LOS is influenced by multiple clinical and organizational factors, accurate prediction may support perioperative planning and resource allocation. This systematic review aimed to identify and critically appraise artificial intelligence (AI)- and machine learning (ML)-based prediction models for postoperative LOS after CRC surgery, with emphasis on outcome definitions, predictor timing, model performance, validation, and clinical applicability. A systematic search of PubMed/Medline, Embase, and Scopus was conducted from database inception to 17 April 2026, with supplementary Google Scholar and reference screening. Peer-reviewed studies reporting AI/ML-based models developed and/or validated to predict postoperative LOS as a continuous outcome, or prolonged/delayed LOS as a categorical outcome, in adults undergoing CRC surgery were included. Risk of bias and applicability were assessed using PROBAST. Owing to substantial heterogeneity in study design, outcome definitions, predictor timing, and reported performance measures, findings were synthesized narratively. Six studies met the eligibility criteria. Included studies were heterogeneous in design, setting, cohort size, LOS definitions, predictor timing, modeling methods, and validation strategies. Outcomes were modeled variably as continuous LOS, prolonged LOS, or delayed discharge, with nonstandardized thresholds across studies. Predictors commonly included age, comorbidity burden, American Society of Anesthesiologists (ASA) score, operative approach, stoma creation, operative duration, blood loss, enhanced recovery pathway variables, and postoperative complications or early recovery measures. Internal validation frequently showed moderate-to-high predictive performance, particularly in models incorporating intraoperative or early postoperative information; however, calibration reporting, external validation, and prospective impact evaluation were uncommon. Overall, methodological concerns included inconsistent reporting of missing-data handling, small sample size in some cohorts, and limited evidence of transportability across settings. Current evidence suggests that AI/ML models may help estimate postoperative LOS after CRC surgery, particularly for perioperative or early postoperative risk stratification. However, the evidence base remains limited and methodologically heterogeneous, and the scarcity of external validation and incomplete reporting reduce confidence in routine clinical implementation. Future studies should use standardized outcomes, align predictor timing with intended clinical use, and prioritize transparent reporting, robust validation, and prospective evaluation.
Die Anastomoseninsuffizienz bleibt eine der folgenreichsten Komplikationen der kolorektalen Chirurgie. Da die Durchblutung eine zentrale Voraussetzung der Anastomosenheilung ist, hat sich die intraoperative Nahinfrarot-Fluoreszenzangiographie mit Indocyaningrün (ICG) als ergänzendes Verfahren zur visuellen Perfusionsbeurteilung etabliert. Der vorliegende Beitrag erläutert Wirkprinzip, Dosierung bzw. Timing, praktische Durchführung und Interpretation (Intensität, Homogenität, Anflutzeit) sowie Limitationen und Fehlerquellen dieses Verfahrens. Randomisierte kontrollierte Studien (RCTs) zeigen heterogene Effekte; in einzelnen Studien wurde eine Reduktion der Anastomoseninsuffizienz nachgewiesen. Insgesamt sprechen RCT-basierte Metaanalysen für einen moderaten Nutzen insbesondere bei linksseitigen Resektionen bzw. tiefen Anastomosen. Dargestellt werden Kontraindikationen, die Dokumentation von Strategieänderungen und Perspektiven .
Background: Hemorrhoids affect a large proportion of adults over the course of their lives and are considered a common condition. With the help of basic measures and conservative or minimally invasive therapies, they can be effectively treated. Objective: This work provides an overview of available treatment options. The correct application of sclerotherapy and rubber band ligation is described. Materials and methods: Evaluation of basic research articles, comparative studies of different techniques, and current guidelines. Results: Sclerotherapy has been shown to achieve a success rate ranging from 52% to 97% after 1 year for grade 1 and 2 hemorrhoids. Rubber band ligation for grade 2 hemorrhoids leads to an improvement of symptoms in 75-90% of cases and is, therefore, superior to surgery due to a lower complication rate. In grade 3 hemorrhoids, the recurrence rate is higher compared to surgical treatment, as demonstrated by the HollAND study. Discussion: In addition to basic therapy, sclerotherapy and rubber band ligation-when selected appropriately according to disease stage-are very well suited for long-term treatment of hemorrhoids and for preventing disease progression. Conclusion: Optimization of bowel habits and stool consistency is essential for preventing hemorrhoidal symptoms. When symptoms occur, there are effective treatment options available.
Background Acne inversa, also known as hidradenitis suppurativa, is a chronic relapsing inflammatory disease of the terminal hair follicles characterized by painful nodules, abscesses, secretions, tunnel formation and scarring. Beyond the considerable somatic disease burden, patients frequently experience pronounced psychosocial distress and relevant socioeconomic consequences. Research question The aim of this review article is to provide coloproctologically active, primarily surgically oriented physicians with a structured guideline-based overview of the pharmacotherapy of acne inversa and to illustrate its importance for early disease control and for improvement of the operative situation. Material and methods This narrative review is based on the Association of the Scientific Medical Societies in Germany (AWMF) S2k guidelines for the treatment of acne inversa (registry number 013-012). In addition, current prescription information of approved medications and selected scientific publications were taken into account. Results In cases of mild disease, topical antiseptic or antibiotic measures, such as clindamycin 1% can be used. In active or recurrent disease activity, systemic antibiotic drugs represent a key treatment component when used for a limited period of time. In moderate to severe disease, biologics are indicated according to the current guidelines. Approved agents include adalimumab from the age of 12 years as well as secukinumab and bimekizumab for adult patients. During inflammatory flares, combination therapy with biologics and antibiotics can be considered. Conclusion The importance of pharmacotherapy is steadily increasing, as early and effective control of inflammation can reduce the progression to chronic tunnels and fistulous structures as well as prevent or delay surgical interventions or improve their extent and predictability. Due to the high prevalence of psychological comorbidities, including depression and suicidal ideation, particular clinical attention is required.
