
The recently published German S3 guideline on salivary gland tumors provides an overview of the current evidence as well as recommendations for managing salivary gland tumors. A distinctive feature compared to other oncology guidelines is that it addresses both benign and malignant tumors. This article aims to highlight the most important aspects of the guideline for practicing otorhinolaryngologists and to outline the corresponding recommendations for management of salivary gland tumors.
Die kürzlich veröffentlichte S3-Leitlinie zu den Speicheldrüsentumoren gibt einen Überblick über die derzeitige Evidenzlage sowie Handlungsempfehlungen zu Raumforderungen der Speicheldrüsen. Eine Besonderheit im Vergleich zu anderen onkologischen Leitlinien besteht darin, dass sowohl benigne als auch maligne Raumforderungen thematisiert werden. Der vorliegende Artikel greift die für den klinisch tätigen HNO-Arzt wichtigsten Aspekte aus der Leitlinie heraus und zeigt die entsprechenden Handlungsempfehlungen auf.
Tiefe Halsinfektionen sind potenziell lebensbedrohliche Erkrankungen, die sich aufgrund der komplexen Anatomie der zervikalen Faszienräume rasch ausbreiten können. Sie entstehen meist aus Infektionen des oberen Aerodigestivtrakts oder odontogenen Herden und gehen trotz moderner Therapie weiterhin mit relevanter Morbidität und Mortalität einher. Ziel dieser Arbeit ist es, die Pathophysiologie, Diagnostik und Therapie tiefer Halsinfektionen darzustellen und die Bedeutung eines zeitkritischen Managements im Sinne des Prinzips „time is tissue“ hervorzuheben. Es handelt sich um eine narrative Übersichtsarbeit auf Grundlage aktueller Literatur und Leitlinien, die anatomische Grundlagen, klinische Präsentation, Bildgebung sowie therapeutische Strategien verschiedener Krankheitsbilder berücksichtigt. Die kontrastmittelverstärkte Computertomographie (CT) stellt den diagnostischen Goldstandard dar. Tiefe Halsinfektionen sind meist polymikrobiell bedingt. Eine frühzeitige antibiotische i.v.-Therapie sowie die chirurgische Drainage bei Abszessbildung sind entscheidend. Der Sicherung der Atemwege kommt dabei eine zentrale Bedeutung zu. Die anatomischen Gegebenheiten begünstigen eine rasche Ausbreitung bis ins Mediastinum. Risikofaktoren wie Diabetes mellitus oder Immunsuppression verschlechtern die Prognose. Ein frühzeitiges, interdisziplinäres und leitliniengerechtes Vorgehen ist entscheidend zur Reduktion von Komplikationen und Mortalität.
BACKGROUND:Deep neck infections are potentially life-threatening conditions that can spread rapidly due to the complex anatomy of the cervical fascial spaces. They most commonly originate from infections of the upper aerodigestive tract or odontogenic sources and remain associated with significant morbidity and mortality despite advances in treatment. OBJECTIVE:The aim of this study is to outline the pathophysiology, diagnosis, and management of deep neck infections, emphasizing the importance of timely intervention in accordance with the principle of "time is tissue." MATERIALS AND METHODS:This work is a narrative review based on current literature and clinical guidelines. It addresses anatomical considerations, clinical presentation, imaging modalities, and therapeutic approaches. RESULTS:Contrast-enhanced CT is the diagnostic gold standard for evaluating disease extent and complications. Deep neck infections are typically polymicrobial. Early intravenous antibiotic therapy and surgical drainage in cases of abscess formation are essential. Airway management plays a central role. CONCLUSION:The anatomical structure facilitates rapid spread, potentially reaching the mediastinum. Risk factors such as diabetes and immunosuppression are associated with worse outcomes. Early guideline-based interdisciplinary management is crucial to reduce complications and mortality.
Sprachbarrieren stellen ein Hindernis im klinischen Alltag dar. Webbasierte Übersetzungssoftware bietet eine barrierearme Lösung. Diese Studie untersucht die Einstellung der Mitarbeitenden einer deutschen Universitätsklinik zu Sprachbarrieren und webbasierter Übersetzungssoftware. Ferner werden datenschutz- und haftungsrechtliche Grundzüge für die Anwendung im klinischen Alltag beleuchtet. Es werden ausschließlich Aspekte der Verarbeitung personenbezogener Daten von Patient:innen untersucht. Mitarbeitende (N = 138) einer Universitätsklinik wurden zu Sprachbarrieren, ihrem Auftreten, Strategien zu ihrer Überwindung und insbesondere der Verwendung webbasierter Übersetzungssoftware befragt. BACKGROUND:Language barriers are an obstacle in everyday clinical practice. Webbased translation software can offer a low-barrier solution. This study examines the attitudes of employees at a German university hospital towards language barriers and web-based translation software. Furthermore, general data protection aspects for their application in everyday clinical practice are adressed. METHODS:Employees of a university hospital were surveyed on language barriers, their occurrence, strategies for overcoming them, as well as their use of web-based translation software. Furthermore, the use of such software was analyzed from a data protection and liability perspective. RESULTS:A majority of 80% (111/138) stated that they encountered language barriers in less than half of all patient contacts. Translation software was used in at least half of such cases by 22% (31/138) of respondents. The usefulness of translation software was rated as predominantly positive by 49% (68/138). From a legal perspective, it is strongly recommended that personal data be excluded when using web-based translation software. The use of anonymized data is permissible under data protection regulations. In emergencies or similar exceptional situations, the processing of personal data is also subject to the applicable data protection legislation in these situations. Provided that data protection legislation requirements are met, the use of personal data is permissible. A general assessment of liability during the use of webbased translation software to fulfill the duty of informed consent is not possible, as it is highly dependent on case-specific factors. However, since translation errors may be attributable in individual cases, a human interpreter should be consulted whenever the treating professional has doubts regarding the patient's understanding or the comprehensibility of the information provided. CONCLUSION:Language barriers pose a potential risk in everyday clinical practice. Webbased translation software is easily accessible and could improve patient safety and treatment outcomes. As the use of anonymized data is feasible in most cases, there are no data protection obstacles to using webbased translation software with such data. In the event of any doubt regarding the patient's understanding of the information provided, the involvement of a human translator is recommended to minimize liability risks.
Wir berichten über eine 64-jährige Patientin, die sich zwecks Klärung der Dignität einer unklaren kontrastmittelaufnehmenden Raumforderung zervikal paraösophageal einer operativen Halsexploration in unserer Klinik vorstellte. Hier ergab sich der Verdacht auf eine arteriovenöse Malformation. In einer postoperativen Katheterangiographie zeigte sich jedoch, entgegen der Erwartung, eine beidseitige Agenesie der V. jugularis interna. In der Literatur wurden bisher einige Fälle einer einseitigen Agenesie der V. jugularis interna beschrieben, allerdings, nach unserem Kenntnisstand, bislang noch keine beidseitige.
Im deutschsprachigen Raum steht zur sprachaudiometrischen Kontrolle nach Cochlea-Implantat(CI)-Versorgung ein breites Methodenspektrum zur Verfügung. Zur Ermittlung der Sprachverständlichkeitsschwelle im Rauschen (SRT) kommen verschiedene Verfahren zum Einsatz. Der Digits-in-Noise-Test (DiN) bietet als adaptiver Test mit einfacher lexikalischer Struktur eine Alternative zu listenbasierten Sprachtests oder Matrixtests. Ziel der Arbeit ist die Bewertung der klinischen Relevanz des DiN in der postoperativen Verlaufskontrolle, insbesondere im Vergleich zum Freiburger Sprachtest (FBS) und Oldenburger Satztest (OlSa) sowie zur Unterstützung individualisierter CI-Einstellungen. In einer prospektiven Studie wurden 73 erwachsene CI-Tragende untersucht. Das Sprachverstehen wurde monaural im Freifeld mit dem FBS, OlSa und DiN erhoben. Die Teilnehmenden wurden anhand des Hörverlusts für Zahlen (HVZ) in drei Gruppen eingeteilt. Der OlSa im Störgeräusch wurde nur bei ausreichendem Sprachverständnis durchgeführt. Die Auswertung erfolgte mit deskriptiver Statistik, Kruskal-Wallis-Test und Bland-Altman-Analyse. Die Test-Retest-Reliabilität des DiN war hoch und unabhängig vom Messzeitpunkt. Die SRT unterschieden sich signifikant zwischen den Gruppen. Bessere Hörbarkeit resultierte in niedrigeren (besseren) SRT und höherem Score für die durchgeführten Sprachtests in Ruhe. Der DiN konnte bei allen Teilnehmenden durchgeführt werden. Der DiN ist in allen Phasen der audiologischen CI-Nachsorge anwendbar, ermöglicht eine frühe Differenzierung und Verlaufskontrolle, liefert Hinweise auf Hörbarkeit, ergänzt den FBS und kann in zukünftigen Studien zur optimalen Einstellung der CI-Systeme verwendet werden.
BACKGROUND:A wide range of methods for speech audiometry to evaluate the outcome after cochlear implant (CI) fitting exist in German-speaking countries. Various procedures are used to determine the speech recognition threshold in noise (SRT). As an adaptive test with a simple lexical structure, the digits-in-noise (DiN) test offers an alternative to list-based speech tests or matrix tests. The aim of this study was to evaluate the clinical relevance of DiN in postoperative follow-up, particularly in comparison to the Freiburg speech test (FBS) and the Oldenburg sentence test (OlSa), as well as for supporting individualized CI settings. MATERIALS AND METHODS:In a prospective study, 73 adult CI users were examined. Speech comprehension was assessed monaurally in a free field using the FBS, OlSa, and DiN. The participants were divided into three groups based on their hearing loss for numbers (HVZ). The OlSa in noise was only performed if speech comprehension was sufficient. The evaluation was carried out using descriptive statistics, the Kruskal-Wallis test, and Bland-Altman analysis. RESULTS:The test-retest reliability of the DiN was high and independent of the time of measurement. The SRTs differed significantly between groups, with better audibility showing lower thresholds. Speech tests in quiet and noise confirmed these differences. The DiN could be performed in all participants. CONCLUSION:The DiN can be used in all phases of audiological CI follow-up care and enables early differentiation and progress monitoring, provides information on audibility, complements the FBS, and may be used in future studies on optimal fitting of CI systems.
Komplikationen der akuten Otitis media (AOM) sind selten, können jedoch intratemporal oder intrakraniell auftreten. Klinisch relevante Mastoiditiden zeigen sich meist durch subperiostale Abszesse, während subklinische Verschattungen der Mastoidzellen häufig auftreten, aber keine spezifische Therapie erfordern. Weitere mögliche Komplikationen umfassen Labyrinthitis, Fazialisparese, intratemporale Abszesse, Sinusvenenthrombosen oder Meningitiden. Die Diagnostik basiert auf klinischer Untersuchung, Laborparametern und gezielter Bildgebung. Intratemporale Komplikationen werden primär antibiotisch behandelt. Chirurgische Eingriffe sind bei Abszessbildung oder Therapieversagen erforderlich. Intrakranielle Komplikationen bedürfen meist einer interdisziplinären operativen Versorgung. Die vorliegende Übersicht bietet praxisnahe Empfehlungen zu Differenzierung, Diagnostik und Therapie und soll die Aufmerksamkeit für potenziell schwerwiegende Verläufe schärfen.
Complications of acute otitis media (AOM) are rare but can occur intratemporally or intracranially. Clinically relevant mastoiditis usually presents with subperiosteal abscesses, whereas subclinical opacification of the mastoid cells is common but does not require specific treatment. Other potential complications include labyrinthitis, facial nerve palsy, intratemporal abscess, cerebral venous sinus thrombosis, or meningitis. Diagnosis relies on clinical examination, laboratory tests, and targeted imaging. Intratemporal complications are primarily treated with antibiotics. Surgical intervention is indicated in cases of abscess formation or treatment failure. Intracranial complications often necessitate interdisciplinary surgical management. This review provides practical guidance on differentiation, diagnosis, and treatment, highlighting the importance of recognizing potentially severe courses.
Die Anwendung von injizierbaren Fillern hat sich in den letzten 20 Jahren als eine der am stärksten wachsenden ästhetisch-medizinischen Interventionen etabliert. Hyaluronsäurebasierte Filler machen den größten Marktanteil aus, aber alternative Materialien wie Calciumhydroxylapatit, Poly-L-Milchsäure und polymethylmethacrylatbasierte Präparate gewinnen an Bedeutung. Fortschritte in Injektionsstrategien, anatomischer Präzision, rheologischer Produktdifferenzierung und Sicherheitsstandards haben zu deutlich besseren klinischen Ergebnissen geführt. Doch das Risiko seltener, jedoch potenziell schwerwiegender Komplikationen – v. a. vaskulärer Okklusionen – bleibt eine zentrale Herausforderung. Ziel der vorliegenden Arbeit ist eine umfassende, evidenzgestützte Analyse der aktuellen Evidenzlage zu Materialien, Mechanismen, anatomischen Risikozonen, Injektionstechniken, Indikationen, Komplikationsmanagement und zukünftigen Entwicklungen in der Gesichtsfillertherapie.
The use of dermal fillers has emerged over the past two decades as one of the fastest-growing interventions in aesthetic medicine. While hyaluronic acid-based fillers account for the largest market share, alternative materials such as calcium hydroxylapatite, poly-L-lactic acid, and polymethylmethacrylate-based products are gaining increasing importance. Advances in injection strategies, anatomical precision, rheological product differentiation, and safety standards have led to a marked improvement in clinical outcomes. However, the profile of rare but potentially severe complications-particularly vascular occlusions-remains a central challenge. The aim of this review is to provide a comprehensive, evidence-based analysis of the current literature regarding materials, mechanisms of action, anatomical risk zones, injection techniques, indications, complication management, and future developments in filler therapy.
