
Aggression is an evolutionarily conserved social behavior that becomes clinically relevant when it loses contextual and inhibitory regulation. Rodent models do not serve to reproduce the full complexity of human violence, with its legal, moral, cultural, and interpersonal dimensions. Their translational value lies instead in making core mechanisms of aggression regulation experimentally accessible. This article discusses how rodent models help to distinguish adaptive from dysregulated aggression; to select clinically informative paradigms; and to identify causal pathways that shape attack threshold, escalation, persistence, reward value, and inhibitory control. Rodent studies have mapped a multilevel control architecture in which social sensory threat is appraised through amygdala–hypothalamic pathways, hypothalamic and midbrain circuits organize attack-related behavior, and corticostriatal systems contribute to motivation and regulatory control. Rather than acting as fixed modules, these circuits are shaped by experience, internal state, sex and endocrine modulation, immune signaling, and aggression-related reward. Next-generation models should therefore complement traditional reductionist assays with longitudinal trajectories, socially embedded phenotyping, dysregulation readouts, and measures of distributed network and body–brain states. Within TRR 379, rodent models enable mechanism-focused translation by helping to identify biosignatures and modifiable pathways for future interventions in dysregulated aggression.
Aggression in mental disorders (AMD) poses a challenge for everyday clinical practice and has far-reaching societal implications. The biological foundations of the different manifestations, developmental trajectories, and transdiagnostic and disorder-specific deficits remain largely unknown. One reason for the apparent lack of understanding of the complex mechanisms underlying AMD is the heterogeneity of both aggression and mental disorders themselves. This, in turn, hinders progress in developing effective prevention and intervention strategies. Therefore, a collaborative approach between clinical and experimental neuroscientists is required to address this issue comprehensively and in a multimodal manner. The proposed initiative aims to advance knowledge on the biological bases of AMD and its developmental course. The Transregional Collaborative Research Center 379 (TRR) will follow the Research Domain Criteria (RDoC) framework, investigating multiple aspects that characterize aggression: genetic and molecular mechanisms as well as hormonal, neural, and associated behavioral systems. Thus, the TRR aims to identify biosignatures of AMD based on multidimensional pathways that underlie the different manifestations of aggression to enable the development of tailor-made biosignature-specific prevention and intervention.
Psychosoziale Krisen treten häufig bei Borderline- oder Traumafolgestörungen auf. Mangels zeitnaher ambulanter Alternativen erfolgt die Versorgung oft über die Notaufnahme und stationäre Akutpsychiatrie, was die Behandlungskontinuität erschwert und Hospitalisierung begünstigt. Entwicklung und Implementierung eines klinik- und sektorenübergreifenden Behandlungskonzepts zur Krisenintervention und zum Aufbau von Therapiecommitment bei Patient*innen mit Borderline- und Traumafolgestörungen am Zentralinstitut (ZI) für Seelische Gesundheit in Mannheim. Darstellung des neuen Versorgungskonzepts vor dem Hintergrund typischer klinischer Herausforderungen in der Behandlung akuter Krisen. Einbettung in das Modell der dreigliedrigen Behandlungspfade sowie Beschreibung der Behandlungslogik und Schnittstellen. Störungsspezifische Elemente der dialektisch-behavioralen Therapie (DBT) ermöglichen eine fokussierte, niederschwellige Krisenintervention und fördern den Übergang in eine störungsspezifische Psychotherapie. Zentrale Struktur ist die klinikübergreifende Ambulanz für Krisenintervention und Therapiecommitment (AKuT-Ambulanz) als Schnittstelle zwischen stationärer Krisenintervention in der psychiatrischen Klinik und den störungsspezifischen Angeboten der psychosomatischen Klinik. Dadurch entsteht eine durchgängige Behandlungskette von der Krisenintervention bis zur störungsspezifischen Psychotherapie. Herausforderungen bei der Implementierung im akutpsychiatrischen Setting einer Klinik mit Versorgungsauftrag und mögliche Effekte auf Behandlungskontinuität und Hospitalisierung werden kritisch diskutiert.
Ketamin ist seit Jahrzehnten als Anästhetikum etabliert. Um die Jahrtausendwende zeigten erste klinische Studien, dass subanästhetische Ketamindosen eine antidepressive Wirkung bei therapieresistenter Depression entfalten können. Diese Befunde begründeten das Konzept einer glutamaterg wirksamen antidepressiven Therapie und leiteten einen Paradigmenwechsel ein. Über viele Jahre wurde intravenöses Ketamin off-label in spezialisierten Zentren eingesetzt. Mit der Zulassung intranasalen Esketamins steht heute jedoch eine standardisierte und regulär erstattungsfähige Therapieoption zur Verfügung, wodurch die Off-label-Infusion im aktuellen Versorgungskontext deutlich an Bedeutung verloren hat. Esketamin ist ausschließlich in Kombination mit einem selektiven Serotoninwiederaufnahmeinhibitor (SSRI) oder Serotonin-Noradrenalin-Wiederaufnahmeinhibitor (SNRI) zugelassen und erfordert ein strukturiertes Behandlungssetting mit definiertem Monitoring. Der Beitrag gibt ein praxisorientiertes Update zu Wirkmechanismus, Indikationen, Kontraindikationen, zugelassenem Therapieschema, Sicherheitsanforderungen sowie zur Einordnung in den therapeutischen Algorithmus.
The S3 guidelines on posttraumatic stress disorder (PTSD) have been updated and expanded under the leadership of the German-speaking Society for Psychotraumatology (DeGPT). Given the high prevalence of trauma exposure, traumatic experiences should be assessed during all clinical evaluations and history taking. Trauma-focused psychotherapy remains the first-line treatment and should also be offered in cases of complex PTSD (cPTSD) with comorbid disorders, in older people, refugees and individuals with mild to moderate intellectual disabilities. Strong evidence is now available for a broader range of interventions, particularly cognitive behavioral therapy (CBT). Trauma-focused interventions can be delivered online and at higher intensity (several sessions per week). For children and adolescents, trauma-focused CBT also remains the treatment of choice. Pharmacotherapy is the second-line treatment and is not recommended for children or adolescents. Benzodiazepines and Z‑drugs should generally be avoided. Among adjuvant forms of treatment the strongest evidence is available for exercise and physical activity. An important treatment principle involves the establishment of trauma-informed basic principles (trauma-informed care) as a standard approach in institutions of the healthcare system.
Globally, nearly 1 billion people are affected by mental disorders. Psychotherapy, despite its demonstrated efficacy, often shows only limited effects. Current estimates suggest that fewer than half of the patients achieve a clinically relevant symptom reduction. Against this background, innovative psychotherapeutic approaches are gaining importance, particularly personalized treatments, such as modular psychotherapy. Modular psychotherapy refers to treatment programs composed of distinct, functionally defined units (so-called modules) that are independently effective and can be flexibly combined in variable sequences within a systematic treatment algorithm. Initial efficacy studies, such as the MATCH study in children and adolescents, showed superior effects of modular psychotherapy compared to standard treatments, although current evidence is heterogeneous. Current developments of modular psychotherapy, such as the MeMoPsy studies conducted within the German Center for Mental Health (DZPG), integrate digital phenotyping, algorithm-based decision processes, and transdiagnostic mechanisms of change. Overall, modular psychotherapy represents an innovative approach to sustainably enhance the effectiveness of psychotherapeutic treatments via personalization.
Weltweit sind nahezu 1 Milliarde Menschen von psychischen Erkrankungen betroffen, wobei Psychotherapie trotz nachgewiesener Wirksamkeit häufig nur begrenzte Effekte zeigt. Aktuelle Schätzungen legen nahe, dass weniger als die Hälfte der Patient:innen eine klinisch relevante Symptomreduktion erreicht. Vor diesem Hintergrund gewinnen innovative psychotherapeutische Ansätze an Bedeutung, insbesondere personalisierte Behandlungen wie die modulare Psychotherapie. Modulare Psychotherapie bezeichnet Behandlungsprogramme aus eigenständigen, funktional definierten Einheiten (sogenannten Modulen), die unabhängig voneinander wirksam sind und innerhalb eines systematischen Behandlungsalgorithmus flexibel und in variabler Reihenfolge kombiniert werden können. Erste Wirksamkeitsstudien, wie die MATCH-Studie bei Kindern und Jugendlichen, zeigen teils überlegene Effekte modularer Psychotherapie gegenüber Standardbehandlungen, wenngleich die Befundlage heterogen ist. Aktuelle Weiterentwicklungen modularer Psychotherapie, wie die im Rahmen des Deutschen Zentrums für Psychische Gesundheit (DZPG) realisierten MeMoPsy-Studien, integrieren digitale Phänotypisierung, algorithmusbasierte Entscheidungsprozesse sowie transdiagnostische Veränderungsmechanismen. Insgesamt stellt die modulare Psychotherapie einen zukunftsweisenden Ansatz dar, um auf dem Wege bestmöglicher Personalisierung die Wirksamkeit psychotherapeutischer Behandlungen nachhaltig zu verbessern.
BACKGROUND:Depressive disorders are associated with a high burden of disease; a substantial proportion of patients show insufficient response despite guideline-based pharmacotherapy and psychotherapy, particularly in treatment-resistant depression (TRD). Neuromodulation has become established as a fourth pillar in the treatment of depression and includes electroconvulsive therapy (ECT), repetitive transcranial magnetic stimulation (rTMS), vagus nerve stimulation (VNS), transcranial electrical stimulation (tES), magnetic seizure therapy (MST), deep brain stimulation (DBS) and neurofeedback (NF). Despite partly well-established efficacy, these interventions remain insufficiently integrated into routine clinical care. The aim of this paper is to summarize the current evidence and to analyze key barriers to implementation. MATERIAL AND METHODS:Narrative review of clinical studies, meta-analyses, and care data on efficacy, safety and cognitive tolerability. RESULTS:The strongest evidence for both acute and maintenance treatment of severe depressive disorders is for ECT; rTMS is effective and cognitively well tolerated and VNS can represent an additive long-term option in TRD. For tES, small and heterogeneous effects have been reported, MST and DBS remain experimental and for NF consistent evidence of efficacy is lacking. Key barriers include partly heterogeneous protocols, insufficient funding structures, particularly in the outpatient setting and the absence of standardized qualification requirements. CONCLUSION:Neuromodulation should be systematically integrated into staged-care models. Standardized protocols and registry-based research are needed to enable the structured and sustainable implementation of neuromodulatory interventions in clinical care.
Depressive Störungen gehen mit hoher Krankheitslast einher; ein relevanter Anteil der Patient:innen spricht trotz leitliniengerechter Pharmako- und Psychotherapie unzureichend an, insbesondere bei therapieresistenter Depression (TRD). Neuromodulation etabliert sich als vierte Säule der Depressionsbehandlung und umfasst Elektrokonvulsionstherapie (EKT), repetitive transkranielle Magnetstimulation (rTMS), Vagusnervstimulation (VNS), transkranielle elektrische Stimulation (tES), Magnetkonvulsionstherapie (MKT), tiefe Hirnstimulation (THS) und Neurofeedback (NF). Trotz teilweiser gut belegter Wirksamkeit sind diese Verfahren bislang nicht ausreichend in die Regelversorgung integriert. Ziel ist die Darstellung der Evidenzlage und zentraler Implementierungsbarrieren. Narrative Auswertung klinischer Studien, Metaanalysen und Versorgungsdaten zu Wirksamkeit, Sicherheit und kognitiver Verträglichkeit. Für EKT besteht die beste Evidenz hinsichtlich Akut- und Erhaltungstherapie schwerer depressiver Störungen; rTMS ist wirksam und kognitiv gut verträglich. VNS kann eine additive Langzeitoption bei TRD darstellen. Für tES werden kleine, heterogene Effekte berichtet. MKT und THS bleiben experimentell; für NF fehlen konsistente Wirksamkeitsnachweise. Zentrale Barrieren sind teilweise heterogene Protokolle, unzureichende Finanzierungsstrukturen, insbesondere im ambulanten Bereich, und fehlende Qualifikationsstandards. Neuromodulation sollte systematisch in gestufte Versorgungsmodelle integriert werden. Standardisierte Protokolle und Registerforschung sind erforderlich, um eine strukturierte und nachhaltige Implementierung neuromodulatorischer Verfahren in die klinische Versorgung zu ermöglichen.
BACKGROUND:Cannabidiol (CBD) has gained sustained public, medical, and scientific attention in recent years. The prevalence of use of commercially available products containing CBD (CPC) is correspondingly high. OBJECTIVES:Overview of the benefits and risks of CPC, taking into account certain aspects related to CBD as a pharmaceutical active ingredient. MATERIALS AND METHODS:Selective literature search in the PubMed database. RESULTS:Currently, only one primarily CBD-containing drug is approved in Germany (Epidyolex®). CBD demonstrates good safety and tolerability. Elevated transaminases, sedation, decreased appetite, diarrhea, sleep disturbances, infections, and weight loss are possible side effects. CBD modulates the endocannabinoid system and interacts with numerous other molecular targets. CBD is metabolized via CYP2C19, CYP3A4, and UGT. The potential for pharmacokinetic interactions with CBD has not been well studied, but is theoretically high. The potential for abuse of CBD is negligible when administered orally, but should be investigated more thoroughly for inhalation and higher-dose use. Studies have found significant discrepancies between the stated and actual CBD content in many CPC; THC was frequently detected. There is no evidence supporting the health benefits typically claimed by manufacturers for ≤ 4.3 mg of oral CBD/kg/day, but there are indications of significant hepatotoxicity at ≥ 4.3 mg of oralCBD/kg/day. CONCLUSION:There is no robust evidence of health benefits associated with CPC. However, given the levels of exposure, product deficiencies, and the EFSA safety concerns, it must be assumed that there are relevant health risks (particularly hepatotoxicity) associated with the use of CPC.
Cannabidiol (CBD) hat in den letzten Jahren anhaltende allgemeine sowie medizinische und wissenschaftliche Aufmerksamkeit erlangt. Die Konsumprävalenz von kommerziell erhältlichen CBD-haltige Präparaten (KCP) ist entsprechend hoch. Übersicht über Nutzen und Risiken von KCP unter Beachtung einiger Aspekte, die CBD als pharmazeutischen Wirkstoff betreffen. Selektive Literaturrecherche in der Datenbank PubMeb. Aktuell ist nur ein primär CBD-haltiges Medikament in Deutschland zugelassen (Epidyolex®). CBD weist eine gute Sicherheit und Verträglichkeit auf. Transaminasenerhöhungen, Sedierung, Appetitminderung, Diarrhoe, Schlafstörungen, Infektionen und Gewichtsverlust sind mögliche Nebenwirkungen. CBD moduliert das Endocannabinoidsystem und interagiert mit zahlreichen weiteren molekularen Zielen. CBD wird über CYP2C19, CYP3A4 und UGT metaboliert. Das pharmakokinetisches Interaktionspotential von CBD ist nicht gut untersucht, theoretisch aber groß. Das Missbrauchspotential von CBD ist bei oraler Anwendung vernachlässigbar, sollte jedoch bei inhalativer und höher dosierter Anwendung stärker geprüft werden. In Studien fand sich bei vielen KCP eine ausgeprägte Diskrepanz zwischen angegebenem und tatsächlichem CBD-Gehalt; häufig konnte THC nachgewiesen werden. Es gibt keine die herstellerseitig typischerweise behaupteten gesundheitsbezogenen Vorteile von ≤ 4,3 mg oralem CBD/kg/d unterstützende Evidenz, aber Hinweise auf relevante Hepatotoxizität bei ≥ 4,3 mg oralem CBD/kg/d. Es gibt keine belastbare Evidenz für gesundheitsbezogene Vorteile von KCP. Unter Beachtung erreichbarer Expositionen, der Produktmängel sowie der Sicherheitsbedenken der EFSA müssen jedoch relevante Gesundheitsrisiken (v. a. Hepatotoxizität) bei der Anwendung von KCP angenommen werden.
