
L'épidémie de pneumonie virale à Covid-19 représente un enjeu de santé publique majeur pour toutes les structures hospitalières y compris la pharmacie hospitalière qui a joué un rôle incontournable dans la prise en charge de cette crise. Cette étude vise à explorer le niveau de stress perçu chez le personnel des PUI pendant cette pandémie. Il s'agit d'une étude transversale qui a évalué 82 professionnels de santé des pharmacies hospitalières du Maroc, en utilisant une technique d'échantillonnage en boule de neige. Elle a évalué leurs caractères sociodémographiques, les conditions de travail, l'échelle de stress perçu (PSS), leurs émotions et leurs inquiétudes pendant l'épidémie. L'analyse statistique a été réalisée à l'aide du logiciel SPSS. La majorité des participants était des femmes (74,4 %) et des pharmaciens hospitaliers (67 %). Plus de la moitié des participants ont montré des niveaux de stress modéré (73,2 %). Parmi les variables examinées, seules les mesures de protection et l'information sur la situation via la hiérarchie ont montré une association significative avec la valeur du PSS (P = 0,036) et le niveau du stress selon le test Anova et le test Chi-2 respectivement. Dans cette étude, le personnel de la pharmacie hospitalière a montré des niveaux de stress modéré au début de l'épidémie de Covid-19. Il est à noter que les caractères sociodémographiques n'ont pas d'impact sur le stress. La prise en considération des conditions sociales et sanitaires et la gestion du stress aiderait à atténuer le stress professionnel chez le personnel de la PUI pendant la pandémie. The Covid-19 viral pneumonia epidemic represents a major public health issue for all hospital structures, including hospital pharmacy, which has an essential role in dealing with this crisis. This study aims to explore the level of stress perceived by hospital pharmacy staff during this pandemic. This is a cross-sectional study that evaluated 82 health professionals in hospital pharmacies in Morocco, using a snowball sampling technique. It evaluated their socio-demographic characteristics, working conditions, perceived stress scale (PSS), emotions and concerns during the epidemic. Statistical analysis was performed using statistical package for social science (SPSS). The majority of participants were women (74.4%) and hospital pharmacists (67%). More than half of the participants showed moderate levels of stress (73.2%). Among the variables examined, only protective measures and information about the situation via the superiors showed a significant association with the PSS value (P = 0.036) and the level of stress according to the ANOVA test and the Chi-2 test respectively. In this study, hospital pharmacy staff showed moderate levels of stress at the beginning of the Covid-19 epidemic. It should be noted that socio-demographic traits had no impact on stress. Consideration of social and health conditions and stress management would help reduce professional stress among hospital pharmacy staff during the pandemic.
The “All American” is a mobile autoclave commonly used in developing countries. The objective of this study was to evaluate the sterility of cotton cloth wrapped instruments up to 7 days of storage. For this purpose, 3 “All American” sterilization cycles were conducted, one cycle weekly during a period of three weeks. Each cycle contained 5 sets composed of three clamps, wrapped in cotton cloth. Sterility maintenance was tested at J1, J2, J3, J4 and J7. The sets were stored in a heating chamber in order to reproduce the storage conditions of tropical climates. Bacteriological and mycological analyses were carried out on the outside and inside of the cotton, as well as on the clamps of each set. Although residual humidity rates up to 12% were recorded on cotton sets after cycles, we did not report bacterial or fungal growth, nor inside the cotton wraps, neither onto the clamps. These results are encouraging, but should be confirmed by increasing repeatability of sterilization cycle and by varying several parameters influencing dryness and sterility to test reproducibility.
Le 11 décembre 2018, une attaque terroriste a lieu à différents endroits de l’Eurométropole de Strasbourg. Le plan blanc est déclenché dans notre établissement. La Pharmacie usage intérieur (PUI) est un des services prioritaires mobilisé automatiquement. L’objectif est de présenter notre retour d’expérience afin d’en dégager les points forts, les points sensibles et de mettre en place des actions d’amélioration. La méthodologie CREX (comité de retour d’expérience) a permis de recueillir les informations sur les évènements selon leur chronologie, de les analyser par rapport aux référentiels internes disponibles. Une réunion de partage associant tous les acteurs présents lors du plan blanc a permis de réaliser ce CREX. La PUI a su répondre aux besoins des services accueillant les victimes en appliquant correctement les procédures prédéfinies. La coordination, la communication et l’organisation sont des atouts majeurs pour la réussite de la gestion de notre réponse au plan blanc. La formation associant théorie et exercices pratiques a été proposée comme axe d’amélioration pour former davantage de personnel à la gestion du plan blanc, mais également pour maintenir opérationnel le personnel déjà formé. On December 11, 2018, a terrorist attack took place at various locations in the Strasbourg Eurometropolis. The emergency action plan called “plan blanc” is launched in our hospital. The hospital pharmacy is one of the priority services requisitioned automatically. The objective, here, is to present our feedback in order to identify its strengths, sensitive points and to implement improvement actions. The CREX (feedback committee) methodology made it possible to collect information on events according to their chronology, to analyse them in relation to the available internal standards. A sharing meeting associating all the players present during the “plan blanc” allowed to carry out this CREX. The hospital pharmacy was able to respond to the needs of services receiving victims by correctly applying predefined procedures. Coordination, communication and organisation are the major assets for the successful management of our response to the “plan blanc”. The training combining theory and practical exercises was proposed as an area for improvement to train more staff in the management of the white plan, but also to keep the staff already trained operational.
L’objectif de notre travail était d’analyser la qualité des prescriptions hospitalières des pansements réalisées dans notre CHU en ciblant certains pansements prescrits et dispensés en ville (PHEV). Grâce à la collaboration avec la direction régional du service médical Alsace-Moselle (DRSM), 256 ordonnances anonymisées du 1 er semestre 2018 ont été analysées, en lien avec des codes LPPR identifiés. Ces ordonnances concernaient 200 patients et 368 références de pansements, soit en moyenne 1,4 types de pansements/ordonnance. Après analyse des prescriptions dispensées en officine de ville, plusieurs anomalies ont été identifiées : près de 50 % des ordonnances présentaient au moins une anomalie d’ordre réglementaire, quasi 12 % au moins une sur la quantité de pansements à dispenser, plus de 40 % au moins une dans la taille du pansement prescrit et près de 30 % une dans le libellé du pansement. Ainsi, seulement 41,80 % des ordonnances ( n = 107/256) étaient dispensées sans ambiguïté. A contrario, près de 2/3 des ordonnances posaient problème à la dispensation du fait d’ordonnances incomplètes. De telles dispensations pouvaient avoir des impacts tant pour le patient (non continuité des soins) que pour les pharmaciens d’officine (pertes de temps voire financières). Nous travaillons sur une ordonnance type pour améliorer le taux de dispensation univoque en ville. The objective of our work was to analyze the quality of hospital prescriptions of dressings made in our Hospital University by targeting certain dressings prescribed and dispensed in the city (PHEV). Thanks to the collaboration with the regional direction of the Alsace-Moselle Medical Service (DRSM), 256 anonymized prescriptions from the 1st half of 2018 were analyzed, in relation to identified LPPR codes. These prescriptions concerned 200 patients and 368 references of dressings, i.e. an average of 1.4 types of dressing/order. After analysis of the prescriptions dispensed in city pharmacies, several anomalies were identified: almost 50% of the prescriptions had at least one regulatory anomaly, almost 12% had at least one on the quantity of dressings to be dispensed, more than 40% had at least one in the size of the dressing prescribed and almost 30% had at least one in the wording of the dressing. Thus, only 41.80% of prescriptions ( n = 107/256) were dispensed unambiguously. On the other hand, nearly 2/3 of the prescriptions were problematic to dispense because of incomplete prescriptions. Such dispensing could have impacts both for the patient (non-continuity of care) and for pharmacists (loss of time and even money). We are working on a model prescription to improve the rate of unambiguous dispensing in the city.
Les enfants ne sont pas des « adultes miniatures ». Les pharmaciens et les étudiants en pharmacie doivent y être sensibilisés pour développer des compétences en pharmacie clinique pédiatrique. Dans le contexte d’un diplôme universitaire de pharmacie clinique pédiatrique et de 6 e année de filière officine, nous avons développé une nouvelle méthode pédagogique de type classe inversée. Ce projet, initié en 2017 par deux étudiantes en pharmacie, les membres de la cellule TICE et deux pharmaciens et enseignants spécialisés en pédiatrie a touché 30 pharmaciens et internes et 86 étudiants. Les thèmes choisis étaient « Le médicament pédiatrique » et « La fièvre chez l’enfant ». Au total, 4 vidéos ont été créées. Après visionnage, les participants devaient réaliser une fiche synthèse puis travailler sur des cas cliniques avec les intervenants des vidéos. Les participants ont été satisfaits par ce nouvel outil V-Idée-Oh (96 %), par la vision de la vidéo avant le cours (92 %). Concernant la préparation de la fiche, 81 % ont été satisfaits par ce travail en amont. Nous avons testé cette année une nouvelle méthode pédagogique sur deux thèmes de base. Grâce aux commentaires recueillis, nous allons raccourcir la préparation des cours cas et nous élargirons cette méthode pour d’autres thèmes pour les prochaines sessions. Cette nouvelle méthode pédagogique est attractive pour les étudiants et pour les enseignants. Cet article décrit ses modalités pratiques pour être transposable dans d’autres universités et sur d’autres thématiques. Children are not “miniature adults”. Pharmacists and pharmacy students need to be sensitized to this fact in order to develop pediatric clinical pharmacy skills. In the context of a “Pediatric Clinical Pharmacy” teaching course and for the 6th and last community pharmacy university year, we developed a new concept of reverse teaching skills. In 2017, a multidisciplinary project involving two pharmacy students, the information and communication technology unit, and two pharmacists and teachers specialized in pediatrics concerned 30 pharmacy residents and pharmacists and 86 students. The topics chosen were “Pediatric medication” and “Fever in children”. Four videos were implemented. The participants had to summarize the important notions and prepare clinical cases. The participants were satisfied by this new V-Idée-Oh tool (96%), by the vision of the video before the course (92%). About the summary, 81% were satisfied by this upstream work. This year we tested a new teaching method on two basic themes. Thanks to the comments collected, we will shorten the summary exchange and we will enlarge this method for other topics for the next session. This new teaching method is attractive for both students and teachers. This article proposes a method to be transposed in other universities and on other themes.
L’enrichissement constant du livret thérapeutique conduit à une multiplication des coûts. Dans un contexte économique strict, un outil d’aide au déréférencement en fonction des dosages et des galéniques a été mis en place afin d’optimiser le livret thérapeutique. Cet outil s’inscrit dans une démarche de gestion des risques. Un enchaînement de sept étapes inspiré par la méthode AMDEC, assisté par l’utilisation de deux échelles, permet l’obtention d’une note d’impact du déréférencement. Celle-ci est analysée dans un tableau à double entrée en fonction de la consommation annuelle du médicament. À partir de son positionnement dans le tableau, la pertinence de la suppression de ce médicament du livret thérapeutique est objectivée. Au total, 311 spécialités ont été analysées. Quarante-neuf d’entre elles ont été déréférencées après accord multidisciplinaire. Sur la base de 150 € de frais de gestion par spécialité, 7350 € ont été économisés ainsi que du temps pour le personnel grâce à un livret thérapeutique rationnalisé. Cette approche pour optimiser le livret thérapeutique évalue a priori les risques potentiels liés au déréférencement. Cette démarche d’amélioration continue de la qualité permet une rationalisation des coûts, du temps et une diminution du risque d’erreurs médicamenteuses. The constant addition of the therapeutic booklet leads to a multiplication of costs. In a strict economic context, a delisting tool according to dosages and galenic forms was implemented in order to optimize the therapeutic booklet. This tool is part of a risk management approach. A sequence of seven steps inspired by the FMECA method, assisted by the use of two scales, provides a delisting impact score. This is analyzed in a double entry table based on the annual consumption of the drug. Based on its position in the table, the relevance of removing this drug from the drug formulary is objectivized. In total, 311 pharmaceutical specialties were analyzed. Forty-nine product references were delisted after a multidisciplinary agreement. On the basis of €150 in management fees per specialty, €7350 was saved as well as staff time thanks to a streamlined drug formulary. This approach to optimize the therapeutic booklet assesses a priori the potential risks related to delisting. This continuous quality improvement approach allows to streamlining of costs, time and a reduction in the risk of medication errors.
Resume Introduction L’objectif cette etude etait de mettre en lumiere l’etat des lieux de l’utilisation des anti-TNF (adalimumab et etanercept) chez les patients atteints de rhumatisme inflammatoire chronique dans une unite de la pharmacie de l’hopital militaire d’instruction Mohammed V. Materiels et methodes Il s’agit d’une etude prospective realisee au niveau du pole pharmacie de l’hopital militaire de Rabat et incluant des patients traites avec l’adalimumab et l’etanercept et qui a dure 67 jours. Le recueil des informations a ete fait grâce a un interrogatoire direct avec le patient et a l’aide d’une fiche contenant les differents points en rapport avec le traitement anti-TNF α. Resultats Au total 76 patients ont ete inclus, avec un nombre egal entre les patients de sexe masculin et feminin. L’âge moyen etait de 49,8 ± 12,8 ans. L’etanercept etait le traitement le plus employe dans la polyarthrite rhumatoide (PR) (79 %) et la spondylarthrite ankylosante (SpA) (59 %). Les anti-TNF alpha etaient prescrits en monotherapie dans 46 % des cas. En cas d’associations, le methotrexate etait le plus employe (39 %). Le site d’injection le plus utilise etait au niveau de l’abdomen (28,9 %). Le traitement par les anti-TNF alpha etait bien tolere dans la plupart des cas (69,7 %). Conclusion Le traitement employe etait en adequation avec les recommandations de la Societe marocaine de rhumatologie. Nos patients avaient une bonne connaissance par rapport a leur traitement anti-TNF α et certaines informations necessaires et jugees utiles ont ete fournies aux patients admis a la pharmacie.
Le circuit des dispositifs médicaux stériles et implantables (DMS/DMI) est complexe et peut entraîner des évènements indésirables associés aux soins. L’objectif de ce travail est d’évaluer puis d’optimiser la sécurisation de ce circuit dans notre établissement. La grille InterDiag DMS® V2.0 de l’Agence Nationale d’Appui à la Performance a été choisie. Elle comprend deux modules évaluant le circuit des DMS, des DMI et les DMS utilisés dans cinq pratiques de soins. Les audits des unités de soins (US) et de la pharmacie (PUI) ont été réalisés entre novembre 2019 et février 2020. Dix-neuf US dont la PUI ont été auditées. Les taux de maîtrise globaux des différents items évoluent entre 62 et 74 %. Les étapes ayant les taux de maîtrise les plus faibles sont la demande (PUI : 50 %), le transport (PUI : 46 %), le stockage (US : 44 %) et l’évaluation de la traçabilité des DMI (PUI : 45 %). Parmi les 45 actions de sécurisation formulées, 16 ont été mises en place comme la réalisation d’audits sur la traçabilité des DMI. La cartographie effectuée de façon multidisciplinaire montre une bonne maîtrise globale du circuit. Les étapes les plus à risques identifiées sont en cours de sécurisation. Le plan d’action formulé devra être suivi et actualisé chaque année. The circuit of sterile and implantable medical devices (MD) is complex and can lead to adverse events associated with care. The objective of this work is to assess and then optimize the security of this circuit in our establishment. The InterDiag ® V2.0 grid from the Agence Nationale d’Appui à la Performance was chosen. It includes two modules evaluating the circuit of sterile MD, implantable MD and sterile MD used in five health care practices. The care unit (CU) and pharmacy audits were carried out between November 2019 and February 2020. Nineteen CU including the pharmacy were audited. The overall mastery rates for the various items vary between 62 and 74%. The stages with the lowest control rates are demand (pharmacy: 50%), transport (pharmacy: 46%), storage (CU: 44%) and evaluation of the traceability of implantable MD (pharmacy: 45%). Among the 45 security actions formulated, 16 have been implemented as audits of the traceability of implantable MD. The cartography carried out in a multidisciplinary way shows a good control of the circuit. The most risky stages identified are in the process of being secured. The formulated action plan will have to be monitored and updated every year.
Les antibiotiques et antifongiques constituent une classe médicamenteuse incontournable dans l’arsenal thérapeutique. L’émergence de résistances en font des médicaments précieux dont l’utilisation doit être correctement maîtrisée, tant dans les indications que dans les modalités d’administration pour en assurer une efficacité optimale. Cependant ces modalités sont complexes et dépendent aussi des situations d’utilisation qui sont, elles, très variables. Notre objectif était de déployer – à destination des professionnels de santé de notre territoire (hospitaliers ou libéraux) – un outil d’aide au bon usage des antibiotiques et antifongiques par voie parentérale et disponibles au livret thérapeutique de notre centre hospitalier. Nous avons réalisé une recherche bibliographique sur les caractéristiques pharmacocinétiques, pharmacodynamiques et de stabilité pour chacun des médicaments ; afin de synthétiser ces informations sur un document pour une diffusion territoriale. Pour chaque antibiotique et antifongique, nous avons détaillé les modalités de reconstitution et de dilution lors d'une administration en continu ou en discontinu, ainsi que la demi-vie, le type de bactéricidie et les modalités de dispensation à l’hôpital et en ville. Notre outil permet de réunir dans un document synthétique les informations nécessaires à tous les professionnels de santé impliqués dans le circuit des antibiotiques injectables. Antibiotics and antifungals are an essential drug class in the therapeutic arsenal. The emergence of resistances makes them valuable drugs; however, their use must be properly controlled in terms of both indications and administration modalities to ensure optimal efficacy. These modalities are complex and depend on the highly variable situations of use. Our objective was to deploy – for healthcare professionals in our territory (hospital or private practice) – a tool to assist in the proper use of parenteral antibiotics and antifungals available in the therapeutic booklet of our hospital. We conducted a literature search on the pharmacokinetics, pharmacodynamics and stability characteristics for each of the drugs, in order to synthesize this information in a document for territorial diffusion. For each antibiotic and antifungal drug, we have detailed the reconstitution and dilution modalities for continuous or discontinuous administration, as well as the half-life, the type of bactericide and the dispensing modalities in hospital and private practice. Our tool assembles in a synthetic document the necessary information for all healthcare professionals involved in the injectable antibiotic circuit.
La gestion péri-opératoire des patients traités avec les anticoagulants oraux directs (AOD) s’appuie sur les recommandations du Groupe d’Intérêt en Hémostase Périopératoire (GIHP) de 2015, avant et après un geste chirurgical invasif programmé ou urgent. L’objectif de notre étude observationnelle rétrospective monocentrique était d’évaluer les pratiques professionnelles liées à la gestion périopératoire des AOD à visée curative chez les patients bénéficiant d’une chirurgie orthopédique programmée ou urgente. Au total, 62 patients ont été audités sur 7 mois. La gestion des AOD était en adéquation avec les recommandations du GIHP en préopératoire chez 36 % des patients pris en charge dans le cadre d’un geste programmé. Concernant la reprise des AOD en post-opératoire, 73 % (n = 62) des prises en charge étaient conformes aux recommandations du GIHP, en d’autres termes l’AOD avait été repris à partir de la 24e heure. En sortie d’hospitalisation, 27 % des patients n’avaient pas d’ordonnance d’anticoagulation curative. Le pharmacien clinicien joue un rôle important dans le parcours de soin de ces patients à risque en améliorant les pratiques concernant la gestion périopératoire des AOD et en renforçant le lien ville-hôpital. The perioperative management of patients treated with oral direct anticoagulants (ODA) is based on the recommendations of the Perioperative Hemostasis Interest Group (PHIG) of 2015, before and after a scheduled or emergency invasive surgical procedure. The objective of our monocentric retrospective observational study was to evaluate professional practices related to the perioperative management of curative ODA in patients undergoing scheduled or emergency orthopedic surgery. A total of 62 patients were audited over 7 months. ODA management was in line with the GIHP recommendations in preoperative management in 36% of patients undergoing scheduled surgery. Regarding the resumption of ODA post-operatively, 73% (n = 62) of patients were compliant with the GIHP recommendations, i.e. ODA was resumed from the 24th hour. At discharge, 27% of patients did not have a prescription for curative anticoagulation. The clinical pharmacist plays an important role in the care pathway of these at-risk patients by improving practices regarding the perioperative management of ODAs and strengthening the city-hospital link.
La conciliation médicamenteuse d’entrée (CME) est une démarche essentielle lors d’hospitalisations non programmées, notamment après passage aux urgences, pour maîtriser le risque de divergence non intentionnelle (DNI) par méconnaissance des traitements ambulatoires des patients. Notre objectif est d’évaluer la faisabilité et l’intérêt de réaliser cette CME précocement au sein d’une unité d’hospitalisation de courte durée (UHCD), plutôt que dans les services de spécialité d’aval. L’étude rétrospective sur données, réalisée durant 9 mois, a comparé le temps passé, le nombre de DNI interceptées et leur taux d’acceptation entre l’UHCD et des services d’aval (cardiologie, gériatrie et rhumatologie). Les données ont été relevées dans les dossiers patients informatisés (DxCare®, Medasys). Pour les 337 patients ayant bénéficié d’une CME en UHCD, 631 DNI ont été retrouvées chez 200 patients (59 % des patients), soit en moyenne 1,9 DNI/patient. Dans les services d’aval, 978 patients ont été conciliés. Le nombre de DNI retrouvé était moindre (moyenne 1,08/patient ; p < 0,01). Le temps nécessaire à la CME était de 22 minutes à l’UHCD contre 42 minutes dans les services d’aval ( p < 0,01). Nos résultats montrent que cette CME est réalisable dès l’orientation des patients en UHCD et permet une meilleure efficience de cette activité, et ce, malgré la multiplicité des médecins urgentistes pour gérer les divergences. Emergency hospitalizations have been described as a major risk factor of unintended medication discrepancy (UMD). The aim was to compare the number of intercepted UMDs and the time needed to perform an admission medication reconciliation (AMR) in both emergency department and different medical wards. The 9-month retrospective data study compared the time spent, the number of intercepted UMDs and their acceptance rate between the UHCD and downstream services (cardiology, geriatrics and rheumatology). Data were extracted from medical files (DxCare®, Medasys). During this period, 337 patients benefited from AMR in the emergency department and 631 UMDs for 200 patients (59 % of patients) were identified (mean of 1.9/patient). In medical wards, 978 patients benefited from AMR (geriatrics – 51 %) cardiology – 32 %, rheumatology – 17 %). For these patients, less UMDs were identified (1.08/patient, P < 0.01). Time needed for AMR was significantly reduced in emergency department (22 minutes) compared to medical wards (42 minutes; P < 0.01). This study allowed us to demonstrate the feasibility and the interest of developing AMR in emergency departments, leading to a higher efficiency of this activity, and this in spite of the multiplicity of emergency physicians to manage the discrepancies.
Medication adherence is an important factor to determine the efficacy of the prescribed therapy. The main objective of this study was to counsel the patients and to assess its impact on medication-taking behaviour among cancer patients visiting the outpatient department. Cancer patients were enrolled and were divided randomly into intervention and control groups. The intervention group received information leaflets, face to face medication counselling from clinical pharmacists during the study period, whereas the control group received only conventional care. The medication adherence was measured by the Morisky medication adherence questionnaire at baseline and at the final follow-up. Suitable statistical measures were used to analyse the data. In total, 130 subjects (65 patients in the intervention and 65 in control) were enrolled, their medication adherence was assessed. In the intervention group, at baseline, the medication adherence was found to be 6.46 ± 1.36. After counselling patients in the intervention group, their medication adherence improved to 7.71 ± 0.503. The independent t-test result revealed that there was a significant difference (P < 0.05) in medication adherence among the intervention group after counselling. It was also observed that there was no significant difference in the medication adherence behaviour among the usual care group from baseline until follow-up. Pharmacist education and counselling have led to clinical and statistically significant differences in the medication adherence behaviour of cancer patients compared to that of the usual care group. L'adhésion médicamenteuse est un facteur important pour déterminer l'efficacité du traitement prescrit. L'objectif principal de cette étude était de conseiller les patients et d'évaluer son impact sur les comportements de prise de médicaments chez les patients cancéreux en consultation externe. Les patients cancéreux ont été recrutés et ont été répartis au hasard en groupes d'intervention et de contrôle. Le groupe d'intervention a reçu des brochures d'information, des conseils en face à face sur les médicaments de la part des pharmaciens cliniciens pendant la période d'étude, tandis que le groupe témoin n'a reçu que des soins conventionnels. L'adhésion médicamenteuse a été mesurée par le questionnaire d'adhésion médicamenteuse Morisky au départ et au suivi final. Des mesures statistiques appropriées ont été utilisées pour analyser les données. Au total, 130 sujets (65 patients dans l'intervention et 65 dans le contrôle) ont été recrutés, leur adhésion médicamenteuse a été évaluée. Dans le groupe d'intervention, au départ, l'adhérence au médicament était de 6,46 ± 1,36. Après avoir conseillé les patients du groupe d'intervention, leur adhésion médicamenteuse s'est améliorée à 7,71 ± 0,503. Le résultat du test t indépendant a révélé qu'il y avait une différence significative (p < 0,05) dans l'adhésion du médicament parmi le groupe d'intervention après le counseling. Il a également été observé qu'il n'y avait pas de différence significative dans le comportement d'adhésion du médicament parmi le groupe de soins habituels entre le départ et le suivi. La formation et le conseil des pharmaciens ont conduit à des différences cliniques et statistiquement significatives dans le comportement d'adhésion médicamenteuse des patients cancéreux par rapport à celui du groupe de soins habituels.
Un processus pluridisciplinaire de gestion des effets indésirables (EI) a été mis en place depuis 2018. Montrer l’intérêt d’une expertise pharmaceutique clinique dans l’amélioration de la prise en charge médicamenteuse des patients et du recueil des EI. Étude rétrospective des EI de janvier à décembre 2019 et évolution du nombre de cas déclarés au Centre Régional de PharmacoVigilance (CRPV) de 2017 à 2019. Résultats–Discussion En 2019, 114 cas d’EI ont été centralisés par le pharmacien. Quatre-vingt-dix patients (78,9 %) ont pu bénéficier d’une intervention pharmaceutique (IP) et les médicaments concernés étaient en majorité des psycholeptiques (73,2 %). Chez 71,1 % des patients, l’IP entraînait une révision du traitement et une amélioration de l’état clinique et/ou biologique était observée dans 62,5 % des cas. Une augmentation du nombre de cas centralisés par le pharmacien était observée (0 en 2017, 46 en 2018, 114 en 2019) et un nombre de déclarations au CRPV passé de 4 cas en 2017 à 33 en 2019 (+ 825 %). L’intégration d’une expertise pharmaceutique clinique dans une démarche de gestion des EI a permis d’améliorer le recueil des EI et la prise en charge médicamenteuse des patients en développant une collaboration entre les professionnels de santé. To demonstrate the interest of implementing clinical pharmacy services in the improvement of medical care and ADRs collecting. Retrospective study of the ADRs from January to December 2019 and changes in the number of cases reported by the pharmacist and notified to the regional pharmacovigilance centre (RPVC) from 2017 to 2019. In 2019, 114 AR cases were centralized by the pharmacist. Ninety patients (78.9 %) received a pharmaceutical intervention (PI) and the drugs involved were mostly psycholeptics (73.2%). In 71.1% of the patients, the PI led to a revision of the treatment and an improvement of the clinical and/or biological condition was observed in 62.5% of the cases. An increase in the number of cases centralized by the pharmacist was observed (0 in 2017, 46 in 2018, 114 in 2019) and the number of reports to the CRPV increased from 4 cases in 2017 to 33 in 2019 (+825%). The integration of a clinical pharmaceutical expertise in an AR management approach has improved the collection of ARs and the drug management of patients by developing a collaboration between healthcare professionals.
La crise sanitaire liée au COVID-19 a précipité le recours à la télésanté. Pour assurer la continuité des soins durant cette période, les pharmaciens d'officine ont eu l'opportunité de recourir au télésoin et de proposer des téléconsultations avec un médecin à leurs patients. Nous établissons ici un bilan des pratiques de télésanté menées à cette occasion dans une officine de quartier, participant aux activités d'une maison de santé pluridisciplinaire multisite (MSP). Après analyse de situation, les pharmaciens de cette officine ont proposé aux patients concernés soit une téléconsultation, soit un télésoin. Les médecins sollicités ont été ceux de la MSP, ceux du territoire et ceux d'une plate-forme de téléconsultation. Des outils numériques ont permis, d'une part, la communication entre médecins, patients et pharmaciens, et d'autre part, ont permis au pharmacien d'assister le médecin lors de la téléconsultation, tout en garantissant la confidentialité et la protection des données relatives aux patients. Treize téléconsultations et 22 télésoins pharmaceutiques ont été réalisés. Quatre types de situations ont motivé le déclenchement de téléconsultations assistées par le pharmacien et trois celui d'un télésoin pharmaceutique. La mise en œuvre de la télésanté dans cette officine a nécessité des modifications au niveau de l'organisation et du management au sein de l'officine, la formation de l'équipe officinale ainsi qu'une communication plus étroite avec les médecins de la MSP. La télésanté porte des ouvertures majeures pour l'évolution du métier de pharmacien et l'amélioration de la prise en charge des patients, au-delà des situations de crise. The COVID-19 health crisis precipitated the use of telehealth. To ensure continuity of care, during this period, pharmacists had the opportunity to use telecare and to offer teleconsultations with a physician to their patients. We report here on telehealth practices in a local pharmacy participating in the activities of a multidisciplinary health center (MSP). After analyzing a situation, the pharmacists of this dispensary offered teleconsultation or telecare to the patients. The doctors solicited were those from the MSP, those from the territory and those from a teleconsultation platform. Digital tools have enabled communication between doctors, patients and pharmacists on the one hand and enabled the pharmacist to assist the doctor during the teleconsultation, while ensuring the confidentiality and protection of patient data. Thirteen teleconsultations and 22 telepharmacy consultations were performed. Four types of situations motivated the triggering of pharmacist-assisted teleconsultations and three of pharmaceutical telecare. The implementation of telehealth in this pharmacy required changes in the organization and management of the pharmacy, training of the pharmacy team, and closer communication with the MSP physicians. Telehealth offers major opportunities for the evolution of the pharmacist's profession and the improvement of patient care, beyond crisis situations.
