Objective We construct an engineering bacterium that can express human interleukin 21 proteins and do the purification of the recombinant protein.Methods Human interleukin 21 gene fragment was required from human tonsil cDNA by means of PCR.The code gene for the mature N terminus of IL-21 was inserted into prokaryotic expressing vector pGEX-4T-2 and transformed into E.coli DH5α.The expressed product was purified by affinity chromatography using GST fusion protein purification bead.Results PCR and digestion by BamH I and Sal I of recombinant plasmid show there are corresponding electrophoresis products same to expectation.After induced by IPTG,E.coli DH5 α also express the right molecular weight protein.The interest protein is about 10 percent in the whole bacteria protein.Conclusion We successfully construct the vector pGEX-4T-2/IL-21 and the engineering bacteria E.coli DH5α which can express human interleukin 21 mature N terminus fusion protein.By purification,we show the recombinant IL-21 may be a new drug in the clinic to regulate the immunological funcition.
Objective To investigate the role of thymosin-α_1(Tα_1) administrated Th1/Th2 balance and suppression of allergic sensitization in asthmatics.Methods Twenty-five asthmatic patients in remission episode were selected(the asthma group).Fifteen healthy volunteers(the control group) was selected.The asthma group was hyodermiced by Tα_1 every other day for 1.5 months.Serum were obtained from the asthma group before and after treatment and from the control group.IL-4,IL-5,and IFN-γ levels in above-mentioned serum samples were measured by ELISA.The results were analyed statistically.Results IL-4 levels of asthma before treatment [(52.15±8.96)pg/mL] were significantly increased compared with those of control group [(13.33±11.16)pg/mL](P<0.05).IFN-γ level in asthmatics after treatment [(1185.52±67.10)pg/mL]was more significantly increased than that in control group [(138.25±6.25)pg/mL] and that of asthmatics before treatment[(144.58±1.06)pg/mL](P=0.001).IL-4 Levels in asthmatics after treatment [(14.15±4.86)pg/mL] were more significantly decreased than those in asthmatics before treatment [(52.15±8.96)pg/mL](P=0.001).Conclusion In this study,Tα_1 treatment established a Th_1-type immune response that thindered allergic sensitization and the development of increased airway reactivity.
肺炎衣原体是衣原体属中的一个新种,是一类严格在真核细胞内寄生,有独特发育周期,并能通过细菌滤器的原核细胞型微生物.自1965年分离出第一株肺炎衣原体以来,对其研究已日趋深入,有文献报道,肺炎衣原体慢性感染与冠状动脉粥样硬化性心脏病(冠心病)的关系密切[1].我们采用间接免疫荧光技术,对68例冠心病患者进行血清肺炎衣原体特异性抗体检测,现将结果报告如下.
Objective To investigate the state of lower respiratory tract infection in patients with severe brain injury after tracheotomy and study its prevention and treatment.Methods To retrospectively study the patients with severe brain injury after tracheotomy at our unit in our hospital from Jun 2000 to Aug 2005.Results The infection rate of lower respiratory in 198 patients was 100%.Bacteria culture of airway secretion and drug sensitive test were used to analyze etiology in 154 patients(77.8%).Among 154 patients,we isolated 194 strains of pathogens and 40 patients had double pathogens(26.0%).65.0% of 194 strains of pathogens were Gram-negative bacteria,27.8% Gram-positive,and 7.2% fungi.These pathogens resisted some antibiotics used usually at diffferent degrees.The effective rate of the control of infection of lower respiratory in 198 patients with severe brain injury after tracheotomy was 80.8%.Conclusion The infection rate of lower respiratory in the patients with severe brain injury after tracheotomy is very high,can be caused by many dangerous factors,is not controlled easily,so we must take effective measures to carefully control and prevent the infection.
Objective To study the economical chemotherapy for acute lymphoblastic leukemia in chidhood.Methods ①Induction remission chemotherapy with CODP for 4 weeks.②The consolidation treatment with VP16+Ara-c.③Prophylactic treatment for extramedullary leukemia.④Early intensification therapy with COADex.⑤Sustain therapy with 6-MP+MTX or VDex in turn.Per three months gave intensive therapy with COADex or VP16(or VM26)+Ara-c,added with HDMTX+CF.Results All the 18 cases achieved complete remission,the average hospital days of initial treatment is 81,the average cost is 23 thousand yuan,in sustain stage is about 6000 yuan per year.In the all 18 cases,8 cases had treatment discontinued,one patient died in varicella,and one patient was plaralysed after intrathecal injection,then gave up.4 cases are near to complete all the course of treatment.The rest 4 cases have been in sustain stage for 1~1.5 years.Now all the cases are no recurrence and no extramedullary leukemia.Conclusion The effect of this protocal was confirmed,costs were lower,and side effects were fewer.This protocol are deserved to be spread.
目的 评价针灸与胃动力药在钡餐透视中增加胃动力的应用价值.方法 136例胃肠钡餐检查所示胃排出迟缓及障碍者,随机分为针刺组76例和口服胃动力药组(对照组)60例,针刺组采用针刺足三里、上巨虚、耳穴,部分兼刺中腕、内关、太冲.对照组给予口服吗丁啉.结果 76例胃排空迟缓及障碍者,经针灸后72例钡剂可迅速排出,占94.74%;60例口服吗丁啉者,50例钡剂短时内排出,占83.3%.结论 针刺足三里或兼刺耳穴等其他相关穴位,对钡餐透视中胃排出迟缓者有较为明显的效果,该方法在增加胃动力,促进胃排空方面,优于胃动力药,且简便易行,无副反应,有推广价值.
残根残冠是临床上十分常见的牙体缺损疾病,以往大多数患牙被拔除.但随着牙科技术及材料的发展,利用桩核冠进行患牙的保存修复已成为口腔修复治疗的一个重要组成部分.桩核冠是由桩核和外冠两部分组成,即残根残冠经过完善的根管治疗后,先完成桩核,后于核上制作全冠[1].由于传统的桩核冠蜡型的制作耗时长,复诊次数多,增加了患者的负担,为此,我们在临床上探索出一次性制取桩核蜡型的简便方法.经多年临床实践,效果明显,现报道如下.
Objective To explore occuring time,pathogenesis,prevention,and management of late postpartum hemorrhage after cesarean section.Methods Clinical data of 43 cases of late postpartum hemorrhage from 1999 to 2004 were analyzed retrospectively.Results The causes were rupture of cesarean incision,subinvolution of uterus,retained placenta or decidua,endometritis,serious anemia,and postpartum choriocarcinoma in order.Most cases(67.44%) had hemorrhage over three weeks after delivery,some even over six weeks(16.28%).Conclusion Late postpartum hemorrhage after cesarean section may occur beyond puerperium and the main cause is rupture of incision.Prevention and treatment for the causes of late postpartum hemorrhage after cesarean section are very important.
Objective To evaluate the results with intercondylar fracture treated with AO internal fixation.Methods Thirty-eight patients were treated from June 1999 to September 2004,and followed up for an average of 24 months.The curative effects were evaluated with modified cassebaum rating system.Results 14 cases were excellent,17 cases were good,4 cases were fair and 3 cases were poor.The excellent and good rate was 81.6%.Conclusion The degree of comminution and displacement of fracture hinders the final result.Early operation,with osteufumy of the clecranon,anatomical reduction,rigid fixation and early exercise are the important factors that can improve the final result.
Objective Through analyzing clinical prognosis of patients with primary liver cancer(PLC) and discussing prognostic factors affecting long-term outcome,so as to provide theoretic evidence in the comprehensive treatment and prognostic judgment of PLC.Methods A retrospective analysis of clinical and follow-up data of 260 patients with PLC in our hospital from January 2002 to December 2005.Sixteen possible factors with long-term survival rate were analyzed in Cox regression(proportional hazard model).Results Univariate analysis showed that treatment pattern,tumor size,tumor nodule number,liver function,liver cirrhosis,distal metastasis and tumor embolus in vein were significant prognostic factors.The multivariate analysis by Cox regression,the independent prognostic factors were treatment pattern,tumor size,tumor nodule number,liver function and tumor embolus in vein.Conclusion The prognosis of PLC was focused on the following aspects: treatment pattern,tumor size,tumor nodule number,liver function.Knowledge of the factors affecting prognosis is helpful for the selection and management of patients.
Objective To investigate the effects of astragalus on serum and liver tissue TGF-β1 level in immature rats with obstructive jaundice(OJ).Methods Forty immature male Wistar rats were randomly divided into normal control group,sham operation group,OJ group,OJ+astragalus group.In OJ+astragalus group,astragalus(250 mg/100 g body weight) were given for 7 days.The content of TGF-β1 in serum and liver tissue in rats was detected by ELISA.Results The level of serum TGF-β1 was higher in OJ group and OJ+astragalus group than that in normal control group and sham group.The level of TGF-β1 of liver tissue in OJ+astragalus was lower in than that in OJ group.The morphologic changes of the liver ameliorates in OJ+astragalus group.Conclusion The astragalus helps to improve the liver morphology,decreases the process of liver fibrosis in immature rats with obstructive jaundice.
OBJECTIVE To investigate patients with traumatic brain injury after tracheotomy lower respiratory infection and explore its countermeasures.METHODS The clinicai data of patients treated with tracheotomy after traumatic brain injury,in our department from January,2004 to August 2008,were analyzed retrospectively.RESULTS All the patients were the lower respiratory tract infections,of which 154 cases were routined bacterial culture(77.8%),cultivate pathogens 194,40 cases were double infections(26.0%);126 gram-negative bacilli accounted for(65.0%),gram-positive cocci accounted for 54 (27.8%),fungi accounted for 14(7.2%).The commonly used antibiotics in clinical strains have different levels of drug resistance.160 cases of lower respiratory tract infection were infection control,and the total effective rate was 80.8%.CONCLUSION The patients with tracheotomy after traumatic brain injury have a high incidence of lower respiratory tract infection,which are difficult to control.So the doctor must take effective measures to control hospital infection.
Objective To evaluate the results of Barton fracture of the distal radius with T plate internal fixation.Methods Twenty-two patients with Bartons fracture of the distal radius were treated with open reduction and T plate internal fixation.Results The follow-up ranged for 5 to 20 months and for 3-4 months after operation.The function evaluations were based on the modified Gartland and Wertey(GW) scoring systenm,with excellent in 15 cases and good in 4 cases.The overall excellents-good rate was 86.36%.Conclusion Operative treatment with tianium T plate for Bartons fracture can firmly hold the reduced fracture segments and hence facilitate early postoperative rehabilitation and improve the wrist function.
Objective To find the effect of misoprostol and duyiwei induced abortion.Methods 680 cases were classified into two groups,the treating group who were treated with 0.4 mg Misoprostol and 1.8 g Duyiwei PO 30 minutes before Induced Abortion,Control group without misoprostol and Duyiwei.Results There are significant difference between the control group and treating group in the relaxation effect of uterus endocervical(P0.05),pain degree,and the amount of lost blood(P0.05 P0.01).Conclusion The misoprostol and Duyiwei could reduce the pain,blood lose,and help uterus contraction,was seemed as an effective and useful methods in induced Abortion.
Objective To investigate the expression of vascular endothelial growth factor-C(VEGF-C) and its receptor in endometrial adenocarcinoma tissue in pursuit of their role in tumor metastasis and other clinical significance.Methods 48 fresh endometrial adenocarcinoma tissue were examined by RT-PCR to detect the VEGF-C/KDR /Flt-4mRNA expression.Results There was a significant difference of VEGF-C/KDR /Flt-4mRNA expression between the tumor and the normal controls.And there was no significant relationship between VEGF-C mRNA,KDR mRNA or flt-4mRNA expression with the pathological types and clinical stage of invasive carcinoma of cervix.But the expression was significant association with the grade of tumor pathology,lymph node metastasis,tumor size and the invasion of deep muscular layer(P0.05).Conclusion The results suggest that the VEGF-C may play an important role in the lymph node metastasis and it could be a useful biological marker to foresee the lymph node metastasis and prognosis for cervical cancer.
Objective To study the expression of TGF-β1 in cholangiocarcinoma tissues and its biological behavior.Methods TGF-β1 was detected by immunohistochemical methods in 47 paraffin-embedded specimens of cholangiocarcinoma and the control tissues near to the tumor.Results Expression of TGF-β1 was significantly higher in cholangiocarcinoma than that of control tissues near to the tumor(P0.001).There was significantly relation between TGF-β1 expression and clinical stages,lymph node and liver metastasis(P0.01,0.05,0.05),and no difference in histological grade.Conclusion Smad4 and TGF-β1,TGF-βRⅡ may play an important role in the occurrence and development of cholangiocarcinoma.Loss of Smad4 and TGF-βRⅡgene may be one of the significant molecular biological mechanism.
Objective To analyze the clinic separable characteristics and compare the activities of fluconazole with other 5 antifungal agents in 383 cases of Candida viginitis.Methods Microbial population was determined by the method guided to the standard M27-A recommended by the National Committee for Clinical Laboratory Standards.Results Ranked from the highest to to the lowest as C.albicans(96.09%),C.glabrata(2.61%),C.troptical(1.04%),C.krusei(0.26%).Candistatin and amphotericin B were most effective against all the tested clinical Candida isolates(>90%).The rates of drug fast were itraconazole(35.8%),fluconazole(17.5%).Conclusion C.albicans,C.glabrata,C.troptical,and C.krusei were identified in all the 383 cases of Candida viginitis.Candistatin and amphotericin B was the most effective antifungal agents against all the tested clinical Candida isolates.The drug resistance of itraconazole and fluconazole should be reconstructed by clinicians.
Objective To discuss the value of mesh-plug tension-free hernia repair in groin hernia.Methods The mesh & plug tenslon-free hernia repair was administered in 58 patients of groin hernia with the Bard Mesh & Perfix plug offered by Bard Company in U.S.A.The operative procedure,surgical indication,postoperative pain,abliligy recovery,complications and recurrence rate were studied.Results The average operation time was 32 minutes.The patients could move 3～14 hours after operation.Postoperation pain lasted about 2～3 days.Five had retention of urine.Three had feeling of foreign matter in the groin region.The follow-up period ranged from 6 months to 6 years and no recurrence was observed.Conclusion The mesh & plug tension-free hernia repair is easy to be daministered,with minor lesion,quicker recovery and lower recurrence.And the operation indication can be widened.It is a most advanced method in treating groin hernia.
Objective To observe the therapeutic effect of Interferon-α(IFN-α) on children suffering from diarrhea.Methods 60 cases were randomly divided into control group(30 cases)and treated group(30 cases).Based on the treatment with routine methods,ribavirin intra-venous-drip was used in control group,and IFN-α 50 000 u/kg intra-muscular injection was added in the treated group once a day for 3-5 days.Results The cure rate of the treated group was 83.3%,which was higher than 56.7% of the control group(χ~2=5.0794,P0.05).The time of stopping diarrhea of the treated group was(2.20±0.80)d,which was shorter than(4.20±1.65)d of the control group(t'=-5.9739,P0.05).As well as the average hospitalization days of the treated group was(5.20±1.10) d,which was shorter than(8.60±1.40)d of the control group(t=10.4595,P0.05).Conclusions IFN-α has a better therapeutic effect in treating children with rotavirus diarrhea than ribavirin and shortens the course of disease,so it is worthy of clinical application.
Objective To provide a stable,convenient and easily observable model of heterotopic heart transplantation in rat,for further studies on transplantation immunology,pathology and organ protection.Methods The procedures were improved based on the models designed by Ono and Heron.Results Each model of heart transplantation has its advantage and weakness.Model should be selected according to different experimental designs and conditions.Conclusion Two kinds of improved models are easy,whole process can be completed by a single man without microscope assistance,convenient and dependable.
