
Objective: To investigate the effect and mechanism of dexamethasone (DX) on axonal injury after traumatic brain injury (TBI) combined with seawater drowning (SWD) in rats. Methods: To gain an in-depth understanding of TBI + SWD in rats, we established the compound injury model of rats by the Marmarou method and intratracheal pumping of seawater to simulate the pathological conditions. Rats in the DX group received intraperitoneal injections of DX (1 mg/kg) immediately after injury, and rats in the sham group and TBI + SWD group received intraperitoneal injections of the same amount of normal saline. Results: Hematoxylin-eosin (HE) showed that DX improved matrix looseness, cell swelling, and nuclear condensation 168 hours after injury. Immunohistochemistry (IHC) staining showed that the protein expression of AQP4 was decreased in the DX group compared with the TBI + SWD group from 12 hours to 168 hours after injury. DX decreased the modified neurological severity score (mNSS) significantly at 24 hours and 168 hours after injury (P < 0.05). At 72 h and 168 h after injury, DX significantly lowered the expressions of IL -8 and TNF-alpha (P < 0.05). Conclusion: DX may play a neuroprotective role by reducing cerebral edema and inflammatory response after TBI + SWD injury in rats.
急性一氧化碳中毒(acute carbon monoxide poisoning,ACOP)易导致中枢神经系统、心血管系统损害 [1]。骨筋膜室综合征(osteofascial compartment syndrome,OCS)是ACOP的严重并发症之一,发病率不高,但病情严重 [2]。OCS是由骨、骨间膜、肌间隔和深筋膜形成的密闭骨筋膜室内压力增高超过组织灌注压,导致肌肉和神经因急性缺血所产生的一系列症候群 [3]。笔者对1例ACOP继发OCS患者的临床资料进行分析,现报道如下。
海军舰艇由于作业环境特殊,空间狭小密闭,人员高度密集,居住环境复杂,病毒一旦在舰艇爆发,会对军队战斗力造成巨大影响,探索海军舰艇传染病的发生、诱因及防护,对于国家安全和军事战略具有重要意义。笔者通过对美国海军舰艇新冠病毒爆发情况进行分析,探讨其产生的诱发因素,并提出防护措施,旨在为我军舰艇相关突发传染病的处置提供建议和借鉴。
Objective:To investigate the navigation fatigue among submariners and analyze its influencing factors.Methods:A questionnaire survey was conducted among the submariners on one day before after a maritime mission and the 15th day of the voyage,to evaluate their navigation fatigue and analyze the related influencing factors. The general information questionnaire was designed independently,including age,years of service onboard,stress levels,physical exercise habits,smoking during port stays,dietary habits,total daily working hours,maximum consecutive working hours per day,and the number of adverse factors in the environment. Fatigue scale-14(FS-14)was used to evaluate the navigation fatigue of the submariners. Pittsburgh sleep quality index(PSQI)was used to evaluate the sleep quality of the submariners during navigation.Results:One day before the voyage,a total of 68 submariners participated in the questionnaire survey,and 60 valid questionnaires were collected,with an effective response rate of 88.2%. On the 15th day of the voyage,the 60 individuals who had previously provided valid responses were surveyed by the same questionnaire. One day before the voyage,the FS-14 score of the submariners was(5.57 ± 3.01),which was not statistically significantly different from that of the general population in China(5.50 ± 3.09)( P>0.05). The score of FS-14 on the 15th day of voyage was(6.85 ± 3.83),which was higher than both the scores of the general population in China and one day before the voyage(all P<0.01). The incidence of fatigue among submariners was 36.67%(22/60)one day before the voyage and 58.33%(35/60)on the 15th day,and the difference was statistically significant( P<0.01). Univariate analysis showed that nine factors,i.e.,physical exercise habits,smoking during port stay,fermented diet intake,carbohydrate intake,stress levels,the number of adverse factors in the environment,total daily working hours,maximum consecutive working hours per day,and PSQI total score were associated with FS-14 total score(all P<0.05). Multiple logistic regression analysis showed that more physical exercise,higher intake of fermented diet,and shorter total daily working hours were protective factors against navigation fatigue(all P<0.05),while excessive pressure,longer consecutive working hours per day,a higher number of adverse factors in the environment,and poor sleep quality were the risk factors for navigation fatigue(all P<0.05). Conclusion:The incidence of navigation fatigue is high in the submariners during the long voyage. Those who smoke more during port stays,experience higher levels of stress,have longer consecutive working hours per day,are exposed to more adverse factors in the environment,and have poorer sleep quality tend to experience more severe fatigue,while those engage in regular physical exercise,have a higher intake of fermented food and carbohydrates,and have shorter total daily working hours tend to experience milder fatigue.
Objective:To explore the current status of social support and quality of life among female officers and soldiers stationed on some islands and reefs of Nansha Qundao.Methods:A cross-sectional questionnaire survey was conducted to collect the basic data and demographic characteristics of female officers and soldiers stationed on some islands and reefs of Nansha Qundao,to assess their quality of life and current status of social support. Pearson correlation analysis was employed to examine the correlation between the quality of life and social support,and logistic regression analysis was used to identify the factors affecting the quality of life of female officers and soldiers on some islands and reefs of Nansha Qundao.Results:The total score of social support for female officers and soldiers stationed on some islands and reefs of Nansha Qundao was(19.1 ± 4.8),and the total score of quality of life was(55.6 ± 10.4). One-way analysis of variance indicated statistically significant differences in subjective support scores among individuals of different ages,objective support scores among individuals of different educational backgrounds,and subjective support scores among individuals of different marital status( P<0.05 or P<0.01). There were also statistically significant differences in the scores related to the duration of being stationed on the island/reef,health condition,and sleep time( P<0.05 or P<0.01). Female officers and soldiers of different ages demonstrated statistically significant differences in the scores of the physical,psychological,and social relationship domains( P<0.05 or P<0.01). There were significant differences in the scores of quality of life in the psychological domain among female officers and soldiers of different marital status and educational backgrounds( P<0.05). There were statistically significant differences in the scores across all four domains of quality of life among female officers and soldiers with different durations of being stationed on the island/reef,health condition,and sleep quality( P<0.05 or P<0.01). Pearson analysis showed significant positive correlations between the scores of the four domains of quality of life and the scores of three domains of social support for female officers and soldiers stationed on the island/reef( P<0.05). Multivariate logistic regression analysis showed that age,educational background,marital status,duration of being stationed on the island/reef,health condition,and sleep quality were the main factors affecting the quality of life of the female officers and soldiers stationed on some islands and reefs of Nansha Qundao( P<0.05 or P<0.01). Conclusion:The quality of life of female officers and soldiers stationed on some islands and reefs of Nansha Qundao is significantly positively correlated with their social support. During routine combat readiness training,troop leaders should strengthen social support in order to improve the quality of life of female officers and soldiers in special environments.
缺氧是大多数实体肿瘤的基本特征,改善肿瘤乏氧可显著提高抗肿瘤疗效。高压氧治疗(HBOT)可以提高血氧张力,改善组织供氧,广泛用于各种缺血缺氧性疾病的临床治疗。近年来,越来越多的研究表明HBOT能改变肿瘤的乏氧环境,增敏放化疗,提高患者生活质量,有望成为肿瘤治疗的有效辅助手段。HBOT操作简单、不良反应少,与现有的肿瘤治疗方案联合,可能具有较高的临床转化潜力。通过综述近年来HBOT辅助化疗、放疗、放射性损伤及抗肿瘤新疗法在常见恶性肿瘤治疗中的应用,详细阐述了缺氧对肿瘤微环境、肿瘤干细胞及肿瘤耐药性的影响及相关机制,以及改善组织缺氧对肿瘤治疗的可能作用。
Objective:To explore the efficacy of early hyperbaric oxygen therapy combined with Schuell language training on patients with aphasia after stroke,and to analyze its influence on the activity function of language center.Methods:A total of 176 patients with aphasia after stroke admitted to the Department of Rehabilitation of Xuzhou Central Hospital from March 2021 to March 2022 were selected as research objects,and were divided into control group and observation group by random number table method,with 88 cases in each group. The control group received Schuell language training,while the observation group was additionally treated with hyperbaric oxygen therapy. The efficacy of the two groups were analyzed by observing the degree of neurological impairment,cognitive function,and language center activity function in both groups before and after treatment,with National Institutes of Health Stroke Scale(NIHSS),Montreal Cognitive Scale(MoCA),and Boston Diagnostic Aphasia Scale(BDAE).Results:After all courses of treatment,the total effective treatment rate of the observation group was 88.64%(78/88),which was significantly higher than that of the control group(73.86%,65/88),and the difference was statistically significant( P<0.05). Before treatment,there was no significant difference in NIHSS,MoCA,and language center activity function score between the two groups( P>0.05). Compared with those before treatment,the NIHSS scores of the two groups were decreased,and the scores of MoCA and language center activity function were increased;furthermore,after treatment,the NIHSS score of the observation group was lower than that of the control group,and the scores of MoCA and various language center activity function were higher than those of the control group,with statistically significant differences( P<0.001). Conclusion:Early hyperbaric oxygen therapy combined with Schuell language training can improve the overall efficacy on patients with aphasia after stroke,and reduce the degree of neurological impairment,thus improving the cognitive function and language center activity function.
Objective:To carry out clinical analysis of hyperbaric oxygen therapy for carbon monoxide(CO)poisoning in 16 cities of Shandong province and explore the effective prevention and control measures.Methods:The clinical analysis was conducted on the patients with CO poisoning treated in Hyperbaric Oxygen Departments of municipal hospitals in Shandong province by collecting their medical records,including gender,age distribution,degree of poisoning,cause of poisoning,site of poisoning,treatment methods,start time of treatment,and incidence of delayed encephalopathy,etc.Results:During the heating season from November 2021 to March 2022,a total of 15,552 patients with CO poisoning were treated in hyperbaric oxygen departments in 16 cities of Shandong province,including 6,992 males(44.96%)and 8,560 females(55.04%);57.74% patients were aged above 50 years old;a total of 9,023(58.02%)cases were mild,while 1,524(9.8%)cases were severe;81.13% poisoning cases were caused by coal heating;89.82% poisoning cases happened at home;a total of 15,410 cases were treated with hyperbaric oxygen therapy,of which,88.20% cases received hyperbaric oxygen therapy within 24 hours after CO poisoning. A total of 358(2.30%)delayed encephalopathy cases were reported.Conclusion:There is a relatively high number of patients with CO poisoning in Shandong province. It is necessary to take effective measures to step up prevention and control effort. Prompt and timely hyperbaric oxygen therapy after CO poisoning can prevent the occurrence of delayed encephalopathy.
海上发生落水事件时,快速组织有效的搜救和航空救治转运对于落水的舰船员和飞行人员具有重要的意义。随着我国远海编队落水飞行员搜救体系的建设,直升机航空救治和转运的重要性逐渐凸显,但目前其治疗要点和对策研究较少。本研究讨论了在搜救落水人员后进行航空救治以及转运的训练中,伤情拟定、人员和器械配备以及技能培训中存在的问题和训练心得,为完善直升机海上救助及卫勤保障提供借鉴。
1.临床资料:患者女,49岁,因“体检发现白细胞减少3 d”就诊于哈尔滨医科大学附属第二医院,2021年11月1日入院后查血常规:WBC 2.8×10 9/L,HGB 103 g/L,PLT 159×10 9/L。外周血分类:分叶4%、淋巴细胞96%;骨穿:原始细胞63%。支持急性粒细胞性白血病未分化型(AML-M1)诊断。骨髓活检:HE及PAS染色示骨髓增生较低下(30%),幼稚阶段细胞增多(约70%),胞质中等大,胞浆中等量,胞核圆形或不规则,染色质细致,偏成熟阶段粒红系细胞散在分布,巨核细胞偏少见;少量淋巴细胞散在分布。网状纤维染色(MF-0级)。免疫分型:表达CD117、CD38、CD33、CD9、MPO、部分表达CD56、弱表达CD123、CD13,可见异常髓系原始细胞65.61,符合AML诊断。白血病43种融合基因:阴性。染色体:46,XX[10]。2代测序:Ⅰ类突变:TET2、NPM1阳性。最后诊断为“急性髓系白血病伴NPM1”,预后良好组。予以去甲氧柔红霉素20 mg,1 d;10 mg,2~3 d;阿糖胞苷200 mg,1~7 d,诱导达CR。免疫残留及TET2、NPM1阴性后,予以2个疗程大剂量阿糖胞苷巩固及MA方案动员,于2022年3月12日给与地西他滨+改良BUCY方案治疗。2022年3月23日回输CD34计数为2.1×10 6/kg,13 d粒系植活,21 d血小板植活。2022年5月15日患者出现尿频、尿急、尿痛,出现肉眼血尿伴凝块,查尿常规示:隐血+++,尿蛋白++,红细胞计数176个/L,BKV-DNA 1.96E+007↑,JCV-DNA 1.35E+007↑,EBV-DNA未检出,HCMV-DNA未检出;予以水化、碱化、利尿,琥珀酸索利那新片、吗啡缓释片对症、更昔洛韦、膦甲酸钠抗病毒治疗2周未见好转,甲强龙1 mg/kg治疗2周症状反复,疼痛加重,于2022年6月16日予行高压氧治疗,治疗压力0.16 MPa(1.6 ATA),升压10 min,稳压70 min,佩戴面罩吸氧,每20 min休息5 min吸舱内空气,减压10 min出舱。1次/d,治疗过程顺利,患者无不适。治疗当日尿痛缓解,连续治疗10 d,膀胱炎症状完全恢复,尿常规恢复正常。随访至今,未复发。
Objective:To investigate the psychological resilience and analyze the influencing factors in COVID-19 patients in a Fangcang shelter hospital in Shanghai.Methods:The convenience sampling method was used to select a total of 609 COVID-19 patients in a Fangcang shelter hospital from April to May,2022 as research subjects. General information questionnaire,brief version of Psychological Resilience Scale,perceived Social Support Scale,General Self-efficacy Scale,and trait coping style questionnaire were used in the investigation. Pearson correlation analysis,multiple stepwise regression analysis,and path analysis were used to analyze the influencing factors of psychological resilience.Results:The psychological resilience score of 609 COVID-19 patients in the Fangcang shelter hospital was 23.93±12.88. The results of One-Way ANOVA showed that the resilience score of the patients with different education background and length of hospital stay had statistical significant differences( P<0.05). Pearson correlation analysis showed that psychological resilience was positively correlated with social support,self-efficacy,and positive coping style( r=0.615, P<0.01; r=0.479, P<0.01; r=0.573, P<0.01). The results of multiple stepwise regression analysis showed that educational background,length of hospital stay,social support,positive coping,negative coping,and friend support were the influencing factors of psychological resilience in COVID-19 patients,which explained 45.8% of the total variation. The path analysis showed that educational background,length of hospital stay,social support,positive coping,and negative coping could directly affect the psychological resilience of patients( P<0.05),while educational background,length of hospital stay,social support,friend support,and negative coping could indirectly affect the psychological resilience of patients( P<0.05). Conclusion:The psychological resilience of COVID-19 patients in Fangcang shelter hospitals is low. Medical staff should pay more attention to patients with lower educational level and longer hospital stay,and give them positive,effective,comprehensive,and individualized psychological intervention,so as to improve their psychological resilience and speed up the recovery.
钙离子结合蛋白S100A8和S100A9形成的异质二聚体复合物,又称钙保护素,是一种主要由中性粒细胞表达的多功能蛋白,介导广泛的生理和病理反应。笔者对S100A8/A9的炎症调控作用及其在呼吸道感染性疾病、哮喘、慢性阻塞性肺病、呼吸机引起肺损伤中的作用进行阐述,探讨其作为诊断、预后判断指标及干预靶点的可能性。
目的:探讨自制轻便型输液应急箱在援潜救生卫勤保障中的使用效果。方法:将自制轻便型输液应急箱应用到援潜救生卫勤保障进舱训练治疗、紧急静脉输液、前出救援保障等实践中。结果:经过1年4个月实践,轻便型输液应急箱箱内物品在位率100%、完好率100%,护士携行其顺利完成多种情况下的静脉输液操作。结论:通过设置轻便型输液应急箱,有利于缩短援潜救生卫勤保障中的静脉输液操作时间,方便日常维护管理,对保障一线伤员静脉通路建立有着积极作用。
潜水员进行潜水、高气压训练或作业时,巨大的静水压或混合气体压力可能引起潜水员的生理状况发生改变,因此需要对潜水员的生理参数进行实时监测。血压是反映人体血流动力学状态的重要指标,血压异常会引发心血管疾病及其并发症,甚至导致死亡。利用高气压环境下血压监测技术可监控潜水员作业过程中的动脉血压水平,及时分析和处理异常情况,为潜水作业安全提供支撑。笔者主要从血压测量入手,通过分析各种血压检测方法的机制、优缺点来评估其在高气压环境下的适用性,并对高气压环境下血压测量技术的发展方向进行分析和展望。
Objective:To study the effects of high intensity flight training on the body metabolism of pilots.Methods:A total of 25 pilots who underwent high intensity flight training were selected into observation group,and 14 pilots who did not undergo any flight mission within two weeks were selected into control group. Liquid chromatography mass spectrometry(LC-MS)metabolomics technology was used to analyze their urine and saliva. Principal component analysis(PCA)and partial least squares discriminant analysis(PLS-DA)were used to screen the differential metabolites between groups. Kyoto Encyclopedia of Genes and Genomes(KEGG)database was used to annotate the differential metabolites and carry out enrichment analysis.Results:Compared with the control group,nine metabolites in the observation group were differentially expressed,including agmatine,hypoxanthine,adenine,xanthine,inosine,carnosine,dopamine,5-hydroxyindole-3-acetic acid,and indole-3-acetic acid. Six main metabolic pathways were involved,including purine metabolism,tryptophan metabolism,histidine metabolism,β-alanine metabolism,tyrosine metabolism,arginine,and proline metabolism.Conclusion:High intensity flight training may cause body metabolic disorder in pilots through multiple metabolic pathways,which may be induced by hypoxia and stress,and may be related to flight fatigue.
Objective:To study the clinical efficacy of hyperbaric oxygen(HBO)combined with internal fixation in patients with ankle fracture and its effect on ankle functional recovery.Methods:A total of 90 patients with ankle fracture admitted to the Department of Joint Surgery of Yuncheng Central Hospital from August 2020 to September 2022 were selected as the research objects. According to the random number table method,they were divided into control group and study group,with 45 cases in each group. The patients in the control group were treated with minimally invasive percutaneous cannulated screw internal fixation,and the patients in the study group were treated with HBO therapy on the basis of the treatment of the control group. The fracture healing time,swelling and pain disappearance time,and full weight-bearing time were compared between the two groups. The levels of serum tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were measured by enzyme-linked immunosorbent assay before and after treatment. The relative peak strength,torque acceleration energy,and endurance of ankle muscle contraction at angular velocity of 120° after treatment were detected in the two groups,and the isokinetic muscle strength was evaluated. Before and three months after treatment,the Olerud-Molander ankle subjective score(OMAS),the American Orthopaedic Foot & Ankle Society(AOFAS)score,and the Kofoed score were used to evaluate the recovery of ankle function in the two groups. The postoperative complications such as nail tract infection,incision infection,and deep vein thrombosis were observed in the two groups.Results:The fracture healing time,swelling and pain disappearance time,and full weight-bearing time in the study group were significantly shorter than those in the control group( P<0.01). Compared with those before treatment,the levels of serum TNF-α and IL-6 in the two groups significantly decreased after treatment,and the above indexes in the study group were significantly lower than those in the control group( P<0.01). After treatment,the relative peak strength,torque acceleration energy and endurance of valgus,varus,toe flexion,and dorsiflexion in the study group were significantly higher than those in the control group( P<0.01). Compared with before treatment,the Olerud-Molander score,AOFAS score,and Kofoed score of the two groups were significantly improved after treatment,and the above indexes in the study group were significantly higher than those in the control group( P<0.01). The total incidence of postoperative complications in the study group(6.67 %)was significantly lower than that in the control group(22.22%),and the difference was statistically significant( P<0.05). Conclusion:HBO combined with internal fixation can significantly reduce the inflammatory reaction of patients with ankle fracture,improve the clinical efficacy,and promote the functional recovery of ankle joint.
笔者从俄罗斯海军军医野战外科学培训情况出发,介绍了俄罗斯海军军医学历教育阶段和在职培训阶段的培训机构,从学习科目、教学资料和科研能力培养三个方面介绍了具体的培训内容,详细分析了临床培训、模拟演练、舰队实习、职业进修和训练等主要的培训形式。通过分析俄罗斯海军军医野战外科学培训特点,得出相关启示以供我海军参考借鉴。
Objective:To investigate the mechanism of hyperbaric oxygen inhibiting further craniocerebral nerve tissue injury through blocking the activation of NOD-like receptor pyrin domain-related protein 3(NLRP3)inflammasome.Methods:A total of 60 healthy adult male SD rats were selected and divided into sham operation group,traumatic brain injury(TBI)group,and hyperbaric oxygen(HBO)group according to the random number table method,with 20 rats in each group. Surgical induction of restricted cortical injury was performed on the rats of the TBI group and the HBO group. On 6 h after modeling,the HBO group was treated with HBO therapy for seven days. On the 1st,3rd,and 7th day after the completion of TBI modeling,the neural function of each group was evaluated by modified neurological severity score(mNSS),and the expression levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),and IL-18 in the brain tissue were detected by enzyme-linked immunosorbent assay. Moreover,Western blotting was conducted to detect relative expressions of NRLP3 and caspase-1 in the brain tissue at the brain injury area,and TUNEL assay was used to test the apoptosis rate of neurons in each group.Results:Compared with the sham operation group,mNSS scores in the TBI group and the HBO group significantly increased at each time point( P<0.01),and the mNSS scores in the HBO group on day 3 and day 7 were significantly lower than those in the TBI group( P<0.05). Compared with the sham operation group,the levels of IL-1β,TNF-α,and IL-18 in the brain tissue of the rats in the TBI group and the HBO group significantly increased on day 7( P<0.01);on day 7,the levels of IL-1β,TNF-α,and IL-18 in the brain tissue of the rats in the HBO group were significantly lower than those in the TBI group( P<0.05). Compared with the sham operation group,the relative expressions of NLRP3 and caspase-1 in the brain tissue of the rats in the TBI group and the HBO group significantly increased on day 7( P<0.01);on day 7,the relative expressions of NLRP3 and caspase-1 in the brain tissue of the rats in the HBO group were significantly lower than those in the TBI group( P<0.05). Compared with the sham operation group,the apoptosis rates of neurons in the brain tissue of the rats in the TBI group and the HBO group significantly increased on day 7( P<0.01);on day 7,the apoptosis rate of neurons in the HBO group was significantly lower than that in the TBI group( P<0.05). Conclusion:HBO therapy can relieve inflammation,reduce neuronal apoptosis,and effectively improve nerve function deficit,through inhibiting NLRP3 inflammasome,thus improving craniocerebral injury with a good therapeutic effect.
Objective:To explore the mediating effect of psychological resilience(PR)on perceived social support(PSS)and subjective well-being(SWB),and the moderating effect of coping style(CS).Methods:A total of 994 undergraduates majoring in navigation were selected for questionnaire survey by a cluster sampling method,using Perceived Social Support Scale,General Well-being Schedule,Adolescent Psychological Resilience Scale,and Coping Style Questionnaire.Results:PSS,positive coping style(PCS),and PR were positively correlated with SWB( r=0.48,0.46,and 0.69;all P<0.001),while negative coping style(NegCS)and neural coping style(NeuCS)were negatively correlated with SWB( r=-0.47 and -0.34;all P<0.001). PR played a partial mediating role between PSS and SWB[mediating effect was 0.38(95% CI:0.33-0.42),accounting for 79.17% of the total effect]. CS moderated the first half path of the mediating effect,and the moderating effect of PCS was 0.09(95% CI:0.05-0.14). The moderating effect of NegCS was -0.08(95% CI:-0.12 - -0.03). The moderating effect of NeuCS was -0.09(95% CI:-0.14 - -0.05). Conclusion:The PSS of undergraduates majoring in navigation not only directly affects SWB,but also indirectly influences SWB through the mediating effect of PR,which is regulated by CS.
Objective:To investigate the relationship between plasma carbon monoxide levels and hypothyroidism and the therapeutic effects of hyperbaric oxygen combined with Roy’s adaptation model nursing.Methods:A total of 100 outpatients with hypothyroidism treated in the Department of Outpatient of the First Affiliated Hospital of Air Force Medical University between January 2022 to January 2023 were assigned to the observation group,and another 20 healthy persons undergoing physical examinations were assigned to the control group. The plasma carbon monoxide levels and serum thyroid function indicators,including thyroid-stimulating hormone(TSH)and free thyroxine(FT4)levels,of the two groups were recorded;and the relationship between carbon monoxide levels and hypothyroidism was analyzed. Then,the patients in the observation group were randomly divided into a conventional treatment and nursing(CO)subgroup and a hyperbaric oxygen combined with Roy's adaptation model nursing(HR)subgroup,with 50 cases in each subgroup. The therapeutic effects of the two subgroups were observed.Results:Compared with the control group,the serum TSH and plasma carbon monoxide contents in the observation group were significantly increased( P<0.001),and the serum FT4 content was significantly decreased( P<0.001). In comparison,the plasma carbon monoxide content in the patients with severe hypothyroidism was significantly higher than those with mild hypothyroidism( P<0.001). Carbon monoxide level was positively correlated with TSH( r=0.863, P<0.01),and negatively correlated with FT4( r=-0.835, P<0.01). Compared with the CO subgroup,the serum TSH and plasma carbon monoxide contents in the HR subgroup were significantly decreased( P<0.001),and the serum FT4 content was significantly increased( P<0.001). The total effective rate in the HR subgroup(92.00%)was significantly higher than that in the CO subgroup(72.00%),and the difference between the groups was statistically significant( P<0.05). Conclusion:There is a certain correlation between plasma carbon monoxide levels and hypothyroidism. Hyperbaric oxygen combined with Roy's adaptation model nursing can significantly reduce plasma carbon monoxide levels,hence significantly improve therapeutic effects.
