Objective:To observe the clinical efficacy of modified Shaoyao Gancao Decoction in the treatment of autoimmune hepatitis(AIH)of liver-kidney Yin deficiency type.Methods:A total of 78 AIH patients with liver-kidney Yin deficiency type diagnosed in the Department of Hepatology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from February 2020 to March 2023 were selected.After removing the shedding cases,a total of 74 patients were divided into two groups according to random number table method(39 cases in the treatment group and 35 cases in the control group).The patients in the treatment group were given modified Shaoyao Gancao Decoction and the patients in the control group were given compound glycyrrhizin tablets.The treatment lasted for 12 weeks.The evaluation criteria of comprehensive efficacy and the improvement rate of TCM syndrome scores were the main efficacy indexes,and the changes of alanine aminotransferase(ALT),aspartate aminotransferase(AST),glutamyltransferase(GGT),globulin(GLB)and immunoglobulin(IgG)antibody levels from baseline were the secondary efficacy indexes.Results:Based on the evaluation index of western medicine,the total effective rate of the treatment group was 94.87%,and the total effective rate of the control group was 65.71%.Based on the improvement rate of TCM syndrome scores,the total effective rate was 94.87%in the treatment group and 57.14%in the control group.GLB after treatment was lower than that before treatment in both groups,and GLB after treatment in the treatment group was lower than that in the control group.The difference was statistically significant(P<0.05).IgG in the treatment group was lower after treatment than that before treatment,and there was no statistical significance in the control group before and after treatment(P>0.05).ALT,AST and GGT were all lower after treatment,but there was no statistical significance between the two groups(P>0.05).No adverse reactions were observed in both groups during treatment.Conclusion:Modified Shaoyao Gancao Decoction can relieve the clinical symptoms of AIH patients with liver-kidney Yin deficiency type,reduce the levels of GLB and IgG,and restore the liver function of patients.And it has long-term stability and safety.Its effect is better than oral compound glycyrrhizin tablets.
According to the theory of"holism of body and spirit"in Chinese medicine,tic disorder belongs to the comorbid medical conditions of"body"and"spirit".Tic disorder damages both body and spirit,and the key lies in the disharmony of heart and spleen.Treatment based on the heart and spleen,the main clinical acupoints along the governor vessel,pericardium channel,spleen and stomach channels are taken to regulate the heart and spleen,as well as body and spirit,which achieves ideal clinical effect.In the practice of acupuncture and moxibustion,we should pay attention to regulating Qi activity and keeping spirit inside.The effect of acupuncture depth and reinforcing and draining on curative effect should be valued.In clinical practice,we can also flexibly take auxiliary methods such as electroacupuncture and press-needle according to the actual condition.According to the different types and onset periods of tic disorder,the body and spirit should be regulated with emphasis.
Venous leg ulcer is a common disease in TCM surgery,which has complex etiology and is difficult to heal.Di-rector Zheng Xuejun,a famous traditional Chinese medicine doctor in Shanxi Province,believes that the key to the pathogenesis of this disease is the toxicity transformed by the accumulated heat from the dampness and stasis stagnated in the vessels over time.The internal treatment is"regulating blood to alleviate water retention and removing blood stasis to dredge collateral".Self-designed Vein Prescription I is the basic formula,and the formula can be modified according to different syndromes in different stages(dampness and stasis blocking collaterals syndrome in the early stage of ulcer,accumulated dampness,heat and stasis syndrome in the stage of ulcer,and root-deficiency combined with pathogenic factors lingering on in the healing stage).The external treatment is based on the principle of"removing slough and promoting the growth of tissue regeneration",and uses a new method to refine Hongsheng Dan.The self-made improved Shengji Yuhong oil yarn is externally used for wound surface.Traditional Chinese medicine fumigation and washing method is used in each stage and dampness-drying detoxification lotion and ulcer lotion are often used for fumigation and washing and promoting the growth of tissue regeneration.Binding therapy is used extensively to promote venous return and reduce lower limb edema.During different therapies,the assistance of the internal and external treatments and the combination of the Chinese and the western medicine have a very good effect.Through the anal-ysis of director Zheng Xuejun's understanding of venous leg ulcer and the experience of medication rules,this paper summarizes his clinical experience and inherits academic thoughts.And a case is attached for reference.
Objective:To explore the clinical curative effect of modified Niubang Jieji Decoction in the treatment of facial allergic dermatitis.Methods:From August 2021 to August 2022,62 patients with facial allergic dermatitis admitted to the Department of Dermatology of the First Affiliated Hospital of Anhui University of Chinese Medicine were selected as the research objects.The patients were divided into the control group and the study group by random number table method,with 31 cases in each group.The patients in the control group took desloratadine citrate disodium tablets,and the patients in the study group took modified Niubang Jieji Decoction on the treatment basis of the control group.The clinical curative effect,symptom score,inflammatory index and immune function of the patients in both groups were observed.Results:The clinical curative effect of the study group was better than that of the control group,and the difference was statistically significant(P<0.05);after treatment,the symptom scores of redness and swelling,papules,exudation/erosion,pruritus,desquamation and other inflammatory factors[C-reactive protein(CRP),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)]in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05);the improvement of immunoglobulin G(IgG),immunoglobulin A(IgA)and immunoglobulin M(IgM)in the study group was greater than that in the control group,and the differences were statistically significant(P<0.05).Conclusion:The treatment of facial allergic dermatitis with modified Ni-ubang Jieji Decoction can improve the immune function of patients,reduce the inflammatory response,and relieve symptoms,which has reliable curative effect.
Objective:To explore the medication rules of professor Lu Bingjiu in the treatment of ascites due to liver cirrhosis(meteorism)with spleen and kidney Yang deficiency syndrome by data mining method.Methods:A retrospective analysis method was used to collect 103 patients with ascites due to liver cirrhosis with spleen and kidney Yang deficiency syndrome diagnosed and treated by Prof.Lu Bingjiu,a famous traditional Chinese medicine doctor in Liaoning Province,with significant curative effect from December 1,2020 to December 1,2022.Excel 2019,SPSS 25.0,SPSS Modeler 18.0 and other software were used to conduct drug frequency statistics,nature,flavor and channel tropism analysis,association rule analysis,cluster analysis and disease etiology analysis.Results:102 Chinese herbs were involved.The drug nature and flavor were mostly sweet and warm,and the channel tropism was mostly spleen channel and stomach channel.Deficiency-tonifying drugs,damp-clearing and diuresis-promoting drugs,and Qi-regulating drugs were commonly used.The common drug combinations were Fuling-Chenpi,Fuzi-Cheqianzi,Jineijin-Haipiaoxiao,Xueyutan+Diyutan-Haipiaoxiao.The core ingredients were:Ren-shen,Fuzi,Guizhi,Baizhu,Fuling,Zexie,Dafupi,Chenpi,Shanzha,Maiya,Shenqu,Tinglizi,Chushizi,Cheqianzi,Xueyutan,Diyutan and Haipiaoxiao.Four cluster prescriptions were obtained by cluster analysis,namely,Tongda Xiaoji Prescription,Dampness-removing and Spleen-promoting Prescription,Qi-blood-tonifying Prescription,and Spleen-strengthening and Lung-controlling Prescription.Conclusion:In the treatment of patients with ascites due to liver cirrhosis with spleen and kidney Yang deficiency syndrome,Master Lu focuses on warming Yang and invigorating spleen,strengthening spleen and controlling lung,and supplements with the drugs to remove blood stasis and stop bleeding,remove food masses and harmonize stomach,and invigorate Qi and generate blood.The overall characteristics of drug use can be summarized as the combined regulation of Yin and Yang,spleen-strengthening and lung-controlling,preventing disease from exacerbating and combination of tonification and promotion.
