
Ein hypophysär bedingter hypogonadotroper Hypogonadismus tritt auf, wenn die pulsatile Freisetzung von LH und FSH aus den gonadotropen Zellen des Hypophysenvorderlappens in Antwort auf eine funktionell intakte hypothalamische GnRH-Stimulation quantitativ reduziert ist, ferner, wenn die ins Blut sezernierten Gonadotropine qualitativ verändert und daher unzureichend wirksam sind. Angeborene Formen eines hypophysär bedingten Hypogonadismus, die aus einer Fehlentwicklung der Hypophyse und/oder aus einer Hypophysenstiel-Aplasie resultieren, sind regelhaft mit Beeinträchtigungen weiterer hypophysärer Achsen, insbesondere der somatotropen, thyreotropen und corticotropen, selten auch der lactotropen Achse assoziiert. Peripartale Hypophysentraumata sind selten Ursache einer angeborenen multiplen hypophysären Insuffizienz mit Beteiligung der gonadotropen Achse. Bei der X-chromosomalen Nebennierenhypoplasie, die durch eine Mutationen im DAX1(=NROB1)- Gen verursacht wird, findet sich neben einer frühkindlich symptomatisch werdenden Nebennierenrindeninsuffizienz eine Beeinträchtigung aller Ebenen der gonadotropen Achse. Bei Syndromen mit hypogonadotropem Hypogonadismus führt eine chromosomale Veränderung oder eine Genmutation zu Syndrom-typischen Merkmalen. Hierunter findet sich auch eine Beeinträchtigung der zentralen hypothalamo-hypophysär-gonadalen Achse und ggf. auch eine zusätzliche primäre Beeinträchtigung der Hodenfunktion. Beim Prader-Willi-Syndrom ist die zentrale und gonadale Ebene der gonadotropen Achse von einer Funktionsstörung betroffen. Beim CHARGE-Syndrom liegt ein Hypogonadismus hypothalamischer Ursache vor. Beim Bardet-Biedl-Syndrom liegt ein Hypogonadismus am ehesten kombiniert zentraler und gonadaler Ursache vor. Beim Gordon-Holmes-Syndrom, Boucher-Neuhauser-Syndrom und Oliver-Mc-Farlane-Syndrom findet sich neben einer Kleinhirnataxie ein hypophysär bedingter hypogonadotroper Hypogonadismus.
Die Grundlage einer korrekten und aussagekräftigen Hodenhistologie ist die qualitative und quantitative Analyse der testikulären Zellpopulationen sowie eine semiquantitative Score-Evaluation. Durch den daraus hervorgehenden Biopsiebericht wird eine kritische Beratung der Patienten in Bezug auf eine erfolgreiche Behandlung der Infertilität mit TESE und ICSI möglich. Außerdem ermöglicht die detaillierte und auch weiterführende Analyse testikulärer Spermien oder des Hodengewebes die Verbesserung der assistieren Reproduktion und auch der Erforschung von Spermatogeneseschäden. Die Hodenbiopsie ist ein invasives chirurgisches Verfahren und hat unmittelbaren Einfluss auf den Patienten und das weitere Vorgehen der Behandlung. Deswegen darf sie nur bei strenger Beachtung der Indikationen und mit validierten Operations- und Untersuchungstechniken durchgeführt werden. Es empfiehlt sich deswegen die Durchführung in qualifizierten andrologischen Zentren, die zum Beispiel durch die European Academy of Andrology (EAA) zertifiziert sind.
XX-Männer mit weiblichem Karyotyp (46,XX) sind phänotypisch männlich und besitzen weder innere noch äußere weibliche Genitalorgane. Die Prävalenz ist mit 1:10.000 bis 1:20.000 deutlich geringer als das Klinefelter-Syndrom. Die meisten XX-Männer weisen eine ursächliche Translokation des SRY-Gens auf (80–90 % SRY-positiv). XX-Männer haben eine schwere Hodenentwicklungsstörung und bilden keine Spermien, so dass eine Hodenbiopsie bei Kinderwunsch nicht indiziert ist. Häufig liegt ein substitutionsbedürftiger Testosteronmangel vor. Männer mit XYY-Karyotyp werden meist zufällig, z. B. wegen Infertilität, entdeckt, die mit konventionellen oder Verfahren der assistierten Reproduktion behandelt werden können.
Die männlichen Keimdrüsen, die Hoden, erfüllen zwei zentrale Funktionen. Sie sind der Ort der Spermatogenese, also die Produktionsstätte der männlichen Gameten, der Spermien. Ebenso zentral ist ihre zweite Funktion als endokrine Drüse, die die Androgene synthetisiert. Die Androgene sind für die Ausbildung des typisch männlichen Phänotyps notwendig. Die männliche Gonade bildet sich embryonal aus, wenn in der Geschlechtsdifferenzierung das Gen SRY exprimiert wird und ein Testosteronsignal auftritt. Das Gewebe des Hodens lässt sich in zwei wesentliche Kompartimente einteilen: Zum einen in die von peristaltisch aktiven peritubulären Zellen umgebenen Tubuli seminiferi, die das Keimepithel beherbergen, in dem die Differenzierung der Keimzellen via Meiose und Haploidisierung abläuft. Das zweite Kompartiment besteht aus dem Interstitium, das die Leydig-Zellen enthält, die steroidogen aktiv sind und die Androgene produzieren. Immunologisch werden die differenzierteren Keimzellen durch die Blut-Hoden-Schranke abgeschirmt, die ebenso von den Sertolizellen gebildet wird, wie die Nische der spermatogonialen Stammzellen, die als unipotente adulte Stammzellen die Spermatogenese nach der Pubertät lebenslang antreiben. Die komplexen Vorgänge der Keimzellreifung sind durch zahlreiche Faktoren reguliert, zu denen Zytokine, Transkriptions- und Wachstumsfaktoren, vor allem aber endokrine, parakrine und autokrine Signale zählen. Die endokrine Funktion ist Bestandteil der Hypothalamus – Hypophysen – Gonadenachse. Entlang dieser Achse wirken zahlreiche Hormone und Rezeptoren in einem balancierten Feed-back Mechanismus. Primär wird das System durch Kisspeptin und GnRH pulsatil angetrieben, das die Sekretion der hypophysären Gonadotropine LH und FSH steuert, die wiederum Wirkungen im Hoden induzieren. LH stimuliert die Produktion von Testosteron, FSH wirkt auf die Sertoli-Zellen und provoziert eine Inhibin-Antwort. Testosteron, das im Hoden in weit höherer Konzentration vorliegt als im Serum, und Inhibin regulieren wiederum in negativer Rückkoppelung die Gonadotropinsekretion, was einen endokrinen Regelkreis entlang der Hypothalamus – Hypophysen – Gonadenachse bewirkt. FSH und Androgenwirkung sind für eine quantitativ und qualitativ vollständige Spermatogenese essentiell. Testosteron und seine verschiedenen Metabolite, vor allem Dihydrotestosteron und Estradiol sind für die normale männliche Ausbildung zahlreicher Zielorgane notwendig, darunter Knochenstoffwechsel, Muskulatur, Haar- und Bartwuchs, Stimmlage, Fettverteilung und bestimmte Hirnfunktionen. Störungen der Hodenfunktion können verschiedene Ursachen haben, etwa genetische oder erkrankungsbedingte. Diese führen pränatal zu Störungen der Geschlechtsdifferenzierung und postnatal zu weiteren pathologischen Konsequenzen, wie Infertilität oder Hypogonadismus, die mit zahlreichen spezifisch männlichen Erkrankungen einhergehen.
Die Gynäkomastie ist eine benigne Vergrößerung des Brustdrüsengewebes beim männlichen Geschlecht und muss von einer reinen Lipomastie und einem Karzinom abgegrenzt werden. Die Gynäkomastie ist kein eigenes Krankheitsbild, sondern ein Symptom für vielfältige Ursachen, die zu einer Dysbalance zwischen Androgenen und Estrogenen führen. Eine Gynäkomastie kann erhebliche psychologische und psychosoziale Probleme (Körperdysphorie, vermindertes Selbstwertgefühl etc.) verursachen. Als nahezu „physiologisch“ werden Gynäkomastien des Neugeborenen, im Pubertätsalter und im Senium angesehen. Wichtig ist zu erkennen, ob die Gynäkomastie durch einen Estrogen- oder hCG-_ produzierenden Hodentumor verursacht wird, der zeitnaher Behandlung bedarf. Häufige Ursachen einer Gynäkomastie sind primärer (z. B. Klinefelter-Syndrom) und sekundärer Hypogonadismus. Zahlreiche Medikamente, Drogen und Produkte des täglichen Bedarfs (Kosmetika und Nahrungsmittel) sowie berufliche Exposition können eine Gynäkomastie verursachen. Die Behandlung besteht zunächst in der Beseitigung von verursachenden Noxen. Darüber hinaus gibt es keine evidenzbasierte pharmakologische Therapie, nur Heilversuche mit nicht für diese Indikation zugelassenen Medikamenten. Eine empathische Führung des Patienten ist Aufgabe des behandelnden Arztes. Letztendlich bleibt eine chirurgische Gynäkomastektomie, die von einem auf dem Gebiet erfahrenen plastischen Chirurgen oder Senologen durchgeführt werden sollte.
Background: Klinefelter syndrome (KS) with the karyotype 47,XXY is one of the commonest types of congenital chromosomal disorder in males, with an incidence of 0.1% to 0.2% of newborn male infants.It causes hypogonadism and infertility.Until now, however, only about one-quarter of all persons with KS received the diagnosis during their lifetimes.Methods: Selective review of the literature.Results: KS is caused by aneuploidy of the sex chromosomes.Small, firm testes, the manifestations of androgen deficiency (sparse development of male-pattern body hair, greater than average height, lack of libido, erectile dysfunction) and, in more than 90% of affected men, azoospermia are its main features in adults.Affected boys may have verbalization difficulties and problems with learning and socialization.KS is often accompanied by other disturbances such as gynecomastia, varicose veins, thrombosis, osteoporosis, the metabolic syndrome, type 2 diabetes, and epilepsy.The most important therapeutic measure is testosterone supplementation, which should be initiated if the testosterone concentration drops below 12 nmol/L and should be given as directed in the guidelines for the treatment of hypogonadism.This recommendation is made even though there have not been any randomized controlled trials documenting the efficacy of testosterone therapy in adolescents or young adults.In some cases, viable sperm can be obtained from individual testicular tubules by biopsy, so that these patients are able to become fathers. Conclusion:The diagnosis of KS would be less frequently missed if doctors were more aware of, and attentive to, its key manifestations, particularly the small, firm testes, erectile dysfunction, and the comorbidities mentioned above.If the diagnosis were made more often, patients would more often be able to receive early treatment, which would improve their quality of life.►Cite this as:
Das Kapitel „Rechtliche Regelungen“ stellt in Grundzügen die rechtlichen Rahmenbedingungen ärztlichen Handelns dar, bevor es auf spezielle rechtliche Aspekte eingeht. Im allgemeinen Teil werden zunächst die generellen Voraussetzungen der rechtmäßigen ärztlichen Heilbehandlung sowie der Arzthaftung erörtert. Im Hinblick auf das Behandlungsziel der Schwangerschaftsverhütung wird insbesondere auf die rechtlichen Voraussetzungen der Sterilisation und Kastration eingegangen. Die Ausführungen zur Herbeiführung von Schwangerschaften durch Maßnahmen der assistierten Reproduktion behandeln deren rechtliche Grundlagen, das Kindschaftsrecht und die Kostenübernahme durch die gesetzliche und private Krankenversicherung. Es folgen Ausführungen zur Präimplantationsdiagnostik, die seit 2011 im Embryonenschutzgesetz geregelt ist. Ausführungen zur Kryokonservierung folgt ein Überblick über die gesetzlichen Grundlagen der Forschung mit Embryonen und embryonalen Stammzellen.
ZusammenfassungDie Fortpflanzungsmedizin ist ein dynamischer Zweig der modernen Hochleistungsmedizin. Die Andrologie ist mit ihr interdisziplinär verbunden. Ethisch sind für fortpflanzungsmedizinische Behandlungsangebote drei Kriterien relevant, die sich mit individuellen Grund- und Menschenrechten überschneiden: das persönliche Selbstbestimmungsrecht und die Fortpflanzungsfreiheit jedes Menschen, der Schutz der menschlichen Gesundheit und das Kindeswohl. Die Gesundheit und das Wohl von Kindern, die mit fortpflanzungsmedizinischer Unterstützung geboren werden sollen, sind bereits vorwirkend, vor ihrer Erzeugung zu beachten. Im Licht der drei Kriterien sind auch strittige oder hypothetische Handlungsmöglichkeiten zu erörtern, etwa die Leihmutterschaft oder die theoretisch vorstellbare Nutzung von Stammzellen für reproduktive Zwecke. Auf Forschungsebene ist es geboten, die Kenntnisse über Gefahren für die menschliche Fortpflanzung z. B. durch Chemikalien zu verbessern. Aufgrund ethischer Verantwortung für künftige Generationen ist dazu zu forschen, wie sich die Erwärmung der Erdatmosphäre auf die reproduktive Gesundheit auswirken wird.
Neben anderen immun-pathologischen Prozessen im männlichen Genitaltrakt können Antispermienantikörper (ASA) zu einer immunologisch bedingten Infertilität führen. Hierbei hängt die spontane Konzeptionswahrscheinlichkeit vom Anteil der mit ASA behafteten Spermien ab. Zu den wesentlichen Pathomechanismen einer ASA-bedingten Infertilität wird die Beeinträchtigung der Fähigkeit von Spermien zur Penetration im Zervikalmukus, die Beeinträchtigung der Kapazitation und der Akrosomreaktion der Spermien sowie die Beeinträchtigung der Spermienbindung an die Zona pellucida der Eizelle gezählt. Der Nachweis der an Spermien gebundenen IgG- oder IgA-Autoantikörper erfolgt nach den Empfehlungen der WHO im Rahmen der Ejakulatanalyse im direkten MAR- oder Immunobead-Test. Da eine Kortikoid-Therapie nur einen fraglichen Therapieerfolg zeigt, können zur Überwindung der mit ASA einhergehenden männlichen Infertilität Methoden der assistierten Reproduktion angewandt werden.
Background Except for testicular cancer and Hodgkin’s disease, baseline data on semen quality in case of cancers as well as systemic pathologies of the young adult are scarce or based on low sample size. Methods Semen quality in patients having testicular cancer (TGCT, n = 2315), Hodgkin’s disease (HD, n = 1175), non-Hodgkin’s lymphoma (NHL, n = 439), leukemia (L, n = 360), sarcoma (S, n = 208), brain tumour (BT, n = 40), Behcet’s disease (Behcet’s, n = 68) or multiple sclerosis (MS, n = 73) was studied and compared to that of 1448 fertile men candidates for sperm donation (CSD) and 208 partners of pregnant women (PPW). All samples were studied following the same methodology in a single laboratory. Post freezing and thawing semen characteristics were also studied. Results The percentage of normozoospermic men was only 37 % for L patients and lower than 60 % for TGCT, NHL, S and BT. The level of sperm production was differently decreased according to pathologies, the median total sperm count in TC and L patients being four times lower ( p < 0.01 when compared to CSD and PPW). The lowest percentage of progressively motile spermatozoa was found for L and BT patients (both, p < 0.01 compared to CSD and PPW). The percentage of morphologically normal spermatozoa was also reduced in cancer patients, especially in BT patients. Progressive motility after thawing in patients was about half that observed among candidates for sperm donation. In almost half of the semen of patients with testicular cancer or leukemia, the total number of motile spermatozoa per straw was less than 0.5 × 10 6 compared to 4.3 × 10 6 in CSD. Conclusions The present data confirm on large series the deleterious impact of various cancers of the young adult on semen quality, establishing thus baseline data for future studies. Owing to the post-thaw quality of the frozen straws, future fertility projects for the majority of the patients studied (in case there is no post-treatment recovery of spermatogenesis) should necessitate an ICSI to provide the best chance of paternity whatever the fertility check-up in the female partner.
BACKGROUND:NRD convertase, also termed Nardilysin, is a Zn(++) metalloendopeptidase that specifically cleaves the N-terminus of arginine and lysine residues into dibasic moieties. Although this enzyme was found located within the testis, its function in male reproduction is largely unknown. In addition, the precise distribution of this enzyme within germ cells remains to be determined.METHODS:To answer these questions, we developed an immuno-gold electron microscopy analysis to detect Nardilysin at ultrastructural level in mice. In addition, we performed a quantitative analysis of these gold particles to statistically estimate the distribution of Nardilysin in the different subcellular compartments of differentiating late spermatids/spermatozoa.RESULTS:Expression of Nardilysin in wild-type mice was restricted to germ cells and markedly increased during the last steps of spermiogenesis. In elongated spermatids, we found the enzyme mainly localized in the cytoplasm, more precisely associated with two microtubular structures, the manchette and the axoneme. No labelling was detected over the membranous organelles of the spermatids. To test whether this localization is dependent of the functional microtubules organization of the flagella, we analysed the localization into a specific mouse mutant ebo/ebo (ébouriffé) known to be sterile due to an impairment of the final organization of the flagellum. In the ebo/ebo, the enzyme was still localized over the microtubules of the axoneme and over the isolated cytoplasmic microtubules doublets. Quantification of gold particles in wild-type and mutant flagella revealed the specific association of the enzyme within the microtubular area of the axoneme.CONCLUSIONS:The strong and specific accumulation of Nardilysin in the manchette and axoneme suggests that the enzyme probably contributes either to the establishment of these specific microtubular structures and/or to their functional properties.
Background Vasectomy is currently the only long-acting contraceptive option available for men, despite increasing demand and potentially significant positive impacts on human health of additional male contraceptive options. Vasalgel ™ is a high molecular weight hydrogel polymer being developed as a non-hormonal long-acting reversible male contraceptive. Vasalgel consists of styrene-alt-maleic acid dissolved in dimethyl sulfoxide, which is distinct from styrene-alt-maleic anhydride materials previously studied. Methods The goal of the study was to determine the contraceptive efficacy of two test articles with different levels of styrene maleic acid (100 %, and 80 % acid/20 % anhydride). The test articles were injected bilaterally in the vasa deferentia of mature male rabbits. Post-implantation analyses of semen parameters were completed over a 12 month period and compared to baseline measures of sperm concentration, motility and forward progression. Results Both test articles were effective in blocking the passage of spermatozoa through the vasa deferentia in the 12 subjects completing the study. A significant decrease in sperm concentration occurred following implantation of the test material, with no measurable sperm concentration except for a few samples in one animal that were markedly oligospermic. Vasalgel produced a rapid onset of azoospermia, with no sperm in semen samples collected as early as 29–36 days post-implantation, and was durable over a 12 month period. Conclusion This study indicated that Vasalgel is an effective non-hormonal long-acting male contraceptive in a rabbit model.
Objective Endothelial dysfunction and microvascular damage play a crurical role in the pathogenesis of erectile dysfunction (ED). Insulin-like growth factor-1 (IGF-1) is one of the growth factors that have a wide range of biologic effects. IGF-1 is an important mediator of cell growth, differentiation and transformation in various tissues. The purpose of the current study was to determine the association between IGF-1 levels and ED. Materials and methods All men were evaluated for ED and divided into two groups: 80 patients suffering from ED for > 1 year and 80 subjects without ED were enrolled as a control group in this study. Diagnosis of ED was based on the International Index of Erectile Function Score-5. IGF-1 levels were measured in serum by an automated chemiluminescence immunoassay. The relationship between IGF-1 levels and ED scores in patients was statistically evaluated. Results The mean age of patients in ED group was 60.4 ± 11.3 years and 55.4 ± 9.6 in control group. The plasma IGF-1 levels were significantly lower in ED than in control group (96.5 ± 38.3 and 132.5 ± 53.3 ng/ mL, respectively, P < 0.001). The IGF-1 levels were positively correlated with ED score (r = 0.623, P < 0.01). Conclusion In this study serum IGF-1 levels were found to be associated with endothelial dysfunction that predicts ED. Serum IGF-1 level appears to be a specific predictor of ED, and it might be used in early prediction of ED in male population.

Background: In couples presenting with retrograde ejaculation refractory to medical treatment, the first choice of fertility treatment should be Assisted Reproductive Techniques using rapidly purified spermatozoa retrieved from post-ejaculatory urine. The Hotchkiss technique and modified variants are simple and efficient for retrieving sperm from the bladder. We developed a new protocol, including a novel modified Hotchkiss technique involving sperm cryopreservation.The aim was to study the pregnancy rate and birth rate achieved by intra cytoplasmic sperm injection (ICSI) using frozen-thawed sperm retrieved from the bladder with this novel modified Hotchkiss technique in patients with refractory retrograde ejaculation.Results: In this descriptive retrospective, single-center study, we analyzed the local database of all patients who banked sperm at the CECOS Laboratory Biology of Reproduction of La Conception University Hospital, Marseille, France, between 2004 and 2014. A total of 2171 patients banked sperm during this time, including 63 patients with retrograde ejaculation, of whom ten patients banked sperm that had been retrieved by the modified Hotchkiss technique. These ten couples underwent 26 ICSI cycles: nine clinical pregnancies were achieved in six couples, including eight after fresh embryo transfer and one after thawed embryo transfer, resulting in seven live births. The average live birth rate per transfer was 28 %.Conclusions: We report the largest series of births using frozen-thawed spermatozoa retrieved from post-ejaculatory urine by a modified Hotchkiss technique. This series of births demonstrates that this new modified Hotchkiss technique allows for successful association with sperm cryopreservation, leading to an efficient and easy management of couples with refractory retrograde ejaculation.
Objectives To investigate the levels of testosterone, luteinizing hormone (LH), follicle-stimulating hormone (FSH), and prostate-specific antigen (PSA) in prostate cancer patients before and after the switch from degarelix to leuprolide treatments. Methods We enrolled 40 treatment-naïve prostate cancer patients who were treated initially with degarelix and were later switched to leuprolide. The subjects were divided into three groups depending on when they were switched to leuprolide: the 3-month group (3m; switched after 84 days, n =10), the 2-month group (2m; 56 days, n =10), and the 1-month group (1m; 28 days, n =20). Patient symptoms and hormone levels were measured after switching therapy. The castration level was defined as a serum testosterone level ≤50 ng/dl. Results Thirty-nine subjects (97.5%) achieved castration levels of testosterone (11±5.8 ng/dl) 2 weeks after degarelix was first administered, and the characteristics of these patients were investigated. Testosterone levels increased and exceeded the castration level in one subject each of the 3m (142 ng/dl), 2m (72 ng/dl), and 1m groups (63 ng/dl). All subjects achieved the castration level by day 5. In contrast to testosterone levels, the LH and FSH surge on day 2 was significantly higher in the 1m group than in the other groups. The clinical symptoms were not exacerbated before or after switching in any patients. Conclusions A testosterone surge was observed in 8.3 % of the study patients; however, it was very short-lived and mild. LH and FSH levels were significantly higher 1 month after administration compared with 2 or 3 months after degarelix administration.
To examine human microRNA expression in fertile men and subsequently to compare expression patterns of miRNAs in fertile and infertile men, specifically men with Sertoli Cell Only (SCO) histopathology.
Treatment of differentiated thyroid cancer usually consists of a total thyroidectomy followed by one or several courses of radioiodine (131I). 131I is known to have deleterious effects on radiation sensitive tissues and irradiation to the testes has been shown after its administration. We investigated effects of such treatment on sperm DNA in a patient with differentiated thyroid carcinoma.
Background: The annulus is a ring-shaped structure located beneath the plasma membrane that connects the midpiece and the principal piece of mammalian sperm flagellum. It has been suggested that the annulus acts as a morphological organizer, guiding flagellum assembly during spermiogenesis, and as a diffusion barrier, confining proteins to distinct compartments of the flagellum in mature sperm. Previous studies on small cohorts of patients have attempted to correlate annulus defects with the occurrence of human asthenozoospermia. An absence of the annulus has been shown to be frequently associated with asthenozoospermia.Findings: We tried to obtain a more precise estimate of the frequency of annulus defects, by screening a large cohort of 254 men presenting asthenozoospermia (mean progressive motility of 24 %) by the immunodetection of SLC26A8, a transmembrane protein that has been shown to be specifically localized to the annulus. By contrast to previous reports, our results indicate that annulus defects are associated with asthenozoospermia in only 1.2 % of cases.Conclusions: We conclude that defects or an absence of the annulus are not frequently associated with asthenozoospermia. The use of annulus defects as a diagnostic endpoint in patients is therefore not appropriate.
To assess the cognitive and sexual/hormonal functioning of prostate cancer patients treated with a luteinizing hormone-releasing hormone (LH-RH) agonist, and the relationships thereof with adrenal and residual testicular androgen levels.
