
Objectives: Cutaneous vasculitis (CV) is common in SLE, but the epidemiology and risk factors remain unclear. We aimed to identify the trends and risk factors for CV in patients with SLE over a period of 20 years. Methods: The Birmingham Lupus Cohort is an observational longitudinal cohort of SLE patients. Patients were enrolled within 3 years of meeting their fourth ACR criterion. Disease activity, laboratory test results and treatment records were collected. A multivariable shared frailty Cox proportional hazard model was used to identify clinical, laboratory and treatment-related variables associated with the development of CV. Results: We included 392 patients: 95.7% were female. The median (interquartile range) duration of follow-up was 9.2 (5.1–14.7) years. CV occurred in 27% of SLE patients, of whom 43.3% had two or more CV events. This study demonstrated a marked decline in the incidence rates of CV, decreasing from 34.4% (95% CI 29.7, 39.3) during the first 3 years after enrolment to 2.1% (95% CI 0.05, 11.5) after 18 years of follow-up. Development of CV was associated with RP, constitutional, mucocutaneous, musculoskeletal, haematological and cardiovascular involvement, anti-Sm antibodies, anti-dsDNA, and hypocomplementemia. However, the use of AZA and antimalarials was inversely associated with the development of CV. Patients with CV were more likely to develop at least one item of organ damage.
Die leukozytoklastische Vaskulitis (LCV) wurde als seltenes paraneoplastisches Syndrom im Zusammenhang mit verschiedenen hämatologischen Erkrankungen, darunter indolente Lymphome wie die Waldenström-Makroglobulinämie, beschrieben. Allerdings sind nur sehr wenige Fälle von LCV im Zusammenhang mit Plasmazellerkrankungen bekannt. Wir präsentieren den Fall einer 58-jährigen Frau, die aufgrund einer LCV eine schnell fortschreitende, ulzerativ-nekrotische Hautläsion am linken Unterschenkel entwickelte. Die Läsion wurde zunächst als chronische Ulkuswunde (Ulcus cruris) vermutet, stellte sich jedoch als eine atypische Manifestation einer LCV heraus, die das einzige klinische Anzeichen eines schwelenden multiplen Myeloms vom Typ IgG kappa darstellte. Nach einer standardmäßigen Induktions-Chemoimmuntherapie mit Daratumumab, Bortezomib, Lenalidomid und Dexamethason wurde die Patientin einer hochdosierten Chemotherapie mit Melphalan unterzogen, gefolgt von einer autologen Stammzelltransplantation zur Konsolidierung. Trotz einer bakteriellen Hautsuperinfektion konnte die Myelombehandlung ohne größere Komplikationen erfolgreich abgeschlossen werden. Parallel zur Senkung der Paraproteinspiegel heilte die Hautläsion, sodass kein plastisch-chirurgischer Eingriff erforderlich war. Ein LCV-ähnliches Ulcus cruris kann eine seltene und atypische Erstmanifestation einer Plasmazellneoplasie darstellen. In diesem Fallbericht erwies sich die systemische Myelombehandlung als wirksam, um eine vollständige Remission der fortgeschrittenen ulzeronekrotischen Hautschädigung zu induzieren. Dieser Fall erweitert das Spektrum der berichteten monoklonalen Gammopathien mit kutaner Signifikanz.
Pyoderma gangrenosum (PG) is a rapidly progressive disease characterized by deep ulcers, predominantly in the lower extremities. Adalimumab, a monoclonal antibody against tumor necrosis factor alpha, is the first drug approved for PG treatment in Japan, ahead of other countries. We conducted a multicenter, open-label, post-marketing observational study to evaluate the safety and effectiveness of adalimumab in Japanese patients with PG. Of 67 patients enrolled, 37 in the safety analysis set and 32 in the effectiveness analysis set were included in this interim analysis. (Nineteen patients whose case report forms were not collected and 11 whose data were not fixed by the data cut-off date were excluded from the study). In the safety analysis set, the mean age was 62.9 years and 86.5% of patients had comorbidities, including ulcerative colitis (21.6%), diabetes mellitus (18.9%), and hypertension (10.8%); subtypes of PG included ulcerative (n = 33), vegetative (n = 2), and pustular (n = 2). Mean exposure duration to adalimumab was 185.5 days. Systemic steroids were used before (70.3%) and during (56.8%) adalimumab treatment. The incidence proportion of overall adverse drug reactions was 18.9%. The incidence proportions of all infections and serious infections reported as adverse drug reactions were 13.5% and 10.8%, respectively. The proportion of patients with a Physician Global Assessment score (total lesions) of 0/1 at weeks 12, 26, and 52 was 42.9%, 36.8%, and 50.0%, respectively. This interim analysis revealed the characteristics of Japanese patients with PG treated with adalimumab in the actual clinical setting and the real-world safety and effectiveness of adalimumab. At the time of the interim analysis, adalimumab treatment was generally well tolerated, and no new safety concerns were detected. Further follow-up of this study will provide a more detailed understanding of the long-term safety and effectiveness of adalimumab in patients with PG refractory to conventional treatments.
Chronic wounds are difficult to treat due to the presence of biofilm which prevents wound healing. Pseudomonas aeruginosa is one of the most common pathogens found in chronic wounds and conventional treatment strategies have been ineffective in the eradication of its biofilm, without harming the surrounding healthy tissue at the same time. Here, we introduced an innovative approach applying the probiotic product Bio-K+ (containing three lactobacilli) topically as an antimicrobial and antibiofilm agent. We identified lactic acid as the main active component. While antibiotics and antiseptics such as silver-ions only demonstrated limited efficacy, Bio-K+ was able to completely eradicate mature P. aeruginosa biofilms established in an in-vitro and ex-vivo human skin model. Furthermore, it demonstrated biocompatibility in the co-culture with human dermal fibroblasts and accelerated the migration of fibroblasts in a cell migration assay promoting wound healing. To enhance clinical practicability, we introduced Bio-K+ into the hydrocolloid dressing Aquacel, achieving sustained release of lactic acid and biofilm eradication. This new treatment approach applying probiotics could represent a major improvement in the management of chronic wounds and can be extended in treating other biofilm-associated infections.
Background/Objectives: Negative-Pressure Wound Therapy (NPWT) is increasingly used to promote wound healing in chronic and complicated wounds, but its use in surgical oncology is still debated due to theoretical concerns about promoting local tumor recurrence. The aim of this study is to review the available evidence on the oncologic safety of NPWT, specifically regarding the risk of local recurrence in patients undergoing surgery for cutaneous melanoma (CM) or soft tissue sarcoma (STS). Methods: A systematic review of the literature was conducted using the MEDLINE/PubMed, EMBASE, and Scopus databases through December 2024 (PROSPERO: CRD42024623405). Case series, retrospective cohort studies, and randomized clinical trials reporting survival data in melanoma and sarcoma patients treated with NPWT were eligible for inclusion. PRISMA guidelines were followed and quality assessment checked. Results: Seventeen studies were eligible for the analysis with a total of 285 patients, 197 affected by soft tissue sarcoma and 88 by cutaneous melanoma. The pooled proportion of local recurrence was 5% in patients treated with NPWT, regardless of the histology considered (STS and CM). When comparing NPWT to conventional wound therapy, both the pooled risk ratio (0.87; 95% CI: 0.24-3.11; Tau2 = 0.14; I2 = 8%) and odds ratio (0.83; 95% CI: 0.20-3.39; Tau2 = 0.18) indicated no statistically significant difference in the recurrence rate.
Objective: The aim of this study was to assess the utilization of surgical interventions in patients diagnosed with superficial vein thrombosis (SVT) and its potential association with the occurrence of venous thromboembolism (VTE) and bleeding events. Methods: INSIGHTS-SVT, a prospective, non-interventional, multicenter study in Germany, investigated the management and outcomes of patients with acute SVT who received conservative and/or invasive treatments at the discretion of the treating physician. Results: Among the 872 patients with 12-month data, 657 had medical therapy only, and 215 patients underwent vascular surgery (70 within 3 months of SVT diagnosis, 136 between months 4 and 12, and nine had an intervention in both periods). The most commonly performed procedures included endovenous thermal ablation, ligation of the saphenofemoral or saphenopopliteal junction, and vein stripping. The primary outcome of symptomatic VTE was observed in 5.8% of conservatively treated patients and 6.3% of those who underwent surgical intervention. Additionally, the secondary outcome of recurrent or extended SVT was documented in 4.7% of conservatively treated patients and 5.3% of invasively treated patients. Bleeding events occurred in 1.4% of conservatively treated patients and 2.1% of surgically treated patients. These differences were statistically not significant. Furthermore, our analysis indicated a potential protective effect associated with surgical treatments, such as ligation of the saphenofemoral or saphenopopliteal junction, stripping and endovenous thermal ablation, concerning the endpoint of VTE for patients when applied after 3 months from the index SVT event.
Akne vulgaris ist eine weit verbreitete dermatologische Erkrankung, die die Lebensqualität von Jugendlichen und Erwachsenen erheblich beeinträchtigt. Traditionell wurde die Pathogenese der Akne mit Faktoren wie übermäßiger Talgproduktion, follikulärer Hyperkeratinisierung und dem Vorhandensein von Cutibacterium acnes (C. acnes) in Verbindung gebracht. Jüngste Studien haben jedoch die Rolle des Hautmikrobioms hervorgehoben und den Fokus von einzelnen Krankheitserregern auf die Dynamik des mikrobiellen Milieus verlagert. In dieser Übersichtsarbeit wird die bestehende Forschung zum Hautmikrobiom und zu seiner Beziehung zur Akne kritisch bewertet, wobei der Schwerpunkt auf der mikrobiellen Vielfalt, der Variabilität der C. acnes-Stämme und neuen Therapien liegt, die auf das Mikrobiom abzielen. Während bestimmte Studien C. acnes mit dem Schweregrad der Akne in Verbindung bringen, zeigen andere Arbeiten, dass dieses Bakterium auch auf gesunder Haut vorhanden ist. Dies deutet darauf hin, dass stammspezifische Unterschiede und das allgemeine mikrobielle Gleichgewicht eine entscheidende Rolle spielen. Neue therapeutische Ansätze wie Probiotika und die Bakteriophagentherapie zielen auf die Wiederherstellung des mikrobiellen Gleichgewichts oder die selektive Bekämpfung pathogener Stämme ab, ohne dabei das breitere Mikrobiom zu stören. Das Fehlen standardisierter Methoden, die begrenzte Verfügbarkeit von Langzeitstudien sowie die enge Fokussierung auf bakterielle Milieus sind jedoch wichtige Einschränkungen der aktuellen Forschung. Die künftige Forschung sollte verstärkt das breitere Hautmikrobiom, einschließlich Pilze und Viren, analysieren, dabei auf einheitliche Methoden zurückgreifen und sich zudem auf Langzeitstudien konzentrieren, um die mikrobiellen Schwankungen im Zeitverlauf besser zu verstehen. Wenn diese Lücken geschlossen werden, kann dies die Entwicklung wirksamerer mikrobiombasierter Behandlungsansätze für Akne ermöglichen. Zusammenfassend lässt sich sagen, dass mikrobiombasierte Therapien zwar vielversprechend sind, aber dennoch weitere Untersuchungen erforderlich sind, um ihre Wirksamkeit und Sicherheit zu validieren – und so den Weg für innovative, personalisierte Strategien im Akne-Management zu ebnen.
Introduction: The effectiveness of psoriasis treatment is assessed by standardized tools such as the Dermatology life Quality Index (DLQI) and Psoriasis Area Severity Index (PASI). However, discrepancies between patients and physicians in terms of treatment success and goals, along with the growing importance of shared decision-making in healthcare, highlight the need for tools specifically designed for psoriasis. Such tools can enhance communication between patients and physicians, encouraging shared decision-making and improving the assessment of patient treatment expectations. Methods: Comparison of the new PSO-TARGET grid, which consists of 12 therapeutic goals evenly distributed across 4 major components commonly used in quality of life (QoL) studies for chronic diseases, with DLQI as a standard tool, was utilized. Results: A total of 143 adult patients with moderate-to-severe psoriasis and treated with brodalumab were included. On the basis of a blind assessment, dermatologists were not able to identify the patients' chosen PSO-TARGET goal in more than 50% of cases. The comparison after 12/16 weeks of treatment revealed some discrepancies between the two QoL tools. Compared with the rest of the population, the patients who achieved their PSO-TARGET goal, but still reported a DLQI > 1, had higher baseline PASI scores (18.6 versus 14.8; p = 0.067), higher DLQI scores (14.1 versus 10.1; p = 0.004), and a higher number of hard-to-treat locations (median of 2 versus 1; p = 0.004). In addition, patients who had not reached their PSO-TARGET goal but reported a DLQI ≤ 1, all had psoriasis on the scalp at the baseline and were generally younger (median of 31 versus 52 years, p = 0.001).
Hintergrund: Clobetasol zeigt bei der Behandlung von Melasma innerhalb kurzer Zeit bemerkenswerte Ergebnisse. Aufgrund möglicher lokaler Nebenwirkungen ist seine Anwendung jedoch eingeschränkt. Bisher liegt keine kontrollierte Studie zur sequentiellen Anwendung von Clobetasol und Hydrochinon bei Melasma vor. Ziel dieser Untersuchung war es, die Verträglichkeit und Wirksamkeit einer Kombinationstherapie mit 0,05 % Clobetasol gefolgt von 4 % Hydrochinon (CLOB-HQ) im Vergleich zur alleinigen Anwendung von 4 % Hydrochinon (HQ) zu bewerten. Methodik: In einer randomisierten, doppelt verblindeten klinischen Studie wurden 50 Frauen mit Melasma im Gesicht eingeschlossen. Die Teilnehmerinnen der CLOB-HQ-Gruppe verwendeten über 14 Tage hinweg täglich abends 0,05 % Clobetasol, gefolgt von einer 46-tägigen Anwendung von 4 % Hydrochinon. Die Vergleichsgruppe (HQ) erhielt über den gesamten Zeitraum von 60 Tagen ausschließlich Hydrochinon. Die Beurteilungen erfolgten zu Beginn sowie nach 14 und 60 Tagen anhand des modifizierten Melasma Area and Severity Index (mMASI), der Melasma-spezifischen Lebensqualitätsskala (MELASQoL) und der Farbmetrik. Zusätzlich wurde die Global Aesthetic Improvement Scale (GAIS) von einer unabhängigen, verblindeten Fachperson bewertet. Ergebnisse: Zwischen den beiden Gruppen zeigten sich zu den Zeitpunkten Tag 14 und Tag 60 keine signifikanten Unterschiede in den Hauptparametern (P > 0,1). In der CLOB-HQ-Gruppe betrug die durchschnittliche Reduktion des mMASI 13,2 % (95 %-KI: 5,1–21,3 %) nach 14 Tagen und 43,1 % (32,2–54,0 %) nach 60 Tagen. In der HQ-Gruppe lagen die Werte bei 10,6 % (5,9–27,5 %) und 44,8 % (33,2–52,3 %). Sowohl die Lebensqualität (MELASQoL), die Hauthelligkeit (Farbmetrik) als auch die ästhetische Gesamtbewertung (GAIS) verbesserten sich in beiden Gruppen kontinuierlich, ohne dass signifikante Unterschiede festgestellt wurden. Schwerwiegende Nebenwirkungen traten nicht auf. Insbesondere wurden keine Fälle von Teleangiektasien, Hautatrophie oder perioraler Dermatitis im Zusammenhang mit Clobetasol beobachtet.
Hintergrund und klinische Bedeutung: Akne vulgaris ist eine häufige entzündliche Hauterkrankung, die klassischerweise mit einer Überproduktion von Talg, follikulärer Hyperkeratinisierung und der Proliferation von Cutibacterium acnes in Verbindung gebracht wird. Neue Erkenntnisse deuten auf einen Zusammenhang zwischen schwerer oder behandlungsresistenter Akne und dem metabolischen Syndrom hin, das durch zentrale Adipositas, Insulinresistenz, Dyslipidämie und Bluthochdruck gekennzeichnet ist. Ziel dieser Fallserie ist es, die klinische Überschneidung zwischen Akne und Stoffwechselstörungen zu untersuchen und die Bedeutung einer multidisziplinären Beurteilung hervorzuheben. Falldarstellung: Drei Patient*innen mit schwerer Akne vulgaris und koexistierenden Stoffwechselanomalien wurden zwischen 2023 und 2024 in einer dermatologischen Klinik in Oradea, Rumänien, untersucht. Jede*r Patient*in wurde dermatologisch untersucht, im Labor auf metabolische und hormonelle Parameter getestet und individuell behandelt. Die Behandlungsstrategien umfassten topische und systemische Akne-Therapien in Kombination mit metabolischen Interventionen (Änderung des Lebensstils, Metformin [in zwei Fällen] und lipidsenkende Mittel). Fall 1 (weiblich, 23 Jahre) litt an Adipositas, Insulinresistenz, Dyslipidämie und polyzystischem Ovarialsyndrom (PCOS). Fall 2 (männlich, 19 Jahre) litt an zentraler Adipositas und atherogener Dyslipidämie. Fall 3 (männlich, 18 Jahre) zeigte Insulinresistenz, Übergewicht und erhöhte Entzündungsmarker. Alle 3 zeigten ein suboptimales Ansprechen auf die Standard-Aknebehandlung. Die zusätzliche Stoffwechselbehandlung führte innerhalb von drei Monaten zu einer teilweisen Verbesserung. Ein Patient benötigte nach der metabolischen Stabilisierung eine Behandlung mit Isotretinoin. Schlussfolgerungen: Diese Fälle unterstreichen das Zusammenspiel zwischen Akne und Stoffwechselstörungen. Insulinresistenz und systemische Entzündungen können zur therapeutischen Resistenz bei Akne beitragen. Die frühzeitige Erkennung von Merkmalen des metabolischen Syndroms bei Patient*innen mit schwerer Akne kann die Behandlungsergebnisse verbessern. Dermatolog*innen sollten ein metabolisches Screening in Betracht ziehen, um eine umfassende, multidisziplinäre Versorgung zu ermöglichen.
Background: A multitude of factors may influence fatigue in psoriasis and psoriatic arthritis (PsA); however, their individual fatigue components have not been thoroughly examined. Objectives: To explore characteristics of fatigue and its potential drivers in a cohort of patients with psoriasis with or without PsA. Methods: Adults with psoriasis and a nonpsoriasis control group completed the Multidimensional Fatigue Inventory-20 questionnaire. Patients with psoriasis also reported joint pain intensity, pruritus, skin pain, and sleep problems using a numerical rating scale. Linear regression models were applied to continuous outcomes, and beta coefficients (β) for the slopes were estimated with 95% confidence intervals (CIs). Results: Among 2741 adults with psoriasis (of which 593 also had PsA) and 3788 controls, the impact on total fatigue was greatest for PsA (β = 5.22; 95% CI, 3.55–6.90), followed by psoriasis (β = 2.10; 95% CI, 0.96–3.25), compared with the general population (Ptrend < .0001). Among patients with psoriasis with or without PsA, increasing joint pain intensity was associated with overall fatigue (β = 2.23 [95% CI, 2.03–2.44] for each 1-point increase in joint pain numerical rating scale score). Limitations: We lacked information on the effect of pharmacotherapy.
Introduction: Bullous pemphigoid (BP) and prurigo nodularis (PN) are chronic pruritic skin diseases that severely impact patients’ quality of life. Despite the widespread attention these two diseases have garnered within the dermatological field, the specific pathogenesis, particularly the molecular mechanisms underlying the pruritus, remains largely unclear. Limited clinical sequencing studies focusing on BP and PN have hindered the identification of pathological mechanisms and the exploration of effective treatment strategies. Methods: To address this gap, we collected a total of 23 peripheral blood mononuclear cell samples from BP and PN patients, as well as healthy controls, and performed RNA sequencing analysis. By integrating bioinformatics and machine learning techniques, we aimed to uncover the shared immune regulatory networks and pruritus-related mechanisms between BP and PN. Results: Our study identified 161 differentially expressed genes shared between BP and PN, which were primarily enriched in immune activation and neural pathways, providing crucial molecular insights into the pruritus-related mechanisms of both diseases. Furthermore, using the machine learning algorithms of support vector machines and random forest, we pinpoint 7 crucial genes shared between the BP and PN databases. Among these, IL-27 emerged as a potential pivotal gene, as its mRNA expression levels strongly correlated with clinical parameters including pruritus scores, immunoglobulin E levels, and eosinophil counts. Validation experiments conducted on clinical samples from an additional 22 participants confirmed the upregulation of IL-27 expression in both BP and PN lesions.
Interferon-alpha ist ein bewährtes Medikament zur Behandlung von kutanen T-Zell-Lymphomen (CTCL). Da die bisher zugelassene Form (IFN-α2a) seit Anfang 2020 nicht mehr hergestellt wird, greifen Ärztinnen und Ärzte zunehmend auf eine alternative, sogenannte pegylierte Form (pegIFN-α2a) zurück – obwohl diese offiziell nicht für CTCL zugelassen ist. In einer rückblickenden Studie, an der zwölf Hautkliniken in Deutschland beteiligt waren, wurden die Daten von 70 Patientinnen und Patienten mit CTCL ausgewertet. Die Mehrheit litt an Mycosis fungoides, ein kleinerer Teil am Sézary-Syndrom. Die Behandlung mit pegIFN-α2a zeigte bei über der Hälfte der Betroffenen eine positive Wirkung. Allerdings musste die Therapie bei der Hälfte der Patient:innen nach durchschnittlich gut einem Jahr abgebrochen werden – meist wegen Nebenwirkungen. Diese traten bei fast 70 % auf, etwa ein Drittel davon waren schwerwiegend. Besonders häufig kam es zu Veränderungen im Blutbild, starker Müdigkeit und Problemen mit der Leber.
Background: Dupilumab, a human monoclonal antibody targeting the interleukin 4 alpha receptor, is used for treatment of moderate to severe atopic dermatitis (AD). Previous studies have reported diagnoses of cutaneous T cell lymphoma (CTCL) after dupilumab use. Objective: Investigate the risk of CTCL after dupilumab use in patients with AD. Methods: Using the TrinetX database, incidence of cutaneous and lymphoid malignancies including CTCL was compared between a cohort of patients with AD who used dupilumab and a cohort of patients with AD who never used dupilumab. A second analysis excluding prior disease-modifying antirheumatic drug use was performed. Propensity score matching was performed to control for covariates. Results: An increased risk of CTCL was found in the cohort of AD patients who used dupilumab (odds ratio 4.1003, 95% confidence interval 2.055-8.192). The increased risk persisted after exclusion of prior disease-modifying antirheumatic drug use. Risk was not increased for other cutaneous or lymphoid malignancies. Most (27/41) cases of CTCL were diagnosed more than 1 year after dupilumab use. Limitations: There is potential for misclassification in the database. Severity of AD could not be assessed. Association between dupilumab and CTCL does not prove causality.
The number of primary cutaneous lymphoma patients receiving low-dose radiotherapy is increasing, though controlled clinical trials defining the standard radiation dose for each specific entity have not yet been completed. Radiation oncologists are left with making highly individualized decisions that would be better enriched by additional clinical evidence. In this expert opinion, we aim to provide a clear recommendation to improve the current practice of radiation oncology. In addition, existing literature has been reviewed to develop recommendations for all types of primary cutaneous lymphoma. A prospective trial is urgently needed to identify the factors influencing patient outcomes following different radiation doses.
Background: No currently approved treatment for paediatric plaque psoriasis selectively targets interleukin (IL)-23. In adults, guselkumab (a selective IL-23 inhibitor that targets the p19 subunit) demonstrated substantial efficacy with a favourable safety profile in treating moderate-to-severe plaque psoriasis. Objectives: To evaluate the efficacy and safety of guselkumab in paediatric patients with moderate-to-severe plaque psoriasis (PROTOSTAR; NCT03451851). Methods: This phase III randomized placebo-controlled study enrolled patients aged ≥ 6 to < 18 years with moderate-to-severe plaque psoriasis. In part 1 [week (W)0-W16], patients were randomized to receive guselkumab, placebo or open-label etanercept (active reference arm). At W16, part 1 patients entered a guselkumab withdrawal/retreatment period or continued/crossed over to receive guselkumab (W16-W52). Co-primary endpoints were Investigator Global Assessment (IGA) 0/1 and ≥ 75% improvement in Psoriasis Area and Severity Index (PASI 75) [or U.S. Food and Drug Administration-required ≥ 90% improvement in PASI (PASI 90) co-primary endpoint] responses at W16 of part 1. Part 2 evaluated continuous open-label guselkumab treatment (W0-W52). Results: Of 92 and 28 patients enrolled in parts 1 and 2, respectively, 86% and 96% continued treatment through W52. In part 1, at W16, significantly higher proportions of guselkumab-treated compared with placebo-treated patients achieved IGA 0/1 (66% vs. 16%; P < 0.001), PASI 75 (76% vs. 20%; P < 0.001) and PASI 90 (56% vs. 16%; P < 0.01). More than one-third of guselkumab-treated patients achieved clear skin [IGA 0: 39% vs. 4% placebo; 100% improvement in PASI (PASI 100): 34% vs. 0% placebo; both P < 0.01]. In part 2, at W52, 86%, 93% and 82% of guselkumab-treated patients achieved IGA 0/1, PASI 75 and PASI 90, respectively. Through W16 of part 1, 42%, 68% and 58% of guselkumab-, placebo- and etanercept-treated patients, respectively, experienced adverse events (AEs). Rates of AEs with guselkumab were similar through W52 in parts 1 and 2; common AEs included nasopharyngitis, upper respiratory tract infection and COVID-19. No serious or opportunistic infections occurred.
Die Zahl der Patienten mit primär kutanem Lymphom, die eine niedrigdosierte Strahlentherapie erhalten, nimmt zu, obwohl kontrollierte klinische Studien zur Festlegung der Standard-Strahlendosis für jede spezifische Entität noch nicht abgeschlossen sind. Strahlentherapeuten müssen hochgradig individualisierte Entscheidungen treffen, die durch zusätzliche klinische Evidenz besser untermauert wären. Mit diesem Gutachten möchten wir eine klare Empfehlung zur Verbesserung der derzeitigen Praxis in der Radioonkologie geben. Darüber hinaus wurde die vorhandene Literatur durchgesehen, um Empfehlungen für alle Arten von primär kutanem Lymphom zu entwickeln. Eine prospektive Studie ist dringend erforderlich, um die Faktoren zu identifizieren, die das Patientenergebnis nach unterschiedlichen Strahlendosen beeinflussen.
