Introduction: Atypical glandular cells (AGC) are rare but clinically significant due to their association with cervical and endometrial lesions. Although human papillomavirus (HPV) is the main etiological agent of cervical lesions, not all AGC are HPV-related, particularly those of endometrial origin. Given the heterogeneity of associated findings, their characterization is essential to guide clinical management. Objective: To describe the distribution of AGC subcategories and evaluate subsequent diagnostic findings, comparing findings between AGC (not otherwise specified) NOS and AGC (favor neoplastic) FN in women evaluated at a Brazilian university hospital. Methods: This is an observational, retrospective, analytical study with a retrospective cohort design, based on the review of cytology reports with an AGC diagnosis and their subsequent diagnostic investigation. Associations between categorical variables were assessed using Fisher’s exact test, with a significance level of 5%. Results: Among 22,246 cytology samples, 161 showed AGC (0.72%). After exclusions, 126 cases were included: 85.7% AGC-NOS and 14.3% AGC-FN. Neoplasia at any site was identified in 22.2% of women and was more frequent in AGC-FN (55.6%) than in AGC-NOS (16.7%) (p=0.001). A statistically significant difference was observed in the distribution of cervical findings between groups (p=0.003), with a higher proportion of neoplastic lesions in AGC-FN. No significant difference was found for endometrial findings (p=0.400). Endocervical cytology was performed in 68 women, showing a significant association between the initial AGC classification and subsequent cytological findings (p=0.003), although 44.4% of AGC-FN cases had negative results. Conclusion: AGC represents a heterogeneous clinical entity, with a higher frequency of neoplastic findings in AGC-FN compared to AGC-NOS, particularly in the cervix. These findings reinforce the importance of a comprehensive diagnostic investigation, particularly in cases classified as AGC-FN. In settings without access to HPV molecular testing, clinical, colposcopic, and histopathological evaluation remains essential.

Introduction: Brazilian public health faces constant challenges to meet demographic and social changes, and despite current policies, rates of sexually transmitted infections, such as syphilis, especially in pregnant women and newborns, are still high. Primary Health Care is recognized as the best way to act on maternal and child health, with prenatal care being essential for the diagnosis and follow-up of pregnant women. Objective: To analyze the social and epidemiological profile of pregnant women and newborns with syphilis in the Southeast region of Brazil between 2019 and 2022. Methods: This is a descriptive and cross-sectional research, with data from Brazilian Epidemiological Indicators and the 2023 Epidemiological Bulletin of the Brazilian Ministry of Health. Results: According to the results, the Southeast region concentrates almost 50% of the cases of gestational and congenital syphilis in Brazil. Although many pregnant women receive prenatal care and are diagnosed in the first trimester, a significant proportion still receive incomplete treatment, which negatively affects the newborn. Conclusion: Syphilis continues to be a serious public health issue, especially in the Southeast, with alarming growth rates. Therefore, it is essential to implement public policies that improve the quality of prenatal care, ensuring continuous follow-up during pregnancy and in the postpartum period for effective diagnosis and treatment of syphilis.

Introduction: Syphilis is a sexually transmitted infection of high public health relevance, with increasing incidence in Brazil. Although screening protocols exist for pregnant women, there are gaps in knowledge regarding non-pregnant women. Objective: To describe the sociodemographic and behavioral profile of non-pregnant women receiving care in health services in Pelotas, Rio Grande do Sul, and the factors associated with positivity in treponemal tests for syphilis. Methods: A cross-sectional study was conducted between August 2023 and December 2024 with non-pregnant women aged 15–70 years attending outpatient clinics and Primary Health Care Units affiliated with the Federal University of Pelotas. Sampling was non-probabilistic. Sociodemographic, behavioral, and syphilis-related knowledge data were collected using a standardized questionnaire and rapid treponemal tests. Descriptive analyses and chi-square tests were performed (p<0.05). Results: A total of 2,021 women were included (mean age: 32.6 years; standard deviation: 8.3 years). The prevalence of reactive treponemal tests was 3.2% (64/2,021). An educational gradient was observed, with higher prevalence among women with lower schooling levels (4.7% in incomplete primary education vs. 0.85% in higher education; p<0.05). Single women and those in relationships with multiple partners showed higher prevalence. No significant differences were observed across age groups. Conclusion: The high prevalence of syphilis among non-pregnant women, particularly those with lower educational attainment, reinforces the need for screening and prevention strategies beyond the prenatal context.

This Editor's Letter, which some may interpret as a manifesto, is about the need to always recognize the progress made by public administrators. However, it is necessary, perhaps fundamental, to demand better public health actions for all population groups, continuously. Here, we discuss equality, equity, and effectiveness in access to HPV vaccination within the Brazilian Unified Health System (SUS). Here, we make demands regarding the updating of modern active pharmaceutical ingredients, the same ones used for populations in countries with a high Human Development Index (HDI). After all, Brazil will join this group of nations in 2026. Here, we recount history, memory, humanities, academic life trajectory, along with a tangle of extension activities and positions on public health policies. In this Letter, there are no restrictions on expressing concerns.
Introduction: Women living with HIV/AIDS (WLHA) are more vulnerable to human papillomavirus (HPV) infection, particularly by high-risk oncogenic subtypes, due to HIV-associated immunosuppression. Objective: To assess the prevalence and distribution profile of HPV subtypes in WLHA, correlating findings with CD4 levels and age group. Methods: A cross-sectional study was conducted with 184 WLHA receiving care at a public referral center in Juiz de Fora, Minas Gerais, from May 2021 to May 2022. Self-collected vaginal samples were analyzed by RT-PCR to detect HPV subtypes. Associations between HPV infection, CD4 counts, and age were assessed using the chi-square test (p<0.05). Results: HPV infection was detected in 72% of the participants. Oncogenic subtypes not included in the quadrivalent vaccine were identified in 49.3% of the infected individuals, with HPV 52 and 58 being the most prevalent. A higher frequency of infection was observed in women with CD4 counts <500 cells/mm³, although no statistical association was found between CD4 levels and specific viral types. There was no significant association between age and the prevalence of major subtypes. Conclusion: WLHA show a high prevalence of HPV, particularly high-risk subtypes not covered by the quadrivalent vaccine. The findings support expanding immunization with the nonavalent vaccine and strengthening cervical cancer screening and control strategies in this population.
Despite more than a century of investigation, syphilis vaccine development has long been hindered by the unusual outer membrane of Treponema pallidum subsp. pallidum (TPA) and the historical inability to propagate syphilis spirochetes in vitro. Early observations using the rabbit model established that protective, antibody-mediated immunity is achievable. The recent definition of the repertoire of TPA outer membrane proteins (OMPs) defined the universe of potential targets for protective antibodies and provided a critical foundation for the ongoing syphilis vaccine development. Built on a “learning from nature” approach, the mapping of antibody responses elicited during natural infection against OMPs allowed the identification and prioritization of extracellular loops (ECLs) as vaccine targets. Immunization of animals with protein scaffolds displaying these targets generates high titers of antibodies able to recognize surface-exposed regions of the spirochete. Recent advances in long-term in vitro cultivation and genetic manipulation of TPA have enabled the development of assays to directly evaluate the functional activity of ECL-specific antibodies in promoting opsonophagocytosis, growth inhibition, impairment of motility, and outer membrane disruption. Next-generation platforms are being explored to enhance immunogenicity, simplify production, and facilitate scalable translation of these immunogens toward clinical evaluation. In parallel, researchers are uncovering the sequence variability in OMPs across circulating TPA strains to understand how mutations can affect antibody recognition and global vaccine efficacy. Collectively, these advances position the field to leverage structural, immunological, and microbiological insights to counter the stealth pathogen and, ultimately, achieve an effective syphilis vaccine.

Introduction: Cytomegalovirus (CMV) is a widely disseminated virus, capable of remaining latent and reactivating, with high global seroprevalence. In Brazil, seroprevalence exceeds 95%, with primary infections predominating in childhood. Objective: To describe the clinical profile at birth and outcome at 12 months of age of patients diagnosed with congenital cytomegalovirus infection in a reference pediatric hospital in Florianópolis from 2017 to 2022. Methods: This was a retrospective, observational and descriptive study with secondary data analysis. The research was approved by the Human Research Ethics Committee. Results: Of the 20 cases included in the study, 12 (60%) were female and all were white coloured. 14/17 (82.3%) of the cases were full-term births and 14/16 (87.5%) had a low birth weight for gestational age. Jaundice was identified in 7/13 (53.8%), microcephaly in 5/18 (27.7%), conjugated hyperbilirubinemia in 5/8 (62.5%) and intracranial calcifications in 9/20 (45%) records. 6/20 (30%) cases failed the neonatal hearing screening and 5/20 (25%) presented bilateral loss of brainstem auditory evoked potential. At 12 months of age, bilateral hearing loss and microcephaly were found in 5/8 (62.5%) and 7/13 (53.8%) cases respectively. Conclusion: Low birth weight for gestational age, jaundice and microcephaly were prevalent changes. Intracranial calcifications were the most common neuroimaging finding. Important late sequelae were present at 12 months of age.
