
The morphological and functional changes of the eighth nerve in patients undergoing cerebello-pontine angle (CPA) surgery are under investigation in the ENT Department of the University of Verona. The main aspects of the research are: ultrastructural examination of the eighth nerve in normals and subjects operated on for vestibular or cochleo-vestibular neurectomy and for acoustic neuroma; monitoring cochlear nerve function during CPA surgery by monopolar and bipolar recording of cochlear nerve action potential (CNAPs). The specific aims of this research are: to acquire information on the pathophysiology of a number of peripheral cochleo-vestibular disorders; to evaluate the role of intraoperative recording of the eighth nerve in preserving hearing.
The authors present two microsurgical options of the reconstruction of the lower third of the face. They outline a systematisation of indications and techniques and emphasise versatility and flexibility of free flaps compared to pedicled flaps, and describe the advantages of the vascularisation and the techniques of the iliac crest osteocutaneous flap and the parascapular cutaneous-latissimus dorsi osteomyocutaneous flap which was developed by Nassif.
Wegener's Granulomatosis occurs rarely. It is an autoimmune disease and as such it is a special form of panarteriitis nodosa. Manifestations are seen in various organs. Granulomatous lesions in the nose, paranasal sinuses and pharynx are very characteristic and often mark the beginning of the disease. However, a painful otitis media with combined hearing loss may also be the first sign of this generalised rheumatic disease. By describing three cases we document clinical constellations of results and summarise them in tabular form. The knowledge of these constellations prevents confusing this disease with otitis media, tuberculosis and sudden deafness.
Zwischen 1978 und 1985 wurden an der Mainzer HNO-Klinik der Universität 30 Patienten mit Paragangliomen im Kopf-Hals-Bereich operativ behandelt. Das gewonnene Gewebe wurde neben den histologischen Routinemethoden auch mit Antikörpern gegen Zellstrukturen untersucht. Verwendung fanden dabei Antikörper gegen Gefäßendothelzellen (UEA-I), neuronale Strukturen, wie Glia- oder Schwannsche Zellen (S-100) und NSE als Marker für endokrin aktive Zellen. Auf Grund der immunhistologischen Befunde wurde die Einordnung der Paragangliome in die histologischen Untergruppen sehr erleichtert. Bei Vergleich von Klinik und Histologie wurde deutlich, dass sich die schweren klinischen Verläufe mit agressivem Wachstum und Filialisierung in der Gruppe der adenomatösen Variante häufen.
Bis Anfang der achtziger Jahre wurden an der Frankfurter Universitätsklinik für Hals-Nasen-Ohrenleiden laterale Mittelgesichtsfrakturen durch Drahtosteosynthese und Einlage von Kunststoffspãnen in die betroffene Kieferhöhle versorgt. tYber Miniplattenosteosynthesen von Mittelgesichtsfrakturen wurde nach unserer Kenntnis erstmals von Michelet (1973) berichtet. Von 1983 bis 1986 führten wir an unserer Klinik Miniplattenosteosynthesen bei Jochbeinfrakturen aller Schwierigkeitsgrade mit dem Ziel durch, die Einsatzmoglichkeiten dieser Technik im Vergleich mit den herkbmmlichen Techniken zu prüfen.
