Needle-free connectors (NFCs) were introduced to eliminate the use of needles in intravascular catheters, and their newest generations were designed to improve patient safety and reduce catheter-related bloodstream infection (CRBSI) risks. The aim of this meta-analysis was to compare NFCs with 3-way stopcocks (3WSCs) and their effects on CRBSI rates.A meta-analysis was conducted using a research protocol consistent with the PRISMA statement for reporting meta-analyses. The Cochrane Database of Systematic Reviews and MEDLINE were searched for relevant randomized studies published from January 2000 to September 2018.We identified and selected for the meta-analysis 8 studies comparing CRBSI rates (according to the Centers for Disease Control and Prevention's National Healthcare Safety Network definition) associated with NFCs utilizing negative-displacement, neutral-displacement, or positive-displacement devices with rates for 3WSCs. Relative risk was 0.53 with a 95% CI of 0.28 to 1.00, and the relative difference was –0.018 with a 95% CI of –0.039 to 0.004.CRBSI risk was statistically higher for 3WSCs compared to NFCs.
L'arsenic, le cadmium, le mercure et le plomb, sont des éléments ubiquitaires qui vont inéluctablement contaminer les aliments. Ils peuvent induire divers effets toxiques à faible niveau d'exposition, via le régime alimentaire. Le risque pour le consommateur est évalué avec une précision croissante ces dernières années, grâce aux études du régime total. Bien que les apports alimentaires de ces éléments se situent en-deçà des valeurs de référence, les marges de sécurité apparaissent faibles pour certains groupes de population. Le choix d'aliments, de nature et de provenance variées, peut contribuer à réduire l'exposition.Arsenic, cadmium, mercury and lead are widespread elements which unavoidably contaminate foods. They may elicit various toxic responses at low exposure level through the diet. Ensuing risk for the consumer has been assessed with growing accuracy in recent years owing to total diet studies. Although dietary intakes for these elements remain below tolerable reference values, margins of safety appear tenuous for some population groups. Varying both types and origins of foodstuff may contribute to lower exposure.
Les fibronectines sont des glycoprotéines, de masse molaire élevée, de demi-vie d'environ 24 heures, qui appartiennent à la famille des glycoprotéines de structure ; on en connaît actuellement au moins 12 isoformes. Elles sont présentes, sous forme soluble, dans le plasma et les autres liquides biologiques et, sous forme insoluble, dans la matrice extracellulaire et certaines membranes basales (fibronectine cellulaire). La fibronectine soluble, formée de 2 sous-unités reliées par 2 ponts disulfure, présente une structure compacte. La fibronectine cellulaire est une forme polymérisée qui possède une structure fibrillaire. La forme plasmatique peut être incorporée à la matrice extracellulaire. Chaque sous-unité de fibronectine est composée d'une série de domaines structuraux, chacun contenant des sites de liaison responsables d'interactions avec d'autres molécules (collagène, fibrine, héparine…) ou avec des cellules (fibroblastes, plaquettes…). Les fibronectines sont des protéines multifonctionnelles qui jouent des rôles biologiques variés. Elles interviennent dans la structure de la matrice extracellulaire et dans les interactions cellules/matrice. Elles sont également impliquées à différents niveaux de l'hémostase et de la réparation tissulaire. La forme plasmatique se comporterait comme une opsonine non spécifique. Son taux varie dans le plasma en fonction de l'âge et du sexe. Une baisse constante est signalée dans les cas de coagulation intravasculaire disséminée. La fibronectinémie diminue aussi au cours de la dénutrition aiguë et sa normalisation permet d'apprécier l'efficacité à court terme d'une assistance nutritionnelle. Une diminution de la fibronectine plasmatique se produit 6 à 24 heures après une agression compliquée ou non d'infection et la persistance d'un taux bas est de mauvais pronostic. Dans certaines formes de lésions vasculaires, on a pu, grâce à des anticorps monoclonaux, mettre en évidence une augmentation dans le plasma d'une isoforme cellulaire soluble (ED1+) alors que la fibronectine totale ne variait pas. La diminution ou l'absence de fibronectine à la surface des cellules tumorales pourrait induire un changement de morphologie et un changement de comportement de ces cellules, pouvant expliquer en partie leur multiplication anarchique et leur potentiel invasif et métastatique. Enfin, la fibronectine serait impliquée dans l'athéro-artériosclérose et dans les micro et macro-angiopathies chez les diabétiques.
Les résultats obtenus avec différents protocoles thérapeutiques appliqués à 651 patients ayant ingéré un produit corrosif ont été analysés. Dans 617 cas, la conduite thérapeutique suivie comprenait, à l’exclusion de tout autre traitement, la séquence suivante : endoscopie précoce - nutrition artificielle dans les formes les plus graves - chirurgie réparatrice en phase cicatricielle. Dans les autres cas, les patients avaient été traités de manière conventionnelle. Mille quatre cent vingt et un examens endoscopiques ont été pratiqués, avec une fréquence d’accidents de 0,6 % des cas. Trois cent quatre vingt huit patients ont été soumis à une nutrition artificielle pendant une période de 20 à 120 jours. La fréquence des sténoses oesophagiennes serrées a été de 20 % des cas, avec ou sans dilatation préventive. Sous nutrition artificielle, seules les formes les plus graves (stades III) ont présenté une telle complication et ce dans 50 % des cas. Une colonoplastie de l’œsophage a été pratiquée chez 81 malades. La mortalité globale a été de 6 % des cas. Le traitement coordonné a permis de maintenir une mortalité à un niveau faible, mais a laissé persister un nombre relativement important de sténoses cicatricielles. Les dilatations ne devraient être réservées qu’aux sténoses oesophagiennes peu étendues.
The role of catheter material in the formation of deep venous thrombosis during parenteral nutrition has been widely emphasized. Systematic venograms show central venous thrombosis in 20 to 33% of cases with polyethylene catheters and in 4% of cases with silicone catheters. Heparin infusion through the catheter diminishes but does not totally eliminate the risk of thrombosis. The aim of this study was to define the conditions under which the risk of thrombophlebitis was minimal. Four series of experiments were carried out, each on five rabbits. Catheters were inserted into the vena cava and, after 10 days, venograms were performed. The animals were then sacrificed, and the vena cava was macroscopically and microscopically studied. Plastic catheters were used in the first series, heparin-Benzalkonium-bonded plastic catheters in the second, silicone catheters in the third, and heparin-Benzalkonium-bonded silicone catheters in the fourth. The results revealed thrombosis of the vena cava and a fibrin sleeve around the catheters in series 1; thrombosis of the vena cava in series 2; a fibrin sleeve around the catheters in series 3; and neither thrombosis nor a fibrin sleeve in series 4. In conclusion, heparin-Benzalkonium-bonded silicone catheters appear to provide the best protection against thrombophlebitis by reducing the damage caused to the intima by the catheters and by slowing down platelet-aggregation around them.
It is difficult to select and combine the nutritional parameters most useful in predicting the outcome of major gastrointestinal surgery. The aim of this study was to define retrospectively a multifactorial prognostic nutritional index adapted to this purpose. Seventy-eight patients on whom one or more total or partial visceral resection were performed in nonemergency conditions were included in this study. Statistical analysis was carried out to determine correlations between the preoperative nutritional parameters and the postoperative complications such as: a) wound rupture and anastomotic leakage; b) severe sepsis; c) death. Delayed hypersensitivity, assessed as normal or abnormal, together with plasmatic albumin and transferrin levels, the thresholds of which were respectively determined at 35 g/l and 2.2 g/l, were selected as the factors with the greatest predictive value. For the prognosis of postoperative severe complications and death, the sensitivity and specificity of an index using the association of these three parameters were respectively 82.7 p. 100 and 51 p. 100 with positive and negative predictive values of 50 p. 100 and 83.3 p. 100.
Ultrasonically guided liver puncture was carried out in 26 patients without overt carcinoma, who presented with a small intrahepatic mass giving an ultrasonographic image of primary or secondary tumour. Puncture was performed with a lumbar or a Chiba needle under local anaesthesia, and a real-time ultrasound apparatus with guide was used. The specimens collected were fixed on slides for histological examination. An aetiological diagnosis was made in 73% of the cases (16 carcinomas and 3 liver abscesses). No abnormal cells were detected and further investigations were conducted in 15%. The failure rate was 12% (2 punctures without specimens and 1 false-negative result). No infectious or haemorrhagic complication was observed.
The present study was carried out on 76 industrial workers divided in four working sections and exposed to cadmium and copper fumes and/or dust for periods ranging from 2 to 32 years. There were no signs of chronic copper poisoning associated with long term industrial exposure. In all cases indeed, normal blood copper level (near 15,5 mumol/l) and normal urine copper level (53 mumol/mol creat) were found. Moreover, in the plasma, the copper/ceruloplasmin ratio was approximately 8, with both normal values in micromoles. The great elimination of copper in feces assured probably the copper homeostasis. On the contrary, particularly near the furnace, the atmospheric cadmium level was to be considered; 9 out of 11 furnacemen who worked during more than 5 years showed neither respiratory or hepatic failure nor bone mineral metabolism disturbances, in spite of urine cadmium level greater than 10 mumol/mol creat, "early warning" for renal damage. There was no significant correlation between urine and blood cadmium levels when the latter was greater than 4,5 micrograms/l (40,2 nmol/l). The high beta 2 microglobulin level for 66 p. cent of furnacemen, was the indicator of tubular damage. Therefore the urine cadmium in combination with beta 2m should be determined to detect early effects of cadmium in exposed workers.
