Objective To investigate P300 potentials in patients with schizophrenia, and the influence of risper-idone and clozapine on P300. Methods A total of 63 inpatients with schizophrenia were randomly assigned to risperidone group(n=32) or clozapine group (n=31) for the double-blind treatment of 6 weeks. Positive and Negative Symptom Scale(PANSS) was used to assess efficacy. The patients and normal controls were measured auditory event-related potential P300. Results After 6-week treatment, the total scores and factor-scores of PANSS in two groups were decreased significantly, and there were no significant differences between the two groups. Compared to normd controls, the patients of two groups had significantly longer latency periods of N1,P3,and P3-N1 ,and lower amplitude of P3. There were no significant differences in P300 between two groups be-fore and after treatment. In elozapine group, the score of positive symptoms in PANSS had significantly nega-tive relation with latency periods of P3 and P3-N1, and the score of negative symptoms had positive relation with latency period of P3. While no significant relations were found between scores of PANSS and P300 in ris-peridome group. Conclusion Risperidone and clozapine both have beneficial effect for patients with schizophre-nia. The patients would have obvious abnormalities of P300. Risperidone could have no effects on P300, while clozapine may influence latency of P300.
Objective To investigate the influence of risperidone and clozapine on cognitive function in patients with schizophrenia using computer maze test and WSCT. Methods A total of 63 inpatients who met the diagnostic criteria of CCMD-2-R for schizophrenia were randomly assigned to risperidone group(n=32) or clozapine group (n=31) for the double-blind treatment of 6 weeks. Positive and Negative Symptom Scale(PANSS) was used to assess efficacy. Treatment Emergent Symptoms Scale (TESS)and Rating Scale for Extrapyramidal Side Effects (RSESE) were used to assess the side effects. Cognitive function was assessed with computer maze test and WSCT. Results At the 2nd weekend, performance of maze test was related to performance of WSCT, while performance of complex maze test was related to number of completed classification and persistent errors in WSCT at the 6th weekend. There were significant improvements in cognitive function in both groups. Score of negative symptoms subscale had significant correlation with cognitive performance, but the improvements in positive symptoms had significant correlation with the improved performance of some cognitive tests. Conclusion Computer maze test is a better tool for the assessment of cognition. Risperidone and clozapine both have beneficial effect on cognitive function of patients with schizophrenia.

目的:探讨阿尔茨海默病与老年精神分裂症患者P300的差异及P300在阿尔茨海默病患者认知功能障碍严重程度判断中的价值.方法:采用电生理技术检测2000-12/2003-12山东省精神卫生中心住院及门诊阿尔茨海默病患者38例及老年精神分裂症患者40例的P300.应用简明精神状态量表评定阿尔茨海默病患者的认知功能.结果:阿尔茨海默病患者P300检测结果中在Fz[(402.50±3.77)ms]、Cz[(425 81±1.90)ms]和Pz[(366.31±1.61)ms]3点P300潜伏期与老年精神分裂症患者相比均明显延长,差异有显著性(t=4.23,2.79,4.56;P均<0 01);在Fz[(5.05±0.65)μV]、Cz[(3.93±0.56 pV]和Pz[(7.97±0.73)μV] 3点波幅明显减低,与老年精神分裂症患者比较,差异有显著性(t=4.02,3.75,2.85;P均<0.01).阿尔茨海默病患者P300在Fz,Cz及Pz3点潜伏期与简明精神状态量表分数显著负相关(r=-0.78,-0.76,-0.77,P均<0.01);在Fz,Cz及Pz 3点波幅与简明精神状态量表分数显著正相关(r=0.77,0.78,0.75;P均<0.01).结论:阿尔茨海默病患者P300在Fz、Cz与Pz3点潜伏期延长及波峰减低的现象与老年精神分裂症患者的差异以及与简明精神状态量表分数的关系,有助于对阿尔茨海默病患者认知功能受损程度的判断.
