AIMS:The aims of this study were to evaluate and compare the pharmacokinetic profiles and establish bioequivalence of test and reference teriflunomide tablets (Aubagio) in healthy Chinese male subjects under fasting and fed conditions. MATERIALS AND METHODS:Subjects were randomly assigned to either the fasting or the fed group and also to one of the two treatment sequences (test-reference or reference-test), according to which they received a single 14-mg dose of the test or reference teriflunomide tablet in the study periods. During each period, blood samples were collected at pre-dose and at intervals up to 72 hours after dosing. After 72 hours post dose, an accelerated elimination procedure using cholestyramine 4 g t.i.d. PO was done. Plasma concentrations of teriflunomide were determined by liquid chromatography-tandem mass spectrometry. The safety of both tablets was monitored throughout the study. RESULTS:48 subjects were enrolled, and all completed the study, with 24 participants each in the fasting and fed groups. In both groups, the 90% confidence intervals for AUC0-72h and Cmax were within the acceptable bioequivalence range (80 - 125%). There were no significant differences in adverse event (AE) reporting between the subjects receiving test or reference tablet. No serious AEs occurred during the study period. CONCLUSION:The test teriflunomide tablet was pharmacokinetic bioequivalent to the reference teriflunomide tablet (Aubagio) in healthy Chinese male subjects under both fasting and fed conditions. Both formulations were well tolerated by all study participants.

During each period, blood samples were collected at pre-dose and at intervals up to 48 hours after dosing. Plasma concentrations of metoprolol were determined by liquid chromatography. The safety of both ER tablets was monitored throughout the study.RESULTS:60 subjects were enrolled and all completed the study, with 30 participants each in the fasting and fed groups. In both groups, the 90% confidence intervals for AUC0-48h, AUC0-inf, and Cmax were within the acceptable bioequivalence range (80 - 125%). There were no significant differences in adverse event (AE) reporting between the subjects receiving test or reference ER tablet. No serious AEs occurred during the study period.CONCLUSION:The test metoprolol ER tablet was bioequivalent to the reference metoprolol ER tablet (Betaloc ZOK) in healthy Chinese subjects measured under both fasting and fed conditions. Both formulations were well tolerated by all study participants.
Clozapine is the effective therapy for treatment-refractory schizophrenia. However, the use of clozapine is limited by its adverse effects. As propranolol is frequently used for the prevention and treatment of clozapine-induced tachycardia, we performed a meta-analysis to evaluate the effects of propranolol on steady state pharmacokinetics of clozapine in schizophrenic patients. We included 16 retrospective studies on the effects of propranolol on steady state pharmacokinetics of clozapine in schizophrenic patients, with data from both generic and brand name treatment phases in eight clozapine bioequivalence studies conducted in a single center in China from 2018 to 2022. The neurotoxicity of Semen Strychni has been reported recently in several clinical cases. Therefore, this study was conducted to investigate the role of HMGB1 in a model of neurotoxicity induced by Semen Strychni and to assess the potential alleviating effects of glycyrrhizic acid (GA), which is associated with the regulation of HMGB1 release. Forty-eight SD rats were intraperitoneally injected with Semen Strychni extract (175 mg/kg), followed by oral administration of GA (50 mg/kg) for four days. After treatment of SS and GA, neuronal degeneration, apoptosis, and necrosis were observed via histopathological examination. Inflammatory cytokines (TNF-α and IL-1β), neurotransmitter associated enzymes (MAO and AChE), serum HMGB1, nuclear and cytoplasmic HMGB1/ph-HMGB1, and the interaction between PP2A, PKC, and HMGB1 were evaluated. The influence of the MAPK pathway was also examined. As a result, this neurotoxicity was characterized by neuronal degeneration and apoptosis, the induction of pro-inflammatory cytokines, and a reduction in neurotransmitter-metabolizing enzymes. In contrast, GA treatment significantly ameliorated the abovementioned effects and alleviated nerve injury. Furthermore, Semen Strychni promoted HMGB1 phosphorylation and its translocation between the nucleus and cytoplasm, thereby activating the NF-κB and MAPK pathways, initiating various inflammatory responses. Our experiments demonstrated that GA could partially reverse these effects. In summary, GA acid alleviated Semen Strychni-induced neurotoxicity, possibly by inhibiting HMGB1 phosphorylation and preventing its release from the cell.
AIM: To evaluate the bioequivalence of cinacalcet hydrochloride tablets in healthy Chinese volunteers. METHODS: A randomized, open, double-period and crossover trial was conducted, 48 healthy volunteers were administered a single dose of cinacalcet test tablets or reference tablets orally under each fasting and fed condition. The concentration of cinacalcet was determined by validated LC-MS/MS method. Pharmacokinetic parameters were calculated by Phoenix WinNonlin 8.0 to study its bioequivalence. RESULTS: The main pharmacokinetic parameters of test tablets and reference tablets under fasting condition were as follows: C max (5.96±4.15) and(6.11±4.08) ng/mL, AUC 0-72h (45.82±30.20) and(46.11±29.50) ng·h·mL -1 , AUC 0-∞ (49.65±33.64) and(49.63±32.01) ng·h·mL -1 , T max (4.5[1.0, 6.0]) and(4.5[1.0, 6.0]) h, t 1/2 (23.15±9.23) and(22.43±8.81) h, respectively. Under fed condition, the main pharmacokinetic parameters of test tablets and reference tablets were as follows: C max (11.14±5.24) and(10.24±5.39) ng/mL, AUC 0-72h (76.70±39.34) and(75.18±34.36) ng·h·mL -1 , AUC 0-∞ (83.28±43.00) and(81.38±38.03) ng·h·mL -1 , T max (3.0[1.5,5.0]) and(4.3[1.5,7.0]) h, t 1/2 (28.07±6.37) and(27.46±5.44) h, respectively. The 90%confidence intervals of the geometric average ratios of C max , AUC 0-72h and AUC 0-∞ were all within the equivalent interval of 80.00%-125.00%. CONCLUSION: Two formulations of cinacalcet tablets are bioequivalent and safe.
Background: Time and space constraints have often hindered the provision of optimal pharmaceutical care, limiting medication therapy management. Social media tools have gained significant popularity in the field of pharmaceutical care. This study aimed to develop a WeChat-based intelligent medication manager platform that facilitates online pharmaceutical care and encourages self-management.Methods: We developed a WeChat-based Internet pharmacy service platform called Xiang Medicine Guidance (XMG). Through the analysis of surveys and user access data, we evaluated the demand and utilization of the XMG platform and assessed patients' satisfaction with its services. Patients' adherence before and after the XMG platform intervention was also investigated.Results: The XMG platform was launched in November 2022, offering medication guidance, reminders, and consultation services through the WeChat mini-program. By the end of April 2023, the platform had attracted 141.2 thousand users, accumulating 571.0 thousand visits. Moreover, 1,183 clients sought online medication consultations during this period. Six months after the launch of XMG, an impressive 91.02% of users expressed their satisfaction with the platform. The medication reminders and consultations provided by XMG significantly contributed to medication adherence, with 56.02% of users categorized as having good adherence, better than the previous 47.26%.Conclusion: Through its services and features, XMG empowers patients to better manage their medications, seek professional advice, and adhere to their prescribed treatment plans. XMG has the potential to positively impact public health on a broader scale.
OBJECTIVE In response to the trend of medical development informatization, expand the scope of pharmaceutical service of pharmacy intravenous admixture service, to realize the closed-loop infusion management of pharmacy intravenous admixture service.METHODS Based on sqlserver database, the pharmacy intravenous admixture service of our hospital developed an infusion closed-loop management system of the whole process connected with the hospital information management system by using Java language.The whole process of the system adopted bar code, personal digital assistant, with Android Studio development environment.RESULTS The developed PIVAS closed-loop management system of intravenous infusion extend pharmaceutical service to infusion execution bedside patients bed, realized information sharing and interaction in the whole process of pharmacy intravenous admixture service, nurse station and work department, ensures the safety and effectiveness of medication for patients.CONCLUSION Information technology can promote the closed-loop management of intravenous infusion and expand the scope of pharmaceutical service, while the closed-loop management and expanded pharmaceutical service can realize the full traceability of information.The close combination of information technology and management is the inevitable trend of the development of pharmacy intravenous admixture service and modern medical care.
BACKGROUND:The study aimed to investigate the safety, pharmacokinetics (PK), and efficacy of YJ001 spray, a candidate drug for diabetic neuropathic pain (DNP) therapy. RESEARCH DESIGN & METHODS:Forty-two healthy subjects received one of four single doses (240, 480, 720, 960 mg) of YJ001 spray or placebo, and 20 patients with DNP received repeated doses (240 and 480 mg) of YJ001 spray or placebo via topical route of administration to the local skin of both feet. Safety, and efficacy assessments were performed, and blood samples were collected for PK analyses. RESULTS:The pharmacokinetic results revealed that the concentrations of YJ001 and its metabolites were low, and most of them were lower than the lower limit of quantitation. In patients with DNP, treatment with a 480 mg YJ001 spray dose significantly reduced pain and improved sleep quality compared with placebo. No serious adverse events (SAEs) or clinically significant findings of the safety parameters were observed. CONCLUSION:Systemic exposure to YJ001 and its metabolites is low after YJ001 spray is applied locally to the skin, which will reduce systemic toxicity and adverse reactions. YJ001 appears to be well tolerated and potentially effective in the management of DNP and is a promising new remedy for DNP.
SHR-1222, a novel humanized monoclonal antibody targeting sclerostin, has been shown to induce bone formation and decrease bone resorption at a single dose ranging 50–400 mg in our previous phase 1 trial. This study was a randomized, double-blind, placebo-controlled, dose-escalation phase 1 trial, which further investigated the safety, tolerability, pharmacokinetics (PK), pharmacodynamics (PD), and immunogenicity of multiple ascending doses of SHR-1222 in women with postmenopausal osteoporosis (POP). A total of 105 women with POP were enrolled and randomly assigned. Twenty-one received placebo and eighty-four received SHR-1222 sequentially (100 mg QM, n=4; 200 or 300 mg QM, n=20; and 400 or 600 mg Q2M, n=20). The most common adverse events included increased blood parathyroid hormone, increased low-density lipoprotein, increased blood alkaline phosphatase, increased blood cholesterol, back pain, and arthralgia, the majority of which were mild in severity without noticeable safety concerns. Serum SHR-1222 exposure (Cmax,ss and AUC0-tau,ss) increased in a greater than dose-proportional manner. Following multiple doses of SHR-1222, the bone formation markers (terminal propeptide of type I procollagen, bone-specific alkaline phosphatase, and osteocalcin) increased in a dose-dependent manner, whereas the bone resorption marker (β-C-telopeptide) was downregulated. Accordingly, BMD gains in the lumbar spine, total hip, and femoral neck were observed. The maximum BMD increase from baseline at the lumbar spine was detected in the 300 mg QM cohort (14.6% vs. 0.6% in the placebo group on day 169). Six (6/83; 7.2%) subjects developed anti-SHR-1222 antibodies with no discernible effects on PKs, PDs, and safety. Thus, multiple doses of SHR-1222 showed an acceptable safety profile and dose-dependent plasma exposure in women with POP, and could improve their BMD rapidly and prominently by promoting bone formation and inhibiting bone resorption. These findings further support SHR-1222 as a potential alternative agent for the treatment of POP.
Semen Strychni has long been used for the treatment of rheumatoid arthritis, facioplegia and myasthenia gravis due to its anti-inflammation and anti-nociceptive properties in China. However, the fatal neurotoxicity of Semen Strychni has limited its wider clinical application. To investigate the acute toxicity induced by Semen Strychni and the detoxification of liquorice, we evaluated inflammation, oxidative stress and the translocation of high mobility group box 1 (HMGB1) in rats. As a result, there were obvious oxidative stress and inflammation in hippocampus after the Semen Strychni extracts (STR) treatment in rats. Liquorice extracts (LE) and its three active monomers - glycyrrhizic acid (GA), liquiritigenin (LIQ), isoliquiritigenin (ISL) showed the potential for mitigating STR-induced neurotoxicity. HMGB1 levels in cytoplasm and serum and the levels of two downstream receptors RAGE and TLR4 were significantly increased after STR treatment. Through using LE and the monomers, the nucleocytoplasmic transport and release of HMGB1 were inhibited. In addition, the binding between HMGB1 and TLR4 was weakened in detoxification groups comparing with the STR group. Taken together, these findings indicated that liquorice and its active components alleviated acute neurotoxicity induced by Semen Strychni partly via HMGB1-related pathway.
Background:Semen Strychni (SS) is an effective Chinese medicine formula for treating myasthenia gravis (MG) in clinics. Nonetheless, its molecular mechanism is largely unknown.Objective:Using network pharmacology, molecular docking, and experimental validation, we aim to identify the therapeutic effect of SS on MG and its underlying mechanism.Methods:The main ingredients of SS and their targets and potential disease targets for MG were extracted from public databases. The protein-protein interaction (PPI) network was constructed using the STRING 11.0 database, and Cytoscape was used to identify the hub targets. In addition, Gene Ontology (GO) and the Kyoto Encyclopedia of Genes and Genomes (KEGG) were used to identify molecular biological processes and signaling pathways. Then, AutoDock Via conducted molecular docking. The experimental autoimmune myasthenia gravis (EAMG) model in female Lewis rats, quantitative real-time polymerase chain reaction (qRT-PCR), Western blot, and enzyme-linked immunosorbent assay (ELISA) were performed to confirm the effect and mechanism of SS on MG.Results:The following active compounds and hub targets were identified by screening and analyzing: isobrucine, vomicine, (S)-stylopine, strychnine, brucine-N-oxide, brucine and AKT1, MAPK1, MAPK14, CHRM1, ACHE, and CHRNA4. KEGG enrichment analyses indicated that the cholinergic synapse and neuroactive ligand-receptor interaction signaling pathway may be necessary. The results of molecular docking revealed that the main active ingredients bind well to the hub targets. In vivo experiments proved that SS could improve the weight loss and Lennon scores in the EAMG model. Experiments in molecular biology showed that SS could treat MG by affecting the cholinergic synapse through the respective antibody, receptor, and key enzymes in the cholinergic pathway.Conclusion:This study provided a preliminary overview of the active constituents, primary targets, and potential pathways of SS against MG. SS ameliorated EAMG by regulating the cholinergic synaptic junction.
目的 制订一组符合我院静脉用药调配中心(PIVAS)实际情况的合理、规范、科学的新型调配模式.方法 收集我院PIVAS采用传统调配模式和新型调配模式一个月的数据分别纳入对照组和观察组,进行统计分析比对.结果 采用新型模式的普通药物和抗菌药物平均排药核对时间和过程差错均显著少于传统模式组,各类药物采用新型配置复核模式后,减少了工作时间,除抗菌药物外,其他各类型药物配置复核过程中的差错数均低于传统模式.结论 新型调配模式提高了调配各环节工作效率,减少了差错率,符合我院PIVAS实际情况.
国际糖尿病联盟(International Diabetes Federa-tion,IDF)最新报告显示,2019年全球约4.63亿20~79岁成人患糖尿病,其年龄标化患病率高达8.3%,预计2030年和2045年上升为9.2%和9.6%[1].最新全国流行病学调查结果显示,我国大陆地区糖尿病总体患病率为12.8%,糖尿病前期患病率为35.2%.胰岛素是治疗糖尿病的重要药物,在我国的使用非常广泛[2].
目的 研究甘草酸对马钱子中毒所致神经损伤大鼠的保护作用,并初步探讨其作用机制.方法 SD大鼠随机分为对照组(0.5% 羧甲基纤维素钠4 mL·kg-1)、马钱子中毒组(0.5%羧甲基纤维素钠4 mL·kg-1+马钱子0.175 g·kg-1)、低剂量甘草酸解毒组(马钱子0.175 g·kg-1+甘草酸75 mg·kg-1)、高剂量甘草酸解毒组(马钱子0.175 g·kg-1+甘草酸150 mg·kg-1),连续给药4d后处死大鼠,收集血液和脑组织.用尼氏和原位末端转移酶标记技术染色观察神经元凋亡和变性;用酶联免疫吸附法测定海马中炎性指标肿瘤坏死因子α(TNF-α)和白细胞介素6(IL-6)的含量;用蛋白质印迹法测定海马中沉默调节蛋白6(Sirt 6)的表达水平及核转录因子κB(NF-κB)在细胞核、细胞质中的表达水平;用免疫共沉淀技术考察Sirt 6与NF-κB的相互作用.结果 对照组,马钱子组,低剂量甘草酸解毒组和高剂量甘草酸解毒组的尼氏染色积分光密度(IOD)值分别为45913.86±599.32,24550.71±1786.94,35482.63±3109.87和46328.51±1656.92;TUNEL阳性细胞率分别为(8.37±1.35)%,(51.28±8.62)%,(22.4±3.97)%和(7.16±1.29)%;海马组织中TNF-α含量分别为(20.72±2.38),(38.17±4.36),(28.43±2.09)和(23.09±2.14)pg·mL-1;海马组织中IL-6含量分别为(5.26±0.09),(10.58±2.27),(7.35±2.08)和(6.42±1.16)pg·mL-1;海马组织细胞核内NF-κB p65蛋白的表达水平分别为0.19±0.09,0.70±0.06,0.48±0.11和0.41±0.11;海马组织细胞胞浆中NF-κB p65的蛋白表达水平分别为0.16±0.01,0.32±0.02,0.67±0.05和0.50±0.10;海马组织细胞中Sirt 6蛋白的表达水平分别为0.38±0.08,0.20±0.05,0.43±0.11和0.50±0.07.马钱子组的上述指标与对照组比较,或低剂量甘草酸解毒组的上述指标和高剂量甘草酸解毒组与马钱子组比较,差异均有统计学意义(P<0.05,P<0.01).结论 甘草酸能缓解马钱子所致神经损伤,其作用机制可能与调控Sirt 6/NF-κB信号通路,抑制炎症反应相关.
The effects of permanent disruption of neuropeptide transmission on the induction (i.e., sensitization) and elicitation (i.e., challenge) phases of contact hypersensitivity (CHS) are described. BALB/c mice were chemically denervated of neuropeptide (i.e., tachykinin) containing sensory C fibers by an acute injection of capsaicin (50 mg/kg) on postnatal day (PND) 2 to 3. As young adults (PND 45-60), these mice and their control littermates were sensitized by topical application of 0.1% 2,4-dinitrofluorobenzene (DNFB) or vehicle. Treatment groups generated from this exposure regimen consisted of untreated, controls (O/O), denervated, controls (CAP/O), untreated, sensitized (O/DNFB), and denervated, sensitized (CAP/DNFB). The elicitation phase of CHS was evaluated in these animals by measuring ear thickness in response to a DNFB challenge. In DNFB-sensitized groups, ear thickness was significantly increased over controls but was additionally increased 2.4-fold in CAP/DNFB compared to O/DNFB mice. The induction phase of CHS was next assessed in young adult mice by measuring lymph node cell (LNC) proliferation. For this, mice were sensitized for 3 consecutive days before their draining, auricular nodes were removed. The LNC were dissociated and cultured for 24 h with tritiated thymidine to assess LNC proliferation. As expected, significantly higher numbers of LNC occurred in both DNFB-sensitized groups (CAP/DNFB, O/DNFB) compared to the unsensitized, controls (CAP/O, O/O). However, LNC proliferation in CAP/DNFB was significantly higher than O/DNFB animals. Flow cytometry on similarly exposed mice failed to demonstrate any significant difference in the population of CD4CD8 or CD3CD45R LNC cells from neuropeptide-denervated (CAP/O, CAP/DNFB) mice or their respective treatment mates (O/O, O/DNFB), suggesting that alterations in T or B cell populations did not underlie these changes. Finally, cytokine release from the LNC from these treatment groups was examined. For this, the auricular lymph nodes were removed from animals, 2 to 4 h after the animals were administered a single application of a sensitizing concentration (0.1%) of DNFB or acetone vehicle. LNC, dissociated from these nodes, were cultured for 24 h. The nutrient media was removed from these cultured cells and examined for the release of proinflammatory cytokines, interleukin (IL)-1 beta, IL-2 and tumor necrosis factor (TNF)alpha, by ELISA. There were no significant increases in IL-2. However, IL-1 beta release was significantly increased in CAP/DNFB mice over O/DNFB by 18-fold and by over 30-fold compare to O/O controls. Levels of TNF alpha were significantly increased in both O/DNFB and CAP/DNFB mice over the nonsensitized controls (O/O, CAP/O). CAP/DNFB values were approximately double that of O/DNFB. There was no significant difference in IL-1 beta or TNF alpha release between the nonsensitized controls (O/O, CAP/O). Collectively, these data indicate that neuropeptide denervation by neonatal administration of capsaicin alters both the induction and elicitation phases CHS and may modify sensitivity to chemically induced CHS.
Considering that neurotransmitters (NTs) and amino acids (AAs) exert pivotal roles in various neurological diseases, global detection of these endogenous metabolites is of great significance for the treatment of nervous system diseases. Herein, a workflow that could cope with various challenges was proposed to establish an extendable all-in-one injection liquid chromatography-tandem mass spectrometry (LC-MS/MS) assay for analyzing these small molecular metabolites with high coverage. To obtain a qualified blank biological matrix for the preparation of standard curves and quality control samples, different absorption solvents, including activated carbon (AC), calcite (Cal) and montmorillonite (Mnt) were systematically evaluated for efficient absorption of endogenous substances with minimum residue. We also firstly proposed a "Collision Energy Defect (CED)" strategy to solve the huge difference of mass signal strength caused by different properties and concentrations of 11 NTs and 17 AAs. The quantitative results were validated by LC-MS/MS. Sensitivity, accuracy, and recovery meeting generally accepted bioanalytic guidelines were observed in a concentration span of at least 100 to 500 times for each analyte. Then the temporal changes of intracerebral and peripheral NTs and AAs in ischemic stroke model and sham operated rats were successfully produced and compared using the described method. All these results suggested that the currently developed assay was powerful enough to simultaneously monitor a large panel of endogenous small molecule metabolites, which was expected to be widely used in the research of various diseases mediated by NTs and AAs.
With the improvements in relevant policies, laws, and regulations regarding drug clinical trials in China, the quantity and quality of drug clinical trials have gradually improved, and the development prospects of drug clinical trials for endocrine disorders and metabolism and nutrition disorders are promising. Based on information from the clinical trials from the online drug clinical trial registration platform of the National Medical Products Administration, we aimed to review and evaluate the development of clinical trials of drugs for endocrine disorders and metabolism and nutrition disorders in mainland China from 2010 to 2019, as well as the trends over time. A total of 861 trials were carried out on 254 types of drugs for endocrine disorders and metabolism and nutrition disorders, among which 531 (61.67%) involved endocrine disorders, and 330 (38.33%) addressed metabolism and nutrition disorders. The annual number of clinical trials has been increasing gradually, with a significant increase in 2017. Among them, the proportion of clinical trials with Chinese epidemiological characteristics was relatively large (Wu, Annual Report on Development Health Management and Health Industry in China, 2018). The largest number of trials were for diabetes drugs (55.63%), followed by trials of drugs for hyperlipidemia (19.4%) and those for hyperuricemia (7.9%). It was found that the geographical area of the leading units also showed obvious unevenness according to the analysis of the test unit data. Based on the statistics and evaluation of the data, comprehensive information is provided to support the cooperation of global pharmaceutical R&D companies and research units in China and the development of international multicenter clinical trials in China. This work additionally provides clinical trial units with a self‐evaluation of scientific research competitiveness and hospital development strategies. At the same time, it provides a reference with basic data for sponsors and stakeholders in these trials to determine their development strategy goals.
目的:研究精神分裂症患者体内普萘洛尔对氯氮平血药浓度及药动学的影响.方法:选取24位精神分裂症患者,进行氯氮平片试验制剂和参比制剂的随机、开放、2周期交叉的空腹生物等效性试验,其中15人早晚服用氯氮平片100 mg,另外9人服用氯氮平片+普萘洛尔,每周期在固定的时间点进行血样采集,应用高效液相色谱-串联质谱法检测血样中氯氮平的浓度,使用WinNonlin 7.0软件计算参比制剂组氯氮平的药动学参数.结果:与单用氯氮平相比,合并使用普萘洛尔后,氯氮平的平均AUG0-12、Cmin,ss及Cav,ss均显著增高(P=0.043,P=0.017和P=0.042);合用普萘洛尔组的氯氮平Cmax,ss均值也较单用氯氮平组高[(854.11±286.47) ng·mL-1 vs.(671.14±160.98) ng·mL-1],但差异无统计学意义;合用普萘洛尔组氯氮平的平均t1/2显著长于单用氯氮平组(P=0.001).结论:合并使用普萘洛尔可显著增加氯氮平的系统暴露量,在临床上需注意普萘洛尔对氯氮平的影响.
