Objective To evaluate the efficacy,toxicity and quality of life of S-1 plus oxaliplatin(SOX)versus 5-FU plus cisplatin(PF)as adjuvant chemotherapy for adenocarcinoma of esophagogastric junction(AEG).Methods Patients with AEG who had accepted radical operation and the pathological stage which was Ⅲ stage were eligible for inclusion.Patients were randomly assigned to receive SOX regimen or PF regimen.Eighty-two patients with AEG who had accepted radical operations were enrolled.38 patients were in the experimental group and 44 patients were in the control group.The major evaluation parameters were disease-free survivaI(DFS),overall survival(OS)and toxicity as well as quality of life.Results For SOX and PF regimens,the median DFS were 29.3 months and 18.4 months,respectively(P<0.05),and the median OS were 35.9 months and 24.8 months,respectively(P<0.05).The major adverse reaction of these two groups was the hematologic toxicity,mainly gradeⅠ~Ⅱwith no significant statistical difference.The incidence of peripheral neurotoxicity in SOX regimen was higher than that in PF regimen(P<0.05).However,the toxicities were tolerable.The condition of improvement of the quality of life was significantly better than PF regimen(P<0.05),with statistical significance.ConclusionCompared with PF regimen,the SOX regimen as the postoperative adjuvant chemotherapy of AEG can prolong the median DFS and OS,toxicity can be tolerated.The drug administration was convenient,and the quality of life was improved significantly.The results of this retrospective analysis indicate that SOX regimen has obvious advantages over PF regimen.

OBJECTIVE To study the influence of the sequence of three-dimensional conformal radiotherapy (3DCRT) and transcatheter arterial chemoembolization (TACE) on the efficacy and toxicity of treatment in patients with hepatocellular carcinoma (HCC) with portal vein tumor thrombus (PVTT). METHODS A total of 65 patients who were diagnosed with primary HCC with PVTT were enrolled in the study from November 2008 to March 2012 and were randomly divided into the following two groups:group A,32 patients treated with 3DCRT followed by TACE; group B,33 patients treated with TACE followed by 3DCRT. RESULTS The total efficacy rates of groups A and B were 68.8% and 69.7% (x² =0.232, P < 0.793). The survival rates,effective percentage of PVTT and AFP remission rates were not significantly different between group A and group B.The exacerbation rate of liver function was significantly higher for group B than for group A (P < 0.05). No serious complication was found in the follow-up period for either group. CONCLUSION The combination of 3DCRT and TACE is a relatively effective local treatment for patients with primary HCC and PVTT.Compared with TACE followed by 3DCRT, 3DCRT followed by TACE may have a negative influence on liver function.
Objective To observe the efficacy and adverse effects of conventionally fractionated (CF) versus hypofractionated (HF) three-dimensional conformal radiotherapy (3DCRT) for hepatocellular carcinoma (HCC) with portal vein tumor thrombosis.Methods A retrospective analysis was performed on the clinical data of 65 patients with HCC suitable for 3DCRT from 2008 to 2012.These patients were randomly divided into HF group (n =32) and CF group (n =32).The HF group received 3DCRT at a total dose of 45-55 Gy (3-4 Gy/fraction,3-5 fractions/week),while the CF group at a total dose of 40-56 Gy (2 Gy/fraction,5 fractions/week).Results The follow-up rate was 100%.For the HF group and CF group,the short-term response rates were 72% vs.55% (P =0.034),the 1-year local control rates were 72% vs.55% (P=0.034),the 1-year overall survival rates were 59% vs.45% (P=0.042),and the numbers of individuals with grade 1-2 acute gastrointestinal reactions,deterioration of liver function,and radiation hepatitis were 14 vs.11,8 vs.8,and 1 vs.0,respectively (P =0.237).Conclusions HF 3DCRT is superior to CF 3DCRT in the treatment of HCC.
INTRODUCTION:We investigated the therapeutic effects of three-dimensional conformal radiotherapy combined with transcatheter arterial chemoembolisation (TACE) for hepatocellular carcinoma (HCC) with portal vein tumour thrombosis (PVTT).METHODS:Sixty-three HCC patients with PVTT were divided into two groups. Group A (30 patients) was treated with three-dimensional conformal radiotherapy followed by 2-3 series of TACE, while group B (33 patients) was only treated with TACE.RESULTS:The 1- and 2-year survival rates of group A were 62.40% and 20.81%, respectively, with a mean survival time of 13.0 months. The 1- and 2-year survival rates of group B were 56.49% and 18.83%, respectively, with a mean survival time of 9.0 months. There were significant differences between the two groups (log-rank chi-square value = 3.950, P = 0.047).CONCLUSION:Three-dimensional conformal radiotherapy combined with TACE can significantly improve clinical outcomes in patients with HCC and PVTT compared with TACE alone.
Objective To observe the therapeutic effects,improvement of life quality,and toxic side effects of double-way chemotherapy combined with radiotherapy in the treatment of NSCLC patients with malignant pleural effusion.Methods 36 NSCLC patients with malignant pleural effusion were randomly and evenly divided into two groups according to the therapeutic method.The group A was treated with double-way chemotherapy combined with three-dimensional conformal radiotherapy,and the group B was treated with double road chemotherapy only.Besides,all of the patients were given routine and symptomatic treatment.The therapeutic effects,life quality and toxic side effects were evaluated and compared between the two groups.Results The effective rates were 94.4% and 61.1% respectively in the group A and the group B(P0.01).There was no significant difference of toxic side effects between the group A and the group B(P0.05).The Karnofsky score of the group A was higher than that of group B(P0.05).The overall survival of the group A was significantly longer than that of the group B(P0.05).Conclusion It is more effective for patients with malignant pleural effusion caused by NSCLC treated with double-way chemotherapy combined with three-dimensional conformal radiotherapy than double-way chemotherapy only.
