Objective:To explore the application value of intracranial pressure monitor in the treatment of hyperten-sive intracerebral hemorrhage(HICH)with soft channel minimally invasive drainage.Methods:The clinical data of 93 cases with HICH who were treated in our hospital from July 2019 to July 2021 were retrospectively analyzed and grouped according to different treatment methods.Among them,cases who received soft channel minimally inva-sive drainage were included in control group(49 cases),and cases who received intracranial pressure monitor com-bined with soft channel minimally invasive drainage were included in observation group(44 cases).The hematoma-related conditions,inflammatory factor levels,oxidative stress indicators,complications and prognosis were compared in two groups.Results:Compared with observation group,the intraoperative suctioned hematoma vol-ume,suctioned hematoma volume-to-volume ratio,and postoperative hematoma emptying time in control group were higher(t=7.373,25.611,6.099,P<0.001).After treatment,the serum CRP,IL-6,and TNF-αlevels in two groups were decreased,and compared with control group,the CRP,IL-6,and TNF-αin observation group were lower(t=48.328,8.992,19.056,P<0.001).After treatment,the serum SOD and GSH-Px in the two groups were increased,and MDA was decreased(P<0.05).Compared with control group,the serum SOD and GSH-Px in observation group were higher,and MDA was lower(t=8.545,5.027,18.791,P<0.05).The rate of complications in control group was higher than that in observation group(30.61%vs 11.37%)(χ2=5.088,P=0.024).The good prognosis rate in control group was lower than that in observation group(63.27%vs 86.37%)(χ2 = 6.460,P = 0.011).Conclusion:Intracranial pressure monitor combined with soft channel minimally invasive drainage can effectively remove hematoma in cases with HICH,reduce oxidative stress and in-flammatory response with fewer complications and better prognosis.hemorrhage;glutathione peroxidase.
5-fluorouracil(5FU)is a pyrimidine antimetabolite, which binds to thymidine synthase(TS)after transformation in vivo and interferes with DNA synthesis, thereby leading to cell damage and death.5FU is widely used in the chemotherapy for colorectal cancer, breast cancer, gastric cancer and other cancers.However, there is still an important limitation of the clinical application of 5FU that patients are prone to develop drug resistance, which leads to low efficacy or failure of chemotherapy.Non-coding RNA(ncRNA)is a class of RNAs that does not encode proteins.Recent studies have found that ncRNA plays important roles in the 5FU resistance mechanism by regulating the expression of downstream target genes.In this review, the mechanisms underlying 5FU resistance, such as nucleotide metabolism, gene repair and tumor microenvironment, are summarized, and how ncRNAs play a role in these mechanisms are illustrated.It provides supports for further research on the mechanisms of 5FU drug resistance and exploring the treatment plans for reversing drug resistance.
Exosomes are membrane vesicles with lipid bilayers and diameters of 30~100 nm secreted by most cells in the body, which can carry and transfer proteins, nucleic acids and lipids.Long non-coding RNA (lncRNA) is a class of non-coding RNAs with the length of more than 200 nucleotides.It has no protein expression, but can regulate gene expression at transcription, post transcription and epigenetic levels.Exosomal lncRNA refers to the lncRNA carried, transported and secreted by exosomes.It can participate in intercellular signal transduction, cause changes in gene expression in cells, and have an important impact on the occurrence and development of tumors and other diseases.Exosomal lncRNA is expected to become a new tumor biomarker, which can be used in the diagnosis, treatment and prognosis of tumor by liquid biopsy.This article reviews the roles of lncRNA in tumor and its potential application in tumor diagnosis, treatment and prognosis.
目的 探讨神经内镜辅助去骨瓣血肿清除术治疗高血压脑出血的效果及对患者神经功能、预后的影响.方法 选取2018年2—10月收治的高血压脑出血92例,根据手术方法不同将其分为观察组(48例)和对照组(44例)两组.观察组予神经内镜辅助去骨瓣血肿清除术治疗,对照组予去骨瓣开颅血肿清除术治疗.比较两组手术指标、血肿清除情况、神经功能评分、预后情况和并发症发生情况.结果 观察组手术时间长于对照组,术中出血量少于对照组,住院时间短于对照组,血肿清除率高于对照组,差异有统计学意义(P<0.01).术前,两组美国国立卫生研究院卒中量表(NIHSS)和格拉斯哥预后量表(GOS)、日常生活活动能力(ADL)量表评分比较差异无统计学意义(P>0.05).术后1周,两组NIHSS评分均较术前降低,且观察组NIHSS评分低于对照组,差异有统计学意义(P<0.01).术后3个月,两组GOS和ADL量表评分均高于术前,且观察组GOS和ADL量表评分高于对照组,差异有统计学意义(P<0.01).术后3个月内,并发症总发生率观察组低于对照组,差异有统计学意义(P<0.05).结论 神经内镜辅助去骨瓣血肿清除术治疗高血压脑出血患者术中出血量少,住院时间短,可提高血肿清除率,改善神经功能及预后,且术后并发症总发生率低.
微小RNA (microRNA,miRNA)是一类由19~24个核苷酸构成的内源性的非编码单链RNA,通过与靶基因完全或不完全结合降解mRNA或抑制mRNA的翻译,在体内起到转录后调控作用,参与多种生理和病理过程.新近报道发现,miR-338-5P在黑色素瘤、消化系统肿瘤、肺癌等各种类型的癌症中异常表达,其通过结合一系列的靶基因来调控细胞增殖、转移、侵袭和凋亡,在不同肿瘤中发挥抑癌或促癌的不同作用,并可能成为癌症诊断的生物标志物和治疗靶点.在其他炎性相关疾病中,比如病理性心肌肥厚、类风湿性关节炎等,也发现miR-338-5P可以通过靶向不同基因,参与疾病的发生发展.现综述miR-338-5P在肿瘤、肿瘤放化疗以及其他炎性相关疾病中的作用和其在疾病诊断、预后判断等方面的应用,为肿瘤等疾病相关标志物以及更有效的治疗靶点的开发提供新思路.
目的:观察参麦注射液治疗急性有机磷中毒性心肌损害的临床疗效。方法选取2010年1月至2015年1月急性有机磷农药中毒伴发心肌损害患者120例,男56例,女64例。随机分为对照组和治疗组,每组60例。对照组采用急性有机磷农药中毒的常规治疗方法,治疗组在对照组常规治疗基础上加用参麦注射液。比较2组患者的临床疗效及心电图,心肌酶恢复时间,胆碱酯酶恢复时间的变化情况。结果治疗组总有效率91%;对照组总有效率74%,治疗组患者疗效优于对照组,差异有统计学意义( P <0.05)。2组患者在症状消失时间、心肌酶恢复时间、胆碱酯酶活性恢复时间,心电图改善时间方面均优于对照组,差异有统计学意义( P <0.05)。结论参麦注射液是防治有机磷中毒心肌损害的有效药物。
目的:研究60例急性有机磷农药中毒( acute organophosphorus pesticide poisoning ,AOPP)患者血清肌酸激酶同工酶( CK-MB)及肌钙蛋白I ( CTnI )的动态变化,探讨CK-MB及CTnI在AOPP患者心肌损害诊断中的价值。方法选取AOPP患者60例,中毒患者分为轻、中度中毒30例,重度中毒30例。患者入院后均予积极抢救治疗,并于中毒后第1、2、3、5、7天分别采静脉血,进行CK-MB及CTnI的检测。对照组为健康体检者30例,于查体当日取静脉血,检测其CK-MB及cTnI水平,对2组结果进行比较。结果 AOPP患者血清CK-MB及cTnI增高,与对照组比较差异有统计学意义( P <0湝.05),且中毒程度越重,升高越明显。结论 AOPP可引起心肌损害,CK-MB及cTnI的动态检测有助于判断心肌损害程度,cTnI的诊断价值可能优于CK-MB。
目的:探讨糖尿病视网膜病变患者血脂和动态血压特点。方法将111例2型糖尿病患者根据眼底检测结果分为3组:无视网膜病变组(NDR组)42例,背景型视网膜病变组(BDR组)36例,增殖型视网膜病变组(PDR组)33例。对所有患者进行血脂和动态血压监测。结果 BDR组患者甘油三酯(TG)、总胆固醇(TC)水平均显著高于NDR组(P<0.05);而PDR组患者TG、TC、高密度脂蛋白胆固醇(LDL-C)水平显著高于BDR组和NDR组,HDL-C水平显著低于BDR组和NDR组(P均<0.05)。 BDR组白天、夜间以及24小时收缩压和舒张压均高于NDR组, PDR组白天、夜间以及24小时收缩压和舒张压又均高于BDR组和NDR组(P均<0.05)。 BDR组中非勺型血压发生率明显高于NDR组(61.11% vs 9.52%,P<0.05),PDR组非勺型血压发生率又明显高于BDR组(81.82% vs 61.11%,P<0.05)。结论血脂和血压在视网膜血管病变过程中起重要作用,血脂检测和血压动态监测有助于临床上糖尿病视网膜病变的防治。
ObjectiveToobservetheeffectsofToutongningcapsuleonthebehaviorsandsymptomsofratswith migraine.Methods Seventy-two SD rats were randomly divided into 6 groups: normal control group,migraine model group, Sibelium treatment group,Toutongning high-dose group,Toutongning median-dose group,Toutongning low-dose group,with 12 rats in each group.The animal models with migraine were established by subcutaneous injection with NTG.The rats in normal control group were given 0.9%sodium chloride solution by subcutaneous injections.The appearing time and disappearing time of redness in both ears of rats after the animal models were established, terminal time of scratching head, climbing cage frequency were observed and recorded in the 6 groups.Results There were no significant differences in the terminal time of redness in both ears of rats, terminal time of scratching head and climbing cage frequency between Toutongning high-dose group,Toutongning median-dose group and Sibelium treatment group ( P >0.05),however,there were significant differences between Toutongning low-dose group and Sibelium treatment group ( P <0.05).Conclusion Toutongnning capsule can improve the behavior and symptom of rats with migraine caused by nitroglycerin,and the therapeutic effects of high-dose and median-dose Toutongning capsule are more obvious.
Objective:To observe the effects of Toutongning capsule on P2X3mRNA expression in trigeminal ganglion of migraine rats.Method:The healthy SD rats were evenly divided into:normal group.model group,three dose groups of Toutongning(760,380,190 mg·kg-1)capsule group and Xibiling group.Migrained rats model was established by NTG subcutaneous injection.Samples were harvested and measured in 3 hours and 6 hours after NTG subcutaneous injection.The P2X3mRNA expression in trigeminal ganglion was determined by RT-PCR.Result: P2X3mRNA expression in trigeminal ganglion increased in the model group compared with that in the normal group(P0.01).P2X3mRNA expression was elevated at hour 3 then decreased at hour 6.P2X3mRNA expression in trigeminal ganglion and thalamencephalon were downregulated in the three dose groups of Toutongning capsule and Xibiling group compared with those in the model group(P0.01).Conclusion: Toutongning capsule could treat migraine by reducing P2X3mRNA expression in trigeminal ganglion.
Objective:By observing the effects of Toutongning capsule on the expression of c-fos and c-jun in periaqueductalgrey(PAG) and thalamencephalon in rats with migraine to discuss the therapeutic mechanism of Toutongning capsule.Method:The SD rats were evenly divided into 6 groups:normal group,model group,Flunarizine(1.04 mg·kg-1)intervention group,high-dose,mid-dose and low-dose Toutongning capsule groups(760,380,190 mg·kg-1).Migraine model in rats was established by NTG subcutaneous injection.After NTG subcutaneous injection the specimen at 4 hours and 6 hours was obtained.The expression of c-fos and c-jun were detected by immunohistochemistry.Result:The expression of c-fos and c-jun in PAG and thalamencephalon were increased in the model group compared with that in the normal group(all P0.01).At 4 hours after NTG subcutaneous injection,there was much more expression of c-fos and c-jun compared with those at 6 hours after NTG subcutaneous injection.The expressions of c-fos and c-jun in PAG and thalamencephalon were decreased in the high-dose Toutongning capsule intervention group,mid-dose Toutongning capsule intervention group and Flunarizine intervention group compared with those in low-dose Toutongning capsule group and in the model group(all P0.01).There was no significant difference between model group and low dose intervention group.Conclusion:By reducing the expression of c-fos and c-jun and restrainning the transmission of pain information in PAG and thalamencephalon,Toutongning capsule could treat migraine.And the effect of high-dose Toutongning capsule and mid-dose Toutongning capsule were much better.
