Objective To evaluate the clinical efficacy of pre-procedure simethicone on detection of pharynx by upper gastrointestinal endoscopy. Methods A total of 100 patients with esophageal squamous cell carcinomas ( ESCCs) were enrolled in this prospective controlled trial and randomly assigned into two groups. The study group was given gargle with 5 mL simethicone plus 5 mL water combined with conventional procedure before gastroscopy. The control group was prepared according to the conventional procedure. The age, gender, tumor stage, lesion size, doctor's satisfaction and examination time between two groups were analyzed. Results The basic conditions between the two groups, including age, gender, and tumor stage were not significantly different (all P>0. 05). The detection rate of superficial lesion in pharynx was higher in the study group than that in the control group, with no significant difference [8. 16%(4/49) VS 2. 04%( 1/49), P=0. 362]. The median time of pharyngeal observation in the study group was less than that in the control group (21. 7 s VS 33. 9 s, P=0. 000). The doctor was more satisfied in the study group than the control group (P=0. 001). Conclusion Pre-procedure with simethicone improves the endoscopic visibility and detection rate of superficial squamous cell carcinoma with less observation time.
Abnormal regulation of apoptosis can lead to carcinogenesis. Single nucleotide polymorphisms in apoptotic genes have been associated with cancer risk, such as the FAS rs2234767G/A polymorphism, which alters transcription of the FAS promoter. Downregulation of FAS, with resultant cellular resistance to death signals, has been found in many cancers. However, the association between the FAS rs2234767G/A polymorphism and cancer risk is still controversial. Here, we performed a meta-analysis including 41 articles (44 case-control studies, 17,814 cases and 24,307 controls) identified from PubMed and Chinese language (CNKI and WanFang) databases related to cancer susceptibility and the FAS rs2234767G/A polymorphism. We used odds ratios (ORs) and 95% confidence intervals (CIs) to assess the strength of the associations. We found that the rs2234767 G-allele was a protective factor for cancer risk (GG vs. AA: OR=0.88, 95% CI=0.79-0.98; GG+GA vs. AA: OR=0.87, 95% CI=0.79-0.96). Similar associations were detected in the "source of control", ethnicity, and cancer type subgroups. Further studies on a larger sample size and considering gene-environment interactions should be conducted to confirm the role of FAS polymorphisms, especially rs2234767G/A, in cancer risk.
A 61-year-old woman sought medical attention after experiencing throat irritation, chest pain, and difficulty swallowing for 1 week. Chest computed tomography (CT) showed diffuse wall thickening in the upper and middle esophagus, as well as an esophageal stricture ([Fig. 1]), suggestive of esophageal carcinoma. An esophagogastroduodenoscopy (EGD) was performed. Before the endoscopy, a slightly tender swelling was observed on the right side of the neck. The endoscopy revealed swelling in the left side of the pharynx ([Fig. 2 a]) and at the entrance to the esophagus ([Fig. 2 b]), and a stenosis was also apparent; however, the lumen and mucosa exhibited normal elasticity and the lumen distended normally after air inflation. Some regions of the esophageal mucosa appeared purplish-blue ([Fig. 2 c]), although the mucosal surface was smooth with no abnormalities.
A 49-year-old woman was admitted to the hospital emergency department having had abdominal pain for 16 hours. She had previously undergone placement of a fully covered anti-reflux metal stent to treat esophageal achalasia (achalasia of cardia). On physical examination, the abdomen was soft with normal bowel sounds. However, there was tenderness on deep palpation of the left lower abdomen. All laboratory test results were within reference ranges. An abdominal radiograph demonstrated an impacted metal foreign body in the left lower portion of the abdomen without signs of pneumoperitoneum ([Fig. 1]). Single-balloon endoscopy was performed (Olympus SIF-Q260; Olympus, Inc., Tokyo, Japan). At approximately 150 cm from the pylorus, the stent was found ([Fig. 2]). It was grasped with a snare and extracted ([Fig. 3], [Fig. 4]). An abdominal radiograph that was performed after the end of the procedure did not reveal pneumoperitoneum. The patient was discharged the following day.
A 51-year-old man was admitted to our hospital with hematemesis after swallowing a chicken bone. An endoscopy was performed, which showed a large submucosal hematoma in the esophagus, which extended from 15 cm down to the esophagogastric junction. The esophageal lumen was occupied by the hematoma. No active bleeding or foreign body was seen ([Fig. 1]; [Video 1]).
Objective To investigate the practical efficiency of a novel disposable-sheathed gastroscope system in visit round of remote areas.Methods A total of 1120 participants were randomized to accept gastroscopy with either a disposable-sheathed gastroscope system (n =568) or a standard system (n =552).The duration of the endoscopic procedure and cleaning and disinfection was recorded.The results of gastroscopy,the feedback of the procedure from patients and any adverse events presented during the procedure were also recorded.Results The mean procedure time was significantly longer with the sheathed system (4.9 ± 1.4 min) than with the standard one (4.1 ± 1.3 min,t =5.689,P =0.000).However,the duration of cleaning and disinfection were significantly shorter with the sheathed gastroscope (4.9 ±0.2 min) than with the standard one (35.0 ±0.2 min,t =-1271.145,P =0.000).So the total instrument turn-over time for sheathed system was only a quarter of the time for standard endoscope (9.9 ± 1.3 vs.39.0 ± 1.4 min).The detection rate of abnormalities and patients'feedback were similar between with two systems.No sheath leakage or rupture was observed.Conclusion The sheathed gastroscope system is safe,convenient and rapid in manipulation,and is valuable in clinical practice,especially in visit rounds of remote areas.

When it comes to the analysis of transient state activities of iron core equipment including transformers and generators, magnetization curves of piecewise linearization are often used to replace hysteresis loop. Yet, there are often calculation errors and oscillation. This article makes analysis on the principle of J-A hysteresis loop model, deriving opposite J-A model with equation transformation method, and analyzing the possible causes for instability in calculations. The simulation research results show that eddy current loss has a major impact on hysteresis loop.
The objective of the present study was to evaluate the application potential of CyberKnife for high-risk tumors of the central nervous system and to analyze the effectiveness of CyberKnife in relation to dose recovery and gain division (times). A total of Eighty-one targeted areas from 139 central nervous tumor lesions in 60 patients were treated with I-VI ranged CyberKnife for 1 week. Following CyberKnife treatment, imaging tests revealed a decrease in tumor volume, reduction of central nervous system symptoms and an increase in the life quality of patients. The advantages of CyberKnife include non-invasiveness, individualized treatment, flexibility, high accuracy and rapid treatment. CyberKnife produces slight damage and a favorable therapeutic effect and expands our concepts concerning precise radiotherapy for tumors. It is an indispensable method for personalized tumor treatment.

Objective To investigate the effects of estrogen on mismatch repiar gene expression in colonic mucosa in vivo. Methods A total of 42 healthy individuals underwent colonoscopy were enrolled in the study. Half an hour before colonoscopy examination, blood sample was taken for determining the serum estradiol (E2) level. N ormal colonic mucosal tissues determined by naked eye under colonoscopy examination were taken in the right hemi colon to detect HMLH1 and hMSH2 gene expression by semi-quantitative RT-PCR and immunohistochemistry staining. Then the correlation of serum E2 levels with hMLH1 and hMSH2 expression in colonic mucosa was analyzed. Results A bimodal curve was presented for the correlation between serum E2 level in healthy individuals and hMLH1 expression in colonic mucosa. A strong positive correlation of E2 level with hMLH1 expression in normal colonic mucosa was observed when serum E2 level was more than 45 pg/ml (For mRNA, P=0. 003, r=0. 701; for immunohistochemistry positivity index, P=0. 000, r=0. 874).However there was no correlation between E2 level and hMSH2 expression. Conclusion High serum E2 level might increase the hMLH1 gene expression in colonic mucosa in vivo.

To solve the complexity of the problem in the choice of vehicles for transportation of flammable liquids in third-party logistics enterprises,the existing problems of flammable liquids transportation facing the third-party logistics system are analyzed.A selection strategy model is put forward for the selection of vehicles for flammable liquids transportation from the strategy viewpoint.Then the key factors about tractor selection,tractor-trailer matching and the cost accounting are analyzed respectively for the real application with the selection strategy model.This paper provides the methodological guidance for the growing third party logistics companies in responding to transportation orders dynamically,transport vehicles selecting and transport costs saving respectively.
Objective To study the proliferation and the tumor cell killing of autologous and allogeneic T cells stimulated by dendritic cell-tumor fusion vaccine. Methods Mature dendritic cells were obtained from peripheral blood of healthy blood donors and fused with SGC7901 gastric carcinoma cells by polyethylene glycol, and then the dendritic cell-tumor fusion vaccine were screened out by HAT/HT medium. The proliferative activity of homogeneic and allogeneic T cells stimulated by dendritic cell-tumor fusion vaccine were detected. T cells with the strongest proliferative activity were isolated and co-cultured with SGC7901 gastric carcinoma cells, finally, the killing effect of T cells on tumor cells were detected. Results About (3. 87 ±0. 31) × 10~7 dendritic cells were isolated from 200 ml peripheral blood, and purified dendritic cell-tumor fusion vaccine was obtained by fusing mature dendritic cells with SGC7901 gastric carcinoma cells. The dendritic cell-tumor fusion vaccine stimulated T cell proliferation effectively, and the proliferative activity of autologous and allogeneic T cells were the strongest at fusion cells to T cells ratio of 1:10, meanwhile the proliferation activity of these T cells were similar. The killing rates of autologous and allogeneic T cells stimulated by dendritic cell-tumor fusion vaccine on SCC7901 gastric carcinoma cells were (77. 16 ±2. 76)% and (77. 70 ±3. 55)% , respectively. In contrast, the killing rates of autologous and allogeneic T cells were (20.45 ±2. 12)% and (21. 31 ±2. 86)% , respectively, significantly lower than the vaccine activated T cells ( P < 0. 01). Conclusion Dendritic cell-tumor fusion vaccine derived from dendritic cells of healthy blood donors can activate autologous and allogeneic T and effectively kill tumor cells in vitro.
OBJECTIVE:To explore the protective effect of calcium channel agonist BayK8644 preconditioning on the lungs against ischemia-reperfusion injury (I/R) and its mechanism in rabbits. METHODS:Forty rabbits were randomly divided into 4 groups (10 in each group): a sham-operated group (Sham), an I/R group (I/R), an ischemic preconditioning (IP) group (IP), and a BayK8644 preconditioning group(BayK8644).The wet to dry weight (W/D) ratio, superoxide dismutase (SOD) activity, myleoperoxidase (MPO), and malondialdehyde (MDA) contents of the lung tissues were measured after the operation. Morphological and ultrastructural changes of the lung tissue were observed by light and electron microscope. The expression of pulmonary surfactant-associated protein-A (SP-A) was examined with immunohistochemistry. RESULTS:The W/D ratio, MPO and MDA contents of the lung tissue in the BayK8644 group and IP group were significantly lower than those in the I/R group (P<0.05), while the SOD activity and expression of SP-A of the lung tissue in the BayK8644 group and IP group were significantly higher than those in the I/R group(P<0.05), The morphological changes and ultrastructural changes were less notable in the BayK8644 group and IP group, compared with the I/R group. There was no significant difference in SOD activity, MPO and MDA contents between the BayK8644 group and IP group (P>0.05). CONCLUSION:Appropriate BayK8644 preconditioning can induce transient Ca²+ influx, and elicit strong protection against the lung ischemia-reperfusion injury, which can simulate the endogenous protective effect of ischemic preconditioning.
Abstract Estrogen is reported to have a protective effect on colon cancer; however, the underlying mechanism is unclear. Impaired mismatch repair plays an important role in colonic carcinogenesis. The purpose of this study was to investigate the association of estrogen on regulating mismatch repair expression in colonic epithelial cells. In cultured COLO205 cells, the effect of estradiol (E2) and antagonist ICI182.780 on the expression of hMLH1 and hMSH2 was studied using reverse transcription-PCR and Western blotting. The correlation between serum level E2 and the expression of hMLH1 and hMSH2 in colonic mucosal tissue of 42 healthy individuals was also examined using reverse transcription-PCR and immunohistochemical staining. E2 increased the expression of hMLH1 in COLO205 cells, which was suppressed by ICI182.780. However, the effect of E2 on hMSH2 expression was not significant in COLO205 cells. In healthy individuals, a strong positive correlation of E2 level with hMLH1 expression in normal colonic epithelial cell was observed when serum E2 level was >45 pg/mL, but no correlation was seen between E2 and hMSH2 expression. E2 affects the expression of hMLH1 but not hMSH2 in vitro, and high serum E2 level correlates with hMLH1 expression in vivo. These findings suggest that the anticolonic cancer effect of estrogen may be related to hMLH1 regulation. Cancer Prev Res; 3(8); 910–6. ©2010 AACR.
The aim of study was to evaluate the function of modified platelet additive solution (PAS-IIIM) with trehalose as a substitute of plasma for the storage of platelet concentrates at low temperature (10 degrees C). Apheresis platelets from 6 donors were divided and added with different media (group A: 100% plasma; group B: 70% PAS-IIIM/30% plasma; group C: 100% plasma/trehalose). Groups A, B, C were stored at 10 degrees C, 22 degrees C and -85 degrees C separately. In addition, group D (platelet concentrates stored with 100% plasma at 4 degrees C) was set up as control group for scan electronmicroscopy. The samples of each platelets were collected on day 0, 1, 5, 7 and 9 after storage respectively, while samples of platelets stored at -85 degrees C (group C) were collected on day 20 after storage. CD62p, hypotonic shock response (HSR), platelet aggregation, lactic dehydrogenase (LDH) and morphology of platelets were evaluated. The results showed that the expressions of CD62p in groups A and B increased in a time-dependent manner, but HSR and platelet aggregations decreased. The expression of CD62p, LDH release, and platelet aggregation in group A were significant higher than that in group B (p < 0.05). HSR in group A was significant lower than that in group B (p < 0.05). LDH release was significant high in samples of group C and the expression of CD62p was lower than that in other two groups (p < 0.05). It is concluded that the protective effects of 70% PAS-IIIM/30% plasma (10 degrees C) and plasma platelets (22 degrees C) on morphology of platelets are similar, but better than those of plasma platelets (4 degrees C) and plasma/trehalose (-85 degrees C). In short, PAS-IIIM serves as a good substitute of plasma for platelet storage, and protects the chilled platelets.
Objective To investigate the changes of vascular endothelial growth factor (VEGF),transforming growth factor-β1 (TGF-β1) and platelet derived growth factor (PDGF) in plasma and apheresis platelets with or without activation by shaking. Methods A total of 10 units of double therapeutic dose platelets were collected and each unit was divided into 3 groups to take samples of plasma (3ml/person,as control group),fresh apheresis platelets (5ml/person,as experimental group1) and activated apheresis platelets (platelets stored by shaking at 22 ℃ for 5 days,5ml/person,as experimental group2). The levels of VEGF,TGF-β1 and PDGF in the samples of the 3 groups were measured by ELISA. The expression of CD62p in 2 apheresis experimental groups was measured by flow cytometry. The differences between the two groups were compared. Results The levels of VEGF,TGF-β1 and PDGF,which were (410.95±95.07) pg/ml,(91.15±19.50) ng/ml and (12.60±2.06) ng/ml,respectively,in group 1 were markedly higher than those in the plasma control group,which were 149.09±28.11,37.38±10.73 and 3.28±0.79,respectively (P 0.01),while the levels in group 1 were significantly lower than that in group 2,which were 495.16±63.49,110.33±19.06 and 16.96±2.71,respectively (P0.05). Platelet counts in these samples showed a positive correlation to the levels of VEGF,TGF-β1 and PDGF growth factors.Conclusion There are high expressions of VEGF,TGF-β1 and PDGF in apheresis platelets,and activation of platelets could promote the production and release of these growth factors.
