Based on studying venous trunks and their syntopia, previously performed on a total of 160 cadavers we elaborated methods of cross shunting. Clinical application thereof in ten patients presenting with severe arterial hypertension accompanied by type 2 diabetes mellitus made it possible to decrease both mortality and complication rates.
The article presents data of experimental study of renoportal venous anastomoses made on cadaver material (160). The use of various techniques allowed to analyze the known variants, to suggest methods which enlarge possibility of Greyhack-Harris anastomosis. Clinical application showed efficacy of surgical treatment of severe and malignant arterial hypertension.
Estimation of Perftoran efficacy for preventive maintenance of ischemia-reperfusion complications in operative treatment of chronic infrarenal aortic aneyrysms was performed. Fourty nine patients were included into the study. Perfrtoran was administered intravenously during preoperative preparation in 20 patients. Control group (n=29) did not receive the drug. Complex estimation of Perftoran efficacy included analysis of postoperative complications and changes of the blood lactate level. The data obtained allow to consider that using Perftoran during preoperative preparation reduces risk of occurrence of ischemiareperfusion complications in operative treatment of infrarenal aortic aneurysms.
Under investigation were the nearest results of using a modified fashine drain in treatment of 57 patients operated laparoscopically for a complicated cholelithiasis. The method was shown to have both advantages and adverse effects as compared with the traditional kinds of drainage. The indications to drainage with a fashine drain after a laparoscopic procedure for complicated cholelithiasis were established and investigated.
Present-day methods of treatment of insulin-dependent diabetes mellitus (IDD) fail to prevent the development of complications in the majority of patients in 5-10 years after the disease manifestation. There are no radical methods for the treatment of this condition, therefore any new treatment modality that may help delay the development of complications and deterioration of the quality of life should be used along with the traditional methods. Deportalization of the pancreatic blood outflow was carried out in 148 IDD patients and its remote (up to 5 years) results analyzed. The surgery was carried out in patients with medium-severe and grave conditions. Under study were carbohydrate and lipid metabolism, liver, kidney, and heart functions, retinal vessel status, and painful manifestations of distal polyneuropathy, as well as changes of the quantity of functioning capillaries and the rheovasographic index. Surgery resulted in reduced insulin requirement, disappearance of hypoglycemic comas, reduced peripheral resistance of the vessels, increased cardiac output index and a higher working capacity of the patients, disappearance of paroxysms and pains in the lower limbs, etc. The authors suppose that changed course of the disease is connected with recovery of the insulin-glucagon coefficient in the liver in glucagon-containing blood shunting into the total blood stream and with the pharmacologic effect of glucagon getting into the systemic blood stream.
The function of the liver was studied on an experimental model of a local purulent process in 40 rabbits. It is shown that a local purulent process may cause a hepatic damaging effect, the presence of which is confirmed by the active egress of the organ-specific enzymes histidase and urocaninase into the blood. Determination of these enzymes in blood serum is an authentic test which characterizes damage to the hepatic cells in a local purulent process. Appraisal of the functional condition of the liver in local purulent processes may be employed in clinical practice for early detection and timely treatment of possible hepatotoxic damage.
An analysis of cineangiograms of lower extremity arteries of 225 patients with occlusive lesions of the abdominal aorta and lower extremity arteries has been made. Symptoms detected by cineangiography are described which allow to judge on stenotic lesions of the arteries with greater reliability than by the data of serial angiography. Quantitative analysis of cineangiograms has been performed with the determination of time of blood flow along arterial segments.
In the experiment, the possibilities for intensification and acceleration of the umbilical vein tissue stabilization, increasing the strength and improving the plastic properties, increasing the resistance of biological grafts to thrombosis due to the use of the improved method of conservation according to the scheme: dynamic stabilization in the glutaraldehyde-enzyme-glutaraldehyde-heparin solution are studied. The developed biological grafts were used in 12 patients with occlusion of the arteries of the femoropopliteal segment. After 18 mos, the grafts are patent in 8 patients.
Experimental evidence on the interaction of liposomes of varying composition with canine serum components has shown that neutral liposomes and those positively and negatively charged bind actively to different serum proteins, among which albumin and heavy and light Ig G chains have been detected. A simple and informative approach to studying the liposome-protein interactions is suggested.
Electrochemically activated 0.8% solutions of potassium chloride are marked by high bactericidal and bacteriostatic activity, do not possess toxic properties, are easily tolerated by the patients, and promote melting of fibrin and necrotic tissues. With their use granulations and epithelialization occur more rapidly and the terms of treatment and the associated economical expenditures are reduced.
Pylorus-preserving resections of the stomach were fulfilled in 118 patients with type I ulcers. The detected drop of the acid production in the stomach with ulcers of such localization was considered to be an indication to the removal of not more than 1/2 of the stomach. A necessary condition of pyloropreserving resection is the leaving of the pylorus zone not more than 2-3 cm. Two patients died (one from peritonitis and one from pulmonary insufficiency), 5 patients had disturbed evacuatory function of the stomach related with anastomosis. Long-term results were followed-up in 113 patients. All the patients returned to their normal activity, the pain syndrome disappeared, the dumping syndrome was absent. Complete inhibition of free hydrochloric acid was noted. There were no recurrences.
An experience with the treatment of 210 patients with consequences of associated chemical burns of the esophagus and stomach (174 of them were operated on the stomach at early terms after burn for impassability) allowed recommendation of operation with the least risk--shunting interventions on the stomach and esophagus. Ante-thoracal lateral jejunostomy is thought to be indicated to patients with massive injuries of the stomach or complications of gastrostomy.
A device is proposed for a noninvasive examination of hemodynamics of the aorto-femoral segment which allows to make compression of the arteries under the inguinal ligament. The arterial pressure is measured with an ultrasonic sensor under decompression of the pneumatic balloon. Under examination there were 369 patients. The examination has revealed different stages of stenosis of the iliac segment from the initial symptoms (50% stenosis) up to a pronounced stenosis. The occlusion of iliac arteries was diagnosed and collateral circulation was quantitated.
