Objectives To verify whether nutritional support is associated with Bronchopulmonary Dysplasia (BPD) by mediating serum cytokines. Methods We recorded in a retrospective cohort study the intake of enteral nutrition (EN) and parenteral nutrition (PN) in the first week, serum cytokines on Day 2–7 and Day 14–28 by examining the nutritional and clinical history of 34 newborns born at gestational age ≤ 32 weeks admitted to the neonatal intensive care unit from January 2018 to October 2019. Results Non-BPD group (24 infants) had a significantly higher gestational age and higher birth weight than the BPD group (14 infants). Among the infants with non-BPD, total proteins, total energy and EN carbohydrates, EN proteins, EN lipids, and EN energy in the first week intake and IL-4, IL-6, IL-8 on Day 14–28 and FiO2 on Day 28 were significantly lower, and IL-10 on Day 2–7 and IL-13 on Day 14–28 was significantly higher. Pearson correlation analysis showed a negative correlation between total protein, total energy and EN proteins, EN carbohydrates, EN lipids, and EN energy in the first week with IL-4, IL-6, and IL-8 on Day 14–28. In mediation analyses, total protein, EN proteins, EN carbohydrates, EN lipid, EN energy, total energy in the first week were related to FiO2 on day 28 partly mediated by IL-6 and IL-8 on Day 14–28. Conclusions Our results showed that inefficient nutrition, especially EN nutrition intake in the first week of life were associated with BPD by mediating proinflammatory cytokines on Day 14–28.
Objective:To analyze the risk factors for premature infants with hemodynamically significant patent ductus arteriosus (hs-PDA) requiring surgical treatment, and to explore the indications for surgical treatment in premature infants with hs-PDA.Methods:A nested case-control study was conducted.The data of premature infants with gestational age<30 weeks who were diagnosed with hs-PDA in the Neonatal Intensive Care Unit of Peking Union Medical College Hospital from January 2007 to May 2020 were analyzed retrospectively.The hs-PDA patients treated surgically were included in the operation group.The hs-PDA patients of the same gestational age and gender who were not treated surgically were taken as the control group.The ratio of the case number between the operation and control groups was 1∶2.The clinical data during pregnancy, at birth and after birth of premature infants were compared between the 2 groups.The measurement data were tested by the independent sample t test or Mann- Whitney U test.The classification and enumeration data were compared by the Fisher′ s exact probability method.The risk factors for premature infants with hs-PDA requiring surgical treatment were analyzed by the conditional Logistic regression method. Results:A total of 182 premature infants with hs-PDA were enrolled in the study, including 10 in the operation group and 20 in the control group.The patients underwent PDA ligation 30.5(22.7, 37.0) d after birth, and the median preo-perative invasive ventilation duration was 9.7(17.5, 27.2) d. Compared with the control group(20 cases), the preterm infants in the operation group had a lower birth weight [(891.5±118.0) g vs.(1 054.4±230.2) g, t=2.091], a wider arterial duct diameter [3.2(2.8, 4.0) mm vs.2.0(2.0, 3.0) mm, Z=-3.300], and longer invasive ventilation duration [25.0(18.7, 38.2) d vs.3.0(1.0, 7.5) d, Z=-3.688]. Besides, the operation group applied the pulmonary surfactant for more times [2(1, 3) times vs.1(1, 2) times, Z=-2.440], and inhaled a higher concentration of oxygen on the 14 th day after birth [29(25, 36)% vs.21(21, 29)%, Z=-2.358] than the control group.Moreover, compared with the control group, the operation group took longer to achieve adequate enteral feeding [48.2(51.5, 63.5) d vs.42.5(23.5, 48.0) d, Z=2.789], and gained a higher maximum vasoactive inotropic score (VIS) [3.0(0, 3.5) points vs.0(0, 0) points, Z=-2.630]. The difference in all the above-mentioned indicators between the 2 groups was statistically significant (all P<0.05). Univariate Logistic regression analysis showed that the arterial duct diameter, application times of the pulmonary surfactant, the maximum VIS score, and the time taken to achieve sufficient enteral feeding were all related to the need for surgical treatment of hs-PDA in the operation group (all P<0.05). Multivariate Logistic regression analysis revealed that the invasive ventilation duration ( OR=0.747, 95% CI: 0.560-0.998, P=0.048) was an independent risk factor for hs-PDA premature infants requiring surgical treatment. Conclusions:The factors related to the need for surgical treatment in preterm infants with hs-PDA include a wide arterial duct diameter, multiple applications of the pulmonary surfactant, a high concentration of inhaled oxygen on the 14 th day, and the long time to achieve sufficient enteral feeding.The independent risk factor for the surgical treatment in hs-PDA children is the long invasive ventilation duration.
目的 探讨早产儿出生后24~72h且医学评估稳定后,接种卡介苗的安全性和效果.方法 选取2021年12月至2022年6月北京协和医院国际医疗部产科病房出生的接种0.1 ml卡介苗的早产儿35例(病例组),另选取同时期出生的体质量N 2 500 g的接种0.1 ml卡介苗的足月新生儿35例(对照组),比较2组接种卡介苗后的卡痕率、卡痕均值和不良反应.结果 病例组胎龄为34~36+6周,平均胎龄低于对照组[(35.5±0.6)w比(38.8±0.9)w],接种卡介苗时日龄高于对照组[(1.5±0.7)d1比(1.3±0.6)d],出生时体质量低于对照组[(2 276.3±140.7)g比(3 125.0±384.1)g],差异均有统计学意义(P<0.05).病例组和对照组的卡痕率(57.1%比68.6%)和卡痕均值[(4.4±1.3)mm比(4.6±1.2)mm]比较,差异均无统计学意义(P>0.05).两组在1、3个月查体时,均未发现接种部位直径>10 mm的局部脓肿或溃疡、同侧腋下局部淋巴结肿大>10 mm或发生脓疡破溃、骨髓炎、全身播散性卡介苗感染和寻常狼疮等不良反应.结论 34~36+6周的早产儿在出生24~72 h且医学评估稳定后,接种卡介苗是安全、有效的,可在临床中推广实行.
BackgroundBronchopulmonary dysplasia (BPD) is a devastating form of chronic lung disease that develops in preterm infants. BPD is speculated to arise from abnormal inflammatory responses, which is related to the composition of commensal microbiota, leading us to hypothesize that BPD susceptibility could be influenced by gut microbiota through inflammatory responses. This study is aimed to detect cytokines and the differences in fecal gut microbial composition in the BPD patients.MethodsBetween June 2018 and June 2020, preterm infants born at gestational age ≤30 weeks were recruited. The clinical data of infant characteristics were collected. On days 3–7 and 14–28 after birth, fresh stool samples and serum were collected. The gut microbiota composition between the BPD group and controls was detected by 16S rRNA sequencing. On days 3–7 and days 14–28, ten cytokines including IL-1β, IL-2, IL-4, IL-6, IL-8, IL-10, IL-12p70, IL-13, IFN-γ, and TNF-α were detected in the serum.ResultsThis study enrolled 38 preterm infants; the number of preterm infants in the BPD group and control group was, respectively, 18 and 20. The gestational age (27.4 ± 1.5 weeks vs. 29.5 ± 0.9 weeks, p = 0.000) and birth weight (971 ± 240 g vs. 1262 ± 335 g, p = 0.000) of the BPD group were lower than those of the control group. The present study found that the BPD group had high levels of IL-1β, IL-4, IL-6, IL-8, and TNF-α, whereas IL-10 was decreased. The Shannon diversity index of the BPD group was lower. The relative abundances of Proteobacteria in BPD group increased significantly from days 3–7 to days 14–28, while the Firmicutes was decreased. On days 14–28, the relative abundances of Proteobacteria in BPD group were significantly higher than those in the control group, while the Firmicutes was lower.ConclusionBronchopulmonary dysplasia could be influenced by gut microbiota through inflammatory responses. More studies are needed to explore the imbalance of cytokines and microbiome in BPD infants and whether it could be reversed by probiotics. This study provided a novel perspective for treating BPD.
The clinical data of 5 cases of chylous ascites in preterm infants admitted in NICU of Peking Union Hospital from 2001 to 2021 were retrospectively analyzed. There were 3 boys and 2 girls with the gestational age of 29 +1 weeks, and birth weight of (1 122±323) g. No peritoneal effusion was found on prenatal ultrasound examination. All the five cases diagnosed with chylous ascites after the initiation of enteral nutrition on d4 to d10. All cases were resolved by conservative treatment, including fasting with total parenteral nutrition for 3 wks. The parenteral nutrition strategy was specified by high protein concentration (4 g·kg -1·d -1) and low lipid emulsion (2.0-2.5 g·kg -1·d -1). Formula containing 50% medium chain triglyceride or human milk was fed sequentially, and no feeding intolerance or abdominal distension were observed. All patients were discharged stable and followed up for 3-5 years,and no recurrence occurred. The PubMed and Wanfang database were searched for cases of chylous ascites in preterm infants, and 7 cases were reported in literature. Six cases were diagnosed by antenatal ultrasound between 21 and 23 weeks of gestational age. Three cases underwent radionuclide lymphoscintigraphy, and 2 of them demonstrated peritoneal lymphatic fistula or lymphatic dilatation. Five cases were treated with fasting plus intravenous infusion of octreotide. Three infants who failed to respond to conservative treatment underwent surgical treatment. Four cases were complicated with sepsis and needed intravenous antibiotic treatment.
We aim to explore the intestinal microbial metabolites in preterm infants with noninvasive methods and analyze the effects of initial feeding methods. Preterm infants with gestational weeks lower than 34 were recruited for fecal sample collection every 7 days. Fecal pH, ammonia, bile acid, and secretory IgA (sIgA) were tested. A 1:10 fecal slurry was inoculated into different culture media containing different carbohydrates as the only carbon source: lactose (LAT), fructooligosaccharide (FOS), galactooligosaccharide (GOS), and 2′-fucosyllactose (FL2). After 24 h of anaerobic culture through an in vitro fermentation system, air pressure difference, carbohydrate degradation rate, and short-chain fatty acids (SCFAs) content in fermentation pots were measured. Preterm infants were assigned into two groups: group A, preterm infants fed by human milk, including mother’s own milk and donor human milk (DHM); group B, preterm infants fed by preterm formula at first 3 days and fed by human milk (including mother’s own milk and DHM) from day 4 to discharge. Group A included 90 samples and group B included 70 samples. Group A had lower fecal pH (p = 0.023), ammonia (p = 0.001), and bile acids (p = 0.025). Group B also had higher fecal sIgA levels, both in OD (p = 0.046) and concentration (p < 0.0001) methods. Carbohydrates degradation rates in group A were higher than group B, especially in LAT medium (p = 0.017) and GOS medium (p = 0.005). Gas production amount had no significant difference in all four media. Several different SCFAs in four kinds of different culture media in group A were higher than in group B, but valeric acid was lower in group A. The initial feeding methods may affect the preterm infants’ intestinal microecology and microbial metabolites for at least several weeks.
Objective To evaluate the strengthens and feasibility of network-based teaching for clinical training in neonatology during COVID-19 pandemic. Methods Because of COVID-19 pandemic, the interns of Peking Union Medical College Hospital accepted the network-based teaching in the training of neonatology from February to May 2020. Compared with those who received traditional training from November 2019 to January 2020, the Systematic Evatuation of Teaching Quality(SETQ) questionnaire and Pediatric Voluntary Self-assessment Forms were conducted to evaluate the quality of teaching and of knowledge mastery. Results There were no significant differences in teaching quality between two groups. While network-based teaching has favored knowledge mastery for rare diseases, complicated clinical operation and the interpretation of auxiliary examinations (P<0.05). Conclusions Network-based teaching and the traditional pathway have their strengthens respectively and can be complementary in the intern training of neonatology.
Anaphylaxis is increasingly in children, which is currently undernotified, underdiagnosed, and undertreated in China.In order to further improved the understanding and management of anaphylaxis, this issue reviews the pathogenesis, triggers and risk factors, clinical diagnosis and management of anaphylaxis, thus offers the recommedations of anaphylaxis in Chinese children based on previous published evidence-based guidelines and practice parameters.Recommendation aims to develop guiding principles for the diagnosis and management of anaphylaxis in children, and provide a framework for the development of new guidelines.
Introduction: Coronavirus disease 2019 (COVID-19) swept Wuhan in January 2020. Other cities in China also suffered during the pandemic. Routine medical services were conducted in the Neonatal Intensive Unit (NICU) as usual, but the follow-up after discharge was seriously affected. Objective: To investigate the feasibility and effectiveness of a combination of online and face-to-face follow-up for preterm infants during the COVID-19 epidemic and to explore a follow-up pattern that can provide follow-up services while maximizing the protection of preterm infants and soothing the fear of their parents. Methods: Preterm infants ( n = 35) whose first follow-up appointment was scheduled from February 1 to April 30, 2020, and preterm infants ( n = 43) in the NICU follow-up group who were discharged from January 1, 2018, to January 31, 2020, who had a second or later routine follow-up appointment scheduled from February 1 to April 30, 2020, were enrolled. We provided a combination of online and face-to-face follow-up for preterm infants surveyed with the Wenjuanxing platform before and after the online follow-up and compared the first-time follow-up rate between the outbreak and the same period of the previous year. Results: Feeding and oral medicine and supplements were the most concerning problems of the parents of preterm infants. The anxiety level of the family was significantly decreased after online follow-up ( P < 0.05). A total of 96.8% of parents were satisfied or very satisfied with online follow-up, and 95.2% of parents thought that online follow-up had answered all their questions. Only 35.5% of parents thought online follow-up could replace face-to-face follow-up. Conclusion: The combination of online and face-to-face follow-up alleviated the anxiety of the parents during the outbreak and achieved a similar first-time follow-up rate as the same period in 2019.
Bronchopulmonary dysplasia (BPD) is the most common complication of extremely preterm birth. This study was aimed at detecting cytokine and fractional exhaled nitric oxide (FeNO) levels to evaluate their mechanisms and predicted significance for BPD. Preterm infants born at gestational age≤32 weeks were recruited, and clinical data were collected. We detected ten cytokines, including IFN-γ, IL-10, IL-12p70, IL-13, IL-1β, IL-2, IL-4, IL-6, IL-8, and TNF-α on Days 1–3, Days 7–14, and Days 21–28 after birth by using the Meso Scale Discovery (MSD) technology. The FeNO levels of infants were measured when they met the discharge criteria. A total of 46 preterm infants were enrolled, consisting of 14 infants in BPD group and 32 infants in the control group. The gestational age (27.5±1.3 vs. 29.9±1.3 weeks) and birth weight (1021±261 g vs. 1489 ± 357 g) were lower in the BPD group. The following were high-risk factors for BPD, as determined by multivariate logistic regression analysis: gestational age<30 weeks, birth weight<1000 g, PDA, longer mechanical ventilation, and higher FeNO. The cytokines of IL-6 and IL-8 on Days 7–14 and IL-4, IL-6, IL-8, and TNF-α on Days 21–28 were also high-risk factors for BPD. IL-6 contributed to BPD disease severity. Conclusion. The preterm infants with PDA and prolonged mechanical ventilation tended to develop BPD. The IL-6 and IL-8 were significantly increased on Days 7–14 and were high-risk factors for BPD. Moreover, the IL-6 level was associated with BPD disease severity. We speculated that NO was related to BPD via Th2 cell-mediated inflammatory responses such as IL-4 and IL-6. Cytokines might predict the occurrence of BPD.
AbstractBreast milk (BM) hormones have been hypothesised as a nutritional link between maternal and infant metabolic health. This study aimed to evaluate hormone concentrations in BM of women with and without gestational diabetes mellitus (GDM), and the relationship between maternal factors, BM hormones and infant growth. We studied ninety-six nulliparous women with (n 48) and without GDM and their exclusively breastfed term singletons. Women with GDM received dietary therapy or insulin injection for euglycaemia during pregnancy. Hormone concentrations in BM, maternal BMI and infant growth were longitudinally evaluated on postnatal days 3, 42 and 90. Mothers with GDM had decreased concentrations of adiponectin (Pcolostrum<0·001; Pmature-milk=0·009) and ghrelin (Pcolostrum=0·011; Pmature-milk<0·001) and increased concentration of insulin in BM (Pcolostrum=0·047; Pmature-milk=0·021). Maternal BMI was positively associated with adiponectin (β=0·06; 95 % CI 0·02, 0·1; P=0·001), leptin (β=0·16; 95 % CI 0·12, 0·2; P<0·001) and insulin concentrations (β=0·06; 95 % CI 0·02, 0·1; P<0·001), and inversely associated with ghrelin concentration in BM (β=–0·08; 95 % CI –0·1, –0·06; P<0·001). Among the four hormones, adiponectin was inversely associated with infant growth in both the GDM (βweight-for-height=–2·49; 95 % CI –3·83, –1·15; P<0·001; βhead-circumference=–0·39; 95 % CI –0·65, –0·13; P=0·003) and healthy groups (βweight-for-height=–1·42; 95 % CI –2·38, –0·46; P=0·003; βhead-circumference=–0·15; 95 % CI –0·27, –0·03; P=0·007). Maternal BMI and GDM are important determinants of BM hormone concentrations. Milk-borne adiponectin is determined by maternal metabolic status and plays an independent down-regulating role in early infant growth.
目的:探讨合并Bartter综合征的小儿囊性纤维化的临床表现、诊断和基因型特点。方法回顾性分析华人中首例合并假性Bartter综合征的囊性纤维化患儿的临床表现、基因型特点等,并复习相关文献。结果患儿女,16个月,临床表现为反复呼吸道感染伴发作性低钾低钠低氯性碱中毒。汗液氯离子( Cl-)左下肢为108.4 mmol/L,右下肢为120.4 mmol/L。囊性纤维化穿膜传导调节因子(CFTR)基因突变分析,发现2个已知基因突变,即R709X(母源)、G970D (父源)。结论合并假性Bartter综合征的囊性纤维化患儿主要表现为反复肺部感染、汗液Cl ->60 mmol/L;突变基因为R709X和G970D。
Objective To prospectively investigate the radiation dose and image quality of pediatric chest CT using Sn100 kV on a third-generation dual-source CT (DSCT)in comparison to standard 100 kV chest CT. Methods From December 12,2015 to June 30,2016,45 consecutive pediatric patients referred for non-contrast chest CT scan in Peking Union Medical College Hospital were prospectively enrolled as study group. They were examined at 100 kV with a dedicated tin filter on a third-generation DSCT. These patients were retrospectively matched with 45 patients who were examined on a second-generation DSCT at 100 kV without tin filter. The radiation dose as well as the lung and mediastinal window image quality(IQ)of the two groups were compared and analyzed statistically. IQ was evaluated using a five-point scale (1=unevaluable,5=excellent). Differences of radiation dose and noise between the two groups were determined with variance analysis and t test,IQ with Mann-Whitney U test,and the consistency of diagnosis with Kappa test. Results The average CT dose index volume of the study group was (0.24±0.11)mGy,which was decreased by 92% compared with the control group [(3.10+1.18)mGy] (t=16.287,P=0.000). Mean dose-length product and mean effective dose for study group were significantly lower than those of control group [(7.13±4.72)mGy·cm vs. (84.78±46.78)mGy·cm,t=11.077,P=0.000;(0.11±0.06)mSv vs.(1.23±0.61)mSv,t=12.334,P=0.000]. There was no significant difference between the two groups in terms of image noise (t=-0.003,P=0.397)and contrast to noise ratio (t=0.545,P=0.488). There was no significant difference between the two groups in lung window IQ (doctor 1:U=796.000,P=0.055;doctor 2:U=889.500,P=0.277),while significant difference was seen concerning of the mediastinal window IQ (doctor 1:U=305.000,P=0.000;doctor 2:U=276.500,P=0.000). Referring to the lung window,the median IQ for the study group and control group was 4 (3-5)and 4 (3-5),respectively. All imaging findings had acceptable IQ. The breath motion artifacts (χ2=13.846,P=0.001)and heart beat artifacts (χ2=53.519,P=0.000)of the study group were significantly lower than those of the control group. Conclusion Compared with standard 100 kV chest CT,the use of tin-filtered Sn100 kV on a third-generation DSCT provided 92% dose reduction in pediatric chest CT examinations while maintaining diagnostically acceptable lung window images.

Objective To retrospectively study the serum IgG and IgM antibodies against toxoplasma, rubella virus, cytomegalovirus and herpes simplex virus type 1&2 in various populations, and analyze the clinical values.Methods From 2008 to 2015, 2 661 pregnant women, 324 infertile women, 2 492 women with abnormal pregnancy history, 623 women with recent abnormal pregnancy, 261 infants with intrauterine growth retardation and other diseases, 170 women for preconceptual examination, and 702 women for physical examination in Beijing were included .Commercial EIA kits were used to detect serum IgG and IgM antibodies to toxoplasma, rubella virus, cytomegalovirus and herpes simplex virus type 1&2. Positive reactions of IgM antibodies to any pathogens were re-tested with another kind of commercial EIA kit. PEMS3.1 software was used for statistical analysis.Results The prevalence of serum IgG or IgM antibodies against toxoplasma, rubella virus, cytomegalovirus and herpes simplex virus type 1& 2 were found within 0.7%-1.6%(0-1.2%) , 85.3%-92.0% ( 0.4%-2.7%) , 89.1%-94.9% ( 0.7%-1.7%) , 74.8%-86.0% ( 0 -0.7%) , 8.1% -17.4% ( 0 -4.1%) respectively in the studied population groups.The prevalence of TORCH IgG and IgM antibodies were not found to be higher in both populations with past suspicious exposure ( infertile women and women with abnormal pregnancy history ) and recent suspicious exposure ( women with recent abnormal pregnancy and infants with intrauterine growth retardation and other diseases) than that in pregnant women and women for preconceptual and physical examination. Conclusion No associations between TORCH infections and the suspicious exposure were found in the populations above.

OBJECTIVETo review the clinical data of enteral feeding of extremely low birth weight infants (ELBWI), and analyze the influencing factors.METHODFrom Jan. 2000 to Jan. 2010, data of 31 ELBWI from Peking Union Medical College Hospital were retrospectively collected. ELBWI were assigned to different groups according to the time achieving full enteral feeding, comparison was done between two groups for enteral feeding.RESULTTwenty-four infants were analyzed, their mean gestational age was (29.0 ± 1.8) weeks (26.14 - 34.43 weeks), birth weight (882 ± 67) g (730 - 970 g), there were 11 infants in group A, whose time for achieving full enteral feeding was (27 ± 6)days, there were 13 infants in group B, whose time achieving full enteral feeding was (46 ± 10)days. The ratio of asphyxia (18.2% vs. 61.5%, P = 0.047), duration of umbilical vein catheterization longer than 10 days (18.2% vs. 61.5%, P = 0.047), and duration of mechanical ventilation longer than 14 days (27.3% vs. 76.9%, P = 0.038) in group A was higher than in group B. The milk volume on the 21st and 28th day in group A was much more than that in group B [(88.9 ± 35.4) ml vs. (37.4 ± 34.9) ml, P = 0.002; (121.1 ± 37.4) ml vs. (53.2 ± 33.1) ml, P = 0.000]. There were no significant differences between the two groups in gestational age, birth weight, patent ductus arterious, erythrocytosis, dysglycemia, sepsis, the time to begin enteral feeding, the beginning milk volume, the adding milk volume in the 1st, 2nd week, and the milk volume on the 3rd, 7th, 14th day.CONCLUSIONAsphyxia, duration of umbilical vein catheterization, and duration of mechanical ventilation are likely to influence the enteral feeding of ELBWI, ELBWI with successful enteral feeding could show good tolerance in the 3rd week. But individual program should be made for enteral feeding of ELBWI, because enteral feeding could be influenced by multiple factors.

The objective of this study was to determine the effects of the level of inhaled oxygen during resuscitation on the levels of free radicals and anti-oxidative capacity in the heparinized venous blood of preterm infants. Forty four preterm infants <35 weeks of gestation with mild to moderate neonatal asphyxia were randomized into two groups. The first group of infants were resuscitated with 100% oxygen (100% O(2) group), while in the other group (reduced O(2) group), the oxygen concentration was titrated according to pulse oximeter readings. We measured total hydroperoxide (TH) and redox potential (RP) in the plasma within 60 min of birth. The integrated excessive oxygen ( summation operator(FiO(2)-0.21) x Time(min)) was higher in the 100% O(2) group than in the reduced O(2) group (p<0.0001). TH was higher in the 100% O(2) group than in the reduced O(2) group (p<0.0001). RP was not different between the 100% O(2) and reduced O(2) groups (p = 0.399). RP/TH ratio was lower in the 100% O(2) group than in the reduced O(2) group (p<0.01). We conclude that in the resuscitation of preterm infants with mild to moderate asphyxia, oxidative stress can be reduced by lowering the inspired oxygen concentration using a pulse oximeter.
Objective To study the pattern of catch-up growth of very or extremely low birth weight infants during their first 24 months of adjusted age.Methods Forty-one very or extremely low birth weight infants admitted to the NICU of Peking Union Medical College Hospital from April,2001 to October,2005 were followed up till 24 months of adjusted age.Intrauterine and postnatal growth retardation was defined as measured growth parameters 10th percentile on the growth curves.The incidence of growth retardation at each time point of follow-up was calculated.The risk factors for growth retardation by 24 months of adjusted age were estimated.Results By weight,the incidence of intrauterine growth retardation was 58.5% on admission(small for gestational age),then the incidence of postnatal growth retardation was 80.5% at discharge,56.1% at 37-42 gestational weeks,39.2% till 3months of adjusted age.The incidence remained 22.0% by 6 months,12 months and 24 months of adjusted age.The risk factors for such growth retardation by 24 months were low gestational age,intrauterine growth retardation,asphyxia and maternal complication of pregnancy induced hypertension.Conclusions Very or extremely low birth weight infants showed high incidence of intrauterine growth retardation at birth and higher growth retardation at discharge.Most of them achieve catch-up growth by 6months of adjusted age.The benefits and possible risks for such catch-up growth need further study.
