PURPOSE:This study is aimed at analyzing the application experience and feedback of the patients with poststroke ankle dorsiflexion disorders for 3D printed AFO with three different materials.METHODS:15 patients were randomly divided into three groups; 3D printed AFO with 3 different materials (PA2200, Somos NeXt, and PA12) was used to each group, according to the crossover study design, in order to ask the three groups of patients to use three different materials of 3D printed AFO. Assessment was taken by the end of each test round. Through statistical processing, the patient feedback data of the three groups of materials of 3D printed AFO were obtained.RESULTS:In the material comfort assessment of the AFO, Somos NeXt was compared with PA2200, and the p value was <0.05; in the item of surface smoothness of the AFO, Somos NeXt was compared with PA2200, and the p value was <0.01; at the same time, PA12 was compared with PA2200, and the p value was <0.05.CONCLUSION:The 3 different materials of 3D printing AFO bring different experience, and we also have sufficient reason to believe that there will be differences in the auxiliary effect of this on patients, which leads the patient's selection too. The material Somos NeXt is much popular and has certain clinical advantages.
Objective To explore the effect of bilateral hand function training on the hand function of stroke survivors.Methods Thirty-one patients with unilateral hand dysfunction after stroke were randomly divided into an experimental group of 16 and a control group of 15.Both groups were given 60 minutes of hand function training of their affected hands daily,five days a week for 4 weeks.The experimental group was additionally provided with 40 minutes of bilateral hand training on the basis of the aforementioned intervention.Before the treatment and after the 4 weeks,integrated surface electromyography (iEMG) signals were recorded from both hands' flexor and extensor muscles.Both groups were also assessed using the Fugl-Meyer motor assessment for the upper extremities (FMA-UE).Results After the treatment,the average iEMG of the flexor and extensor muscles during flexion and extension were significantly better than before the treatment in both groups.There was no significant difference between the two groups in their average iEMGs before the treatment,but afterward significant differences were observed in all the iEMG measurements.After the treatment the average total FMA-UE score of the experimental group was significantly higher than that of the control group.Conclusion Bilateral hand function training is significantly more effective than unilateral training in promoting the recovery of flexor and extensor muscle function after a stroke.
目的:探讨早期应用淋巴引流手法结合持续被动活动(CPM)对膝关节周围骨折术后膝关节功能康复的临床疗效.方法:41例膝关节周围骨折术后患者随机分为观察组21例和对照组20例,对照组予常规康复治疗(包括关节松动 、肌力训练 、冰敷等),观察组在常规康复治疗的基础上应用淋巴引流手法及CPM治疗.治疗前及治疗4周后测量患肢膝关节周径评定水肿情况,采用关节活动度及lysholm膝关节功能评分(LKSS)评定膝关节功能.结果:治疗4周后,2组膝关节周径较治疗前均降低(均P<0.01),其中观察组降低更明显(P<0.01);2组ROM、LKSS评分较治疗前均提高(均P<0.01),其中观察组提高更明显(P<0.01).结论:膝关节周围骨折术后早期应用淋巴引流手法结合CPM能减轻膝关节肿胀 、改善膝关节活动度及膝关节功能.
Objective To observe the effect of acupuncture and family exercise on the pain and quality of life in the patients withcervical spondylotic radiculopathy. Methods A total of 104 patients with cervical spondylotic radiculopathy were randomly divided into 4 groups: the control group was treated with cervical traction therapy, the exercise group were with traction therapy and family exercise, the Acupuncture group were with traction therapy and acupuncture, and the combined group were with traction therapy, family exercise and acupuncture. The short-form of McGill Pain Questionnaire (SF-MPQ), Northwick Park Neck Pain Questionnaire (NPQ) and WHO quality of life (WHOQOL-BREF) were compared between two groups. Results After treatment, the SF-MPQ were (5.8 ±1.0, 7.1 ±1.1, 7.9 ±2.1 vs. 9.3 ±1.1, F=44.897) and NPQ (12.9 ± 2.0, 15.5 ± 1.6, 15.9 ± 3.4 vs. 17.8 ± 1.9, F=69.730) in Comb. G, Acu. G, Exe. G were significantly lower than those in the Ctrl G. (P<0.01). The scores of physiology, psychology, society and environment of WHOQOL in the Comb. G, Acu. G and Exe. G after treatment were significantly higher than that in the Ctrl G. (F=154.216, 190.314, 65.227, 344.897, all Ps<0.001). Conclusions Four groups’ interventions are effective for cervical spondylotic radiculopathy. The combined interventions have more positive impact on pain management and quality of life in patients.
目的:观察互动式头针联合易化技术对卒中患者下肢运动功能的影响.方法:90例卒中恢复期伴下肢运动障碍患者随机分为互动式头针组、传统头针组和对照组,各30例,分别给予对应治疗,连续治疗4周后按5米折返试验评定移动能力、Berg平衡量表评估平衡功能、改良Barthel指数综合评价整体康复疗效.结果:治疗后3组5米折返试验时间、Berg评分、改良Barthel指数均较治疗前显著改善(P<0.01);治疗前后差值比较,互动式头针组5米折返试验、Berg评分均大于传统头针组和对照组(P <0.05),改良Barthel指数大于对照组(P<0.05),与传统头针组比较无显著差异(P>0.05);治疗前后差值比较,传统头针组5米折返试验、Berg评分与对照组比较无显著差异(P>0.05),改良Barthel指数大于对照组(P<0.05).结论:互动式头针方案有助于卒中患者下肢运动障碍改善,提高其移动能力.
骨化性肌炎( myositis ossification,MO)是一种异位性骨化[1]. 在外伤等多种诱因下,于骨骼系统以外的组织、骨骼肌、韧带、肌腱、关节旁、腹膜腔内发生的异常异位骨化,常表现为局部肿块、肿胀疼痛、受累关节活动受限,是一种自限性良性的局限性骨化性软组织肿块[2]. 而神经源性骨化性肌炎( neurogenic ossifying fibromyopathy )指患者持续遭受严重大脑或脊髓损伤后在肌肉组织、皮肤、皮下组织、筋膜、肌腱骨膜、血管壁、肌腱、韧带及关节附近的纤维组织内发生,通常发生在四肢肌肉及关节连接部,以髋关节最多见,其次为肘、肩及膝关节部位多见[3]. 常见于男性. 病程长短不一,发病范围逐渐波及全身,临床上较为少见,易与多种疾病混淆,为康复科少见疾病[4].
Objective To study the protective effect of scalp acupuncture combined with exercise training on neurons in rats with ischemic brain damage.Methods The experiment contained five groups:negative control group,positive control group,acupuncture group,rehabilitation group and experimental group.The behavioral score of rats was evaluated by modified Bederson's neurological score.The expression of BDNF protein was detected by immunohistochemistry and the expression of BDNF mRNA was detected by RT-PCR.Results Compared with the acupuncture group and the rehabilitation group,the neurological function scores of the experimental group were significantly decreased (P <0.05) 7 days and 14 days after operation.The numbers of BDNF positive cells in experimental group were significantly increased compared with that of rehabilitation group and acupuncture group (P <0.05) 3 days and 21 days after operation.The expression of BDNF mRNA was significantly increased compared with that of the acupuncture group and the rehabilitation group (P <0.05) 7d and 14d after operation.Conclusion Scalp acupuncture combined with exercise training can increase the synthesis of BDNF with up-regulating the expression of BDNF mRNA,and protect the injured neurons and improve the behavior of rats.
目的:比较单纯牵引与综合物理疗法治疗腰椎间盘突出症的疗效。方法:116例腰椎间盘突出症患者分为观察组62例和对照组54例,均采用腰椎间歇牵引治疗;观察组同时配合微波及温热式低周波疗法。结果:治疗1个月后,观察组治愈显效率明显优于对照组(77.4%与50.0%,P<0.05),总有效率,2组比较差异无显著性意义(91.9%与90.7%)。结论:综合物理治疗更能显著提高腰椎间盘突出症的临床疗效。
