目的 设计并制作基于证据的社区居家失能老年人吞咽安全科普作品.方法 采用循证护理及德尔菲法,经文献系统检索、文献质量评价、证据提取及证据级别评定,形成最佳证据总结,并通过两轮专家函询,构建吞咽安全教育内容体系,基于该体系,对文本内容进行细化及科普性转化,由多学科团队共同设计并制作系列科普作品.结果 从安全教育目标、吞咽困难识别、吞咽功能锻炼、口腔健康管理等8个方面总结共31条最佳证据;2轮函询专家积极性、意见权威性及协调度均较好,最终确立的社区居家失能老年人吞咽安全教育内容体系包含4个一级指标、11个二级指标、21个三级指标;吞咽安全系列科普作品包括手册、视频、音频,具有通俗易懂、生动形象、要点突出等特点.结论 社区居家失能老年人吞咽安全科普作品研发过程科学,注重科普作品的友好化设计,为其他学者制作健康科普类作品提供方法借鉴.
目的 分析近10年康复辅助器具领域国内外研究现状、研究热点和趋势.方法 检索2012至2021年中国知网和Web of Science(WoS)数据库核心合集相关文献.应用CiteSpace及VOSviewer可视化分析作者、国家/地区、机构、研究热点及趋势.结果 与结论 国内外年发文量均呈逐年上升趋势.国内发文最多的团队为喻洪流团队,发文最多的机构为中国残疾人辅助器具中心.国外发文最多的团队为Maysam G团队,发文最多的机构为匹兹堡大学.国家发文量最多及中介中心性最高的是美国.国内外康复辅助器具相关研究热点主要集中于康复辅助器具产品的设计与研发、康复辅助器具在特殊/脆弱人群中的应用、康复辅助器具及其相关专业服务的应用效果研究;预测基于智能感知、类脑计算、深度学习及虚拟/增强现实的智能辅助器具产品设计研发及应用研究是本领域的发展趋势.
Objective To discuss the effects of minimally invasive plate fixation and interlocking intramedullary nail for treatment of ulnar fractures. Method 85 patients with ulnar fractures were selected as the subjects. 51 patients treated with minimally invasive plate fixation were included in the minimally invasive group while 34 patients treated with interlocking intramedullary nail were included in the intramedullary nail group. The status of surgery,postoperative recovery and the incidence of postoperative complications were compared between the two groups. The wrist function was evaluated by Gartland-Werley wrist scoring system,forearm rotation angle and grip strength at 3 months after surgery and at the last follow-up. Results The surgical time,hospitalization time and fracture healing time of minimally invasive group were shorter than those of intramedullary nail group ( P<0. 05) . The excellent and good rate of Gartland-Werley wrist score in minimally invasive group at 3 months after surgery (74. 51% ) was higher than that in intramedullary nail group (50. 0% ),and the forearm rotation angle and grip strength were larger than those of intramedullary nail group (P<0. 05),but there was no significant difference at the last follow-up ( P>0. 05 ) . The incidence of complications in minimally invasive group ( 5. 88% ) was significantly lower than that in the intramedullary nail group (23. 53% ,P<0. 05). Conclusion Compared with interlocking intramedullary nail, minimally invasive plate fixation can shorten the surgical time and fracture healing time,recover the wrist function in a short period and reduce the incidence of postoperative complications.
目的 探讨股骨髓腔内使用微孔多聚糖止血球止血,对于减少全髋关节置换术失血的安全性和有效性.方法 选择初次单侧人工生物型假体全髋关节置换患者90侧.采用抛硬币法随机分为试验组(术中股骨髓腔内止血),对照组(不予股骨髓腔内止血)两组.两组均常规使用氨甲环酸(术前30 min,1g/100 mL静脉滴注,术毕术区灌注0.5 g/50 mL,术后6h,1 g/100 mL,静脉滴注)止血基础上,股骨髓腔内选用微孔多聚糖止血球止血.手术均采用后外侧入路(改良Gibson入路),由同一术者完成.结果 术中股骨髓腔内止血组的总出血量、术中出血量、隐性失血量、术后引流量均显著少于对照组,差异有统计学意义(P<0.05),两组均无输血患者,均未发生感染,均有一例患者发生深静脉血栓,两组患者术后半年均无假体松动,术后半年患者髋关节功能评分(Harris评分)差异无统计学意义.结论 试验表明股骨髓腔使用微孔多聚糖止血球止血,能有效、安全地减少初次单侧人工生物型假体全髋关节置换的出血.
