帕金森病是第二常见的神经退行性疾病,好发于老年人,并且随着人口老龄化的发展其重要性日益增长,其症状包括运动症状如静止性震颤、肌强直、运动迟缓和姿势步态异常等,非运动症状如嗅觉减退、便秘、抑郁、焦虑、睡眠障碍等,其临床表现严重影响老年人的生存质量,因此,帕金森病的早期诊断十分重要.本文对2016年最新的帕金森病诊断标准及近年来帕金森病常规与功能影像诊断综述如下.
目的采用CT和MRI图像融合技术评价帕金森病(PD)患者深部脑电刺激(DBS)术后电极位置准确性的应用价值。方法收集接受双侧DBS疗法的32例PD患者,采用术后颅脑薄层CT和术前MRI图像融合(融合组)技术并与常规术后复查MRI图像(常规组)比较,分析电极位置精确度及检查耗费时间。结果融合组与常规组测量的电极位置相关性较好(P均<0.008);融合组与常规组电极尖端位置比较,除左侧电极y轴、z轴方向差异有统计学意义外(t=-2.34、-3.08,P均<0.05),余差异均无统计学意义(P均>0.05)。术后复查MR检查时间为(7.65±0.33)min,术后复查CT时间为(2.85±0.29)min,差异有统计学意义(P<0.05)。结论 DBS术后应用头部薄层CT与MRI图像融合能精确定位电极位置,并可避免PD术后复查MRI的潜在风险,缩短术后复查的时间。
Objective Getting the thalamus segmentation atlas by using the theory of probability tractography algorithm to deal with diffusion tensor imaging data,provides reference clata to research the connectivity between thalamus and cortex.Methods We convert and analyze the data,which is collected from thirty healthy volunteer scanned with regular T2 sequence and DTI sequence,to obtain the thalamic sementation atlas by using the theory of probability tractography algorithm.Results The thalamus segmentation arias we got was divided into seven parts with different colors,which reveal the connection type between thalamus and cortex respectively.We compare the thalamus segmentation atlas with the general thalamus nucleus atlas,to find that their connection type are very similar.Conclusion The thalamus segmentation atlas has a strong consistencY with the general thalamus nucleus arlas,which is helpful for usto understand further the anatomy and function of the thalamus,and it also can provide an anatomical basis for the surgery which targets on the thalamus.
