OBJECTIVETo evaluate the effectiveness of open reduction and internal fixation and repair of palmar ligment in treating trans-scaphoid perilunate dislocation.METHODSFrom June 1995 to June 2001,14 patients with trans-scaphoid perilunate dislocation were treated with open reduction and internal fixation and repair of palmar ligment. Among them,there were 13 males and 1 female,the ranging in age from 21 to 38 years,averaged 25.4 years. All patients were posterior dislocation and all operations were performed within 2 weeks after injury.RESULTSAll patients were followed up from 24 to 60 months with an average of 28.3 months. Thirteen scaphoid fractures were primary healed and functions of wrist joint were good. Bone disunion was found in 1 case and part functions of wrist joint were limited. No found necrosis of lunate and scaphoid. According to clinical scoring system of Cooney, 9 case got excellent results, 3 good, 1 fair and 1 poor.CONCLUSIONOpen reduction and internal fixation and repair of palmar ligament is effective in treating trans-scaphoid perilunate dislocation,which can early provide steady fixation for scaphoid,and profit to recover blood supply of lunatum and subterminal scaphoid.
Objective: To evaluate the preliminary result of clinical application in the treatment of completely dislocated acromioclavicular joint by allogenic tendon repair of coracoclavicular ligament. Methods:From Oct. 1997 to Dec. 2001,21cases of completely dislocated acromioclavicular joint were treated with allogeneic flexor tendon to repair the coracoclavicular ligament, which was stored in deep hypothermia. Reconstruct it through tissue hole,in the direction of coracoclavicular ligament fibers,using double-S inter-crossing way. The allogenic flexor tendons were used to rebuild the coracoclavicular ligament for vertical stability of the acromioclavicular joint. Results: Nineteen cases were followed up from 12 to 40 months,averaged 16.8 months. Evaluated according to the criteria of Lazzcano and Karlsson, 15 out of 19 cases got excellent result and 4 good. Conclusion:Allogenic tendon repair of coracoclavicular for treatment completely dislocated acromioclavicular joint is effective. It coud be procured before operation, and avoid the injury and complication caused by the autograft. It is effective when the artification tendon produced with tissue engineering was not completely succeed.
患者男性,15岁.1995年9月因左小腿中1/3前内侧有一肿物.疑为血管瘤,在当地医院行左动脉造影检查术,所用造影剂不详,未发现异常影像,故未进一步治疗.胫前肿物自然消失.然而造影后却发现左小腿渐肿胀,左足出现下垂畸形,并逐年加重.于1999年11月收住我院诊治.
