目的:探讨采取自制袋式减压约束带用于防止神经外科非计划性拔管(UEX)的价值.方法:选取300例神经外科烦躁病人并随机分成A、B、C三组,每组100例,三组均使用约束带进行固定,A组用传统约束带固定,B组用球拍式约束带固定,C组用自制袋式减压约束带固定,比较三组不良事件发生情况.结果:在非计划拔管次数、自解约束带次数及损坏床单元次数、约束部位皮肤损伤、经济成本上,C组均明显低于A组与B组,对比差异显著(P<0.05).结论:对神经外科烦躁病患,使用自制袋式减压约束带进行肢体固定,可有效预防UEX的发生,值得推广.
[目的]探讨深静脉输液导管采血对血常规和生化检验结果的影响。[方法]选择危重病人40例进行研究,用每例病人的深静脉导管血与对侧肢体浅静脉血标本作自身对照。观察组经深静脉导管采血送检,同时在深静脉导管对侧常规采血进行检验。两组标本同时送检,对两组结果进行统计分析。[结果]两组红细胞计数、白细胞计数、血小板计数、血红蛋白检查结果比较,差异无统计学意义(P>0.05);两组血钾、血钠、血氯、血钙、尿素氮、二氧化碳结合力比较,差异无统计学意义(P>0.05)。[结论]正确采用深静脉输液导管采血方式不影响血常规和生化检验结果,深静脉输液导管采血可作为血常规和血生化检查的血液标本采集方式。
Objective To analyze the application of blood samples collected through deep cervical venous catheter to the care of critically ill patients. Methods A total of 40 cases of critically ill patients in our hospital from April 2014 to June 2016 were collected and analyzed. Blood was drawn from different parts, namely shallow venous blood and the contralateral limb deep vein blood. The blood routine, blood biochemistry and the incidence of disease complications of the two ways were compared. Results The differences in terms of the RBC, WBC, Hb and PLT of the contralateral limb deep vein blood group and the superficialx venous blood group were not statistically significant ( P>0. 05). The differences in terms of the blood biochemical, sodium, serum chloride, calcium, blood urea nitrogen and carbon dioxide binding force of the contralateral limb deep vein blood group and the superficial vein group were not sta-tistically significant ( P>0. 05 ) . The rate of complications of the contralateral limb deep vein blood group was 10%, which was significantly lower than that of superficial venous blood group 32. 5%, and the difference was statistically sig-nificant ( P<0. 05 ) . Conclusions Blood samples collected through deep cervical venous catheter can solve such prob-lems, for example, peripherally repeated puncture, damaged blood vessels and surrounding tissue, meanwhile it can reduce complications with simple operation and less occupational exposure, and can improve the venous blood safety as well, which has good clinical value.
