In this data, we analyzed patients’ histories of 6466 outpatients with gastrointestinal (GI) diseases who visited gastroenterologist in Minsk Clinical Consulting Diagnostic Centre. 1698 patients with irritable bowel syndrome (IBS) were selected. It was revealed that IBS is a wide spread functional bowel disorder and its proportion among GI diseases was 26,3%. IBS in females is 2.5 times more common than in males. IBS occurred in all age groups with a small difference in the frequency of subtypes by age, mainly at the overage age.
The article presents a review of Russian and international literature dedicated to the problem of painful ejaculation. Mentions of this type of disorders are quite rare. Its real incidence remains unknown. Connections between painful ejaculation and infectious and inflammatory diseases (orchitis, epididymitis, prostatitis, vesiculitis, urethritis), previous surgeries (in particular, transurethral resection of the prostate), administration of certain drugs (for example, antidepressants), rare congenital developmental anomalies are considered. Data on the effectiveness of conservative and surgical treatment are presented.
Introduction. Priapism is a prolonged painful erection lasting for more than four hours, which either persists after the termination of sexual stimulation or is not associated therewith. Priapism may be the first clinical manifestation of chronic myeloid leukemia (CML).Aim. To describe a clinical case of ischemic priapism that occurred in a patient with a previously undiagnosed CML.General findings. We present a clinical observation of ischemic priapism having lasted for 18 hours, which occurred in a patient with a previously undiagnosed CML. The diagnosis of CML was subsequently established on the basis of a study of peripheral blood and bone marrow. The therapeutic tactics consisted in an immediate puncture aspiration of blood from the corpora cavernosa and an intracavernous injection of a phenylephrine solution. After priapism had been relieved, an CML therapy was applied, which allowed the recurrence of priapism in the patient to be avoided.
Delayed ejaculation is a form of sexual disorders, which is characterized by constant or intermittent delays or absence of ejaculation and orgasm, despite normal sexual arousal and erectile function. Delayed ejaculation is one of the most studied and rare types of male sexual dysfunctions, which leads to depression, anxiety, and often is a reason of low self-esteem, reduced satisfaction of a man with his partner, and worsening of relationships between partners. In some cases, delayed ejaculation and anejaculation cause infertility. Current views on epidemiology, diagnostics and treatment strategy of delayed ejaculation are presented in the article.
Blood disorders, in particular chronic myeloid leukemia (CML), are among the rare etiological factors of priapism. A clinical observation of ischemic priapism lasting 18 hours, which developed in patients with previously not diagnosed CML, is presented in the article. The CML was subsequently diagnosed based on the results of blood analyses and bone marrow aspiration. Treatment strategy consisted of immediate aspiration of blood from the cavernous bodies (19G) followed by the intracavernous injection of phenylephrine solution. After resolving priapism, CML therapy was carried out, which allowed to avoid a recurrence of priapism.
Priapism may occur in patients with hematological disorders with excessive white blood cell counts, such as chronic myeloid leukemia (CML). Priapism may be the first clinical manifestation of CML. At this work we report a case of a young adult who presented with ischemic (low-flow or veno-occlusive) priapism which had persisted for the last 18 hours as the first manifestation of CML. CML was diagnosed on the basis of peripheral blood and bone marrow examination . Treatment of the priapism was initiated by corporal aspiration and intracavernous phenylephrine irrigation. Furthermore, along with management of priapism, appropriate treatment of CML allows to reduce risk of recurrence of priapism in this patient.
Breath ammonium test is an alternative method for diagnostic of H. pylori infection. For several years we work with this test. Our studies are showing high levels of sensitivity and specificity for this test. Also we see decreasing prevalence of H. pylori infection since 2009 to 2014. Breath ammonium test shows a high efficacy in comparison to histological method and polymerase chain reaction and can be recommended to use in diagnostic of H. pylori infection.
Протокол № 2, Приложение 2 УТВЕРЖДЕНЫ XVI Съездом НОГР Москва, 24-25 ноября 2016 г. Президент НОГР д.м.н., проф. Л. Б. Лазебник
Relevance. Currently, there is a need to study the genetic diversity of H.pylori in patients with variety of acid-related diseases to develop new strategies for the treatment of patients with H.pylori to predict high efficiency of treatment. Objective: To assess the effectiveness of schemes of eradication therapy in patients with chronic pancreatitis and concomitant H.pylori infection. Materials and methods. The study included 108 patients with H.pylori infection: 63 patients had chronic pancreatitis and were concomitant with H.pylori-infection and 45 patients were without chronic pancreatitis and had H.pylori infection with a chronic gastritis. All patients were determined by factors of pathogenicity of H.pylori by immunoblotting. After forming the group randomized patients received eradication therapy scheme I and the scheme I with inclusion of bismuth tripotassium dicitrate. Conclusions. The effectiveness of H.pylori eradication therapy is dependent on the genetic component of H.pylori. In the presence of H.pylori pathogenicity factors p33, p30, p26, p19, p17 in order to increase the effectiveness of treatment the scheme of eradication therapy I line drugs bismuth tri dicitrate should be included.
