The article shows the possibilities of multispiral computed tomography (MSCT) studies in the assessment of osteoregeneration of bone tissue after application of hydroxyapatite for filling of bone defects at different surgical operations. The comparative assessment of the rate of consolidation according to X-ray computed tomography (CT) in patients with bone fractures treated with the use of hydroxyapatite and without it is presented in the article. We studied 17 people (10 males and 7 females; average age - 39,3 ± 12,4 years). Revision arthroscopic plasty of anterior cruciate ligament of knee (n = 7 (58,3 %)), hydroxyapatite plasty of defect of bone tissue at the osteosynthesis of long bone fractures by metal constructions (n = 5 (41,6 %)) were realized. In 7 (58,3 %) cases the operation was performed with use of hydroxyapatite and in 5 (41,6 %) cases the surgery of osteosynthesis was realized with use of titanium plates. The dynamics of consolidation was determined by X-ray CT in 1, 2 and 4 months after surgery. As a result we received significant difference on the results of studies that testified to the stimulating effect of hydroxyapatite in the process of consolidation of fractures.
The aim. of the work was to introduce implants with bioactive coating in the practice of treatment of long tubular bones and. to assess speed of consolidation in comparison with bioinert constructions with use of clinical data, radiation and biochemical methods of examination. Use of bioactive calcium-phosphate coatings obtained by the method of microarc oxidation for the osteosynthesis of long tubular bones is accompanied by more effective consolidation of fracture area in comparison with titanium plates. In 4 months after the operation with use of bioactive implants 95,2 % of patients had. active formation and. formed callus and. only 4,8 % of patients didn't have signs of consolidation. In the same terms at the fixation by bioinert plates adhesion of fracture didn't happen in 24 % of cases. Main mechanism of positive action of bioactive implants is connected with increase of calcium depot in blood and. decrease of markers of bone resorption in the first months after their installation.
In our opinion, determination of the levels of RANTES-CCR5 system expression on blood mononuclear leucocytes in vitro can be used for individual evaluation of the perspective of implant material as inflammatory/osteoinductive reaction trigger in patients with osteogenesis imperfecta (OI). Besides, RANTES gene expression on blood mononuclear leucocytes may be effective in the development of the methods to predict the degree of inflammatory (osteolytic) response, while CCR5 gene expression in the development of the methods to predict the process of reparative bone tissue remodeling in response to using different implants.
