Eleven adult patients with stable sickle cell disease (SCD) were investigated twice at a 15-day interval by nail fold capillaroscopy (NFC), bulbar conjunctival angioscopy (BCA), and laser Doppler flux metering (LDF). NFC and BCA are essentially descriptive. NFC revealed, in some patients, network patterns similar to those observed in vasculitis. BCA provides a reproducible method for the visual assessment of the microcirculation. When using a rating scale with five criteria it identified abnormalities related to both altered red blood cells and vascular morphology but failed to discriminate between patients. LDF, in contrast, is quantifiable and showed reproducible characteristic reactive alterations of vasomotion in response to ischemia after 3 min of occlusion. The SCD patients had a delayed response, best measured by the recovery half time, which was four to five times longer than that of normal subjects. In addition, the cold provocative test was always abnormal, as evidenced by a significant drop in the contralateral flux, and was often poorly tolerated. The observed trends suggest that BCA and LDF should be performed in a large cohort of patients, both in the steady state and during vasoocclusive crises. These methods seem to be useful complementary noninvasive procedures to cellular and clinical evaluation monitoring for therapeutic trials.

Проведенное исследование подтвердило исключительно большую роль расстройства микроциркуляции при ХВН и показало чрезвычайную важность неинвазивных методов исследования, особенно ККС, в диагностике нарушения микроциркуляции при варикозной болезни и ХВН, выявило характерные закономерности изменения ККС картины при различных стадиях болезни и дало основание делать соответствующие прогнозы на дальнейшее течение и развитие данного заболевания. Будущие исследования углубят наши знания о генезе ХВН, что будет служить к улучшению диагностики и лечения данной патологии.
This article gives a brief analysis of present-day techniques for the microcirculatory examination of patients with chronic venous insufficiency (CVI) of lower limbs. The last several years have seen a growing interest in the microcirculatory examination of patients with this pathology. This article considers the application of laser Doppler flowmetry (LDF) to the microcirculatory examination in terms of CVI severity and LDF capabilities. Broad experience in this area enabled us to specify recommendations on LDF application to the early diagnosis of microcirculatory disorders in CVI patients. The results obtained demonstrated promising trends in the LDF use for the further study of the CVI problem.
Microcirculation in the patient's foot skin with different stages of arterial ischemia, was examined with the aid of TV-capillaroscopy, laser dopplerography, polarography of transcutaneous oxygen tension. The effect of semiconductor laser (wave length 0.98 mcm) on microcirculation system was studied. The dependence of capillary blood flow on the transcutaneous oxygen tension was determined. The laser therapy proved to be successful in the patients with the II and III stages of arterial ischemia.
The effects of a low-intensity laser beam in the IR-range of the spectrum (wavelength 0.89-1.3 μm) on blood microculation in man were studied when the projection of large blood vessels (femoral, carotid, popliteal arteries) and acupuncture points were irradiated. A stimulating effect of laser irradiation on the microcirculation was demonstrated, characterized by the activation of capillary bloodflow, arteliole distension, and by the reduction of the level of intravascular erythrocyte aggregation, A highly effective therapy with low-intensity lasers has been noted for treating obliterating lesions of vessels in the lower extremities and for correcting venous impairments.
