Purpose. The purpose of the study was to assess the intensity of endogenous intoxication and oxidative stress in the coronary sinus and peripheral blood of coronary artery disease patients undergoing coronary artery bypass surgery. Materials and methods. The study involved 30 patients with coronary artery disease. Serum concentrations of lipid peroxidation products, total antioxidant activity and endogenous intoxication parameters, i.e. substances of low and medium molecular weight, medium molecular weight peptides, were assessed in coronary sinus and peripheral blood. Results. Long-existing cardiac ischemia certainly accompanied by a significant accumulation in the blood of patients with lipid peroxidation products and the products of endogenous intoxication. In peripheral blood total antioxidant activity is reduced by 2 times unlike sinus. In the study of indices of lipid peroxidation in the reperfusion period there was a reduction of primary and secondary lipid peroxidation products – diene conjugates and malondialdehyde, and later during reperfusion is increasing lipid peroxidation products. Conclusions. Coronary artery bypass surgery in coronary artery disease patients is accompanied, on one hand, by the increase in the activity of lipid peroxidation processes and decreased antioxidant protection, and, on the other hand, by increased levels of endogenous intoxication products.
In the experiments on the isolated perfused rat heart the effects of liposomes, containing different concentrations (0.25 and 0.1 mg/mL) of emoxipine, on coronary flow restoration after total normothermic ischemia and reperfusion were studied. The coronary flow, levels of nitrates and nitrites in the outflowing perfusate from heart and level of free radical processes were assessed, The obtained results showed that 0.1 mg/mL liposomal emoxipine provide with stronger increase coronary flow during reperfusion mostly due to the increase concentration of endothelial nitric oxide compare with treatments at 0.25 mg/mL.
В экспериментах на изолированном перфузируемом сердце крысы изучали влияние липосом, содержащих в своем составе различные концентрации (0,25 мг/мл и 0,1 мг/мл) эмоксипина, на сократительную функцию и свободнорадикальный статус изолированного сердца, подвергшегося тотальной нормотермической ишемии и реперфузии. Оценивали сократительную функцию сердец, уровень миокардиальных маркеров в оттекающем от сердца перфузате и уровень свободнорадикальных процессов в миокарде. Полученные результаты свидетельствуют о том, что меньшие концентрации эмоксипина (0,1 мг/мл) в составе липосом обеспечивают уменьшение реперфузионной сократительной дисфункции сердца, максимальное снижение степени повреждения сарколеммы кардиомиоцитов и более выраженное (по сравнению с 0,25 мг/мл) ингибирующее влияние на процессы перекисного окисления липидов в ишемизированном миокарде в период реперфузии.
