Целью исследования было изучение предсказательной ценности факторов риска в отношении развития цереброваскулярных осложнений у больных артериальной гипертонией по данным пятилетнего наблюдения. Исследование включало 80 пациентов в возрасте от 25 до 60 лет, страдающих эссенциальной гипертонией. Всем больным исходно и повторно через 5 лет наблюдения были проведены клинико-лабораторное обследование, ЭхоКГ, УЗИ сонных артерий, суточное мониторирование АД. После 5-летнего периода наблюдения у 11 (13,7 %) пациентов течение артериальной гипертонии осложнилось развитием мозгового инсульта. Большинство этих пациентов являлись курящими, у них отмечался повышенный уровень холестерина и высокая степень AT. Выявлено, что повышение систолического АД явилось предиктором развития инсультов. Кроме того, у пациентов с осложнениями наблюдалось повышение вариабельности АД, недостаточное ночное снижение АД или его ночное повышение. Осложнения ТВ чаще наблюдались у пациентов, которые не принимали гипотензивные препараты. Полученные данные свидетельствуют, что огромное значение в развитии цереброваскулярных осложнений придается курению, гиперхолестеринемии, степени повышения систолического АД и нарушению суточного профиля АД.
Structural changes of brain in patients with hypertension depending on the clinical manifestations of chronic cerebrovascular pathology according to MRI are investigated. It is shown that in hypertensive patients structural changes of brain considerably advance the development of clinical manifestations of cerebrovascular pathology. MRI signs of hypertensive encephalopathy are registered in patients with subclinical stage of disease. This group of patients should become of high priority when carrying out preventive and rehabilitation medical actions.
35 hypertensive patients with IHD, in 6 months after CABG, were bollowed-up in the study. The tolerance increase in physical activity in patients who have reached of optimal level the BP with use metoprolol and irbesartan is noted. Improvement in a daily profile the BP associated with long controllable treatment is presented. Necessity of 24-hours ambulatory blood pressure monitoring in hypertensive patients with IHD after CABG is shown.
The study included 15 patients with arterial hypertension and verified periventricular enhancement of Т2-weighted MR image in a zone around the horns of lateral brain ventricles. All patients as well as control nonhypertensive group underwent MRI study enhanced with injection of paramagnetic contrast agent as 2 ml of 0,5M saline sloution per 10 kg of BW. The enhancement of periventricular T1-weighted image after contrast injection was quantified as ratio of MRI intensities after and before injection. Increase of permeability of blood-brain barrier in zones of periventricular hypostasis was revealed.
The aim of this study was to test the hypothesis that sympathetic hyperactivity may be conducive to early brain damages in hypertensive patients. We examined 49 asymptomatic, non-complicated essential hypertensives aged from 26 to 57 years. Positive correlation between the parameters describing sympathetic activation depth of PVH and between the parameters describing sympathetic activation and linear sizes of subarachnoidal space was revealed. Our results showed that degree and depth of LA correlated with high sympathetic and low parasympathetic tone. Correlation between sympathetic hyperactivation and liquorodynamics disturbances was revealed too.
Original mathematical algorithm of detecting real nighttime periods of relatively lower HR according to ABPM data series which also allows to estimate traditional BP dip was developed. Calculation of nighttime BP dip based on detection of individual nighttime directly from ABPM data produces more effective clinical efficacy compared to standard method.
