Оценить влияние перегрузки жидкостью на сроки пребывания в ОРИТ, длительность механической вентиляции и функцию легких в раннем послеоперационном периоде у детей после колоэзофагопластики. Материалы и методы. Ретроспективное исследование, включающее 40 детей возрастом 12,0 (10,0; 18,0) месяцев и весом 9140±1444,1 г. Мы оценивали параметры вентиляции, длительность механической вентиляции и длительность пребывания в детском отделении реанимации и интенсивной терапии (ОРИТ). Результаты. Слабая прямая корреляция выявлена между общей перегрузкой жидкостью и перегрузкой жидкостью в ОРИТ за первые сутки, с одной стороны, и длительностью ИВЛ и длительностью пребывания в ОРИТ, с другой. Уровень SvO2 на 1-е сутки имел слабую отрицательную корреляцию с интраоперационной перегрузкой жидкостью. Уровень SvO2 на 2-е сутки имел слабую отрицательную корреляционную связь с перегрузкой жидкостью в 1-е сутки в ОРИТ. Уровень SvO2 на 1-е и 2-е сутки коррелировал с общей перегрузкой жидкостью на 1-е сутки (R=-0,414 и R=0,402 соответственно). Рin на 1-е сутки слабо коррелировало с интраоперационной перегрузкой жидкостью, с перегрузкой жидкостью в первые сутки в ОРИТ. Рin на 1-е и 2-е сутки коррелировало с общей перегрузкой жидкостью. We evaluated the ventilation parameters, the duration of mechanical ventilation, and the duration of intensive care unit stay. Results. A weak positive correlation was found between total fluid overload and fluid overload in the ICU on the 1st day, on the one hand, and the duration of mechanical ventilation and the duration of stay in the ICU, on the other. SvO2 on the 1st day had a weak negative correlation with intraoperative fluid overload. SvO2 on the 2nd day had a weak negative correlation with fluid overload on the 1st day in the ICU. SvO2 levels on 2nd and 3rd days correlated with total fluid overload on the 1st day (R=-0.414 and R=0.402, respectively). Aim. To identify the relationship between fluid overload in children who received infusion therapy in the postoperative period at surgical intensive care unit (ICU), the development of acute kidney injury (AKI), and the length of ICU stay. Methods. A retrospective cohort observational study was conducted. Our study includes 75 newborns with malformations of the gastrointestinal tract, diaphragmatic hernias, gastroschisis, necrotizing enterocolitis and sacrococcygeal teratomas treated after surgery in the ICU. The Kidney Disease: Improving Global Outcomes (nKDIGO) scale was used to detect AKI. To assess the fluid overload, the ratio of the difference between the introduced and released liquid and entering the ICU weight was used (in %). Significant overload was adopted value more than 5%. The indicator injected fluid consisted of the volume of infusion and enteral nutrition. The indicator of released fluid included diuresis, fluid loss from the gastrointestinal tract, intestinal stomas, and drainages. Results. The frequency of AKI was 28.0% (21 of 75). The average time of AKI development from the moment of surgical intervention was 5.9 3.29 days. The duration of ICU stay after surgery was 8.0 [5.0; 16.0] days. In 18 of 75 (24%) children, the fluid overload of more than 5% was observed on the 1st postoperative day. In 12 out of 75 (16%) children, the fluid overload of more than 5% was observed on the 2nd postoperative day. A fluid overload of more than 5%, observed on the 1st day after the operation, was associated with a longer ICU stay. The rate of AKI is higher when a fluid overload of more than 5% on the 2nd day after surgery was observed. Conclusion. Severe fluid overload can be a significant factor of the AKI development. Fluid overload more then 5% on the 2nd day after surgery may lead to an increase in the incidence of AKI. Liquid overload more then 5% on the 1st day after surgery may lead to an increase in the length of ICU stay. In newborns, it is necessary to take into account the fluid load at the first two days after the operation and correct the infusion therapy in case of fluid overload detection.
Extracorporeal membrane oxygenation (ECMO) was firstly used in 1977 for the treatment of congenital diaphragmatic hernia (CDH) in a neonate. The first time ECMO was administered in our country to a child with CDH on the 10th of January, 2013, at the intensive care department of Municipal Pediatric Hospital No. 13 named after N.F. Filatov. However, the first patient with CDH who had received ECMO, had survived decannullation and had been discharged from the hospital arrived to this hospital only on the 12th of October, 2015. The aim of ECMO was to stabilize the child’s condition and to obtain an opportunity of surgical correction of the congenital malformation. Indications included increasing cardiopulmonary failure, hypoxemia (РаО 2 50, the alveolar-arterial oxygen gradient> 600 for 8 hours), combined acidosis, hypotension, suprasystemic pulmonary hypertension (main pulmonary artery pressure was higher than 100 mm Hg). There were no technical issues or complications during the procedure. Hemostatic therapy under the Activated Clotting Time (ACT) supervision, artificial lung ventilation (ALV), intravenous fluid and transfusion therapy were carried out. Cardiotonic therapy was canceled after the beginning of veno-arterial ECMO due to hemodynamic stabilization. Protective artificial ventilation was applied: VR-20; Pin-18mbar; PEEP-7mbar; FiO 2 -0,21. ACT was kept at 203 seconds in average at the average heparin infusion speed of 4 unit/kg/hour. Acidbase balance indicatiors: рН-7,38; РаО 2 -121,7 mm Hg, РаСО 2 -44,0 mm Hg, ВЕ-3,3, lactate 1,3. Average central venous pressure was 6,6 mm Hg., average invasive blood pressure - 51 mm Hg. After 72 hours of artificial circulatory support ECMO was canceled and surgical intervention was started after gas exchange and hemodynamic stabilization.
