The review takes into consideration the problems of effectiveness and safety of anticoagulant therapy in patients with venous thromboembolic complications. together with literature data we provide the results of our own study of optimization of long-term anticoagulant therapy in venous thromboembolic comlications. Particularly the results provided of comparison of standard therapy by high-molecular weight heparin (5 days or more) and warfarin and therapy by treatment dosages of enoxaparine with the prolonged treatment to one month in outpatient practice. Also the opportunities discussed for the usage of novel oral anticoagulants — dabigatran elexilate, rivaroxaban, apixaban — as for the treatment of acute thrombosis episode, as in long-term period. the detailed view is presented at the defining of optimal duration of anticoagulant therapy and the factor influencing the effectiveness and safety of long-term treatment. As the latter we recommend to use laboratory markers: d-dimer and thrombin-activating fibrinolysis inhibitor.
В обзоре представлены результаты исследования RELY-ABLE, в котором были изучены возможности длительной терапии дабигатраном при фибрилляции предсердий. RELY-ABLE является продолжением исследования RE-LY, спланированного для сравнения двух доз дабигатрана (110 и 150 мг 2 раза в день) с вар-фарином у больных фибрилляцией предсердий с высоким риском инсульта. Согласно протоколу, участие в RELY-ABLE продолжили больные из RE-LY, получавшие дабигатран, поэтому в RELY-ABLE сравнили между собой эффективность и безопасность двух доз дабигатрана (110 и 150 мг 2 раза в день). В целом частота ишемических, геморрагических и фатальных событий в RELY-ABLE оказалась вполне сопоставима с таковой в RE-LY. При сравнении двух доз дабигатрана в RELY-ABLE не выявлено существенных различий по частоте инсульта или уровню смертности, но отмечена более высокая частота больших кровотечений для дозы 150 мг. Таким образом, RELY-ABLE предоставляет дополнительную информацию об эффективности и безопасности длительной (в среднем 4,3 года) терапии дабигатраном в дозах 110 и 150 мг дважды в день у больных фибрилляцией предсердий. Также в обзоре представлены результаты обсервационного исследования Medicare, подтвердившего результаты RE-LY и RELY-ABLE в реальной клинической практике на более чем 130 тыс. больных фибрилляцией предсердий.
Aim. To study the factors associated with elevated D-dimer levels in patients with acute venous thromboembolic events (VTEE). material and methods. The study included 111 patients (76 men and 35 women aged 18–76 years) with a first or repeat episode of deep vein thrombosis (DVT) and/or pulmonary embolism (PE) in the last 2 months. The majority of the patients (n=80) received unfractionated heparin (UFH) for at least 5 days, followed by warfarin (international normalized ratio (INR) control at least once a month; target INR 2,0–3,0). Some patients (n=31) received therapeutic doses of enoxaparin (1 mg/kg subcutaneously, every 12 hours) for at least 30 days, followed by warfarin treatment. D-dimer levels (norm ® D-DI” reagents (Diagnostica Stago). Results. D-dimer levels varied from 0,02 to 9,96 mkg/ml (median 1,05 mkg/ml, interquartile range 0,49–1,99 mkg/ml) and exceeded the upper norm limit in 74% of the patients. There was a positive association between D-dimer levels and thrombus “size” (r=0,304; p < 28 days, and thrombus “size” < 6 points were independent predictors of D-dimer elevation in the acute period of DVT/PE. Conclusion. D-dimer levels, measured 32 (23–44) days after the development of DVT/PE symptoms, were elevated in 74% of the patients. D-dimer elevation in the acute period of VTEE was associated with female gender, CHF, “age” and “size” of the thrombus.
