Aim. To study the clinical peculiar features of sleep and its structure by the data of night polysomnographic study and peculiarities of development of higher cortical functions (HCF) in children with obstructive sleep apnea/hypopnea syndrome (OSAHS). Materials and methods. Clinical, neuropsychological and polysomnographic examination of 25 preschool-age children with OSAHS was carried out. The group of сomparison included 10 healthy children. Clinical study of sleep was assessed, using specially developed by the author sleep questionnaire; HCF status was estimated by means of neuropsychological study, adapted for child age (E.G. Simernitskaya). Results. Some peculiar features of organizing sleep in children with obstructive night apnea were detected. There was demonstrated a reduced duration of drowsiness, decreased duration of fast sleep phase, reduced number of sleep cycles and vegetative disorders in the form more frequent episodes of tachycardia, confirming sympathicotonia during night sleep. Kinesthetic dyspraxia was registered in 76 % of observations among apnea children. Differences in anxiety level were registered in extreme values of anxiety index (AI); AI > 50 %, considered as high, was revealed in 72 % of patients with OSAHS. High productivity and stability of attention ( S = 1.00-1.25) was noted in 35 % of control children, in OSAHS group this index was 4 %. Conclusions. Reduced duration of fast sleep phase in children with apnea is a matter of principle. Not only snore is a marker of OSAHS, apnea can occur without marked concomitant snoring. The role of connective tissue dysplasia in OSAHS formation is to be discussed. Muscular hypotonia formed can increase constriction of the upper airways, especially within the fast sleep phase and provoke apnea/hypopnea episodes.

Изучить клинические особенности сна и его структуры по данным ночного полисомнографического исследования и особенности развития высших корковых функций у детей с синдромом обструктивных апноэ/гипопноэ сна (СОАГС). Материалы и методы. Проведено клиническое, нейропсихологическое и полисомнографическое обследование 25 детей с СОАГС в дошкольном возрасте. Группу сопоставления составили 10 здоровых детей. Клиническое исследование сна проводилось по специально разработанной авторской анкете сна. Оценка состояния высших корковых функций осуществлялась с применением нейропсихологического исследования, адаптированного для детского возраста (Э.Г. Симерницкая). Результаты. Выявлены некоторые особенности организации сна у детей с обструктивными ночными апноэ. Продемонстрировано сокращение продолжительности стадии дремоты, уменьшение продолжительности фазы быстрого сна, сокращение количества циклов сна и вегетативные нарушения в виде учащения эпизодов тахикардий, свидетельствующих о симпатикотонии во время ночного сна. Кинестетическая диспраксия имела место в 76 % наблюдений у детей с апноэ. Различия уровня тревожности зарегистрированы в крайних значениях индекса тревожности (ИТ). ИТ выше 50 % расценивается как высокий, выявлен у 72 % пациентов с СОАГС. Высокая продуктивность и устойчивость внимания ( S = 1,00-1,25) отмечаются у 35 % детей контрольной группы, в группе с СОАС данный показатель составил 4 %. Выводы. Принципиальным является уменьшение продолжительности фазы быстрого сна у детей с апноэ. Не только храп является маркером СОАГС, апноэ может иметь место без выраженного сопутствующего храпа. Требует обсуждения роль соединительнотканной дисплазии в формировании СОАГС. Формирующаяся на этом фоне мышечная гипотония может усиливать сужение верхних дыхательных путей, особенно в фазу быстрого сна, и провоцировать эпизоды апноэ/гипопноэ.
Peculiarities and structure of sleep of children suffering from adenoid and tonzillar pathology, which were revealed during night polysomnographic test are discussed. The presence of non-specific neurological and logopaedic complaints, dissomnic disorders and sleep macrostructure disorder were proved. The researchproves shortening of the somnolence phase, increase of latent period of rapid eye movement sleep, while it is decreased in night sleep cycles, increase of delta-sleep, increase of the time of being awake at night. The disorders mentioned above reveal some pthologic and genetic aspects of development of children suffering from obstructive apnoea, this necessitates the use of individual program of rehabilitation.
Children's speech syndromes are rarely an independent disorder. They often combine with other pathologic conditions and present a complex clinical picture. The article describes two clinical observations of Landau-Kleffner acquired epileptic aphasia. Two types of syndromes singled out on the basis of their aetiology are presented, they are: idiopathic and symptomatic. Clinical observations underline clinical polymorphism of Landau-Kleffner syndrome, which makes it difficult to diagnose.
The paper describes clinical cases of symptomatic and idiopathic acquired epileptiform aphasia (LandauKleffner syndrome). The necessity of carrying out the EEG procedure during the night and day sleep in all children with psycho-speech retardation for the early diagnosis and efficient treatment of epileptiform aphasia is discussed.
