Our study is a retrospective analysis of 96 cases of medulloblastoma in children and adults, followed up in the department of radiotherapy of the National Institute of Oncology, over a period of 9 years between January 2002 and December 2010. Medulloblastoma represent, according to literature data, less than 1% of brain tumors in adults, but is the most frequent brain tumor of children. Median age at diagnosis was 8 years for children and 24 years for adults. 63 cases (65.6 %) were males. Symptoms was dominated by intracranial hypertension syndrome occurred in 87 cases (97.8%), associated with a cerebellar syndrome noted in 83 cases (94.3%).The most frequent localization seat in cerebellar vermis in 43 cases (76.8%) for children and in the cerebellar hemisphere in 28 cases (70%) for adults. Diagnosis was based on histological examination of the surgical specimen. Classical histological sub type was predominant 88 patients (91.7% of cases). The first step in the treatment of medulloblastoma is resection of the tumor. Surgical resection was complete in 53 cases (55.2%). Adjuvant radiotherapy associated or not with chemotherapy is essential. Irradiation of the entire central nervous system at the dose of 36 Gy, followed posterior fossa boost to 56 Gy was performed in 74 cases (77.1 %). The 5 year overall survival rate for the whole patients was 50%. Therapeutics advances, especially in radiation therapy techniques, improve the efficiency of the treatment, increasing survival and reducing sequelae
BACKGROUND: Malignant triton tumor (MTT) is a variant of the typical malignant peripheral nerve sheath tumors with rhabdomyoblastic differentiation what mostly occur in the context of neurofibromatosis type 1. CASE PRESENTATION: Here, we report on a case of a 15-year-old male with a clinical history of neurofibromatosis type 1 who presented to the pediatric emergency department with an intraperitoneal tumor of about 20 cm of diameter. The biopsy of this tumor confirmed histology of rhabdomyosarcoma. The patient received six cycles of chemotherapy. After that, resection of the tumor was performed. Histological findings revealed a malignant peripheral nerve sheath tumors (MPNST) of Triton type. Then he received 28 sessions of radiation therapy. The child died 11 months after radiation therapy. CONCLUSION: Malignant triton tumor is a very aggressive tumor. A multidisciplinary approach is required for diagnosis and treatment of this rare pediatric tumor.
L’irradiation des cancers de la prostate est une indication principale de la radiothérapie guidée par l’image. L’objectif de notre travail était de savoir si les marges du volume cible prévisionnel (PTV) appliquées dans notre centre sont suffisantes pour couvrir le volume cible. Notre étude inclue les patients pris en charge pour un cancer de la prostate par arcthérapie volumétrique modulée entre septembre et décembre 2017. Lors de la première consultation, nous avons insisté auprès des patients sur l’importance du respect des conseils hygiénodiététiques. Les marges du volume cible prévisionnel appliquées étaient de 7 mm dans toutes les directions sauf en arrière, où elle était de 5 mm. Les traitements ont été faits en appliquant les décalages calculés. Les données générales ainsi que les déplacements appliqués pour tous les malades inclus dans les trois plans ont été recueillis. Notre étude a inclus dix patients. La moyenne d’âge était de 71 ans ± 5. Tous les cancers étaient classés de haut risque selon la classification de D’Amico. Au total, 260 tomographies coniques ont été analysées, 12,6 % (n = 33) ont était refaites le même jour (en raison de la distension rectale pour 90 %). La médiane des décalages dans la direction verticale était de 1,5 mm ± 0,9 avec un minimum de 0,9 mm et un maximum de 4,5 mm, 1,7 % des décalages étaient de plus 7 mm. Pour la direction latérale, la médiane était de 2,5 mm ± 1,2 avec un minimum de 1 mm et un maximum de 5 mm et 3,9 % des décalages > 7 mm. La médiane des décalages dans la direction longitudinale était de 3,2 mm ± 1,2 avec un minimum de 1,6 mm, un maximum de 5,2 mm et 17,9 % étaient de plus de > 5 mm. Les médianes des déplacements durant les trois premières tomographies coniques et les deux dernières ont été analysées ; il existait une différence significative des médianes des déplacements entre le début du traitement et la fin dans l’axe antéropostérieur. Notre étude nous a permis de montrer que la marge choisie de 7 mm dans toutes les directions était suffisante. Sauf en arrière, cette marge de 5 mm semblait être insuffisante. Il y a une nécessité de contrôles plus réguliers et d’une meilleure préparation des patients.
Aim: To determine the effect of waiting time for radiotherapy on the overall survival of patients with glioblastoma Patients and methods: We examined records of patients with glioblastoma who were referred to radiotherapy after surgery or biopsy, any age, radical intent or palliative intent with or without chemotherapy. Waiting time was defined in two ways: time from biopsy to radiotherapy and time from presentation to radiotherapy department to start of radiotherapy Results: There were 102 patients in the study with a median follow up of 9 months. All patients have a biopsy or debulking surgery and postoperative radiotherapy. The surgical procedures were as follows: biopsy only in 34 (33,3%) patients and total resection in 68 (66,6%) patients. 78 patients received radiotherapy alone and 24 patients received radiotherapy plus continuous daily temozolomide, followed by six cycles of adjuvant temozolomide. Median times and ranges of waiting times from biopsy and presentation time to radiotherapy were 52 days (11±110 days) and 32 days (4±80 days) respectively. In univariate analysis older age and absence of total resection were all significantly associated with a reduction in overall survival. While in multivariate analysis, only the absence of total resection was the variable significantly associated with reduced overall survival. Conclusion: It was not possible in this analysis to evidence of the impact of delays in initiating radiotherapy for patients with glioblastoma.
Évaluer la radiothérapie hypofractionnée adjuvante en cas de cancer du sein avec envahissement ganglionnaire massif (de stade N2-N3), aussi bien en termes d’efficacité (contrôle locorégional, survies globale et sans rechute) que de toxicité. Les dossiers de 113 patientes, traitées entre octobre 2009 et juin 2011, d’âge moyen de 48,2ans ± 10,9 ans ont été analysés. Toutes les patientes ont reçu une radiothérapie hypo fractionnée (42 Gy, soit 2,8 Gy/fraction, cinq fractions/semaine) dans le sein (20,4 %, n = 23) ou la paroi thoracique (79,6 %, n = 90) et la région susclaviculaire (99,1 %, n = 112) ou axillo-sus-claviculaire (0,9 %, n = 1). En cas de traitement conservateur, un boost a été délivré. Toutes les patientes ont reçu une chimiothérapie. Le nombre médian de ganglions axillaires prélevé était de 15 (7–33), avec un nombre moyen de 8,4 ± 4,6 de ganglions métastatiques. Par ailleurs, 68,1 % des cancers (n = 77) étaient de stade N2, pour 62,8 % (n = 71), le ratio ganglions envahis/ganglions prélevés était entre 20 et 65 %, pour 31,9 % (n = 6) de plus 65 %. Le suivi médian était de 61 mois. À cinq ans, les probabilités de survie globale, de survie sans récidive, les taux de contrôle locorégional et de contrôle à distance étaient respectivement de 81,1 %, 80,9 %, 90,3 % et 77,9 %. En analyse multifactorille, la survie globale et sans rechute à 5 ans des malades atteintes de cancer triple négatif et avec un ratio ganglionnaire supérieur à 65 % différaient statistiquement significativement. En ce qui concerne toxicités, 8,4 % des patientes ont souffert d’un lymphœdème. Il n’a été observé aucune plexite radique. Nos résultats sont encourageants, la radiothérapie hypofractionnée peut être proposée en cas d’envahissement ganglionnaire de stade N2 ou N3.
