Background Within the group of inflammatory myopathies, Inclusion-Body Myositis (IBM) is a distinct pathology with clinical and histological features very different from other entities. In the literature, an association between primary Sjögren’s syndrome (pSS) and IBM has been previously reported. Objectives The aim of our study was to compare the inflammatory infiltrates between IBM associated or not with pSS and, among patients with pSS, between muscle and salivary gland. Methods We conducted a translational research project, from formalin-fixed and paraffin-embedded muscle biopsies of patients with IBM, associated with Sjögren’s syndrome (IBM-pSS) and sporadic (sIBM) forms, from 6 French expert centers, and salivary glands were collected when available. Imaging mass cytometry (IMC) multiplex immunostaining (37 markers) was used to quantify and decipher the composition of the inflammatory infiltrates within samples (Figure 1). Supervised and unsupervised statistical analyses were performed to compare IMC data between IBM-pSS and sIBM patients and muscle and salivary glands among patients with pSS. Results No statistically significant difference was encountered in our comparisons between IBM-pSS and sIBM muscle biopsies. Nevertheless, some trends were pointed and could merit further work in larger case series. Indeed, IBM-pSS samples (14 patients) tended to have more widespread inflammatory infiltrate than sIBM samples (7 patients) involving on average 4.8% versus 1.6% of muscle biopsy surface respectively (p=0.12). Macrophages and T cells were the predominant populations in both IBM-pSS and sIBM patients’ groups. Among T cells, CD8+ cells predominated compared to CD4+ cells in both groups. The proportion of plasma cells (CD138+ CD38+ CD27+) was higher in IBM-pSS than in IBMs patients (medians of 14.7% and 8.5%, respectively, p=022), as was the proportion of B cells (median of 3.1% and 0.5%, respectively, p=0.14). Comparing salivary gland and muscle biopsies in patients with pSS, the plasma cells were predominant in the salivary gland with more numerous plasma cells and B cells and less numerous macrophages and T cells in salivary glands than in muscle samples. Nodular inflammatory infiltrate pattern containing B cells appeared to be restricted to pSS patients both in muscle and salivary gland biopsies. Conclusion Using for the first time IMC on muscle biopsy and salivary gland has allowed us 1) to point difference between IBM-pSS and sIMB and 2) similarities between salivary and muscle inflammatory infiltrates in patients with pSS samples. These differences suggest that IBM associated with pSS has a distinct pSS-related pathophysiology compared to sIBM, which could raise potential diagnostic and therapeutic applications. Au sein du groupe des myopathies inflammatoires, la myosite à inclusions (Inclusion-Body Myositis, IBM) est une pathologie à part, présentant des caractéristiques cliniques et histologiques très différentes des autres entités. Dans la littérature, une association entre le syndrome de Sjögren primitif (pSS) et l’IBM a été suggérée par plusieurs travaux. L’objectif de notre étude a été de caractériser l’infiltrat inflammatoire des IBM associées au pSS, en le comparant à celui de patients atteints de formes sporadiques de la maladie. Il s’agit d’un projet de recherche translationnel, conduit à partir de biopsies musculaire fixés en formol et inclus en paraffine, de patients issus de 6 centres experts et ayant fait l’objet d’une caractérisation clinique détaillée, rétrospective. Ainsi, 14 patients présentant une myosite à inclusion associée au syndrome de Sjögren (IBM + pSS) ont été inclus et comparés à 7 patients présentant une myosite à inclusion sporadique (IBMs). De nouvelles coupes tissulaires ont été réalisées pour étude de l’infiltrat inflammatoire en immunomarquage multiplexe par imagerie par cytomètre de masse (imaging mass cytometry, IMC) HYPERION. Après comparaison histologique en IHC, des analyses statistiques supervisées et non supervisées ont été utilisées dans la comparaison des données d’IMC entre les deux groupes de patients IBM + pSS et IBMs. Les biopsies des patients IBM + pSS présentent d’avantage d’infiltrat inflammatoire que celles des patients IBMs avec une surface relative occupé par l’infiltrat inflammatoire respectivement de 5,1 % versus 1,9 %). Les macrophages et lymphocytes T sont les populations prédominantes au sein de l’infiltrat inflammatoire musculaire au sein des 2 groupes (avec une médiane respectivement à 52,7 % et 35,0 % dans le groupe IBMs et 49,1 % et 17,8 % dans le groupe IBM + pSS). La proportion de plasmocytes (CD138+ CD38+ CD27 + ) au sein des patients IBM + pSS est supérieure à celui des patients IBMs (respectivement médiane à 14,7 % et 8,5 %), de même pour la proportion des lymphocytes B plus importante dans le groupe IBM + pSS (respectivement médiane à 3,1 % et 0,5 %). Concernant les lymphocytes T (LT), il prédomine principalement des LT CD8+ par rapport aux LT CD4+ dans les 2 groupes. Par rapport aux patients IBMs, l’infiltrat au sein des patients IBM + pSS est globalement caractérisé par une prédominance des mêmes types cellulaires que chez les patients IBMs avec, comme décrit dans la littérature des patients IBMs, une prédominance de macrophages et de lymphocytes T. Cependant cet infiltrat semble plus abondant chez les patients IBM + pSS et chez qui il existe également une augmentation de la proportion des lymphocytes B et des plasmocytes par rapport aux patients IBMs. Cet infiltrat musculaire des patients IBM + pSS présente ainsi des caractéristiques pouvant se rapprocher de l’infiltrat lymphoplasmocytaire observé sur les glandes salivaires des patients pSS. Cette étude suggère qu’il existe une augmentation de la proportion des lymphocytes B et des plasmocytes au sein de l’infiltrat inflammatoire des patients IBM + pSS, par comparaison aux patients atteints de formes sporadiques de la maladie.
